3 Health Savings Account (HSA) About the Health Savings Account Q. What is a Health Savings Account (HSA)? A Health Savings Account, or HSA, is a separate, tax-advantage savings account available to you only if you are covered under what s called a qualified high deductible health plan, such as the Dover Blue Cross Blue Shield (BCBSIL) CDHP, or SelectHealth CDHP. An HSA works in the same way as a personal savings or checking account only the money is used to pay for current and future qualified medical, prescription drug, dental and vision expenses for you and your eligible dependents tax free. If you are enrolled in SelectHealth CDHP, contact HealthEquity at for more information on your HSA. If you enroll in Dover CDHP, a Health Savings Account (HSA) is automatically opened in your name. Q. What are some advantages of an HSA? You don t pay taxes on money going into your HSA. HSA money used towards eligible health care expenses isn t taxed. You control how HSA money is spent and decide on the right care for you based on quality and cost. You own the HSA; the money is yours to keep even if you terminate employment. Any unused money at the end of the year rolls over and stays in your HSA to use at any time. The HSA is portable, and goes with you when you terminate employment. HSA money can be used to pay for eligible medical, prescription, dental and vision expenses, and more. Q. Who owns the HSA? The account belongs to you. As the account holder, it is up to you to keep track of deposits and withdrawals, and using the account for qualified health care expenses to get the full tax-advantages of your HSA. Q. Who administers the HSA? The HSA administrator bank or supplier partner depends on the Dover CDHP in which you enroll: For BCBSIL, it is HSA Bank; for SelectHealth CDHP (employees at USSynthetic and Quartzdyne) it is HealthEquity. Q. How can I get the best value from an HSA? Know how your Dover CDHP coverage works so that you can best estimate how much to set aside in your HSA to help pay for out-of-pocket expenses; Make pre-tax contributions to your HSA through payroll deduction (you can change your contribution amount at any time); Take advantage of preventive care covered at 100% in your CDHP to stay healthy and keep small health problems from becoming serious; 3

4 HSA eligibility Q. Who s eligible for an HSA? The IRS has rules about who s eligible to own and contribute to an HSA. You are not eligible if: You are not covered by a single or family qualified high-deductible plan, such as Dover BCBSIL CDHP or SelectHealth CDHP (for employees at USSynthetic and Quartzdyne); You are enrolled in any part of Medicare; You are claimed as a dependent on someone else s tax return (except your spouse s); You are covered by TriCare or have received VA benefits within the last 90 days. (If you have received VA benefits, you are eligible for the HSA after 90 days.) Q. When can I open an HSA? The HSA is set up automatically once you enroll in the Dover CDHP during annual enrollment or as a new hire. Note Once you enroll in the CDHP, you will receive a Welcome Kit from your HSA Bank or HealthEquity administrator that will include the information you need for managing your account. You will also receive a debit card to use for reimbursement of eligible expenses. Q. Can I enroll in both an HSA and FSA? Yes, but if you enroll in both an HSA and flexible spending account, or Health Care FSA, IRS rules say you cannot have a standard FSA. But you can have what s called a limited-use FSA to use only for dental, orthodontia and vision expenses. If you have both accounts, be sure to set aside just the amount you need in your Health Care FSA because you will lose remaining FSA contributions not used by year end. To learn more, refer to the section and chart on FSAs describing how the accounts differ later in this FAQ. HSA contributions Q. Who can put money in an HSA? You, the Company, or anyone, can contribute to your HSA. However, only the account holder receives tax deductions on the money contributed. Q. Will my company make contributions to my HSA? To help with your deductible and eligible medical expenses under the CDHP, your Operating Company and Dover Corporate will contribute to your HSA. For more information about Company contributions to your HSA, refer to the HSA information provided in your enrollment materials or contact a Benefits Advocate. Remember Employer contributions, and any deposits you make to your HSA, are yours to keep and use for eligible health care expenses at any time. Q. As a new hire that began working mid-year, how will the company contribute to my HSA? Company HSA contributions are pro-rated for new hires based on when eligible and when the quarterly funding occurs which is the first business day of each quarter. Q. How do I contribute to an HSA? You can elect to have contributions automatically deducted before taxes from your pay. You can also make deposits on your own by depositing or transferring funds from a personal banking or checking account, then taking the tax deduction when you file your taxes. 4

