3 2017 EMPLOYEE RATES OUC is proud to offer competitive benefits to our employees and retirees. One of the most important of these is comprehensive health care for you and your family. To assist employees with costs in 2017, OUC will be responsible for the largest share, but there will be a small increase in premiums for dependents, which we believe are offset by significant enhancements to benefits. THE FOLLOWING WILL TAKE EFFECT JANUARY 1, 2017: 2017 EMPLOYEE Weekly Medical, Prescription and Dental Insurance Premiums 2017 Medical, Dental & Prescription Options Core Plan ($1,000 Deductible) Health Reimbursement (HRA) Plan ($3,000 Deductible) Employee $ 0.00 $ 0.00 $ Employee plus one dependent $ $ $ Employee plus family $ $ $ Premium Plan ($300 Deductible) 2017 EMPLOYEE Weekly Aetna Vision Preferred Eyewear Insurance Premiums* Employee $0.98 Employee + 1 Dependent $1.86 Employee and Family $2.74 *This coverage is a pre-tax plan and is subject to IRS rules and qualifying status change. NEW THIS YEAR - Effective in the 2017 Plan Year: Employees will now be able to roll over $500 of unused 2017 funds into their 2018 medical FSA without penalty and without losing those funds.
