1 GUIDANCE NOTES ON APPLICATION FOR VOCATIONAL TRAINING COUNCIL (VTC) GRADUATION FEE WAIVER (2011/2012) IMPORTANT NOTES The data in the application will form the sole basis of determining the appropriate level of waiver to be awarded to the applicant. It is an offence to obtain property/pecuniary advantage by deception. Any person who does so commits an offence and shall be liable on conviction to imprisonment for a maximum of 10 years under the Theft Ordinance (Chapter 210 of the Laws of Hong Kong). 1. Eligibility: You should: (a) be a registered student of the VTC who has paid the Graduation Fee in AY 2011/12; and (b) pass the means test GF 2. Provision / Handling of personal data: (a) It is obligatory for you to provide your personal data to the VTC as required in the application. Insufficient information and misrepresentation of facts will render your application being disqualified for further processing. (b) The VTC will use the personal data in the application for the following purposes: Activities relating to the processing and counter-checking of your application for Graduation Fee Waiver; Activities relating to recovery of overpayments, if any; and Statistics and research. (c) The personal data and other supplementary information that are provided in the application may be disclosed to the Government policy bureaus and departments for the purposes mentioned in section 2(b) above, or where such disclosure is authorized and required by law. (d) The VTC will contact the government departments and organizations (including the employers of your family members) to (i) obtain any information; and verify the data provided in the application, for the purposes mentioned in section 2(b) above. (e) All documents submitted are not returnable. However, you have the right to access and make corrections to the personal data provided in your application. You can request for a copy of the personal data provided in this application. Such request should be addressed to the relevant campus principal/centre manager, and the VTC reserves the right to charge a fee for the processing of such data access request. 3. Important points to note: (a) The VTC has the right to review the application for Graduation Fee Waiver and will conduct investigation, including home visits, for authentication of the application data; and based on the findings, the VTC will make adjustments to the level of waiver already awarded. You will be required to refund to the VTC overpayment of wavier, if any. You and your family members are requested to co-operate with the staff members of the VTC regarding the review of your application; otherwise, you may have to refund all the Graduation Fee Waiver already received from the VTC. Any wilful misrepresentation and concealment of facts will lead to disqualification, full restitution of the total waiver received as well as possible prosecution (b) You must provide sufficient and correct information requested by the VTC within stipulated deadline to process your application, otherwise, you will NOT be offered any Graduation Fee Waiver. 2 4. Notes on completing the form: (a) You should write clearly in black or blue ink when completing the application form. (b) Each applicant should submit one applicant form. (c) The applicant must be the student. However, both you and your parent/guidance must sign on Part VI of the application form. (d) You should note the following points when completing the application form: Part I Financial Assistance Scheme (2011/12) (i) This part serves to remind you that if you have application results of financial assistance schemes as stated in the following table for AY2011/12, you are not required to submit this application form. Please apply using the Brief Form which can be obtained from Campus Secretariat or General Office of your Centre, or via the Graduation Ceremony Webpage ( the completed application form together with supporting documents should be returned to Campus Secretariat or General Office of your Centre. Course Financial Assistance Scheme Full-time Subvented Higher Diploma / Diploma programmes Full-time Fly-high Higher Diploma programmes Full-time Foundation Diploma / Diploma in Vocational Education Programme Tertiary Student Finance Scheme Publicly-funded Programmes (offered by SFAA*) Financial Assistance Scheme for Post-secondary Students (offered by SFAA*) or VTC Tuition Fee Remission Scheme VTC Tuition Fee Remission Scheme *SFAA - Student Financial Assistance Agency On the other hand, if you have not yet applied, or are not eligible for these financial assistance schemes, you are required to use this Additional Application Form, the completed application form together with supporting documents (including your family income and economic status) should be returned to Campus Secretariat or General Office of your Centre. Part II Particulars of Applicant (i) The names in English and Chinese must follow those in the Hong Kong Identity (HKID) Card. For applicant who does not possess a HKID Card, the Hong Kong Birth Certificate (HKBC) Number should be entered in Item 6. If the applicant does not have a HKBC neither, this item should be left blank. (iii) For the Bank Account Number and Name of Holder in Item 8, please pay attention to the following: The bank account could be your own account or the joint account of you and your parent(s) or guardian. The account must NOT be a time deposit account, credit card account or foreign currency account. The bank account must be valid (it must be recently used). The account number and name of holder should be entered correctly, any error made may cause delay in payment. You could provide the bank account of your parent(s)/guardian if you do NOT have one. You are required to complete and sign a Notification of Bank Account Name for the Credit of Vocational Training Council Graduation Fee Waiver (2011/12) form obtainable from the Campus Secretariat/General Office of your centre. 