2 Overview Here at Rogers, we support you as you grow your family through the birth or adoption of a child. As you prepare for your new addition, painting the baby s room and keeping up with the doctor s visits, it s important that you also know how we support you while you re off caring for your child. As part of your Total Rewards offering, you may be eligible for a maternity, parental or adoption leave benefit from Rogers, providing you with income, in addition to the amount you receive from Employment Insurance (EI) or the Quebec Parental Insurance Plan (QPIP), if you reside in Quebec. It s important that we plan appropriately for your leave, so we ask that you provide your manager at least 4 weeks notice of your absence for a maternity, parental or adoption leave, in addition to your expected duration. Rogers Maternity, Parental and Adoption Leave Benefit Eligibility Length of Service Less than 3 months 3 to 12 months 12+ months Maternity/Parental Leave (Birth Mothers Only) Parental or Adoption Leave (Birth Fathers or Adoptive parents) Full-time Part-time Full-time and Part-time Not eligible Not eligible Not eligible 4 weeks of pay at 100% of your Base Pay* minus the benefit you re eligible to receive from EI/QPIP**; and 2 weeks of pay at 70% of your Base Pay minus the benefit you re eligible to receive from EI/QPIP 4 weeks of pay at 100% of Base Pay, minus the benefit you re eligible to receive from EI/QPIP; 2 weeks of y at 70% of Base Pay, minus the benefit you re eligible to receive from EI/QPIP; and 11 weeks of pay at 70% of your Base Pay which exceed the years annual maximum insurable earnings Not eligible 2 weeks of pay at 70% of Base Pay, minus the benefit you re eligible to receive from EI/QPIP; and 15 weeks of pay at 70% of your Base Pay which exceed the years annual maximum insurable earnings Not eligible 2 weeks of pay at 70% of Base Pay, minus the benefit you re eligible to receive from EI/QPIP; and 15 weeks of pay at 70% of your Base Pay which exceed the years annual maximum insurable earnings Note: * Base pay is your regular salary or Annual Benefits Base Rate (ABBR) if you are compensated in part or wholly by commissions. ** The benefit you re eligible to receive from EI/QPIP means the weekly earnings you receive from EI/QPIP upon approval. 2 Maternity, Parental and Adoption Leave
3 Applying for the Rogers Maternity, Parental and Adoption Leave Benefit Eligible employees must submit confirmation of EI or QPIP payments to myhr to start receiving payments through the Rogers Maternity, Parental and Adoption Leave Benefit. You have up to 3 months after you return to work (at the end of your leave) to apply. You must be in receipt of EI or QPIP to be eligible for this benefit. Remember to submit your application for government (EI/QPIP) benefits on time! Applying late to these programs may affect the amount you receive on EI or QPIP. For example, you should apply as soon as possible after you start your leave. If you delay applying for benefits later than four weeks after your last day of work, you risk losing government benefits. For more information on government benefits, visit the Service Canada website, or if you reside in Quebec, visit the Quebec Parental Insurance Plan website. Employee Benefits & Wealth Accumulation Participation while on Maternity, Parental and Adoption Leave During your period of Maternity, Parental and Adoption Leave, your employment seniority and vacation entitlement continue to accumulate. You will also continue to enjoy your employee discount. Participation in the following Employee Benefits & Wealth Plans are continued uninterrupted, provided that you continue to make full and regular monthly contributions over the period of the leave: Pension plan Employee share accumulation plan Basic and optional life insurance Accidental death and dismemberment insurance Dental plan Health plan B.C. medical plan Long-term disability plan If you choose to discontinue or waive your benefits coverage, you must notify myhr. You may re-enroll in Employee Benefits within thirty one (31) days of your return to work. If you re-enroll after thirty one (31) days from your return to work, you will be required to complete and submit a medical questionnaire for approval. Based on this information, the insurance carrier may approve or deny health coverage and will impose a $ month restriction to dental benefits. Add your new Child to your Employee Benefits If you would like to add your new child to your Employee Benefits coverage, you must do so within ninety (90) days from their date of birth or the day in which the child comes into your legal custody. Applications received after ninety (90) days will be considered a late entrant. In this case your dependent child s enrolment will be subject to medical underwriting. Based on this information, the insurance carrier may approve or deny health coverage and will impose a $ month restriction to dental benefits. 3 Maternity, Parental and Adoption Leave
