1 Combi Oven IFB Invitation for Bid Bid Number (FS ) Date: Thursday, February 23, 2017 Contract period: school year, ending on June 30, 2017, with two year minimum warranty of equipment installation date Closing Time of Bid: Thursday, March 9, 2017, 9 a.m. CST Instructions 1. Delivery of Bids Sealed bids will be accepted at Grayson County Board of Education, ATTN: Kristy Hodges, 790 Shaw Station Rd. (P.O. Box 4009), Leitchfield, KY 42754, until the date and time shown above, at which time the bids will be publically opened. All bids must be received by the designated time, and bids will not be considered thereafter. Sealed bid envelope must include BID NAME (Combi Oven IFB) and BID NUMBER (FS ). 2. Compliance with Specifications Bids must be submitted in strict accordance with the specifications contained herein. 3. Corrections of Mistakes on Bid Form Erasures or the use of correction fluid on bid forms are not acceptable and may result in the rejection of the bid. Prior to submission of openings, errors may be crossed out, corrections entered, and initialed by the person that signed the bid. No bids shall be altered or amended after the specified time for opening. 4. Review and Award of Bids After public opening of bids, Grayson County Board of Education may require at least 10 days for reviews of low bid prior to awards. Any supplier must show suitable evidence for acceptance of any item challenged upon review and assessment. Grayson County Board of Education reserves the right to accept any bid, to reject any or all bids, to waive any irregularities or informalities in bids received where such acceptance, rejection or waiver is considered to be in its best interest. Grayson County Board of Education also reserves the right to reject any bid where evidence or information submitted by the bidder does not provide satisfactory proof that the bidder is qualified to carry out the details of the contract. 5. Award of Contract Contract will be awarded to the lowest bidder, meeting all specifications, conditions, and provisions.

Delivery Unit and all applicable accessories shall be delivered to Grayson County Middle School Kitchen, 726 John Hill Taylor Blvd., Leitchfield, KY 42754, and set in place for final hookup (water, electric, and gas) to be completed by the district s maintenance department. This location does have a dock located outside the rear kitchen door. Delivery is expected within four (4) weeks of contract award letter. 7. Installation Start-Up Check Successful bidder is to provide on-site start-up check through authorized service agent to ensure correct hook-up for water, electric, gas, overall operation and installation. 8. Training Successful bidder must provide thorough training (minimum of 2 hours) by a manufacturer corporate chef, to include operation and cleaning procedures, to all cooks at Grayson County Middle within one week of installation, as scheduled by Food Service Director. 9. Price Delivery fee, installation, and training must be included in total price. Deliveries against this contract must be free of state, excise or transportation taxes, except when such a tax is part of a price and school districts are not exempt from such levies. 10. Payments Successful bidder is to send invoice for equipment once all requirements of the bid have been met, to include delivery of equipment, installation start-up check, and training. Grayson County Board of Education will mail a check following a regularly scheduled board meeting (monthly). 11. Penalties In case of default by the vendor, Grayson County Board of Education may procure the articles or services from other sources and may deduct from any unpaid balance due the vendor the amount of the excess cost so paid, and the price paid shall be considered the prevailing market price at the time such purchase is made. 12. Suspension and Debarment By signing this Invitation to Bid, the bidder certifies that neither it nor any of its key officials have been proposed for debarment, debarred, or suspended by a Federal agency. 13. Buy American To the maximum extent practical, Grayson County Schools will purchase units manufactured in the United States. Please provide documentation to show that product is made in the United States, if applicable. 14. Civil Rights Contractors providing services under this Invitation to Bid herewith assure the school district that they are conforming to the provision of the Civil Rights Act of 1964, as amended.

Access to Records Grayson County School Food Service, Kentucky Department of Education, United States Department of Agriculture, the Comptroller General of the United States, or any of their duly authorized representatives, shall have access to any books, documents, papers, and records of the contractor which are directly pertinent to that specific contract, for the purpose of making audit, examination, excerpts, and transcripts. Vendors must maintain records for 3 years after the final payment on this contract and all other pending matters are closed. 16. Termination a. Grayson County Board of Education may terminate the resulting contract for nonperformance for such causes as: i. Failing to keep and perform, within the time period set forth herein, or violation of, any of the covenants, conditions, provisions or agreements herein contained; ii. Failing to provide satisfactory quality of service; iii. Failure to comply with the terms of the contract b. Grayson County Board of Education reserves the right to terminate this contact at any time, by thirty (30) day written notice to Contractor. Upon receipt by the Contractor of the notice of termination, the Contractor shall discontinue all services with respect to the applicable contract. Grayson County Board of Education, after deducting any amount(s) previously paid, shall pay for all services rendered or goods supplied by the Contractor, as well as any reasonable costs incurred by Contractor up to the time of termination but not including Contractor s loss of profit. c. In the event that either party deems it necessary to take legal action to enforce any provision of the contract, and in the event Grayson County Board of Education prevails, the Contractor agrees to pay all expenses of such action, including attorney s fees and costs at all stages of litigation. 17. Questions will be accepted until Friday, March 3, 2017, 3 p.m. CST. Written responses will be provided, including addenda to bid specifications, terms and conditions as needed, by Monday, March 6, 2017, 4 p.m. CST.

Hot-air steamer (combi-steamer mode) for optimal use of steam and hot-air, individually, in succession or in combination 3. Zero clearance design, meaning that the oven can be placed right next to another piece of equipment 4. Graphically supported overview of the cooking chamber climate (temperature, cooking mode, timer) 5. Record mode that determines the ideal cooking path for calibrated products using a core temperature probe for subsequent use without a core temperature probe, with automatic consideration of load quantity 6. Removable core temperature probe 7. Individual programming of common cooking items (chicken strips, pizza, hamburgers, vegetables, etc.) 8. (11) 18 x 26 full size sheet or (22) 12 x 20 half size pan capacity 9. Automatic steam venting at end of cooking cycle 10. Fully automated cleaning with a minimum of 5 cleaning levels 11. Cartridge to include cartridge filter monitor 21. Specifications were based on Alto Shaam CTP10-20G. Bids for this model or equal will be considered. 22.
