It describes the benefits and features specific to Family members. It also states that some benefits and features in the PDS and Factsheets may not apply to you. Therefore, you should read this Plan Summary carefully and in conjunction with the PDS and all the Factsheets to understand how SignatureSuper works. Unless defined in this Plan Summary, terms defined in the PDS and Factsheets have the same meaning in this Plan Summary. Terms used in this Plan Summary include: Participant means D'Orsogna Limited who has who has agreed with the trustee to set up the Plan with the specific benefits and features outlined in this Plan Summary; and Plan means the D'Orsogna Superannuation Plan in SignatureSuper. 3 Your benefits at a glance The purpose of this Plan Summary is to give you important information about the benefits you receive as a member of the D'Orsogna Superannuation Plan. Key features and benefits FEATURE/ BENEFIT EXPLANATION Joining SignatureSuper You can apply to join the Plan as a family member if you are aged 13 years or more and you have an existing family relationship with an employee member either as: a. The employee member s spouse, child (including an adopted child, a step child or an ex-nuptial child), parent, sibling, grandparent, grandchild, father/mother-in-law, brother/sister-in-law, son/daughter-in-law, or b. A person in an interdependency relationship with the employee member. If you would like to join the Plan as a family member, simply complete the Family member application forms booklet. As a family member you must start with an account balance of at least $1,000. Your application must be accompanied by $1,000 or more made up of: a. A contribution (by cheque payable to AMP Life Limited SignatureSuper ), or b. An instruction to transfer (rollover) superannuation money you currently have in another fund into SignatureSuper (use the Request for transfer of member superannuation benefits form ). You can remain a member of the Plan in SignatureSuper only as long as the employee member remains a member of the Plan, you continue to have a family relationship with the employee member, and your account balance remains above $1,000 (please see Dormant accounts transfer to a personal account in the Member Guide Fact Sheet). After you become a family member of the Plan, if you no longer have a family relationship to the employee member (e.g. the employee member and spouse divorce or separate) you need to let us know and we will transfer your benefit according to the process in Leaving the Plan the payment process section of this Plan Summary. Contributions Your current employer may agree to make employer contributions to your account. How your super is invested You may be able to arrange with your current employer to make personal/salary sacrifice contributions via payroll deduction. You can also make personal contributions via BPAY or cheque. You are able to rollover superannuation money into this Plan that you have in other superannuation funds outside of SignatureSuper. Your account balance and all future contributions will be invested using the SignatureSuper LifeStages investment approach until you choose to change your investment options. The Investment Guide Fact Sheet describes the investment options available to you. Fees Details of the fees charged to your account are shown under the Fees and other costs section of this Plan Summary. Insurance cover If you are age 16 or more and are otherwise eligible, you may apply for insurance cover in the event of your Death or Total and Permanent Disablement ( TPD ). For more information, see the Insurance cover section of this Plan Summary. Insurance premium rates The current insurance premium rates are included in the Insurance Guide Fact Sheet. Who provides the insurance AMP Life Limited ABN provides the insurance for your Plan. Please refer to the Insurance Guide Fact Sheet for details. 2

4 Key features and benefits FEATURE/ BENEFIT EXPLANATION Accessing your super You can access (subject to preservation requirements) your super at any time. Annual review We will review the Plan once each year effective 1 January. Death benefit nominations We will provide you with a Member Statement that sets out details of your account and transactions applied to your account during the review period. Your Plan allows either binding or non-binding death benefit nominations. Cooling off We want this financial product to meet your needs. Therefore if you join SignatureSuper as a family member you are entitled to a cooling off period. A cooling off period gives you the opportunity to return the product if it doesn t suit you. For more information, see the Cooling off a Family/Spouse member s rights section of the Member Guide Fact Sheet. 3

5 Fees and other costs The range of fees that may apply to SignatureSuper are explained in the Fees and other costs section of the Member Guide Fact Sheet which you should understand before reading this section of the Plan Summary. The actual fees that apply specifically to you in this Plan are set out below. TYPE OF FEE OR COST Establishment fee Contribution fee Withdrawal fee Termination fee Management costs AMOUNT Nil. Nil. Nil. Nil. Management costs consist of: Administration Fee. 0.70% p.a. of your account balance. LESS Large Plan Discount of 0.45% p.a. negotiated between the Participant and us. PLUS Investment Management Fee. The amount you pay for specific investment options ranges from 0.20% p.a. to 2.20% p.a. of the amount you invested in the investment option. The amount you pay for specific investment options is shown in the Investment Guide Fact Sheet. For the Future Directions Geared Australian Share Investment option the fee is paid on the gross amount invested. PLUS Performance Based Fees of up to 25% of the out-performance over the benchmark index for the relevant investment manager. Investment options with a Performance Based Fee are shown in the The fees that apply to your investment options section in the Investment Guide Fact Sheet. Service fee PLUS Member Fee. This fee is currently $6.00 per month. This fee will increase on 1 July each year in line with the Consumer Price Index (CPI). Investment switching fee. Nil. The actual amount of fees you pay (or rebates you receive) will be reduced by 15%. This is because superannuation funds currently receive a 15% tax deduction for deductible expenses. The benefit of this tax deduction is passed on to you. The fees and rebates shown in the above table are before the 15% tax deduction. The fees described in the table of fees and other costs above include, if applicable, GST. Financial Planning Member Advice Fee You may agree with a financial planner on a Member Advice Fee for financial planning services in relation to your SignatureSuper Account. Please see the Advice and Services Fee section of the Member Guide Fact Sheet for more information. 4

