DESTINATION SHIPPING AND HANDLING CHARGES PREPAID Federal Tax Exemption Number: Introduction and Instructions to Bidders: REQUEST FOR QUOTE FROM COOPERATIVE VENDORS The Lockhart Independent School District is requesting quotes for new Student Computers, Teacher Laptops, headphones, Chromebooks and Carts for our new elementary, Strawn Elementary, that will be opening up in August. Proposals (herein referred to as bid, RFP, Quote or proposal ) will be received by the Lockhart Independent School District at the office of the Purchasing Department, 105 South Colorado, Lockhart, TX The following instructions by the Lockhart ISD are intended to afford bidders an equal opportunity to participate in the request for proposals. Lockhart ISD reserves the right to reject any and all proposals submitted, to request additional information from one or all proposers, and negotiate any and all terms of the proposal if deemed necessary. All bidders must be a Purchasing Cooperative vendor. On the bid sheet please specify which purchasing cooperative the pricing is reflective of. Scope Lockhart Independent School District (LISD) requests proposals for desktop computers, Teacher Laptops, Chromebooks and carts from qualified vendors. The proposal should include a 3 year on-site parts and labor warranty for each computer and laptop. The computers and laptops will need to be setup and delivered to the locations indicated in this RFQ. One image will need to be loaded on all computers and laptops. One computer and one laptop will need to be sent with hard drives to our Technology department for them to load the image and then they will send it back to the vendor for duplication. Lockhart ISD has a Microsoft agreement. This quote request is a line item quote. There may be multiple vendors awarded. LISD reserves the right to increase/decrease the quantity of each item due to budget. Our current budget is $144,000.

The district currently has Dell, HP and M&A desktops and laptops. (Trash removal needs to be offsite) There is no migration of existing computer files or application loads included in this proposal. Installing one image for each of the campuses on the appropriate computers is the only requirement. The district will provide the barcodes. The vendor will be required to provide a listing of serial numbers, asset tag, model, expiration date and warranty. This should be provided in an excel spreadsheet. The vendor is responsible for making sure all addendums are acknowledged. Visit the Lockhart ISD website at to view all addendums RIGHT TO INCREASE OR DECREASE QUANTITIES The DISTRICT reserves the right to increase or decrease quantities subject to the availability of funds during the period the bid/proposal is guaranteed to be firm. RIGHT TO REJECT PROPOSALS The DISTRICT reserves the right to accept or reject any or all proposals or any part thereof and to waive any and all minor informalities or technicalities in any part of the proposal not deemed to be in the best interest of the DISTRICT. TERMINATION FOR CAUSE The DISTRICT shall have the right to cancel or default all or part of the undelivered portion of the order if the contractor breaches any of the terms hereof, including warranties, if the contractor becomes insolvent or commits acts of bankruptcy. Other factors shall include service performance. CERTIFICATION OF PAYMENT Payment by the DISTRICT will be made within 30 days after the date on which the District receives the goods under the contract or the date on which the performance of services under the contract was completed, or the date on which the District received the invoice for the goods or services, whichever is later. FELONY CONVICTION NOTIFICATION A person or business entity that enters into a contract with the DISTRICT shall notify the DISTRICT if the person or an owner or operator of the business entity has been convicted of a felony. Such notice shall include a general description of the conduct resulting in the conviction. Failure to provide such information may result in termination of the contract. Vendors must complete and submit the Felony Conviction Affidavit included with the Bid/Proposal form. Furthermore, District Policy CH Local states that The District shall not consider for contract approval a business whose owner or operator has a criminal record that includes a conviction, deferred adjudication, plea of guilt, or no lo contendere for any felony offense. SUBMISSION OF RFQ All quotes, whether delivered by hand, mail or are due no later than Tuesday, May 17, 2016 at 2:00 PM, at the DISTRICT s Purchasing Office located in the Lockhart ISD Administration Office at 105 S. Colorado, Lockhart, TX to the attention of Tanya Homann, Purchasing Manager. Quotes can be ed to Tanya Homann at Ta nya.homann@lockha rt.t xe d.ne t LATE PROPOSAL All bids delivered will be stamped with the time and date as proof they were received on or before the requested time and date. Bids received after the requested time and date will be considered late and returned unopened. If a return address is not provided, a late bid will be opened for identification and returned. PROPOSAL FORM All proposals must be submitted on the proposal form furnished to insure uniformity. If there is a substitute product that vendor would like to propose, in addition to the product referenced, please make a copy of the quote form and submit it with your original bid. Please provide response on this bid even if you do not wish to participate, failure to send back quote form may result in deleting that company s name off our vendor list records.
