4 1 Purpose of this data dictionary IDI Data Dictionary: Population cohort name and demographics data (August 2015 edition) documents the content of this dataset that the Ministry of Health (MOH) provides to Statistics NZ to use in the Integrated Data Infrastructure (IDI). This dictionary gives information on the variables contained in the dataset from 1 December 2004 (updated quarterly and usually three months delayed) including technical information and descriptions. Use this data dictionary if you are interested in understanding and accessing the population cohort name and demographics dataset in the IDI for your research. Background The MOH seeks to improve, promote and protect the health of New Zealanders through: its sector leadership of New Zealand s health and disability system; advising the Minister of Health, and government as a whole, on health issues; directly purchasing a range of important national health and disability support services; providing health sector information and payment services for the benefit of all New Zealanders. The objectives of the MOH s data and metadata are to: measure and describe the information available within the National Collections; promote uniformity, availability and consistency across the National Collections; support the use of nationally agreed protocols and standards wherever possible; promote national standard definitions and make them available to users. 4

5 2 About the population cohort name and demographics data Overview The National Health Index (NHI) number is a unique number assigned to each person using health and disability support services (healthcare user). The NHI is an index of demographic information associated with the NHI number that is assigned to each healthcare user using health and disability support services. The primary purpose of the NHI is to positively and uniquely identify healthcare users for the purposes of treatment, care and maintaining medical records. For the purposes of the IDI, this unique number allows linkage across national health collections. Observed population/ Analysis Unit: People with a National Health Index (NHI) number, who had interactions with the Healthcare system between 2004 and December 2014 Methodology Type of data: Administrative data capture Data collector: IT Services, MOH. Mode of data collection: Both service providers (eg District Health Boards [DHBs] private hospitals, screening providers, some General Practitioners) and the Ministry of Health (MOH) have the ability to update information in the NHI. Service providers generally update the NHI when a person has contact with secondary services and their information has changed. This information will be more accurate if a person has more contact with secondary health services. The NHI also receives monthly updates about births and deaths from Births, Deaths and Marriages (BDM) and from 2004 to 2012 received quarterly updates from the Primary Health Organisation (PHO) Enrolment Collection with change of address information. Frequency of data collection: The NHI system is updated via real-time transactions. The NHI datamart is updated daily from the NHI system. The IDI data in this table was extracted from the datamart on 3 December

6 IDI Data Dictionary: Population cohort name and demographics data (August 2015 edition) Quality information When multiple records for a healthcare user are identified (ie one healthcare user is found to have two or more different NHI numbers), the NHIs are linked, and one is deemed to be the primary (or master), while the other(s) become secondary (or event) NHI numbers. To avoid the use of multiple NHI numbers, only primary NHI numbers were provided in this extract. Although the NHI only records the most up to date information for each healthcare user, there is a historical table which includes all names the healthcare user has been known by (alternative names). The names in the population cohort name and demographics dataset come from this table. In 2012 the NHI moved to a new platform and this, coupled with other planned changes, should improve the quality of future data. For instance, currently only hospitals and some General Practioners can update NHI information (ie have read-write access). All other frontline providers such as pharmacists, dentists, and plunket nurses etc, only have read access. Although they can update the NHI via a call centre they usually only use this facility to create new NHIs, rather than to update address, ethnicity etc. It is hoped that by extending read-write access to all frontline providers, the NHI will be more frequently updated. Privacy, security, or confidentiality issues Privacy, security and confidentiality issues are covered by the IDI Confidentiality Rules and the IDI Output Guide. The population cohort name and demographics tables that are accessible to researchers do not contain any name or address information to identify an individual. All researchers who have access to the population cohort name and demographics data have had their research proposals assessed using Statistics NZ s microdata access protocols. Only approved researchers who have been granted access by Statistics NZ and the Ministry of Health may view the population cohort name and demographics data. All outputs produced from population cohort name and demographics data must be aggregated and counts suppressed if the underlying unrounded count is fewer than six. It includes the most up to date data available at the time of extraction about each healthcare user in the population cohort. Fields include demographic information, plus up to 12 alternative names (such as maiden names). This identifier will remain the same for an identity across refreshes. Where we receive more information during a subsequent refresh that indicates that two or more identities represent the same identity, the identifier may change. The snz_moh_uid represents a distinct identity in all of MOH tables in IDI. Format: Integer Notes: Variable name: Gender IDI variable name: moh_pop_sex_code Definition: The person's biological sex. Format: Char, 1 gender_code Notes: Sex reported for transsexuals and transgender people should be based on their biological sex. For instance, it is possible for a person to move from one sex to another following sex/gender reassignment surgery. Variable name: Ethnicity 1-3 IDI variable name: moh_pop_ethnicity_1_code, moh_pop_ethnicity_2_code, moh_pop_ethnicity_3_code Definition: Ethnicity is the ethnic group or groups people identify with or feel they belong to. Thus, ethnicity is self-perceived and people can belong to more than one ethnic group. They can also change their ethnic affiliation, both over time, and in different contexts. Format: Char, 2 ethnic_code Notes: From 1 July 1996 up to three ethnic group codes can be collected in the NHI for each healthcare user. Where more than three ethnic group codes are reported, the Statistics NZ prioritisation algorithm is used to report only three values. Format: Char, 1 nz_resident_status_code Notes: 'Y' = Permanent resident (New Zealand citizen or classified as ordinarily resident in New Zealand ); 'N' = Temporary (not a New Zealand citizen, does not have New Zealand ordinarily resident status) Variable name: moh_pop_birth_month_nbr IDI variable name: Definition: The birth month in which the person was born. Format: Tinyint Notes: Partial dates are permissible. At a minimum the century and year must be supplied. If day is provided but month is omitted then the day will not be recorded. Incomplete dates are stored as 'yyyy0101' or 'yyyymm01' and a partial date flag associated with the date is assigned.
