1 To Parents & Guardians From the Nagoya City Board of Education Fiscal Year 2014 Notice for Assistance with School Attendance Fees for Fiscal Year 2014 The City of Nagoya operates a means-tested program to assist persons experiencing difficulties paying for meal services, school supplies and other necessary expenses involved in sending children to elementary or junior high school. 1. Households eligible for Persons who fall under one of the Category 1 to 7 listings on page 2 and Documents for submission A. The Application to Receive Assistance with School Attendance Fees (Please submit the form at the bottom of this document to the school to acquire this application form.) B. Documents proving the information required for the category under which you are applying (refer to the back side of this page) Note: Those wanting certification for payment starting in April should submit all completed documents to your school by Friday, April 18 Note: Applications to Receive Assistance with School Attendance Fees are also accepted as necessary from May onwards. However, payments for applications received in or after May will begin from the month following the application.) 3. Notice 1. All the members of a household, including children and parents/guardians, must be listed in the Application to Receive Assistance with School Attendance Fees. 2. Persons who received for fiscal year 2013 and who have applied for continuation of do not need to submit a new application as it would overlap with the application for continuation of. (A new application must be submitted for students entering the first grade of elementary school.) 3. Persons with more than one child who wish to receive this must submit applications for each child. If you have children attending both elementary and junior high school, you must apply separately to each school. 4. Persons who are unable to provide the necessary supporting documents by Friday, April 18 should consult with the school. (Persons who are deemed to receive for April will have submitted their application and all necessary supporting documents by Wednesday, April 30.) 5. Approval will be based on the contents of your newest application. Please notify the school immediately if there is a change in your household s situation (such as change of guardian, re-marriage, change of address, living with grandparents, etc.). 6. If submitted documents are found to have been altered or to contain errors, the qualifying conditions will not be met and qualification may be revoked. 7. Victims of the Great East Japan Earthquake may be able to receive even if their income surpasses the Total Net Income. Please consult with your school. 2 Households eligible for with school attendance fees and necessary documents Pertaining to with school attendance fees, a household refers to everyone living in the same house. Even if a grandparent s Certificate of Residence is separate from the family members, as long as they live in the same house, they are included in the household. Also, even if a family member whose income supports the household lives in a different location, he/she is still included in the household. All persons in the same household as the child must fall under the same category below, regardless of who is listed on the Certificate of Residence. For example, a family cannot apply under categories that do not match, such as a case in which the father falls under category 3 Persons exempt or partially relieved from paying Municipal Residents Tax and the mother under category 4 Persons exempt from contributing to the National Pension. ) Category Persons who receive Public Financial Assistance Persons whose public financial was either suspended or discontinued on or after April 2, 2013 Persons exempt or relieved from paying Municipal Residents Tax Persons exempt from contributing to the National Pension Persons whose National Health Insurance premiums were reduced, or who were relieved from paying said premiums Persons who received Child Rearing Allowance Necessary Supporting Documents (copies acceptable for all documents) Applications must be accompanied by documented evidence that certifies the circumstances of everyone in your household. No certificate necessary No certificates necessary (as a general rule) A person whose social was discontinued due to a change of household (ex. re-marriage) is not applicable. Fiscal year 2013 Municipal and Prefectural Residents Tax Certificate (based on income during FY 2012) or Fiscal year 2014 Municipal and Prefectural Residents Tax Certificate (based on income during FY 2013) National Pension Insurance Premium Exemption Approval Notice Document (postcard) sent to each individual. NB Exemption must have been approved on or after April 1, NB Those who have lost this postcard must obtain and submit a Proof of Exemption Period for National Pension Insurance Premium. NB: We need an Exemption Approval Notice from everyone who is 20 years of age or older. NB: You cannot apply under this category if your household includes someone under 20 years of age who is not a student, someone 60 years of age or older, or someone who is having their insurance payments deferred through the Special Pension Payment Exemption for Students. Fiscal year 2013 National Health Insurance Premium Payment Notice/Determination Notice or fiscal year 2014 National Health Insurance Premium Payment Notice/Determination