3 Part III Particulars of Applicant s Parent(s)/Guardian Please provide the details of your mother and father or legal guardian. Part IV Particulars of Family Members of Applicant (i) Enter particulars of unmarried sibling(s) residing with the family. Do not enter particulars of unmarried sibling(s) residing with the family but is in receipt of Comprehensive Social Security Assistance (CSSA) (not referring to Disability Allowance). (iii) If unmarried sibling(s) residing with the family (excluding sibling(s) studying full-time course) report themselves as self-employed / with no fixed income / no income proofs / unemployed for more than 3 consecutive months during the period to , they must make sworn declarations at the Home Affairs Department. (iv) Fill in the information of the grandparent(s) who are dependent on your parent(s). Dependent grandparent means your grandparent, who is not in receipt of CSSA (not referring to Old Age/Disability Allowance), and, for a period of at least 6 consecutive months during to , (a) has resided/been residing with your family and is supported by your parent(s); or (b) has taken up permanent residence at another premise owned or rented by your parent(s); or (c) has lived/been living in his/her own premise, rented premise or resided/been residing in elderly homes and is totally supported by your parent(s). Part V Information on Family Income Annual Income includes earnings from full-time/part-time/casual jobs (please specify the kind of work) and other sources such as contribution from relatives/friends, income of unmarried sister/brother not residing with your family, rent from property, etc. However, Old Age/Disability Allowance or Severance Pay, etc. should be excluded. If your parent(s)/guardian, spouse of your guardian and unmarried sibling(s) residing with the family (excluding sibling(s) studying full-time course) claim themselves as self-employed / with no fixed income / no income proofs / unemployed for more than 3 consecutive months during the period to , they must make sworn declarations at the Home Affairs Department. Types of incomes that need to be reported are listed below. For your reference, the items that need not be reported are also listed. Items need to be reported 1. Loans Part VI Part VII You and your parent/guardian must read through the paragraphs; agree to the terms and conditions set out in this Guidance Notes; and sign in the spaces provided in the application form or else the application will not be processed. You can provide additional information (such as special family circumstances, self-reliance or change in family economic status, etc.) to support your application. 5. Notes on submitting the application form: (a) The completed application form and copies of the following supporting documents should be returned to the Campus Secretariat/General Office of your Centre on or before 5 October 2012 (Friday): Applicant s HKID card and student card; The HKID card(s) of applicant s parent(s) / guardian and spouse of the guardian (Part III); Supporting document(s) for deceased, divorced or separated parent(s), if applicable (Part III); The HKID card(s) of applicant s unmarried sibling(s) residing with your family / dependent grandparent(s) (Part IV); The page of the bank statement / passbook / ATM card showing the account holder s name and account number (Part II); Receipts of living expenses, or expenses paid to elderly homes or premises owned or rented by the applicant's parent(s) for the living of the dependent grandparents from to , if applicable (Part IV); Income proofs of applicant, applicant s parent(s) / guardian, spouse of the guardian and unmarried sibling(s) residing with the family for the period to (Part V). Examples are listed below: Salaried employed person: Salary Statement or Tax Demand Note or Bank Statement showing payment of salary / wages or Income Certificate, etc. Sole proprietor or partner of Profit and Loss Account or partnership business: Self-employed person/ employee with no fixed income/ employee with no income proofs: Person unemployed for more than 3 consecutive months during the period Personal Assessment Notice, etc. Income Statement Attached Documents (if applicable): self-explanatory letters, medical certificates, documents issued by SWD, etc. Statement of sworn declarations at the District Office Page 4

5 (b) You and your parent(s) / guardian may be requested to attend an interview with responsible staff of VTC and to give evidences on family income and identity. 6. Granting of fee waiver: (a) The result of your application will tentatively be announced in December 2012 via Graduation Ceremony Webpage ( Successful applicants who have paid the Graduation Fee will be refunded the amount (or part of it, depending on the result of application) through autopay. However, there may be delay owing to the limited time available for processing applications. (b) VTC will assess if you are eligible for Graduation Fee Waiver according to the Adjusted Family Income (AFI) mechanism. However, VTC has the discretion to adjust or cancel the amount of waiver. For successful applications of Graduation Fee Waiver, the levels of waiver will be either full or half [Note]. Note: If it happens that the total amount of Graduation Fee to be waived exceeds VTC s budgeted amount, a waiver percentage lower than 50% may be applied to all half-waiver awardees, subject to the decision by VTC. Full waiver awardees will not be affected. (c) An applicant who is not satisfied with the result of the application and has sufficient justification may submit the form of Application for Review through the Campus Secretariat/General Office of your Centre to the Campus Principal for a review. The form is obtainable at Campus Secretariat/General Office of your Centre. Page 5