5 Q. Can I change my HSA election if I contribute through payroll deductions? You can change your contribution election at any time by adding, increasing, decreasing or even stopping your contributions. The new election will take effect, as soon as administratively practicable, at the next payroll period. Q. Are there contribution limits to an HSA? The IRS sets an annual maximum for deposits to an HSA each year. The maximum is tied to the CDHP coverage level in which you enroll single or family. For 2018, the total contribution limits (from all sources you and the company) are shown here. Also, if you are age 55 or older, you can make additional catch-up contributions above the annual limit. If you enroll in a Dover CDHP with: Employee only coverage Employee & children or Employee & spouse or Employee & family coverage The 2018 HSA annual contribution limit is: $3,450 from all sources combined $6,900 from all sources combined If you are age 55 or older, you can make additional catch-up contributions over the annual limit of up to $1,000. Note Your deposits, company deposits and any other deposits count toward the annual limit. Q. What happens if my HSA deposits go over the IRS annual contribution limit(s)? You are responsible for making sure that you do not exceed the HSA annual maximum. If at the end of the year your deposits exceed the limit, you must report the excess to the IRS on your federal tax return. You will also have to pay taxes on the excess money and any earnings attributable to the excess money. Q. If I don t reach the annual limit by year end, can I still add contributions to my HSA for the prior year in the next year? Similar to an IRA, you can make contributions for the prior year until the tax filing deadline (April 15) of the following year. This timeframe excludes tax-filing extension deadlines. Contact the HSA administrator for more information. Q. Can I rollover money to the Dover HSA from another employer s HSA or from other savings vehicles, such as 401(k) or IRA? You may roll over money to your Dover HSA from some savings vehicles as shown here. You may wish to consult with a tax advisor if you have any questions. If I have another HSA and keep that account, can my employer contribute to the other HSA? No. Employer contributions will only be deposited to your HSA associated with Dover CDHP. 5

6 Q. My CDHP and HSA takes effect January 1, can I start contributing to my HSA later in the year, say in May? As long as you are enrolled in the CDHP, you can elect to contribute to your HSA at any time through automatic pre-tax deductions from your pay or by making your own deposits directly to your account. (For any direct deposits you make, for example, by check, you would take the tax deduction when filing your taxes.) Q. Does the money in an HSA earn interest? Yes, and it is tax deferred. Your interest rate will depend on the balances in your account. Please remember that the maximum an employee can contribute to an HSA in 2018 is $3,450 for Employee only coverage, and $6,900 for Employee & child, Employee & spouse and Family coverage. Q. Can the money in an HSA be invested? Yes. The HSA administrator bank will make investment options available to you. No minimum HSA balance is required to start investing. Please contact HSA Bank customer service representatives at or visit for more information on investing HSA funds. Q. Is the money in an HSA FDIC insured? Yes. HSA deposits are FDIC insured. However, if you choose to invest the HSA money in your account, those investments are not guaranteed. Q. What if I don t use all the money in my HSA? Unlike Flexible Spending Accounts (FSA) where you lose unused funds, the unused money in your HSA rolls over and accumulates year to year. Remember, you own the HSA, and the money is yours to keep and to use at any time, now or in the future, for eligible health care expenses. Q. What happens to the money in my HSA if I switch to a different medical plan? If you switch to a different Dover medical plan, such as a PPO, or enroll in your spouse s or partner s nonqualified HSA plan, you can continue to draw on existing HSA funds to pay for qualified health care expenses but cannot make contributions to your account. Q. What happens to my HSA if I leave my job? If you leave Dover, whether through termination or job elimination, any funds deposited into your HSA your contributions and company contributions are portable and go with you to use for eligible health care expenses at any time. Q. Do my payroll deposits and/or company contributions stop when my employment ends? If you leave Dover, whether through termination or job elimination, any payroll deductions you ve elected and company contributions will end. Company HSA contributions will end on the last date of the month in which the employee is terminated. Also, administration fees for the account that are paid for by Dover Corporate will be the terminated employee s responsibility starting the first of the month following the employee s termination date. Please keep in mind that any funds deposited into your HSA are portable and yours to keep and use for eligible health care expenses at any time. 6

