1 1. Earned Income Please supply the following information for the year ended 5 April 2017: Type of Income 1.1 Salary, wages, bonuses or other type of earnings Send us form P60 from your employer. If you have left your job in the tax year to take up employment elsewhere, your P60 should have details of your previous earnings, in which case tell us the name of your previous employer and send us form P60. If not, send us a copy of form P45 from your old employer. 1.2 Lump sum and compensation payments and any supporting correspondence explaining the makeup of the payment. Please let us know if the whole payment was taxed and included in your P60/P45 figure. 1.3 Benefits in kind, such as company car, medical insurance, expenses Send us form P11D from your employer. 1.4 Share options (for example, Enterprise Management Incentive Schemes) If you have been granted and/or exercised any share options in the tax year let us know and we will contact you to request the further information we require. 1.5 Pension from a former employer or other scheme Send us form P60 or send us a certificate confirming what you received and the tax that was deducted, if any. 1.6 State pension (including widow s benefit) or any relevant documentation from the DSS confirming the amount currently payable. 1.7 State benefits, such as unemployment benefit or any relevant documentation from the DSS e.g. P Self employed income and other earnings such as casual wages, one-off payments, etc. Let us have details of such earnings and any expenses incurred in relation to selfemployment. P60 P45 P11D Certificate of pay, income tax and Employee leaving copy of Return of expenses, payments national insurance contributions employer s certificate and benefits, etc 1
2 2. Investments For each type of investment, tell us if it is held in your name or jointly with someone else. Tell us the total interest on joint accounts. We will assume 50:50 sharing unless you tell us otherwise. Type of Investment 2.1 Rents from UK property including rooms rented out in your main home A list of rents and expenses (e.g. agent s fees, insurance, repairs, mortgage interest) relating to the tax year, whether or not they have been received or paid for during the year. (We do not need details of any items paid or received by you which do not relate to that year.) The property is let furnished*/unfurnished* *(delete as necessary). Unused losses brought forward if known. Holiday lets of UK property, not necessarily in a holiday resort 2.2 Bank or building society interest (excluding ISA, TESSA and PEP investments) These are short lets of less than 31 days. The property must be available for letting for at least 210 days a year and be let for at least 105 days a year. If this applies to you, provide the same information as requested above. EITHER send us certificates from your bank or building society showing the interest credited to your accounts; OR send us a list of the interest received on each account. Send the same information about accounts held by your children, where you provided the funds, if they are under 18 and the accounts earned interest of 100 or more in the year. 2.3 National Savings Send us a list of interest received on each account. 2.4 Shares, government stocks, unit trusts or investment trusts Send us all your dividend vouchers. Also details, and the notification, of any dividends paid as shares so called scrip dividends. Alternatively, send your stockbroker s statements or a list of your dividends. 2.5 Annuities Send us a tax voucher from the person paying you the annuity. 2.6 Trusts and estates EITHER send us form R185 from whoever runs the trust or estate (unless we do the trust accounts); OR tell us who to contact to get the information. 2
3 2.7 Life policies Send us confirmation from the insurance company of any policy you have cashed in, taken money from or given to someone else in the year. 2.8 Overseas (eg, property, bank accounts) and other investment income (eg insurance commission, one-off receipts) Send us a list of each source of income and tax paid. If any foreign tax was paid on your income send us details of this also. If you are in doubt about overseas income follow the instructions for the equivalent UK investment. R185 Tax certificate Beneficiary of trust or estate 3. Other Items Type of information 3.1 Pre-owned Assets Benefits received where you now occupy land/property or use items which you have previously owned and given away or which have been purchased using cash you have previously given away. e.g. You have given away cash to someone who has purchased a property and you now live in that property. Please contact us if you think this applies to you. 3.2 High income child benefit charge (HICBC) Were you, or your spouse or partner entitled to receive child benefit during the year? If yes, how many children have you received child benefit for and what was the total amount received in the year? Please do not hesitate to ask for further details about the HICBC and when it can apply if required. 3.3 Student Loan Let us know if you have a student loan which has not been repaid in full and confirm the amount of repayments made in the tax year. Please let us know if you anticipate your loan will be repaid within 2 years. 3.4 Residency status Has your tax residency status changed during the year? 3
4 4. Capital Gains Tell us about assets you have sold or given away. Tell us if any chargeable assets you ve sold or given away in the year were jointly owned. We will assume a 50:50 sharing unless you tell us otherwise. Type of asset 4.1 Shares, unit trusts, investment trusts or other investments Contract notes for the sale of these investments and details of the cost and date of each acquisition. If you sold shares acquired via a rights issue or sold the rights nil paid, tell us when and how much you received and how much, if anything, you paid for the shares. Alternatively, send us your stockbroker s statements. It is also important that you provide us with details of all acquisitions during the year. 4.2 Property other than your main home whether freehold or leasehold, in the UK or abroad Details of; Sale proceeds Costs of sale (eg. legal or estate agent s fees) Date property purchased Original cost of property Dates and costs of any improvements (eg. extensions) Valuation at March 1982 if held at that date 4.3 Your home If the property was your only or main residence throughout your ownership and the land it included was less than 0.5 hectares (around 1.2 acres), just tell us you ve sold your home. If either of these conditions does not apply, send us details as in 4.2 and a note of the area of land included and/or the periods during which you used the property as your home. Please specify the use of the property during any period when it was not used as your main home. 4.4 Shares in private companies For each disposal of shares (including sale or gift), tell us how much you received and how much the shares were worth at the time if this was different. We shall also need the acquisition details as in Other assets eg, jewellery, paintings, antiques For each item sold or given, tell us when you acquired it and disposed of it and in each case for how much. 4
5 5. Allowances and expenses For each type of expense, follow the instructions in the second column to enable us to work out your allowances. Type of expense 5.1 Employment expenses eg travel, entertaining Send us details of expenses to be claimed against your earnings, including those reimbursed by your employer and shown on form P11D. Also send details of contributions you have made to the cost of benefits provided by your employer and details of subscriptions paid to professional bodies or societies (include the name of the organisation). 5.2 Loans to buy shares in a private company or business interest Send us a note of the lender, the amount of the loan, the date it was taken out and the account number. Tell us if it is held in joint names. Also send us a certificate of interest paid from your lender, together with confirmation of what you used the loan for. 5.3 Pensions, excluding company pension If you contributed to a personal pension or retirement annuity plan or made additional voluntary contributions (AVCs), independently of your employer s pension scheme, send us the certificate from the pension company showing the amounts you paid. Please provide a copy of the pension statement if you believe that your contributions, combined with your employer contributions, exceed 10,000 for the year or for defined benefit schemes the growth in benefits exceed 10, Donations under Gift Aid Please provide details of any payments made to charity under Gift Aid. 5.5 Other allowances and expenses: Investments under Enterprise Investment Scheme or in venture capital trusts If you made an investment under the Enterprise Investment or Seed EIS Scheme or in a venture capital trust send the certificate confirming the investment or a note of the name of the fund/company, the date and amount of the investment. 5