6 Insurance cover Details of the definitions, terms and conditions applicable for your category of membership under the Plan are available in the Insurance Guide Fact Sheet and the Member Guide Fact Sheet. Below is a summary of the insurance cover specifically available to you. INSURANCE FEATURES Voluntary Death and TPD insurance cover Death and TPD premiums If you are age 16 or older, you can apply at any time for a nominated amount of Death only insurance cover (as long as you are age 65 or under) or Death and TPD insurance cover (as long as you are age 55 or under), subject to the terms and conditions of the Insurance Policy. Please refer to the Insurance Guide Fact Sheet for further information. The minimum amount of cover you can apply for is $30,000. The provision of any insurance cover is subject to providing the insurer with satisfactory medical evidence, the maximum cover limits and acceptance of your application. Please contact us to obtain the applicable form (see the Contact us section of this Plan Summary). The cost of your Death and TPD insurance cover (if any) is deducted from your account on a monthly basis. The current insurance premium rates for standard lives are included in the Insurance Guide Fact Sheet. If there is any inconsistency between the Insurance Guide Fact Sheet, this Plan Summary and the Insurance Policy, the terms and conditions of the Insurance Policy will prevail. To obtain a copy of the Insurance Policy, please contact us (see the Contact us section of this Plan Summary). 5

7 Leaving the Plan the payment process Access to your superannuation benefits is subject to your preservation requirements and meeting certain conditions of release. When you advise us that you choose to cease membership in the Plan, we will transfer your account to any complying superannuation vehicle that you nominate. Your account balance will remain invested in the Plan until the date we process your exit. No fee will be charged for the transfer and the value of your account will be the same as at the date of transfer. You may be subject to entry or establishment fees in the new superannuation arrangement. We will automatically transfer your account from the Plan when: The employee member ceases to be a member of the Plan, You cease to have a family relationship with the employee member (e.g. if the employee member is your spouse and you divorce or separate), or Your balance falls below $1,000 and no contribution or rollover/transfer has been received for 12 months or more. Unless you have notified us that you wish to be transferred to another superannuation arrangement, we will transfer you as outlined below. We will automatically transfer your account as follows: if your account balance is $1,000 or more and we have your postal address, your account will be transferred to SignatureSuper Personal and your insurance cover (if applicable) will continue. The process described in the Continuing in SignatureSuper Personal section of this Plan Summary will apply to you; or if your account balance is less than $1,000 or we don t have your postal address when you cease membership of the Plan, your account will be transferred to AMP Flexible Super and any insurance cover you have will stop. You should consider obtaining cover appropriate to your circumstances. You may be eligible to apply for a Continuation Option to continue your cover (conditions apply). Please see the process described in the Continuing in AMP Flexible Super section of the Member Guide Fact Sheet. We will confirm your membership of your new superannuation arrangement and explain the differences between your current Plan and the new superannuation arrangement. Continuing in SignatureSuper Personal SignatureSuper Personal is a product issued by AMP Superannuation Limited ( ASL ) and is part of the AMP Superannuation Savings Trust ( SST ). On transferring your membership to SignatureSuper Personal you belong to the Superannuation Category of the SST. Insurance, investments and fees are some of the features that may change if you transfer from the Plan to SignatureSuper Personal. We will write to you to confirm the details of your membership and to provide you with details of any differences between your plan and SignatureSuper Personal. A summary of these differences is outlined below. Insurance cover Your Death cover (if applicable) will continue automatically. If, at the date of your transfer, you are not yet age 60, your TPD cover (if any) will continue at the same amount of cover you had with your SignatureSuper employer plan. Your insurance cover amount is subject to indexation and the premiums for any insurance cover will be deducted from your account. Any insurance cover continued under SignatureSuper Personal will be at the applicable personal premium rates for that personal insurance. These rates are charged from the start date of your SignatureSuper Personal membership. These are the same as those detailed in the Insurance Guide and do not include commission. Where a claim for a TPD benefit arises, AMP Life will assess your claim under the insurance cover in place at the time the illness or injury commenced: If disablement results from an illness or injury that commenced before the date you transferred to SignatureSuper Personal, your claim will be assessed against your insurance in this Plan, or If disablement results from an illness or injury that commenced after the date you transferred to SignatureSuper Personal, your claim will be assessed against your insurance in SignatureSuper Personal. If you satisfy the eligibility criteria, you can apply for additional insurance cover in SignatureSuper Personal. 6

8 Investment options Your SignatureSuper investment options will be retained. Fees The Member Fee in SignatureSuper Personal will be $6.95 per month. This fee will increase on 1 July each year in line with the Consumer Price Index ( CPI ). The Administration Fee of 0.70% p.a. of your account balance will continue. However the Large Plan Discount in SignatureSuper Personal will be based on the balance of your account as detailed in the following table. ACCOUNT BALANCE Less than $100,000 LARGE PLAN DISCOUNT 0.30% p.a. of your account balance $100,000 or greater but less than $1,000, % p.a. of your account balance $1,000,000 or greater but less than $3,000, % p.a. of your account balance $3,000,000 or greater 0.70% p.a. of your account balance All other fees in SignatureSuper Personal are the same as they apply in this Plan. Death benefit nomination Your SignatureSuper death benefit nomination will automatically carry over to your new account in SignatureSuper Personal. If you did not make a SignatureSuper death benefit nomination, we must pay your SignatureSuper Personal benefit upon your death to your estate, except if your estate is insolvent. 7
