4 Unsustainable Inflation During the last 25 years, general inflation CPI-U averaged 3.0% while healthcare inflation CPI-M averaged almost 6% 5 years prior to ACA CPI-M inflated at 1.5 times that of CPI-U 5 years right after ACA CPI-M inflated at 1.8 times that of CPI-U Significant increases in premiums since ACA Healthcare Spending as a % of GDP Impact on Wages Rand Study 1999 to 2009 Wage increase after paying for healthcare $95 per month. Part of this is artificial because of the federal debt used to finance healthcare. If taxes had increased to cover cost increases, average wage would have decreased by $295 per month. 4

5 Impact on Deficit 1992 Federal healthcare $297 Billion or 19% of tax revenue 2015 Federal healthcare $936 Billion or 32% of tax revenue From 1992 to 2015 tax revenue doubled and healthcare expenses more than tripled. If healthcare expenses had only doubled, it would have saved the government $342 Billion. (Interest, Education and Energy) Government Spending as a % of GDP Impact on the Economy: Medical costs are the tapeworm of American economic competitiveness Crowding out Full time vs. part time 50 employee barrier International competition Adds almost $1,000 to the price of every U.S. made auto Incentive for outsourcing 5

Increase in the uninsured Shift of more cost to consumers No attention to cost control Federal budget helped but cost shifted to states Payers flee the exchanges? Elderly and poor impacted the most. Remember, expense reduction equals revenue reduction If Interest rates increase healthcare begins the death spiral What Should Physicians Do? Understand the environment Focus on cost and value Plan for the future Watch debt Be part of the solution not the problem Be flexible and agile Prepare for the shift to even more patient involvement 7
