2 Membership Checklist TASK Get USA Waterski Insurance You MUST have continuous coverage from February 1 st - October 1 st. No insurance = no participation Fill out Membership Form found in the members section of the website. Sign the appropriate waiver and it to twincitiesriverrats@yahoo.com. Pay Dues & Fundraising ed invoice will be sent shortly after completion of paperwork. Please be patient, this is not generated automatically. Get Friends of the Rats Donations. See requirements on following pages. Flotation Requirements Female skiers and pyramid climbers need a show vest (flo-mo). These can be purchased through Lake Elmo Sports. Male skiers need a team barefoot or jump suit. These are ordered through the team each spring. Approximately $250.
3 Membership Types Member Type Description USA Waterski Insurance River Rat Dues Skiing Active Any Member of the team who skies in a practice or a show Yes Yes Non-Skiing Active Any member of the team who have a role in the performance of a ski practice or show. This includes tow boat and pick-up boat drivers, tow boat and pickup boat spotters, dock crew, concessions crew (must work 50% of shows), sound crew and announcer. Yes No Non-Skiing Supporting These members play a huge role on our team. Supporting Members fall outside the Skiing Active and Non-Skiing Active categories. This includes the costume support crew, prop building, equipment maintenance, fundraising support. 4 Friends of the Rats All returning Skiing Active team members are required to participate in Friends of the Rats Fundraising. $100 requirement for individual, $25 for each additional family member. The money raised through Friends of the Rats goes 100% directly to our team. Number of Skiing Active Members Amount Required $100 $125 $150 $175 $200 You may choose to buy out of this by paying the required amount above, or you may solicit funds. Your required amount is due with your dues. Any amount you raise over your required amount can be used as dues reduction: Funds received by January 31, 2019= 50% back toward dues Funds received by February 1, 2019 or later = 30% back toward dues Sponsorships Sponsorships can also help to reduce your dues. Up to half of the money raised by a member through sponsorships can be applied to your dues. 5 FRIENDS OF THE RATS Dear Friend of the Rats, I am a member of the Twin Cities River Rats Water Ski Show Team. The River Rats perform themed water ski shows every Thursday of the summer, and ski on the Mississippi River in Northeast Minneapolis. As we continue to strive to provide free, family entertainment in the Twin Cities, we rely on donations and sponsorships to help with our expenses. We look to our friends and family to help show their support of the Twin Cities River Rats. Just a simple donation of $25 (or more) will get your name in our ad book for the upcoming ski season. We are a 501(c)(3) non-profit organization with taxexempt status. Anything you give is tax-deductible! Please let us know if you would like a receipt. Thank you for your support of the River Rats! 6 Registration please use the form on the website. Membership fees, Friends of the Rats and any Sponsor payments will be invoiced and payable online via the link in each invoice. Additional Information Non-Member Alumni May ski with proof of current USA Water Ski Insurance will be allowed to ski and participate in the following events at no cost: Aquatennial Alumni Show Any future scheduled away shows Will need to pay $50.00 for each and every home or tournament show they wish to ski and participate in. It will be mandatory for a non-member alumnus to ski and participate in the immediately preceding practice (exceptions to this rule are the Aquatennial Alumni Show and all away shows). Payment must be received* prior to the preceding practice. Potential New Member / Visiting Show Skier With payment of $10.00 for one day USA Water Ski Insurance will be allowed to ski or participate in one practice or show at no cost. Payment Plans Please contact team treasurer Tom Zalewski at t.zalewski97@gmail.com if you would like to make arrangements for a payment plan. Dues need to be paid and received in full prior to any deadline to receive any corresponding reduced dues rates regardless of when the payment plan begins. All payment plans must be paid in full by June 30, 2019