7 HSA qualified medical expenses Q. What eligible medical expenses can I use an HSA to pay for? You can use your HSA for what s called qualified health care expenses. Some examples include: Doctor and hospital visits Medical equipment Dental care, braces, dentures Vision care, glasses, contacts lenses Prescriptions and medications, including over-the-counter drugs, when prescribed by your doctor For a full list of qualified medical expenses, see IRS Publication 502 at Q. Are over-the-counter medicines eligible under an HSA? Yes, but they require a prescription to be an eligible HSA expense. IRS rules require that over-thecounter (OTC) medicines and drugs are only eligible for reimbursement under an HSA if prescribed by a doctor. If your doctor suggests an OTC medicine, just ask for a prescription. These rules do not apply to insulin (including over-the-counter insulin). Some examples of over-the-counter medicines include: Allergy and sinus: Actifed, Benadryl, Claritin, Sudafed Antacids: Mylanta, Pepcid AC, Prilosec, TUMS Aspirin and pain relievers: Advil, Excedrin, Motrin, Tylenol To find a list of the OTC medicines that are eligible under an HSA, see IRS publication 969, Q What if I use my HSA for an expense not considered a qualified expense? If you use an HSA for an expense that the IRS does not consider qualified, you will be required to pay tax on the withdrawal and a penalty may apply. If you are unsure about an expense, go online to the HSA administrator for your CDHP (see below) or call the customer service representative: For BCBSIL CDHPs Go to hcsc.hsabank.com, or call , 24/7 For SelectHealth CDHPs Go to healthequity.com, or call , 24/7 Using my HSA and CDHP Q. How do the HSA and CDHP work together? The Dover BCBSIL and SelectHealth CDHPs are designed with higher deductibles to keep medical contributions low for you and your family. While paying lower medical contributions, you can put those savings into your HSA to then use to help pay for eligible health care expenses. With the CDHP, preventative services, such as annual physicals, screenings and vaccinations, are covered in full, without having to meet your deductible. For services other than preventative and some preventative prescription drugs, you would pay out-of-your pocket until you meet the deductible. You then pay the applicable coinsurance for the covered expense up to the annual out-of-pocket maximum. You can use HSA contributions to help cover the out-of-pocket expenses, such as your deductible, doctor visit fees, prescriptions and other eligible expenses. Or, if you don t have major health expenses beyond preventative care in a year, an HSA lets you keep unused contributions and build up savings to use at any time later, even in retirement. 7

8 Q. When can I begin using my HSA? HSA contributions are available for use as soon as funds are deposited. Q. If I have a large medical bill early in the year but not enough money in my HSA to cover the expense, can I add money to my HSA to take out later to pay for it? Yes, as long as you incurred the expense once your HSA has been opened and add additional funds in the same calendar year, you can build up your HSA balance to cover prior expenses and get reimbursed at a later date. HSAs work differently than flexible spending accounts (FSA) where you re able to get up to the annual amount you ve elected, even while funds are not yet deposited. With an FSA, you only have access to the contributions in your account, but in an HSA you have the option of covering the health care expenses throughout the year as long as the expense was incurred and additional funds added in the same calendar year. Q. Should I save my itemized receipts? While you are not required to submit receipts for HSA reimbursements, it s always a good idea to keep your receipts as proof of your eligible expenses. If the IRS asks for proof that your account was used only for qualified health care expenses, you will be able to provide the receipts. Q. I am enrolled in a CDHP and have employee-only coverage; can I use my HSA for the medical expenses of my spouse or child who is covered by another medical plan? Yes, you can use your HSA to pay the qualified expenses of your spouse and/or child (deductibles, coinsurance, etc.), even though they are enrolled in another company s medical coverage regardless of whether it s a PPO, HMO or high-deductible plan. It s the HSA account holder, in this case the Dover employee, who cannot be covered by a health plan that is not considered a qualified high deductible plan. Q. Can my spouse and I both have HSAs? There are certain IRS rules that apply to married couples who both have HSAs. You can each have a separate HSA. However, IRS rules say that if either spouse has family coverage in a high deductible plan, then both spouses are considered to have family CDHP coverage. This IRS rule applies even if one spouse has single coverage and the other spouse has family coverage. Note If either spouse has family coverage under a plan non-qualified high deductible plan that covers both spouses, such as a PPO or HMO, or has a standard FSA, then both individuals are ineligible for an HSA. For more information, you may wish to consult a tax advisor. Q. My domestic partner is covered under my Dover CDHP; can I use my HSA for my domestic partner s medical expenses? If your domestic partner and/or domestic partner s children meet the IRS qualifications to be considered your tax dependent, then you can use your HSA to pay for their eligible health care expenses. Q. Are there any fees associated with having an HSA? Listed below are the fees associated with an HSA. 9 Q. What happens if my HSA is closed? If an account is closed due to the employee contacting HSA Bank and requesting the account be closed or HSA bank closed the account because of additional information required, a paper application must be completed and sent back to HSA Bank to re-open the account. In order to receive funding for the current year the account must be opened no later than 11/1 otherwise no funding can be added to the account and the next funding will occur in the following plan year. If an employee s account is closed because they fail to provide proof of identity, a paper application is required and two forms of ID are needed to re-open the account. HSA Bank communicates directly with the employee when an account is closed due to failure to provide proof of identity. HSA Bank will send a letter at 4 days, another letter at 3 weeks, and a phone call will be placed to the employee at 5 weeks. For example, if an account is closed to a negative balance, the employee will receive negative balance letters, the suspected fraud account would be frozen, and a call directly to the employee is made. Below is a chart of accepted forms of identification to re-open an account: If the employee fails to keep the account in good standing then the employee must contact the Client Assistance Center to find out the reason the account is not in good standing. HSAs and dependent children Q. I am eligible to carry my dependent children up to age 26 on my Dover CDHP; can I use my HSA for my child s medical expenses to age 26? This is only true if the adult child is considered a dependent for tax purposes. The Affordable Care Act, allows adult children to be covered until age 26 under medical plans; however, the IRS has not changed HSA requirements to coincide with updated health care regulations. Put another way, you can use your HSA for your child s medical expenses if you claim your child as a dependent on your federal tax return. The IRS makes some exceptions in the case of divorce or separation see the next FAQ for details. 9