Notice (Final Assessment) NB We cannot accept a tentative payment notice. NB Amount must be recorded in the exemption / reduction column. NB Please make a copy of the entirety of this notice. NB You cannot apply under this category if your household includes someone on or over the age of 75 years old (Medical Care System for Older Senior Citizens). Child Rearing Allowance Certificate NB: Copy the page on which the Social Welfare Office director s seal is stamped. NB: Allowance must have been received on or after August 1, NB: A household must be comprised of only the person(s) receiving Child Rearing Allowance (the person(s) raising children) and any children eligible for this allowance (18 years of age or younger). NB: Should there be other people in the household such as grandparents or an ineligible child, you cannot apply under this category. NB: Child Allowance, Allowance for Single-Parent Families, Aichi Prefecture Orphan Allowance, and Special Child Rearing Allowance are not applicable. NB: If this allowance is being issued due to a severe disability of a parent, please attach a copy of his/her Disabled Person s Handbook. 3 7 Persons with financial difficulties other than those detailed in categories 1-6 above We will conduct a screening using the sum total of income over the last year (Jan. 1, Dec. 31, 2013) of each person residing with the child. Please submit one of the supporting documents for each person (except for those in high school, those in junior high school or below, or those who are dependents under the Income Tax Act). Persons whose total income over the past year for everyone living with the child was lower than a predetermined level of income (see Total Net Income for Household in the table below) are eligible for. Persons applying for under this category must explain the problems experienced with school expenses, in the Reason for Applying on the attached Application for the Continuation of Assistance with School Attendance Fees. Note: The amount of Total Gross Employment Income for Household in the top line is given as a guideline only; the Total Net Income for Household in the bottom line is the qualifying amount. Necessary Supporting Documents (copies acceptable for all documents) Applications must be Issuing Authority accompanied by documented evidence that confirms the circumstances of everyone in your household If you are applying in April or May (one of 1-3) NB If you cannot apply with 1, please submit 2 or 3. However, there are cases in which you do not have to file a final income tax return due to total amount of income or amount of income deductions, etc. In that case, please file the Municipal and Prefectural Residents Tax Return Form and submit Employment Income Withholding Slip You can submit the Employment Income Withholding Slip only in the case that you can tell the entirety of your income (from Jan. 1, Dec. 31, 2012) with a single document. In the case that you can't tell everything with a single slip or you have 2 or more Employment Income Withholding Slips, you should submit 2 or 3. (If you have the Public Pension Income Withholding Slip and Employment Income Withholding Slip, you can apply under this category.) NB In the case that an amount is not written in the employment income after deductions ( 給与所得控除後の金額 ) column, you cannot submit this document because your year-end tax adjustment has not been finalized. You should submit 2 or 3. However, if the Payment Amount ( 支払い金額 ) is on or below 1,030,000, the start of employment (mid-year)/date of leaving the company ( 中途就 退職 ) is an empty column, and you have no other income, this column can be blank. Your employer NB In the case that your date of employment is written (that is, if there is a 〇 in the 就職 column). The total payment amount by your previous workplace must be written in the "Note ( 摘要 )" column. If it is not written, or if you were unemployed before being employed by your current company, then please submit 2 or 3. *However, people who began employment on April 1, 2013 after graduating from high school in March 2013 and are still employed as of December 2013 can submit this Employment Income Withholding Slip. NB If the date that you left the company is written (that is, if there is a 〇 in the 退職 column), we cannot accept 1. Please submit 2 or 3. *We cannot accept a Payment Record for Remuneration, Fees, Contract Money, or Prize Money Final Tax Return (Copy) NB Please submit forms 1 and 2. NB The Final Tax Return (copy) must be stamped as received by the tax office. If it is not stamped as such, then please be aware that we may inquire about your income situation in the days following. 3 Photocopy of fiscal year 2014 Municipal and Prefectural Residents Tax Return Forms NB Copies of Return Forms, NOT Certificates. NB Please submit the original to the municipal tax office and a copy to the school. Please make copies beforehand as you will not receive a copy when submitting to the tax office. Applications submitted from June onward(one of 4-6) NB In this case, we cannot accept Fiscal year 2014 Municipal and Prefectural Residents Tax Certificate (based on income during FY 2013) Municipal and Prefectural Resident Tax Payment Notification ( 市民税県民税納税通知書 ) and your Tax Statement Form ( 課税明細書 ) (please submit both) Notification for Determined Special Collection Amount of Municipal and Prefectural Resident Taxes for Employment Income, etc. ( 給与所得等に係る市民税県民税特別徴収税額の決定通知書 ) Copies of documents submitted to the tax office Photocopies of documents submitted to a municipal tax office, tax office branch, ward office, or ward office branch Counter at Municipal Tax Office, Tax Office Branch, Ward Office, or Ward Office Branch Sent by Municipal Tax Office Distributed by your workplace

