Best practices dictate that any device that you seek to recovery information be placed in "Airplane Mode" immediately to prevent the phone from writing over the data you seek or being remotely wiped. Some legacy or analog phones will erase data if powered of. In this case we recommend charging the phone completely before transporting or shipping. Do you know the devices operating system? Please select the purpose of the service you are seeking from the choices below: Litigation- This forensic acquisition and/or recovery is intended to be used to resolve a legal matter and I understand the estimated cost to be $499, which does not include media to return the data to me, written reports, court appearances, and expenses. Client understands that forensic acquisitions and analysis for litigation purposes requires additional processes, procedures, and reporting.! 2

3 Non-Litigation - This acquisition and/or recovery are for personal reasons to resolve a domestic matter and will cost $400. I understand the processes and procedures used to recover the information I seek will not be admissible in court. The above price(s) are for work completed at our office. All onsite engagements are billed at our regular hourly and travel rates. See our price sheet for details. Please tell us what information you wish to recover? Authorization I authorize Surveillance Specialist Group to provide forensic acquisition and/or data recovery services as required. I will not hold Surveillance Specialist Group, or its technicians responsible for any and all data losses related to acquisition, analysis, recovery, and or repair. I understand that given the nature of electronic data storage and data recovery service(s), problems could arise and are not the fault of Surveillance Specialist Group but are coincidental and could surface at any time, even when a previous problem is corrected. No rush service is available for forensic examinations or data recovery. No guarantee is made that all the data believed to be on the device or desired will be recovered, particularly data that has been intentionally deleted. Formatting a storage device (ex. Hard Drive, Floppy, USB) will cause all data on that device to be erased. In some cases logical devices (using solid state memory) can delete data and or clear registries when powered off or powered on after being powered off or suffering a loss of power due to battery failure. Equipment left will be finished by date promised if possible. However, no guarantee is made to when item will be finished to the extent the processes and procedures required to recover the data varies by device. All items opened may void all manufactures warranties. All memory drives tested may void manufacturer s warranties. All data uncovered will be transferred to a PDF report, DVD, or CD. Additional charges apply for these items which are $15 per 650MBs, DVDs are $25 per 4.5GBs, DL-DVDs are $35 per 8GBs. Data recovery service, CD and DVD media are not covered under any warranty, expressed or implied of merchantability or fitness for a! 3

4 particular purpose. Surveillance Specialist Group will not be responsible for items left over 30 days. Any equipment left over 30 calendar days will become property of Surveillance Specialist Group and will be disposed of to recover storage space or will be stored at your cost. Customer will not receive the original device(s) pursuant to litigation after forensic data recovery service unless requested in writing which may deem the item as well as the data recovered by our services inadmissible to the extent the device was outside of our care custody and control. A second acquisition for the purpose of validating the original acquisition may be required at your cost. Customer's copy or sufficient identification must be shown in order to claim device(s) left for data recovery. Retainer is due at time of drop off. There is a charge for report(s) beyond the report(s) generated from forensic software. Our services are for Labor Charges and are non-refundable. We are providing a service not a result. We are not responsible for lost data after service is complete and data is picked up. I agree to hold Surveillance Specialist Group, its owners, investigator(s) and technician(s) harmless for any coincidental damages to include, but not limited to, business interruption or other liabilities that result from permanent loss of data, corruption of data or delayed delivery of recovered data. WE ALSO HAVE NO WAY OF VERIFYING YOU ARE THE LEGAL OWNER OF THE DEVICE. AS SUCH, TO THE MAXIMUM EXTENT PERMITTED BY APPLICABLE LAW, IN NO EVENT SHALL SURVEILLANCE SPECIALIST GROUP AND/OR ITS INVESTIGATOR, EXAMINER, TECHNICIANS OR OFFICERS BE LIABLE FOR ANY DIRECT, INDIRECT, PUNITIVE, INCIDENTAL, SPECIAL, CONSEQUENTIAL DAMAGES OR ANY DAMAGES WHATSOEVER INCLUDING, WITHOUT LIMITATION, DAMAGES FOR LOSS OF USE, DATA OR PROFITS, ARISING OUT OF OR IN ANY WAY CONNECTED WITH THE USE OR PERFORMANCE OF THE FORENSIC EXAMINATION OR DATA RECOVERY SERVICES, WITH THE DELAY OR INABILITY TO USE THE RECOVERED DATA OR PRODUCT OF DATA RECOVERY OR RELATED SERVICES, THE PROVISION OF OR FAILURE TO PROVIDE SERVICES, OR OTHERWISE ARISING OUT OF THE USE OF THE FORENSIC EXAMINATION OR DATA RECOVERY SERVICES, WHETHER BASED ON CONTRACT, TORT, NEGLIGENCE, STRICT LIABILITY OR OTHERWISE, EVEN IF SURVEILLANCE SPECIALIST GROUP OR ANY OF ITS TECHNICIANS HAVE BEEN ADVISED OF THE POSSIBILITY OF DAMAGES. TO THE EXTENT WE HAVE NO IDEA WHAT IS OR IS NOT ON THE DEVICE YOU ARE SUBMITTING, WE CANT RESPONSIBILITY FOR WHAT IS ALLEGEDLY STORED ON IT.
