1 Senegal Malaria Financial Landscape PATH MACEPA DECEMBER 2015 MALARIA FUNDING IN SENEGAL AT A GLANCE The estimated cost to fully fund Senegal s Strategic Framework is 124,086,654, or approximately $165 million. The estimated cost to fully fund the Strategic Framework for the years is approximately $119.5 million. The US President s Malaria Initiative (PMI) estimates that there will be a gap of approximately 1.6 million long-lasting insecticidal nets (LLINs) in 2016, when a planned nationwide LLIN distribution campaign will occur, and a gap of approximately one million LLINs in In recent years (the period ), PMI provided approximately 60 percent of Senegal s malaria funding, spending over 101 million on malaria programs and activities. During this same period, the Global Fund provided approximately 30 percent of malaria funding, spending more than 50 million from BACKGROUND The funding requirements for Senegal s National Malaria Control Program (NMCP) are based on the goals and objectives included in its National Strategic Framework. 1 Senegal s Strategic Framework General Objectives 2 (1) To reduce malaria-related morbidity in order to achieve the epidemiological pre-elimination threshold by (2) To reduce malaria-related mortality by 75 percent by Strategic Framework Specific Objectives (1) To protect at least 80 percent of pregnant women through intermittent preventive treatment (IPT) using sulfadoxinepyrimethamine (SP, 3 doses). (2) To protect at least 80 percent of pregnant women through long-lasting insecticidal nets (LLINs). (3) To provide seasonal malaria chemoprevention (SMC) to 98 percent of targeted children. (4) To get at least 80 percent of the population sleeping under LLINs. (5) To protect at least 90 percent of the population through indoor residual spraying (IRS) in targeted areas. (6) To treat at least 95 percent of productive breeding sites in targeted areas. (7) To diagnose 100 percent of suspected cases via rapid diagnostic tests (RDTs), microscopy and, where necessary, molecular biology in the pre-elimination areas. (8) To diagnose at least 95 percent of suspected cases by RDT or thick smear. (9) To treat 100 percent of confirmed cases of malaria according to national guidelines. (10) To provide pre-referral treatment to 100 percent of severe malaria cases among children under the age of 10. (11) To detect 100 percent of epidemics and emergency situations early on. (12) To control 100 percent of epidemics and emergency situations in the two weeks following their detection. Priority Areas The Strategic Framework identifies seven priority areas to support the achievement of the Strategic Framework objectives. 1. Case management: strategy development to ensure universal access to diagnostic testing (by RDT or microscopy) and treatment by artemisinin-based combination therapies (ACTs). 2. Prevention: working toward universal coverage of LLINs through mass campaigns and routine distribution. Routine distribution to be achieved through antenatal care (ANC), community based-organizations, schools, and the private sector. SP will be administered to pregnant women under direct observation (IPT). IRS will be implemented in selected areas to ensure a greater impact in terms of lowering transmission and thus lowering the number of cases. 2 program will be rolled out in areas where this is possible. SMC will be implemented in parts of the country where the rainy season lasts 4 months, with 60 percent of malaria cases occurring during this period. This will be through the administration of SP and amodiaquine to children aged 3 months to 10 years. 3. Management and coordination: specific interventions to focus on strengthening the management unit and coordination mechanisms. 4. Information, education, and communication (IEC)/Behavior change communication (BCC): IEC/BCC and health promotion will be strengthened by focusing on evidence-based strategies. 5. Surveillance and response: weekly surveillance in areas of low transmission and monthly surveillance in the rest of the country. Investigation of cases will be conducted in the pre-elimination areas. Reactive monitoring resulting from the passive detection of cases will be vital. Specific surveillance of mobile populations (pastoralists/migrants) will be provided by establishing a system of home-based care (PECADOM) in the different communities. The DSDOMs will, in particular, notify the morbidity data gathered. 6. Supply and inventory management: improvements to avoid stock-outs through good planning and also good distribution of supplies. 7. Strengthened monitoring/evaluation and research: enabling better monitoring of progress and a consolidation of lessons learned. SENEGAL S MALARIA FINANCING REQUIREMENTS The Government of Senegal has estimated the cost of fully funding the Strategic Framework at 124,086,654, or $165,035, The average annual cost to implement Senegal s malaria program for is approximately 24.8 million, or $33 million. The Government of Senegal has estimated the cost of fully funding the Strategic Framework for the years at approximately $119.5 million. Senegal s Concept Note to the Global Fund for states that the 43,025,915 malaria funding request to the Global Fund would meet 50 percent of Senegal s Strategic Framework funding needs for this period. 4 Doubling the request yields a total malaria funding need of 86,051,830, or approximately $114,448,934, for Adding Senegal s projected domestic contribution of $5,076,396 for yields a total funding requirement of $119,525,330. SENEGAL S MALARIA FINANCING COMMITMENTS HISTORICAL Historically, PMI and the Global Fund have been major donor partners to Senegal. Other major partners include the World Health Organization (WHO), UNICEF, and the Islamic Development Bank (IDB), as shown in Table 1. Table 1. The Global Fund (which spent more than 50 million in ) provided approximately 30 percent of funding for malaria efforts during this period. The Government of Senegal contributed approximately 6.9 million in , or approximately 4 percent of total spending. MALARIA FINANCING COMMITMENTS Total estimated malaria funding commitments for in Senegal are approximately $118.9 million. Figures for commitments are based where possible on publicly available data concerning future spending (as for the Global Fund). Otherwise, projected spending is based on 2014 funding levels. Additional detail about the methodology for developing funding estimates is provided in footnote citations. Table 2 below is intended to provide only a general picture of the malaria financial landscape in Senegal for Funding figures may change as additional data about future spending become available. Global Fund website, Funding Decisions page In its Concept Note to the Global Fund, the Government of Senegal reports that PMI spent 15,988,700 in Senegal in Multiplied by 1.33, the average exchange rate for 2014, this spending level is equal to $21,264,971. $21,264,971 x 3 = $63,794, In its Concept Note to the Global Fund, the Government of Senegal reports spending 1,272,280 on malaria activities in Multiplied by 1.33, the average exchange rate for 2014, Senegal s 2014 malaria spending totals $1,692,132. $1,692,132 x 3 years = $5,076, PMI notes that the Islamic Development Bank is finalizing a $10 million loan to Senegal to be disbursed in US President s Malaria Initiative. FY 2016 Senegal Malaria Operational Plan, p In its Concept Note to the Global Fund, the Government of Senegal estimates that 2014 malaria funding from UNICEF, WHO, and PATH MACEPA totals 363,047 (UNICEF spending of 117,386 + WHO spending of 7,331 + PATH MACEPA spending of 238,330). 4 COMMODITY GAP ANALYSES PMI has developed gap analyses for key malaria commodities in its FY 2016 Senegal Malaria Operational Plan. The tables below summarize PMI gap analysis estimates. PMI estimates that there will be a gap of approximately 1.6 million LLINs in 2016, when a nationwide LLIN distribution campaign is planned to occur, and a gap of approximately one million LLINs in Table 3.