(Not paid directly to students.) Junior high schools offering school lunches will pay expenses for actually consumed food and drink. The amount for April will be paid around June 1, the amount for March will be paid at the end of March, and the amount for the rest of the months will be paid around the 25th of the next month. Paid for participating students deemed eligible for at time of activity Paid after the activity (normally takes 2-3 months) Paid for participating students deemed eligible for at time of excursion Paid after the activity (normally takes 2-3 months) Paid for students deemed eligible for in April Paid for students who use public transportation to travel to school whose distance is 4km or greater for elementary schools and 6km or greater for junior high schools due to special educational considerations (distance to the school is disregarded for children attending classes for students with disabilities). Paid in July, September, and January. Actual expenses incurred for the treatment of illnesses common amongst elementary and junior high school students (see Notes below), per the school s instructions for treatment, are paid to the medical facility via the school principal. (Not paid directly to students.) All years (Only at schools which offer school lunch) Expenses incurred for the issuance of a doctor s certificate in relation to receiving an allergy-free school lunch service. The receipt issued by a medical institution is needed to receive the. The amount for April and May will be paid around June 25, the amount for the rest of the months will be paid around the 25th of the following month. (Max. 3,000yen) Notes For households receiving public financial, this program only provides financial for School Excursions and School Medical Care. Other categories are covered under the public financial program and paid out by the social welfare office (Public Welfare & Child Division of the Ward Office). Illnesses common amongst elementary and junior high school students refers to trachoma, conjunctivitis (including pharyngoconjunctival fever), tympanitis, chronic nasal empyema, cavities, adenoids, parasitic disease, and specified skin diseases. Please make sure to receive the Details of Treatment and any other documents from the school beforehand and then go to a medical institution for treatment. (However, if you have a Children s Medical Care Card or Single Parent Household Medical Care Card, that will take priority for treatment.) Payment times are an approximate estimation. In reality, payment may be a little earlier or later. If a parent or guardian has requested that be paid into a bank account, in principle it will be paid directly into the account of the parent or guardian. However, in the event that fees to be collected by the school and subject to are unpaid, this amount may be directly paid to the school regardless of whether or not account transfer has been applied for. Assistance for the Food Allergy Guidance Expenses is paid to a parent or guardian via the school. (Not deposited directly into their bank account.) The name of the deposit account is: エンジョナゴヤシ (ENJO NAGOYASHI) 5. Q&A Q1: I want to receive Assistance with School Attendance Fees, but am worried that the other children will find out about it. A1: As information concerning Assistance with School Attendance Fees is personal information, it is managed administratively in such a way that no-one else will be able to find out about it. Avoiding other children finding out that their classmates are receiving Assistance with School Attendance Fees is something that is taken into
5 consideration. Q2: My income during 2013 exceeded the qualifying amount, but I recently lost my job and am having difficulty paying for expenses such as school supplies. Can I receive Assistance with School Attendance Fees in this case? A2: If the income of a person responsible for supporting a household decreases as the result of unavoidable circumstances such as illness, injury, or unemployment, this may be taken into consideration even if their income for 2013 exceeded the qualifying amount. In this case, you should consult the school concerned. (In principle, resigning from a job for personal reasons or retirement do not count as unavoidable circumstances.) Q3: I received Assistance with School Attendance Fees when we were a single-parent family, but remarried after it was confirmed that I qualified for this. What happens to the payment of Assistance with School Attendance Fees in this case? A3: If a new member joins your household as a result of remarriage or other circumstances, payment of Assistance with School Attendance Fees will be temporarily halted. If you wish to continue receiving this, you must reapply listing the new composition of your household. Please notify the school immediately if there is a change in your household s situation. Assistance with School Attendance Fees will continue to be paid if your household loses a member as a result of divorce or for some other reason. Printed on recycled paper including recycled pulp