7 PARTICIPANT WAIVER AND RELEASE OF LIABILITY, ASSUMPTION OF RISK, AND INDEMNITY AGREEMENT Water Sport Participation Waiver Form for ADULTS (Age 18 or older) I ACKNOWLEDGE THAT BY SIGNING THIS DOCUMENT, I AM AGREEING TO RELEASE THE RELEASED PARTIES FROM LIABILITY. I HAVE THEREFORE BEEN ADVISED TO READ THIS DOCUMENT CAREFULLY BEFORE SIGNING IT. For and in consideration of The Twin Cities River Rats ( River Rats ) allowing me, the undersigned, to participate in any water sport training or event, including, but not limited to, any training, practice, clinics, and related activity (the Activities ); I, for myself, and on behalf of my spouse, children, guardians, heirs and next of kin, and any legal and personal representatives, executors, administrators, successors and assigns, hereby agree to and make the following contractual representations pursuant to this Waiver and Release of Liability, Assumption of Risk, and Indemnity Agreement (the Agreement ); I hereby represent that: (i) I am at least eighteen (18) years of age or older; (ii) I am in good health and in proper physical condition to participate in the Activities; and (iii) I am not under the influence of alcohol or any illicit or prescription drugs which would in any way impair my ability to safely participate in the Activities. I agree that it is my sole responsibility to determine whether I am sufficiently fit and healthy enough to participate in the Activities, and that I am responsible for my own safety and well-being at all times and under all circumstances while participating in the Activities. I understand and acknowledge the risks and dangers associated with participation in the sport of water skiing and related water sport disciplines, including without limitation, the potential for serious bodily injury, sickness and disease, permanent disability, paralysis and loss of life; loss of or damage to equipment/property; exposure to extreme conditions and circumstances; contact with other participants, spectators, boats, animals or other natural or manmade objects; dangers arising from adverse weather conditions; imperfect ski site conditions; water and surface hazards; equipment failure; inadequate safety measures; participants of varying skill levels; situations beyond the immediate control of the River Rats; and other undefined, not readily foreseeable and presently unknown risks and dangers ( Risks ). I understand that these Risks may be caused in whole or in part by my own actions or inactions, the actions or inactions of others participating in the Activities, or the negligent acts or omissions of the Released Parties defined below, and I hereby expressly assume all such Risks and responsibility for any damages, liabilities, losses or expenses which I incur as a result of my participation in any Activities. I agree to be familiar with and abide by the rules and requirements of participation established by the River Rats for the Activities. I accept sole responsibility for my own conduct and actions while participating in the Activities, and the condition and adequacy of my equipment. I hereby Release, Waive and Covenant Not to Sue, and further agree to Indemnify, Defend and Hold Harmless the following parties: The Twin Cities River Rats, its members, directors, officers, volunteers, agents, sponsors, advertisers; water sport coaches and boat operators; and the property owners and lessees of the property upon which the Activities takes place (Individually and Collectively, the Released Parties ), with respect to any liability, claim(s), demand(s), cause(s) of action, damage(s), loss or expense (including court costs and reasonable attorneys fees) of any kind or nature ( Liability ) which may arise out of, result from, or relate in any way to my participation in the Activities, including claims for Liability caused in whole or in part by the negligent acts or omissions of the Released Parties. I further agree that if, despite this Agreement, I, or anyone on my behalf, makes a claim for Liability against any of the Released Parties, I will indemnify, defend and hold harmless each of the Released Parties from any such Liabilities which may be incurred as the result of such claim. I hereby warrant that I am of legal age and competent to enter into this Agreement, that I have read this Agreement carefully, understand its terms and conditions, acknowledge that I will be giving up substantial legal rights by signing it (including the rights of my spouse, children, guardians, heirs and next of kin, and any legal and personal representatives, executors, administrators, successors and assigns), acknowledge that I have signed this Agreement without any inducement, assurance or guarantee, and intend for my signature to serve as confirmation of my complete and unconditional acceptance of the terms, conditions and provisions of this Agreement. This Agreement represents the complete understanding between the parties regarding these issues and no oral representations, statements or inducements have been made apart from this Agreement. If any provision of this Agreement is held to be unlawful, void, or for any reason unenforceable, then that provision shall be deemed severable from this Agreement and shall not affect the validity and enforceability of any remaining provisions.
