1 SilverScript Employer PDP sponsored by Southern California Edison Frequently Asked Questions Q: What is SilverScript Employer PDP sponsored by Southern California Edison? A: SilverScript Employer PDP sponsored by Southern California Edison (SilverScript) is the prescription drug plan for Medicare-eligible retirees and Medicare-eligible spouses/registered domestic partners who are not enrolled in the Kaiser Permanente Senior Advantage plan. It is a self-insured Medicare Part D prescription drug plan with additional coverage provided by Southern California Edison. This additional coverage means that you have more coverage than the standard Medicare Part D plan. The plan is offered by SilverScript Insurance Company and is administered by CVS/caremark, Edison s current pharmacy benefit manager for active employees and pre-medicare retirees. Q. How do I enroll in the plan? What do I need to do? A: You will be automatically enrolled in the plan. You don t have to do anything. Edison is handling your enrollment. Q. When does my coverage in SilverScript start? A: Your coverage in SilverScript will begin on your effective date. Q: What does the Employer PDP mean in SilverScript Employer PDP? A: Employer PDP means it is an employer-provided Medicare Part D prescription drug plan. Q: What is Medicare Part D? A: Medicare Part D is prescription drug coverage that helps to cover the cost of prescription drugs for anyone who qualifies for Medicare Part A and/or Medicare Part B. It can be offered as a group benefit sponsored by a former employer, such as Edison. 1

2 Q: How do I qualify for a Medicare Part D plan? A: To be eligible for a Medicare Part D plan, you must: Be eligible for Medicare Part A and/or enrolled in Medicare Part B Be a U.S. citizen or permanent resident of the United States Live in the plan s service area Q: Who is covered by SilverScript Employer PDP sponsored by Southern California Edison? A: To be eligible for the SilverScript Employer PDP plan sponsored by Southern California Edison, you must: Be enrolled in Medicare Part A and/or Part B Be eligible for Medicare Part D Be an Edison retiree or a covered spouse of an Edison retiree Be enrolled in an Edison retiree health plan Live in the plan s service area which includes the United States and its territories Q: My spouse will not be eligible for Medicare until next year. What information will you need to enroll him/her in the plan? A: We need your spouse s Medicare Claim Number from his/her red, white and blue Medicare Health Insurance card. This number is referred to as the HICN Health Insurance Claim Number. It is used to identify your spouse when his/her enrollment is sent to Medicare. Q: We are moving soon to a new home outside the city. We are going to get a Post Office Box to make it easier to get our mail. Will that be a problem? A: Medicare requires that we provide your street address to prove that you permanently reside in our service area all 50 states, the District of Columbia or the U.S. territories. If we have a P.O. Box as your home address, we will need to ask you for your street address. Q: Why are my spouse and I both getting information from SilverScript? A: Medicare is an individual benefit. That means that every person has his or her own account with Medicare. Each of you will receive your own ID card with your own unique ID number. When you get your prescriptions filled, you need to make sure you use your ID card and your spouse needs to use his/her own ID card to get his/her prescriptions filled. 2

3 Q. Could my enrollment in this plan affect any other coverage I may already have? A. Yes, Medicare does not permit you to be enrolled in two Medicare prescription drug plans at the same time. Your enrollment in SilverScript Employer PDP sponsored by Southern California Edison may cancel your enrollment in the following types of plans: Another Medicare Part D plan A Medicare Advantage Plan with or without prescription drug coverage Likewise, if you enroll in another Medicare Part D plan or Medicare Advantage Plan in the future, you will automatically be disenrolled from SilverScript Employer PDP sponsored by Southern California Edison. You will lose your Edison-provided prescription drug coverage, unless you enroll in the Kaiser Senior Advantage plan provided through Edison. Q: What happens to my spouse/registered domestic partner or child who is covered on my plan but is not eligible for Medicare? A: Your spouse/registered domestic partner or child will continue to be covered on his or her current prescription drug plan through Edison until he or she is eligible for Medicare. He or she will continue to use the ID card they currently use to fill their prescriptions. Children are not eligible for SilverScript and will remain on the plan they are currently enrolled in, if eligible. Q. What happens if I don t want to be enrolled in SilverScript Employer PDP sponsored by Southern California Edison? A. If you are eligible for Medicare, you must be enrolled in SilverScript prescription drug coverage to keep your Edison-provided prescription drug coverage, unless you enroll in the Kaiser Senior Advantage plan provided through Edison. Q: Will I get a new ID card? A: Yes, you will get a new SilverScript ID card after your enrollment is processed. Do not use this card until your effective date. At that point, you should discard your old prescription drug ID card. If your covered spouse is also eligible for Medicare, he or she will also get a new ID card after his or her enrollment is processed. Your spouse s ID card will have a different number than your ID card. You each need to use your own ID card when you go to the pharmacy to fill your prescriptions. Q. How does my prescription drug benefit work with Medicare Part D? A. In the materials you receive from SilverScript, you will see information that shows the different stages or benefit levels for a standard Medicare Part D plan. You don t have to worry about the different stages. 3