4 Calculating your Maternity/Parental Leave Benefit from Rogers (Birth Mothers) Your maternity/parental leave benefit is calculated using three components: 1. Your regular earnings (base salary or ABBR for employees who are compensated wholly or in part by commission); 2. The amount you re eligible to receive through Employment Insurance (EI) or the Quebec Parental Insurance Plan (QPIP) for residents of Quebec; and 3. The years Maximum Insurable Earnings. In 2018, the maximum insurable earnings for all provinces, excluding Quebec, is $51,700. The maximum weekly amount payable from EI is $547. In Quebec, the maximum insurable earnings is $74,000 and the maximum weekly amount payable from QPIP is $996. When calculating your maternity/parental leave benefit, the following four steps must take place: 1 2 Payment for the first week of your leave is based on 100% of your regular earnings. In all provinces, excluding Quebec (payments start immediately), there is a 1-week waiting period before you start receiving EI benefits. If you reside in Quebec, the amount you re eligible to receive from QPIP is deducted from the amount paid by Rogers. Payment for weeks 2 to 4 of your leave is based on 100% of your regular earnings. However, in this step, the payment you re eligible to receive from EI is deducted from the amount paid by Rogers. If you reside in Quebec, the amount you re eligible to receive from QPIP is deducted from the amount paid by Rogers. 3 Payment for weeks 5 and 6 of your leave is based on 70% of your regular earnings. In this step the payment you re eligible to receive from EI or QPIP is deducted from the amount paid by Rogers. 4 Payment for weeks 7 to 17 is based on 70% of your earnings above the years annual maximum insurable earnings. 4 Maternity, Parental and Adoption Leave

6 Calculating your Parental or Adoption Leave Benefit from Rogers (Birth Fathers or Adoptive Parents) Your parental or adoptive leave benefit is calculated using three components: 1. Your regular earnings (base salary or ABBR for employees who are compensated wholly or in part by commissions); 2. The amount you re eligible to receive through Employment Insurance (EI) or the Quebec Parental Insurance Plan (QPIP) for residents of Quebec; and 3. The years Maximum Insurable Earnings. In 2018, the maximum insurable earnings for all provinces, excluding Quebec, is $51,700. The maximum weekly amount payable from EI is $547 for Standard Leave and $328 for an Extended Leave. In Quebec, the maximum insurable earnings is $74,000 and the maximum weekly amount payable from QPIP is $996. When calculating your parental or adoption leave benefit, the following two steps must take place: 1 Payment for the first 2 weeks of your leave is based on 70% of your regular earnings. In all provinces, excluding Quebec, there is a 1-week waiting period before you start receiving EI benefits, unless the waiting period has already been satisfied (i.e. when EI benefits are shared and the waiting period is satisfied by the birth mother through the maternity leave portion of the leave). In this step, if you reside in Quebec, the amount you receive from QPIP is deducted from the amount paid by Rogers. 2 Payment for weeks 3 to 17 of your leave is based on 70% of your regular earnings above the years annual maximum insurable earnings. Example C: Regular full-time and part-time employee residing outside of Quebec (standard leave is elected) Maximum weekly EI payment = $547 Weeks 12 plus Months of Service [($1, x 70%) $547 from EI] x 2 = $ Total Benefit from Rogers $ $75,000 - $51,700 / 52 = $ ($ x 70%) x 15 = $4, Total Benefit from Rogers $5, Grand Total Benefit from Rogers $5, Note: the example above assumes the EI waiting period has been satisfied.
