(Underwritten by Lincoln Financial Group Rated A+ Superior by AM Best) Railroad Retirement Benefits fall short of giving you and your family complete income protection from death and disability. This plan helps fill the gaps and protects you and your family from loss of income due to: Injury Illness Death Dismemberment This presentation is not a contract or policy of insurance. It contains general information and intended to explain some of the major features of the coverage provided by the group policy and certificate. The specific terms of your group policy and certificate will govern any claim for benefits you may make. Visit for specific information regarding RRB disability benefits.
2 Short Term and Long Term Disability - Option A STD and LTD Option A are provided by SEPTA at no cost to Division 71 members STD pays 60% of income to a maximum of $100 Per Week Starts paying on DAY 8 for injury Starts paying DAY 15 for sickness STD pays for a maximum of 12 weeks after waiting period (ends on day 91) No pre-existing condition limitations for Short Term Disability Option A STD has no offsets (stackable with RRB) until 100% of pre-disability earnings are attained LTD pays 60% of monthly income up to $500 per month LTD pays for up to 24 months Pre-existing condition limitations may apply (see next page) to Long Term Disability Short Term and Long Term Disability - Option B STD and LTD Option B is voluntary to all Div 71 members at a monthly cost of $64.02 Pays 60% of weekly income to a maximum of $692 Per Week Starts paying on DAY 8 for injury Starts paying DAY 15 for sickness Pays for a maximum of 12 weeks after waiting period (ends on day 91) No pre-existing condition limitations for Short Term Disability Option B STD has no offsets (stackable with RRB) until 100% of pre-disability earnings are attained LTD pays 60% of monthly income up to $3,000 Per Month (offset by RRB benefits) LTD pays for up to 24 months Pre-existing condition limitations may apply (see next page) to Long Term Disability Lincoln covers you on and off the job (24hr coverage)
3 Pre-Existing Condition Limitations A twelve (12) month pre-existing condition limitation applies to Long Term Disability. Specifically, members who are electing the buy-up LTD Option B for the first time during this open enrollment or those members who have not yet been covered by the current UNUM plan for more than twelve months will be subject to this limitation. You must have twelve months of continuous LTD coverage to have satisfied this pre-existing condition limitation. If you have been covered under the UNUM plan for more than twelve months then you have met the pre-existing condition limitation with Lincoln Financial Group as well. Lincoln Financial i credit s you for the time you have been insured on the previous plan with UNUM. Newly diagnosed conditions are covered from the plan effective date (February 1, 2015) for all participating i members.
4 Group Voluntary Life Insurance & Accidental Death & Dismemberment (AD&D) Guaranteed Issue amounts up to $250,000 Member can elect limits of $10,000 (1 unit) up to $500,000 or 5x annual income (whichever is less) May increase both member and spouse life insurance election by as much as two units each year on the anniversary of the policy. May exceed Guaranteed Issue limit in future years. Includes equal amount of AD&D (Double Indemnity) No pre-existing condition limitations Premium rate is the same for all members Can add spouse and children to policy Spouse is guaranteed issue up to $50,000 Spouse can elect limits of $5,000 (1 unit) up to $250,000, but may not exceed 50% of life coverage member elects Children are guaranteed issue $15,000 (per child) **Electing child life insurance covers all children at one fixed cost ($2.85 per month), regardless of how many children you have Guaranteed portability options available upon leaving the union prior to retirement (must elect within 31 days of departure) Guaranteed conversion options available upon retiring i (must elect within 31 days of retiring) Lincoln covers you on and off the job (24hr coverage)
5 Accidental Death & Dismemberment 24 hour accident coverage Included in the cost of life insurance Pays 100% of coverage in the event that an accident causes loss of life, two limbs, the loss of sight in both eyes, one limb and the loss of sight in one eye, speech and hearing loss in both ears, or quadriplegia Pays a graded benefit for less severe injuries
6 Premium Payments Due to recent changes in BLET By-Laws Division 71 can no longer payroll deduct d your insurance premium along with your union dues. Moving forward (starting February 2015) your monthly premium will be automatically deducted from a personal checking or savings account of your choice. Vanco Services, LLC, which is a PC-1 Level Service Provider and a licensed, bonded and insured ACH transfer company will automatically draft your account each month for the amount of your monthly premium. You may choose to have your monthly premium deducted once per month or split equally in two drafts per month. You may choose whichever day(s) you would like your draft to occur each month. When you enroll (either online, over the phone or by completing the enclosed forms) you will need to provide the routing and account number for the account you wish to pay your premium from.
7 Eligibility Requirements Are you eligible? All working members of BLET Division 71 SEPTA are eligible Must be working when the policy goes into effect Non working members who are out because of injury, illness or loss of job status (discipline, furlough, etc.) are covered the day they go back to work if they enroll during open enrollment. Non-working members who do not enroll during open enrollment are treated as new members when they return to work.
