1 香港專業教育學院 ( 摩理臣山 ) 申請豁免修讀單元須知 (2017/1 /18 學年 ) 索取表格學生電郵系統 ( 申請日期 2017 年 9 月 1 日至 15 日 星期一至五 遞交表格時間上午 8 時 30 分至晚上 7 時 30 分 ( 下午 12 時 30 分至 1 時 30 分除外 ) 遞交表格地點 查詢電話 工程系 (417 室 ) 工程系網站 ( 星期六 日及公眾假期 休息 注意事項 : 1. 同學在遞交申請表前, 請儘快向課程主任查詢有關個別課程的豁免單元要求 ( 不符合要求, 而引致申請不成功, 所繳之申請費, 恕不退還 ) 2. 每一單元的申請費為港幣一百元, 繳費方法請參閱附頁 請注意 : 無論申請成功與否或中途退學, 此申請費將不獲退還 3. 請將填妥之 豁免修讀單元 申請表格 (2017/18 學年 ) 連同下列文件, 於 2017 年 9 月 15 日或之前親自遞交所屬學系 逾期申請, 概不受理 i) 學生證或身份證副本 ; ii) 申請費收據收據副本 / 正本, 如以 繳費靈 繳交申請費, 請在申請表格內填上已繳費的參考編號 ; 及 iii) 可支持你豁免修讀某些單元的工作證明, 公司信等 4. 申請人如無合理理由而不能提供證明文件, 申請一般不會受理 5. 學系將會在 2017 年 10 月下旬下旬寄出審批豁免修讀結果給予申請人 ( 最終的審批結果將由課程委員會決定, 申請人不得 異議 ) 在未收到學院通知前, 學生須照常上課 若同學成功豁免修讀單元, 有關退款將在下一期學費反映 ( 如適用 ) 6. 以上申請程序並不適用於殘疾學生申請特別豁免 / 特別考試安排 如有需要, 請聯絡所屬學系, 索取有關資料

Fees for Module Exemption Exemption Fee $100 per module (Please note that the fee paid is not refundable even if the application is not supported or the applicant is de-registered from course.) 3. Duly completed application for module exemption together with the following documents must be sent to the Department (Room 417) on or before 15 Sep Late application will NOT be considered. i) Copy of student card or HKID; ii) Please attach the original/copy of the payment receipt; and iii) Copies of documentary proof which can substantiate your request for exemption (e. reference letter etc.) 4. The deadline for submitting the application form for module exemption based on relevant experience is 15 Sep Applicants will normally be notified of the result by mail by late Oct (The final decision of the application results will be made by the Course Board. The decision of the Course Board is final and no appeal for whatever reason will be entertained.) You must attend classes until exemption is formally granted to you. If your application for module exemption is successful, the Campus Secretariat will adjust the tuition fee in next installment (if applicable).
