2 THE LAKEWOOD CLUB TERMS AND CONDITIONS FOR MEMBERSHIP APPLICATION By signing The Lakewood Club Membership Application (the Application ), the Applicant and his/her legal spouse listed therein, agree to the following terms and conditions to their application and to their membership if their application for membership is accepted by The Lakewood Club (the Club ): 1. The Applicant hereby authorizes the Club to check credit and employment history and to obtain other information the Club deems necessary to extend credit to the Applicant and his/her legal spouse. 2. The Applicant will be liable for the payment of monthly dues (or annual dues if chosen) at the rate established by the Club for the applicable membership classification or until the membership is canceled as set forth herein below. The Applicant agrees to pay all fees and charges for programs signed up for by the Applicant, the Applicant s legal spouse or their qualifying children, but not listed below under the applicable membership classification as a Member Benefit. The Applicant hereby agrees to be liable for any fees and/or charges incurred by the Applicant, the Applicant s legal spouse, and their qualifying children, including food and beverages, purchases of merchandise, guest fees for golf, tennis, fitness, and the pool and damage to Club property caused by the actions of the Applicant, the Applicant s legal spouse, or their qualifying children. 3. The Applicant may only cancel his/her membership by giving written notice of such intent no later than 30 days prior to effective cancelation date. 4. The Applicant agrees to comply with the provisions of the Application, the Terms and Conditions thereto and with all rules and regulation promulgated by the Club and in effect from time to time. 5. All dues, fees and charges shall be billed to Applicant monthly. Account balances due the Club shall be payable no later than 30 days following the statement date. If any payment is not received within 90 days of the due date, the Applicant hereby authorizes the Club to charge the amount due, plus any applicable fees or penalties, to the credit card account listed in the Application. The Applicant shall pay any and all costs and expenses incurred by the Club in collecting any unpaid charges, including the Club s reasonable attorney s fees. 4 Applicant and Legal Spouse Must Provide the Following Document(s): Valid driver s license *The Lakewood Club reserves the right to ask for additional identification/documentation in the form of a driver s license, passport, marriage license, birth certificate, etc. for any person/persons listed on this application. Non-Resident Applicant and Legal Spouse Only: Non-Resident Members shall be individual members of the Club who are domiciled outside of Mobile and Baldwin Counties, who do not regularly occupy a home in Mobile or Baldwin County, their spouses and unmarried children under the age of 23 years, all of whom shall be entitled to all privileges of the Club. I understand that in the event my Non Resident status changes, I will notify the Membership Office within 30 days and I understand that additional initiation fees may apply upon changing to a Resident Member. I also understand, the Club maintains the right to administer disciplinary action, up to termination, should they find a Non- Resident Member in contravention of the terms and conditions set forth for Non-Resident Member status. Non-Resident applicants must provide two forms of proof of residency outside of Mobile and Baldwin Counties. A Valid Driver s License and a Voter s Registration Card or a W-2 form must also be provided (please black out any financial information). I acknowledge that I have received, have read, fully understand and agree to the attached Club Terms & Conditions to Membership Application. I understand that my membership right may, from time to time, be modified by duly adopted changes to the Club s rules and/or regulations and that my membership is expressly subject to such rules and/or regulations and any changes thereto. I recognize Lakewood s right to administer disciplinary action, up to termination, for violation of said rules and regulations. Upon signing this Application for Membership, I further understand that the initiation fee is a nontransferable, nonrefundable fee. I also understand that a credit card is required for Club charging privileges. If my account becomes 90 days past due, The Lakewood Club is authorized to charge my credit card the past due balance and my membership will be subject to termination. Members do not have any interest or ownership in the Grand Hotel & Lakewood Golf Club. The initiation fee must accompany this application. This Application must be complete in every detail before it will be accepted for consideration by the Hotel/The Lakewood Club Management. Non-Resident: $7, Non Mobile or Baldwin County Resident Tennis: $10, Initiation fee is a non-equitable, non-refundable fee and must be paid in full at time of application.
