Pharmacy Prescriptions If you are currently receiving your prescriptions at the pharmacy, you must present the new OptumRx card. CVS90 Saver OptumRx will also provide a maintenance medication program called CVS90 Saver. You will continue to have three fills before you need to take action on maintenance medications. Once you have received three fills of maintenance medications, you must get your 90-day supplies through your local CVS Pharmacy or through OptumRx home delivery or you will pay more. Medication home delivery You could save time and money by getting maintenance medications through the mail through OptumRx home delivery. Enroll in OptumRx home delivery to get up to a three-month supply of the medications you take regularly often for less than what you d pay at a retail pharmacy. Your medication will come right to your mailbox. To start home delivery, log in to optumrx.com, us the OptumRx App or call TTY 711. Specialty Pharmacy BriovaRx, the OptumRx specialty pharmacy, is part of your benefit program. BriovaRx provides specialty medications and some clinical support for complex conditions, including cancer, arthritis and others. 3 Board of Trustees - Mid Central Operating Engineers Health and Welfare Fund 1100 Poplar Street Terre Haute, IN Summary Annual Report for MID CENTRAL OPERATING ENGINEERS HEALTH AND WELFARE FUND This is a summary of the annual report for Mid Central Operating Engineers Health and Welfare Fund, EIN , Plan Number 501, for the period August 1, 2017 to July 31, The annual report has been filed with the Department of Labor, as required under the Employee Retirement Income Security Act of 1974 (ERISA). Basic Financial Statement The value of plan assets, after subtracting liabilities of the plan, was $111,725,115 as of July 31, 2018, compared to $104,547,553 as of August 1, During the plan year the plan experienced a change in its net assets of $7,177,562. This change includes unrealized appreciation or depreciation in the value of plan assets; that is, the difference between the value of the plan s assets at the end of the year and the value of the assets at the beginning of the year or the cost of assets acquired during the year. The plan had total income of $86,654,211 which included employer contributions of $73,274,589, employee contributions of $8,929,646, other contributions of $0, gain/loss of $-304,240 from the sale of assets, and earnings from investments of $4,754,216. Total plan expenses were $79,476,649. These expenses included $4,417,515 in administrative expenses and $75,059,134 in benefits paid to participants and beneficiaries, and $0 in other expenses. Your Rights To Additional Information You have the right to receive a copy of the full annual report, or any part thereof, on request. 6 Gentlemen, rev up your engines! Guys, do you wait until you run out of oil to get an oil change? Probably not, so when it comes to fine-tuning your health, put yourself in the driver s seat. Eat right, stay active and get regular checkups, screenings and shots to make sure you stay running as smooth as your wheels. Choose the right fuel Your car needs the right gas to run properly and your body needs the right fuel to stay in great shape. Eating the right foods for your body can help you keep health issues under control. Here are some easy-to-follow heart-health tips 1 : Add more omega-3 fatty acids, which are essential fatty acids your body needs to work normally. Include them easily by consuming foods like fish, almonds and flax seeds. Limit your sugar intake and cut out processed foods and soft drinks. Eat at least 2 cups of fruit and 3 cups of vegetables daily. Stay active for better overall health. Take good care of your body, and it ll run like a well-oiled machine for years to come. Get your tune up Tune ups are essential to your car, and checkups are essential for you. As you get older, your risk to develop diabetes, testicular or prostate cancer and heart issues increases. By taking control of your well-being you can lower those chances. 2 Early detection can save your life. Here are a few things you can do to prevent health issues: Schedule preventive checkups. Know your family history. Get regular cancer screenings. Don t smoke. Maintain a healthy weight. Exercise regularly aim for 30 minutes, five days a week. No matter whether you drive a new car or an old one, you have to take care of it to get the most out of it. In fact, the older your car gets the more you have to do to maintain it. 7 Pay attention to the check engine light Sometimes, your car s engine needs a bit of extra help to keep going.