10 Here are the IRS requirements for dependent children: Age must be under age 19 at the end of the tax year, or under age 24 if a full-time student for at least five months of the year, or be disabled at any time during the year. Relationship must be the taxpayer s child or stepchild. Residence must have the same principal residence as the taxpayer for more than half the tax year. Exceptions apply for children of divorced or separated parents, temporary absences, and for children who were born during the year. Support must not provide more than one-half the child s own support for the year. Refer to Section 152 of the IRS Code, which provides details and defines dependent tax qualifications. You may also wish to consult a tax advisor if you need more information. Q. I m divorced from my child s other parent, can I use my HSA for my child s medical expenses, even if my former spouse claims our child as a tax exemption? Yes. IRS rules for HSAs offer an exception if you are a non-custodial parent and/or your child s tax exemption is claimed by your former spouse. Specifically, a child of parents who are divorced, separated, or living apart for the last 6 months of the calendar year, is treated as the dependent of both parents (see IRS Publication Keep in mind you must be legally divorced or separated at the end of the year or have lived apart during the last six months of the year in order for this rule to apply. Q. Last year I claimed my adult child on my tax return, but my child is no longer eligible having turned 26; to claim this year, can I use my HSA for their medical expenses? Remember, there are four requirements of qualifying child that must apply in order for you to use your HSA to pay for your child s medical expenses (see previous questions). Once your child no longer meets these requirements, you can no longer pay their medical expenses from your HSA see also IRS Code Section 152(d). You may also wish to consult a tax advisor for further clarification. Q. My adult dependent graduates from college in June; can I use my HSA for medical expenses for the period before or after my child s graduation? Your dependent child must meet the definition of dependent as defined by the IRS, under Code Section 152(d) as described in the previous questions; that is, you can use your HSA as long as your child meets the requirements of an eligible dependent. You may also wish to consult a tax advisor for further clarification. Q. My adult child will open his/her own individual HSA; can I contribute and/or transfer money from my HSA to my child s HSA? Transfers are not permitted from one individual s HSA to another s HSA not even to the account of your child, spouse or other family member. However, you can contribute to your child s HSA with deposits from your personal accounts such as savings or checking, or even with cash. Remember that only your child (the account holder) will receive the tax benefits. 10

11 Paying for Eligible Expenses/Using the HSA Debit Card Q. How do I pay for eligible medical expenses from my HSA? You can pay your provider for a qualified health care expense or reimburse yourself if you ve paid for the expense out of your pocket. These payment options are available through your HSA administrator bank: Debit card Your HSA debit card lets you pay for eligible expenses at most pharmacies, hospitals and doctors offices. You should select Credit at the register (transaction fees apply when used with a PIN) and amounts will be deducted from your account balance. You can also use your HSA debit card to withdraw funds from an ATM. Checks With an HSA Bank check, you can pay your provider when you receive a bill for eligible expenses or pay yourself back for out-of-pocket medical expenses you ve already paid. (Paper check ordering fees apply and vary by quantity requested.) Online transfers When you sign up for Internet banking, you can pay your provider or reimburse yourself for eligible expenses you ve already paid by transferring funds from your HSA to your personal checking or bank account. (There s no cost for this option.) Paper request form You can submit a withdrawal form to request payment for an expense. (A processing fee applies for this option.) Q. How should I use my HSA debit card? Your HSA debit card can be used as credit (signature-based) or debit (PIN-based) card to pay for eligible medical expenses at health care providers, hospitals or pharmacies. Whenever possible, always select credit when paying for eligible expenses to avoid transaction/pin fees. Keep in mind that a transaction fee applies whenever you use your PIN. Additionally, if you make withdrawals from an ATM, an ATM fee may apply also. When you open the HSA, two debit cards will be mailed to your home if you have an authorized signer in addition to yourself, on the account. An authorized signer can be added on the Employee Portal to your account. To obtain extra cards, contact your HSA administrator bank for forms and instructions. A fee will apply for any additional or replacement cards requested. Q. What if I use my HSA debit card for a cash withdrawal to pay for an ineligible expense? Remember that you are responsible for using the HSA for qualified health care expenses. If you use your account to pay for ineligible expenses, you will be taxed on the money at your income tax rate, plus a 20% penalty if you re under age 65. If you re 65 or older, you will be taxed on the money but won t be assessed the additional penalty. Q. Will my HSA debit card automatically decline payments for non-eligible medical expenses, such as groceries? Your HSA debit card is medical-merchant coded. This means the merchant must provide medicalrelated care, services or products in order for the card to work. If you were to use your card at a restaurant or gas station, your transaction would be declined since these are not medical merchants. Q. What if my HSA debit card doesn t work or my transaction is declined? Your HSA balance may be too low to cover the cost. If this occurs, you will need to pay with another form of payment (cash, check or credit card); and pay yourself back later. If you write an HSA check and your account is overdrawn, overdraft fees will apply, similar to a personal checking account. 11