4 You pay the same copayment or coinsurance percentage you currently pay through all the Medicare Part D stages. The additional coverage provided by Edison covers the gaps between Medicare Part D and your current pre-medicare coverage. Q. What does the additional coverage from Edison cover? A. The additional coverage from Edison fills the gap between the standard Medicare Part D plan and the pre-medicare coverage, such as: Paying the difference in cost between what you would pay with a standard Medicare Part D benefit and the pre-medicare copayment or coinsurance. Covering drugs not on SilverScript s standard formulary, the list of drugs covered by the plan Covering certain drugs that are not covered by Medicare Part D Allowing you to use an out-of-network pharmacy to fill your prescriptions. Q. How does a Medicare Part D plan work? A. The standard Medicare Part D plan has four stages or benefit levels. This is how the plan works in 2016 without the additional coverage provided by Edison: Stage 1 is the Deductible stage. This is when the individual must pay a certain amount $360 in 2016 before the plan pays any part of the cost Stage 2 is the Initial coverage period that begins after the individual meets the deductible. During this stage, the individual pays 25 percent of the drug cost until he/she reaches $3,310 in total drug costs. Total drug costs is the combined total of the amount the individual paid and the plan paid during the calendar year for covered drugs. Stage 3 is the Coverage gap, also called the Donut Hole. This is when an individual pays a large portion of the cost, either 45 percent of the cost of brand name drugs or 58% of the cost of generic drugs. The individual stays in this stage until he/she has $4,850 in Medicare out-of-pocket costs. Stage 4 is the Catastrophic coverage stage that begins after an individual reaches $4,850 in Medicare out-of-pocket costs. This is the stage when the plan pays most of the cost. The individual pays the greater of 5 percent of the prescription drug cost or a minimum copayment of $2.95 for generic drugs or $7.40 for brand-name drugs. Medicare s out-of-pocket costs include the amount the individual paid, the amount others paid on the individual s behalf and any manufacturer discounts received in the Coverage Gap. It does not include the amount the plan paid. You do not have to worry about these different stages. The additional coverage provided by Edison covers the gaps between Medicare Part D and the pre-medicare 4

5 coverage. Just remember that you pay the same copayment or coinsurance percentage through all the Medicare Part D stages. Once you reach Medicare s out-of-pocket cost, you pay the lower of your current copayment or Medicare s catastrophic coverage coinsurance or copayment. Q. What is a formulary? A. For SilverScript, the formulary is the list of drugs covered by the standard SilverScript plan. It includes both brand name and generic drugs selected by the plan with the help of doctors and pharmacists. The additional coverage provided by Edison will cover any drugs that are covered on your current plan but are not included in the SilverScript formulary. A 2016 Abridged Formulary (List of Covered Drugs) will be included in the Welcome Kit you will receive after you are enrolled in the plan. If your drug is not listed in this booklet, you can call SilverScript Customer Care at (866) to obtain more information. TTY users should call (866) Q. How do I find out if SilverScript covers my drugs? A. You will receive the 2016 Abridged Formulary (List of Covered Drugs) in your Welcome Kit after you are enrolled in the plan. This Formulary will list only the most commonly used drugs covered under the Medicare Part D portion of the plan. It will not list the drugs that are available through the additional coverage provided by Edison. If you have any questions, contact SilverScript Customer Care at (866) , 24 hours a day, seven days a week. TTY users should call (866) Q. What do I have to pay when I get my prescription filled? A. You will have the same copayment or coinsurance percentage you currently have under your plan today, if enrolled. Costs for medication can change over time and vary by pharmacy and other factors. Q. Which pharmacies can I use? A. SilverScript has many of the same network pharmacies that are in the CVS/caremark network for active employees. You must use a network pharmacy to have your drug costs count toward your Medicare total drug costs and Medicare out-of-pocket costs, except in an emergency or non-routine circumstance. Due to the additional coverage provided by Edison, you will continue to be covered when you use an out-of-network pharmacy. 5

