If you are an operator of inflatable or zip line devices only, your Operating Licence and all your Device Permits expire on May 31, Important Notice TSSA no longer accepts cash payments. (As of May 2, 2011) The following payment methods will continue to be accepted: Credit Card Visa and MasterCard Debit Cheques Money Orders Please note the following to complete your renewal process: STEP 1: Required Information and Renewal Documents a) Licence Renewal Application An Application for Ontario Licence to carry on the business of operating Amusement Devices must be completed. The application is enclosed. A fillable PDF version is available on TSSA s website. Remember to include a list of mechanics that will erect and maintain your amusement devices for the season. If a maintaining contractor/independent mechanic is the designated mechanic(s), the Declaration of Maintaining Contractor/Mechanic for Amusement Devices, Form No. AD 09064, must be completed and submitted as proof of services retained. The declaration is enclosed. b) Insurance TSSA requires applicants to submit, with their completed application, an original Certificate of Insurance, in Acord or CSIO form, evidencing a minimum of $2 Million of Liability Insurance.The Acord or CSIO Certificate of Insurance may be issued by an insurance broker or other intermediary on behalf of the Insurance company but this should be clearly indicated on the certificate.certificates of Insurance issued in a form other than an Acord or CSIO will not be accepted. Please refer to the Insurance Document enclosed for further clarification.
2 c) Collections Applicable fees and outstanding 2017 invoices must be paid. We have provided you with an invoice for your licence renewal and devices. Please indicate the devices you wish to renew when you submit payments so that we may process the permits for only those devices that will be operating this year. **Please note that all overdue fees must be paid prior to your licence renewal. An Account Summary Notice has been included with this package, along with copies of overdue invoices** d) Operating Schedules Owners are required to submit a copy of their Operating Schedule (to the extent known) in an MS-Excel format as denoted in the Amusement Devices Operating Schedule Season sample template shown below: ADL # ADL 1234 Licensee Name Arriving Date Event Stats / Show Opens Departure / Closing Date Fun Rides May 28, 2018 May 30, 2018 June 6, 2018 Location Address 345 CarlingView Drive,Toronto Event Name (Optional) FunFest Copies of this MS-Excel template can be obtained from the TSSA website at: Owners of traveling shows must complete all fields. Owners of Fixed Devices may omit the Arriving Date. Please note a licence will not be issued until TSSA is in receipt of the required operating schedule in the approved electronic Excel format. The Excel form is available for download on the TSSA website. Paper versions or modified electronic versions will not be accepted. Completed electronic files must be sent by to adoperatingschedules@tssa.org. Where an operating schedule is incomplete the submitter is required to update their operating schedule accordingly, over the course of the season through the designated address. Where an inspector discovers a company without an up-to-date operating schedule, orders will be issued, and a device may be shut down. Applications received for a licence without the Operating Schedule(s) will receive written notification of the requirement and the application will be placed on hold. STEP 2: Wait for confirmation that TSSA has approved your licence renewal and that the correct devices have been renewed. Please allow 2-3 weeks to process this information. If you have not received your renewed licence within 3 weeks, please contact our office. It is advisable to submit items to TSSA as quickly as possible to avoid any delays. Page 2
3 STEP 3: Call for an Inspection Once your licence has been renewed and you have received confirmation from TSSA, please call your TSSA inspector to perform the inspection to obtain your 2018 operating permit. Once the inspection is successfully completed the inspector will validate your permits on site. Please remember that all devices must pass inspection before they may be operated. A list of TSSA amusement devices inspectors and their contact information is available at Please ensure the appropriate inspector with the Amusement Device Designation is contacted. 4 AD Renewal Package Insurance Requirements Insurance Requirements O.Reg 221/01, s. 5 (3) Insurance Document In keeping with its commitment to streamline its processes and level the playing field, TSSA has simplified its process to validate / verify compliance with the insurance requirements detailed in O.Reg 221, s. 5 (3) which states; Every person who carries on the business of operating amusement devices shall obtain and maintain liability insurance in respect of the business in the amount not less than $2,000,000 per occurrence with a carrier licensed in Ontario and/or Canada. O. Reg 221/01, s. 5 (3). It is an important component of TSSA s mandate to protect the public interest by ensuring that there is insurance available to protect the Public. A Certificate of Insurance, in Acord or CSIO form, is required to evidence compliance with O. Reg 221/01, s. 5(3). Requirement for a Certificate of Insurance Insurance agents, brokers and managing general agents are recognized by the insurance industry as authorized intermediaries. To acknowledge this relationship, and as a means to simplify its processes, TSSA will once again accept certificates of insurance from these parties. The certificates of insurance must conform with industry accepted standards, that is, they must be prepared on an Acord or CSIO form. To obtain an Acord or CSIO certificate of insurance, simply contact your insurance agent or broker and ask them to issue and sign a Certificate of Insurance, in Acord or CSIO form, noting TSSA as the certificate holder and detailing the devices to be insured, the limits required under Regulation 221(5) 3 and confirmation that a 30 day notice of cancellation and/or material change in coverage clause has been endorsed onto the policy. It is also recommended that the Certificate of Insurance reference your Amusement Devices Licence Number to help ensure timely processing. A sample Certificate of Insurance is attached for your reference. The Certificate of Insurance must then be submitted to TSSA with the annual licence application. Notification of Changes to Insurance Whenever there is a change to insurance coverage including cancellation or renewal, TSSA must be notified to keep insurance information current at all times. Please provide a new Acord or CSIO Certificate of Insurance within 30 days of renewals or changes. Failure to notify TSSA will result in invalidating the licence.