Do transaction limits apply to my HSA debit card? For security purposes, your HSA debit card comes with a daily limit on the dollar amount you can use with your HSA as credit and will pay for medical expenses of up to $3,000 per day. There s also a daily limit if using your HSA with your PIN for eligible expenses. This amount is $300 per day and includes PIN transaction fees. For transfers of HSA funds to your personal savings or checking account(s), a $2,500 per day limit applies. For more information and fee schedules, check with your HSA administrator bank (see Managing My HSA later in this FAQ). Quick Tip If there are not enough funds in your account and you try to withdraw, the transaction will be denied. (See, Managing My HSA later in this FAQ) Managing My HSA Q. How do I manage my HSA? Your HSA Bank administrator makes it easy and convenient by offering the tools you need to manage your account online. Go to the HSA online portal of your HSA Bank administrator or call the customer service representative if you need assistance. For HSAs associated with: BCBSIL CDHP HSA Bank is the supplier/partner Go to hcsc.hsabank.com or Call customer services representatives at ,24/7 HSA Bank mobile app Through your online account, you can: See your HSA balances, contributions and withdrawals Get reimbursements Enter claims for electronic or check reimbursement Make transfers Invest funds Update contact information, order debit cards, order checks View online tax statements Find useful information and more *New users are required to create a Username and Password. Q. Will I have an HSA Bank account number? Yes, your HSA will be assigned an account number, similar to any personal savings or bank checking account. The account number will be included in the welcome kit that you will receive in your home mail approximately 10 days after you first enroll in the Dover CDHP with HSA. 12

13 HSAs if you re retired, have Medicare, Medicaid or VA benefits Q. I m age 65 but not yet retired; can I contribute to an HSA? Yes, as long as you are enrolled in a qualified health plan, such as the Dover CDHP and have not enrolled or applied for Medicare, you can contribute to an HSA. Q. My spouse has Medicare but I m enrolled in the CDHP with HSA through Dover, can I contribute to an HSA? Yes, you can contribute to your Dover HSA as long as you have not enrolled in Medicare yourself and are enrolled in the Dover CDHP. You can use the HSA for eligible expenses for yourself and your spouse. Q. Can my HSA be used to pay Medicare premiums? Yes, you can pay for Medicare premiums, but once the accountholder enrolls in Medicare Part A or Part B, he or she can no longer contribute to the HSA. Q. My child or spouse has Medicaid; can I enroll him or her in Dover family CDHP and participate in an HSA? Yes. CDHP and HSA eligibility does not depend on your income, your earned income, or the insurance coverage of your child. You can enroll in family Dover CDHP even if your child or spouse has Medicaid. Q. I m a Dover employee who s received VA benefits, can I open an HSA? You can if you wait at least 3 months after last receiving VA benefits before you open an HSA. If you have an HSA and receive VA benefits in the future, you are not entitled to contribute to your HSA for another 3 months (this includes accepting contributions from any source). However, you are still able to use existing funds to pay for qualified health care expenses during the 3 month period. Tax reporting and HSAs Q. What tax reporting will the HSA administrator bank provide? You will receive IRS Form 1099 SA, which reports the withdrawals, called distributions you took from your HSA; and IRS Form 5498-SA for accountholders. The forms will be mailed to your home by the HSA administrator bank each year in January. Q. Do I need to file special forms with my federal tax return to show contributions to my HSA? Yes, you ll need to file IRS Form 8889-Health Savings Accounts with your federal tax return. Please consult a tax advisor if you have specific questions. General HSA Information Q. What happens if I pass away should I name a beneficiary? Yes. You should designate a beneficiary by going online to the HSA Bank portal or call the HSA customer service representative (see Managing My HSA). Q. What should I do if I change my address or my legal name (marry or divorce)? Remember, you own the HSA, so it s up to you to keep your personal information current, just as with any savings or checking account. To change any personal information, go online to the HSA Bank portal or call the customer service representative (see Managing My HSA). Reminder If you move or have a name change, you ll also want to make certain that you advise your local HR representative about the change. Remember to make any other beneficiary changes to life insurance or other benefits. 13

