Dear Sir, I am the holder of above referred trading account with you which is mapped with my bank savings / current account bearing A/c No. with Bank. The said Bank account is being held by me along with other holders as detailed in Schedule I. Pursuant to SEBI communication received through various mutual funds, I wish to add Joint holder(s) (who are also joint holder(s) in my above bank) as detailed in Schedule II to my current & future mutual funds holdings procured through The details of my current mutual funds holdings are mentioned in Schedule III. I understand that: 1. Kotaksecurities.com shall only be recognizing the first holder for placing various types of mutual fund transaction requests either through its websites or through call n trade facility. 2. All communications will be sent to the trading account holder i.e. First holder. 3. It will be my/our responsibility to obtain independent advice regarding the tax implication of changing my Mutual Funds investment from single mode to Joint holder mode. 4. Kotaksecruities.com will forward my request for processing to the AMC and therefore my request shall be processed by respective AMC at their discretion. 5. Any discrepancy or error in bank detail or other details provided to Kotaksecurities.com may make my request liable for rejection. Kotaksecurities.com may not specifically inform me about such rejection/discrepancy. I will not have any claim on Kotaksecurities.com for any such rejections. 6. Any of the holders of the mutual fund units may directly approach the AMC for transactions of such units. 7. The aforesaid holding pattern will be applicabl e to all my current and future subscriptions in various mutual funds schemes through kotaksecurities.com. 8. By this request my current & future mutual funds holdings subscribed through will be converted in aforesaid joint names in Either or survivor mode. 9.