6 Q. Can I fill my prescriptions at a Veterans Administration (VA) pharmacy with SilverScript? A. No, your prescriptions will not be covered under this plan if they are filled at a VA pharmacy. VA pharmacies cannot be included in Medicare Part D plan networks. You can still use a VA pharmacy and use your VA benefits. However, the cost of those medications will not count towards your Medicare total drug costs or Medicare out-ofpocket costs. Q. Can I fill my prescriptions at a pharmacy that is not part of SilverScript s pharmacy network? A. Yes, although we encourage you to use a network pharmacy. You must use a network pharmacy to have your drug costs count toward your Medicare total drug costs and Medicare out-of-pocket costs, except in an emergency or non-routine circumstance. Due to the additional coverage provided by Edison, you will continue to be covered when you use an out-of-network pharmacy. If you get your prescription filled at an out-ofnetwork pharmacy, you will have to pay for the full cost for your drug and request reimbursement from SilverScript. You will need to send your request for payment to SilverScript, along with your receipt showing the payment you have made. You will be reimbursed for the plan s share of the cost for your medication. Please refer to the Summary of Benefits or Evidence of Coverage that you will receive from SilverScript. Q. When do I need to get a prior authorization? A. Some drugs may need prior authorizations under the plan. If you take a drug that may be covered under Medicare Part B or Medicare Part D, you may have to get a new prior authorization before the drug can be filled after your effective date. This process is required in order to determine which coverage Part B or Part D covers that use of the drug, based on your medical condition. If you need to get a prior authorization, you can call SilverScript Customer Care after your membership goes into effect to start the prior authorization process. You can reach Customer Care at (866) , 24 hours a day, seven days a week. TTY users should call (866) If you currently take a drug that requires a prior authorization and you will need to refill your prescription soon after your effective date, you may want to refill your prescription through CVS/caremark (unless you are currently a Kaiser member) to allow enough time for your prior authorization to be processed after your effective date. 6

7 Q. What is Extra Help? A. Extra Help is a Medicare program that helps individuals who have low income pay for prescription drug costs. In 2015, those with an annual income no more than $17,655 for an individual or $23,895 for a married couple may qualify. Q. How do I know if I am eligible for Extra Help from Medicare? A. If Medicare identifies you as an individual that qualifies for Extra Help to pay for your prescription drug costs, you will receive a letter from Medicare or the Social Security Administration. To see if you qualify for Extra Help, you can: Call Medicare at MEDICARE ( ). TTY users should call Available 24 hours a day, 7 days a week. A: If you qualify for Extra Help, your share of the cost will be reduced. Copayments range from $0 to 15 percent coinsurance. The LIS Rider, which is included in your Welcome Kit that you will receive in December, will tell you the exact amount of your copayment or coinsurance in You will pay the lower of: Your Extra Help copayment or coinsurance Your Edison copayment or coinsurance Q: What is the Late Enrollment Penalty? A: The Late Enrollment Penalty (LEP) is the amount an individual must pay if he/she: Did not enroll in a Medicare prescription drug plan when first eligible for Medicare Did not have creditable prescription drug coverage coverage at least as good as Medicare s standard plan Had a break in coverage of more than 63 consecutive days Q: I have been covered on Edison s retiree medical plan since I retired. Do I have to worry about the late enrollment penalty? A: No, Edison s retiree prescription drug coverage has always met or exceeded Medicare Part D coverage standard and has therefore been considered creditable coverage. 7

8 Q: What happens if I am contacted about a late enrollment penalty? A: If SilverScript contacts you, they may need information about your past prescription drug coverage to send to Medicare. Please make sure you provide the information requested in the notice. You may also ask that any decision about your late enrollment penalty be reconsidered. Edison has decided to cover any late enrollment penalty at this time. But if you leave the plan or are disenrolled from the plan, you may have to pay the late enrollment penalty in the future. Q: Will my income affect what I have to pay for my Medicare coverage? A: Similar to Medicare Part B, some people may have to pay an extra amount for their Medicare Part D premium because of their annual income, called the Part D Income- Related Monthly Adjustment Amount (D-IRMAA). If your modified adjusted gross income in 2014 and reported on your tax return in 2015 is more than $85,000 for an individual and $170,000 for a married couple filing jointly, Medicare requires that you pay an additional premium based on your income. The additional amount ranges from $12.70 to $72.90 per month in It is deducted from your Social Security check or you will be billed by Medicare. The additional amount is not paid to Edison or to SilverScript. It is important that you pay this additional amount if required. If you don t, Medicare will disenroll you from SilverScript Employer PDP sponsored by Southern California Edison. If you are disenrolled from the plan, you will lose your Edisonprovided prescription drug coverage, unless you enroll in the Kaiser Senior Advantage plan provided through Edison. Edison will reimburse you for this additional premium. Q: How do I get reimbursed for the additional premium? A: If you have to pay the additional D-IRMAA amount to Medicare for your Part D coverage, you will receive a notice from Social Security or an invoice from Medicare. To be reimbursed by Edison for the additional premium, you need to provide proof of your Medicare Part D IRMAA payment annually with your request for reimbursement. Medicare requires Medicare prescription drug plans to offer an optional, free service called Medication Therapy Management (MTM). If you take multiple medications, have a chronic condition or high drug costs, you may be invited to participate in a MTM 8

9 program designed for your specific health issue. You may choose not to participate, but you should carefully consider taking advantage of this free service.