14 Flexible Spending Accounts (FSAs) About Flexible Spending Accounts Q. How do Flexible Spending Accounts (FSAs) work? Flexible Spending Accounts (FSAs) help you pay for eligible expenses not covered by your health plan. If you enroll, the money you contribute is automatically deducted from your pay before taxes, and set aside for you to use to pay for certain medical, dental, pharmarcy, vision or dependent day care expenses. You can enroll in one or both accounts: Healthcare FSA Dependent Day Care FSA Q. Who administers the FSA? WageWorks administers the account(s) and processes FSA reimbursements. Q. What does contributing to FSAs pre-tax mean? You may think of salary in terms of annual or hourly gross wages but the amount you take home is after you pay taxes state, federal income, Social Security and Medicare withholdings. The tax rate, based on your earnings, is applied to gross pay. When you set aside money in an FSA, it s taken from your gross pay before taxes are deducted. This reduces the total amount of taxes you pay and puts more money in your pocket. Q. How much tax savings can I expect with an FSA? You can save on average as much as 30% on taxes but the actual amount will depend on your household income, number of dependents and tax filing status. You can even calculate how much more money you could take home when you use an FSA by using the WageWorks FSA calculator tool online at wageworks.com, click on I m an Employee, then What are WageWorks Benefits. Q. What is a Health Care Flexible Spending Account (FSA)? The Health Care FSA can be used to pay for eligible medical expenses for yourself and your eligible dependents (including your tax-dependent domestic partner and your partner s qualifying children). If your domestic partner and/or domestic partner s children are not your tax dependents, their expenses are not reimbursable under the Health Care FSA. Q. What is a Dependent Day Care Flexible Spending Account (FSA)? If you need help with daycare expenses in order to work or look for work, or for your spouse to attend school full-time, a Dependent Day Care FSA allows you to pay for daycare for qualified dependents that include your spouse and children, or your tax-dependent domestic partner or partner s qualifying children. To be eligible for reimbursement, dependent day care expenses must be necessary for you (and your spouse/partner) to work or to look for work. Work may include actively looking but does not include unpaid volunteer work, or volunteer work for a nominal salary. Your spouse is considered to have worked if he or she is a full-time student for at least five calendar months during the tax year, or if physically or mentally incapable of self-care. 14

15 FSA Eligibility Q. Who s eligible to enroll in FSAs? Employees who work at least 30 hours per week are eligible to enroll in FSAs. Even if you do not enroll for medical and/or dental coverage through Dover, you can enroll in one or both flexible spending accounts. Q. Who is eligible for reimbursements from the Health Care FSA? You, your spouse and dependent children that are qualified tax dependents are eligible for reimbursements under the Healthcare FSA, even if you do not have medical coverage through Dover. If your domestic partner and/or domestic partner s children qualify as your tax dependents, their expenses are also eligible under the Health Care FSA. Q. Who qualifies for the Dependent Day Care FSA? Dependent Day Care FSA can only be used for qualified dependents that include: Your children (or domestic partner s children if tax-dependent) under age 13 Your spouse, domestic partner or other dependent of any age, who is physically or mentally incapable of self-care and claimed as an exemption on your federal income tax. Q. Do I need to enroll to participate in an FSA? Yes, you must enroll to participate in an FSA. To continue participating, after your initial enrollment, you are required to re-enroll each year during annual enrollment. Your elections do not carry over from one year to the next. Can I enroll in an FSA if I don t have medical and/or dental coverage through Dover? Yes. You are not required to be enrolled in a medical or dental plan in order to participate in an FSA. Q. I am enrolled in a Dover CDHP with an HSA, can I also enroll in the Health Care FSA? Yes, but if you enroll in both a Health Care FSA and and are enrolled in a CDHP with an HSA IRS rules require your FSA to be a limited-use Health Care FSA and can be used only for eligible dental, orthodontia and vision expenses until you have met the CDHP deductible. Once you have met the CDHP deductible your Health Care FSA can reimburse you for medical expenses. Alert Plan your FSA contributions wisely and put in only the money you will need for the year. HSA contributions roll over to use at any time but you lose unused FSA contributions if not used by year end. Contributing to an FSA Q. How much can I contribute to an FSA? The FSA contribution limits are set by the IRS each year. Spending Account Annual Minimum Annual Maximum Health Care FSA $100 $2,500 Dependent Day Care FSA $100 $5,000 if married and filing jointly; $2,500 if filing separate tax forms* *Expenses reimbursed under your Dependent Day Care FSA may not also be reimbursed under your spouse s Dependent Day Care FSA, and vice versa. 15

