The sum of R is payable in advance for 4000 shares in above mentioned Co-op. If membership is not granted by the board of Directors, the amount will be credited. To ensure compliance with the provisions of the National Credit Act, the following documents must also be provided to SS-Co-op, together with the membership application form. In the event of the applicant being a CC, - the applicant s / CC s most recent (not older than 12 months) financial statements; - copies of the CK1 form, - the most recent CK2 form, - the members agreement (if any), (members names and addresses) - a resolution authorising the membership application of the CC, - the identity documents of the members, - short motivation why an account for the CC has to be open, - VAT certificate, - proof of members residential address, for example; Utility bill, municipal water & lights account, or Bank statement on an official Bank document/form, - proof of the business physical address from where the Close Corporation operates for example; Utility bill, (municipal water & lights account), Bank statement on an official Bank document/form, or letter from auditor verifying the business address, - proof of bank account details, attach Bank statement, a letter from bank on official Bank document or cancelled cheque. If the CC wants to apply for a credit facility and or hire purchase: - a resolution authorising the opening of an account and credit facility with SS-Co-op must be provided, (pro forma resolution attached) completing the information document relating to legal entities.
