The Federal Regulations require that before awarding Federal Student Aid, we must ask you to confirm the information you reported on your FAFSA. A. 2 C. Independent Student s Income to Be Verified Student Federal Tax Return Filers Complete this section if the student and spouse, filed a 2015 IRS income tax return(s). These instructions apply to the student and spouse, if the student is married. Notify the Eastman School of Music Financial aid Office if the student or spouse filed separate IRS income tax returns for 2015 or had a change in marital status after December 31, Important Note: If the student and spouse filed an amended 2015 IRS income tax return, the student must contact a financial aid counselor before completing this section. Check the box that applies: The student has not yet used the IRS DRT in FAFSA on the Web, but will use the tool to transfer 2015 IRS income tax return information into the student s FAFSA. The student used the IRS DRT in FAFSA on the Web to transfer 2015 IRS income tax return information into the student s FAFSA. The student is unable to use the IRS DRT in FAFSA on the Web, and instead will provide the Eastman School of Music Financial Aid Office a 2015 IRS Tax Return Transcript. To obtain a 2015 IRS Tax Return Transcript, please see Section G of this form for further instructions. Check here if the student s 2015 IRS Tax Return Transcript is provided. 4 G: Instructions How to Request IRS Verification of Non-filing Letter? What is an IRS Verification of Non-filing Letter? An IRS Verification of Non-filing Letter provides proof that the IRS has no record of a filed Form 1040, 1040A or 1040EZ for the year you requested. Non -Tax filers can request an IRS Verification of Non-filing of their 2015 tax return status, free of charge, from the IRS in one of three ways: Online Request Available at Note: This is typically not available if you have never filed taxes before in prior years. If this is the case, please follow the instructions to use the paper request below. Under Tools, click Get a tax transcript. Click Get Transcript ONLINE. Enter the non-filer s Social Security Number, address, filing status, account numbers for loan or credit card associated with your name, and mobile phone associated with your name. (This information is used to validate your identity). Click Continue. Select Verification of Non-filing Letter and in the Tax Year field, select If validated successfully, you will be able to view your IRS Verification of Non-filing Letter. (If you are unable to validate your identity, you may use the Get Transcript by mail instructions below). Sign and submit the IRS Verification of Non-filing Letter to the Eastman Financial Telephone Request Available from the IRS by calling Note: This is typically not available if you have never filed taxes before in prior years. When you receive the Non-filing Letter, sign and submit the IRS Verification of Non-filing Letter to the Eastman Financial Paper Request Form IRS Form 4506-T (Available at Complete lines 1-4, following the instructions on page 2 of the form. Line 3: enter the non-filer s street address and zip code. Line 5 provides non-filers with the option to have their IRS Verification of Non-filing Letter mailed directly to a third party by the IRS. DO NOT have your IRS Verification of Non-filing Letter sent directly to the Eastman Financial Line 7: Select the checkbox on the right hand side for Verification of Non-filing.

5 Line 9: Year or period requested field, enter 12/31/2015. The non-filer must sign and date the form and enter their telephone number. Only one signature is requested when requesting a joint IRS Verification of Non-filing Letter. Mail or fax the complete IRS Form 4506-T to the address (or FAX number) provided on page 2 0f the Form 4506-T.