16 Q. Can I change my FSA contribution election(s) during the year? Once made, you cannot change your FSA contribution elections during the year that is, increase, reduce or stop contributing, unless you have a qualifying family status change. This is a life event that may affect your coverage needs, such as marriage, divorce or birth of a child. Additionally, any change you make must be consistent with that life event. For example, if you have a baby, you can add or increase contributions* to a Healthcare or Dependent Day Care FSA but you cannot stop contributions if already elected. You have 30 days from the date of the status change to notify your local HR representative of the change. You must also provide proof, such as a birth certificate or marriage certificate to verify the change. Q. What IRS restrictions apply to my FSA contributions? In exchange for the tax savings, the IRS puts some restrictions on your FSA contributions: You cannot change your contribution amount during the year, except for certain qualifying status changes. Contributions you don t used by the end of the year are forfeited. You cannot transfer money from one flexible spending account to another, that is from the Health Care FSA to the Dependent Day Care FSA, and vice versa. Q. What if I don t use up the contributions in my FSA(s) by the end of the year? IRS rules require that any money you contribute to your FSA only be used for eligible expenses incurred during that same plan year. However, you can submit claims incurred in the prior plan year up to March 31 of the following year. Any money left in your account(s) after the claims filing deadline (March 31) is lost. You cannot use one year s contributions to pay for the next year s expenses. Unlike health savings account (HSA), the money in an FSA does not roll over each year. Q. What happens to unsued contributions in my FSA if I terminate employment? Your contributions to the FSA end when your employment ends. Any money left in your Healthcare and/or Dependent Day Care FSA that is not used for reimbursement of eligible expenses incurred by your termination date will be lost. However, if there s a balance in your Health Care FSA, you can request reimbursement for expenses incurred while you were still working by submitting a request for reimbursement to WageWorks. COBRA-continuation coverage for your Health Care FSA is also available through the end of the plan year in which you terminated or retired. Q I have both a Health Care FSA and a Dependent Day Care FSA, can I shift money between my FSA accounts? No. IRS regulations require the accounts operate separately. You cannot transfer funds or use your Health Care FSA for eligible dependent day care expenses, or the reverse. FSA Eligible Expenses Q. What are considered eligible Health Care FSA expenses? The IRS determines which expenses are eligible for reimbursement. Some examples include: Deductibles/coinsurance for medical and dental plans Office visits and prescriptions drug copayments Expenses that exceed plan maximums; for example, if not covered by your medical plan, or expenses that exceed plan limits Dental treatments (but not teeth whitening or cosmetic dental work) Orthodontia Vision (eyeglasses, exams, contact lenses, and LASIK surgery) For a full list of eligible expenses, go to irs.gov, see IRS Publication 502 Medical and Dental Expenses. 16

17 Q. What are considered eligible Dependent Day Care FSA expenses? The IRS determines which expenses are eligible for reimbursement. Generally these include: Preschool or nursery school (but not kindergarten) Summer day camp Child care Sick child care Elder care for qualified tax-dependents Q. What providers are eligible for reimbursement under a Dependent Day Care FSA? To be eligible for reimbursement under a dependent care FSA: The services must be provided by an eligible provider of child care. This includes a licensed child care facility that complies with applicable state and local laws and any licensed individual who is not an employee s tax dependent or is their child who is 19 or older. The expense must be for services already received and not services to be provided in the future. For example: If an employee prepays for a summer day camp for their dependent, reimbursement cannot be provided until after their dependent attends the camp. FSAs and Life Events Q. What is a qualifying status change and how does it impact my FSA? Once made, your FSA contribution election remains in effect throughout the calendar year, unless you experience a qualifying event or special enrollment event. A qualifying event or special enrollment event that could affect your coverage needs includes marriage, divorce, or birth of a child. If you experience a change in status, you have 30 days from the date of the event to change your FSA election(s) by adding, increasing, decreasing or stopping contributions to your account. The change you make must be consistent with the life event. For example, if you adopt a child, you can add or increase your contributions to the health and/or dependent care FSA; but you cannot stop your contributions. If you have a life event, contact your local HR representative within the 30-day timeframe to make any benefits changes. Q. I married in May and enrolled in a Health Care FSA starting June 1, can I be reimbursed for expenses incurred anytime during the year? No. You would only be able to request reimbursements for expenses incurred going forward from the date you participated in the FSA for that calendar year. For example, if you married in May and your Health Care FSA took effect on June 1, you can submit medical expenses incurred on and after June 1; that is for the period from June 1 through December 31, in which you were an actively participating in the Health Care FSA. Using FSAs Q. How am I reimbursed for Health Care FSA expenses? When you request a payment from WageWorks, you are reimbursed for the amount of eligible expenses up to the amount of your annual election, minus any previous reimbursements. For example, let s say you elect to set aside $480 each calendar year ($40 a month) for your Health Care FSA. In January, you request reimbursement for a $50 prescription and will be reimbursed for the $50. In February, you buy eyeglasses and contact lenses, and your share of this expense is $600, and request reimbursement for $600. Because your annual FSA balance is $430 ($480 total minus the $50 already reimbursed January), you will be reimbursed for $430. Your account is now empty for the rest of the calendar year. 17

18 Q. How am I reimbursed for Dependent Day Care FSA expenses? When you request a payment from WageWorks, you are reimbursed for the amount of the eligible expense up to the current balance in your account (contributions to date minus any previous reimbursements). For example, let s say you elect to set aside $480 each calendar year ($40 a month) for your dependent care FSA. In January, you file a claim for $30 and are reimbursed for the $30. In March, you file a claim for $200 and are reimbursed for $50 ($80 contributions for January and February minus $30 claim already reimbursed. The $150 difference ($200 claim minus $50 reimbursed) remains an eligible expense and will be reimbursed as payroll contributions to your account are deposited. Q. If I have an expense late in the year but don t receive my bill until the next year, can I still request payment? An FSA claim (health care and/or dependent day care) is eligible for reimbursement in the year in which it is "incurred." The expense is considered "incurred" on the date of service or treatment, not the day you pay for it. If the service or treatment extends beyond the end of the year, only expenses incurred during the plan year for which you are contributing to your FSA will be eligible for reimbursement in that plan year. You have until March 31 of the following plan year to request reimbursement for expenses incurred in the prior year. Requesting FSA reimbursments/using the FSA debit card Q. How do I request FSA reimbursements? You can request reimbursements for both Healthcare and Dependent Day Care FSAs through WageWorks, the FSA administrator. To obtain FSA Reimbursement Forms visit wageworks.com. Once logged in, go to the navigation bar (left side of screen), then select Submit Receipt or Claim. (New users must register and select a Username and Password.) You can also call WageWorks representatives at to request the Forms. Customer service representatives are available Monday through Friday, 7 a.m. to 9 p.m. CT. Note You can also access your account through WageWorks mobile app. Q. What are my FSA reimbursement choices? WageWorks gives you a choice of reimburement options: WageWorks Healthcare debit card works like a debit card and can be used for health care-related expenses at hospitals, providers and pharmacies, such as prescriptions or eyeglasses. Pay My Provider lets WageWorks pay your health care provider directly when you fill out an online form. Pay Me Back allows to you pay yourself back for expenses you ve already paid for, either through deposits to your checking account or by check mailed to your home. Important To make sure your reimbursement or health care debit card verification is approved without delay, make sure you submit the right documentation. Also, be sure to save your receipts to submit to WageWorks (by fax or mail). See the next FAQ. 18

19 Q. What proof or claim documentation does WageWorks need for reimbursements? For payment requests or Healthcare Debit Card Verification, follow the instructions below (also found online at wageworks.com in the Claim & Card Use Verification Documentation Checklist ). 1. Prepare your documentation and include: Provider name (facility or person who provided service; or if a purchase, where the item was bought, e.g., hospital, doctor, pharmacy) Patient name Service date(s), for care or item purchase Type of service (bag tag receipt can be used for prescriptions) Patient name Financial proof (show amount you paid and portion paid by insurance by including an EOB from your medical plan) Important Credit card receipts and cancelled checks are not valid forms of proof and not accepted documentation. 2. Submit claims/documentation (forms are available online or by WageWorks customer service) Log into your account at wageworks.com Click on the right claim link Healthcare or Dependent Care or Card Use Verification form for the fax number or mailing address to submit your documentation Speed up claim payments! Use your EOB ( explanation of benefits from the insurance company); Use itemized receipts for services or prescriptions but make sure they include the information listed above (if an over-the counter medication, you must include the prescription from your doctor); Make sure handwritten documentation includes stamped provider information; Make sure information is clear and legible. Q. How do I use WageWorks Health Care FSA debit card? Your WageWorks Health Care FSA debit card works like any bank debit card. It s a convenient way to pay for medical expenses and prescriptions at health care providers and pharamcies. You can also purchase health care products and services at approved providers and merchants. When you use the card, always select Credit and the cost will be deducted from your health FSA. Q. Where can I use the WageWorks Health Care FSA debit card? WageWorks Healthcare debit card is good at select providers and merchants who have an IRS-approved inventory system in place. This includes most doctor and dentist offices, hospitals, pharmacies, and hearing and vision care providers. The card can also be used for health-related products and prescriptions at select discount stores and supermarkets. To find a list of approved providers and merchants, go online to wageworks.com. TIP Before using the card, you may wish to check the list of approved merchants at wageworks.com or by calling WageWorks customer service at Q. Do I need to submit my receipts to WageWorks for Health Care FSA debit card transactions? In most instances, debit card transactions at approved providers and merchants are automatically verified at checkout, which means you may not need to submit your receipt to WageWorks but should always retain a copy in case they are requested. If you use your card at a provider or merchant that does not meet the IRS-approved inventory standard, WageWorks will require that you submit receipts or your health insurance explanation of benefits (EOB) to verify the transaction. Important WageWorks will notify you if the transaction can t be approved and give you verification instructions (all card purchases must be verified within 90 days of the transaction date). 19
