The other is the pride and self-esteem that comes from working to earn an income. The Fundamental Series Disability Income Protection provides quality income protection at an affordable price. It offers income replacement benefits designed to replace lost income during disability. Also included in this policy is a return-to-work assistance benefit which is designed to help you transition back to work. Your Loss of Income - Injury disability policy is guaranteed renewable to age 75. Contract provisions are fully guaranteed to age 75 while premiums may be subject to change. Your policy cannot be singled out individually for a premium change. Any adjustments to premiums will be made for an entire group of policy owners who share common characteristics. Your 24 Hour Loss of Income - Injury coverage is designed for clients who do not have Workers Compensation or similar coverage, and need coverage for work and non-work related disabilities. The Business Overhead Expense - Injury and or Illness policy covers the fixed costs of keeping your business or practice open while you are disabled. Premiums are tax-deductible as a normal business expense and claim benefits, when received, are taxable income to the business. Most common and customary fixed expenses incurred in the operation of your business are covered including: - scheduled instalment payments of principal or debt allocated to business use; - rent, leased and rented equipment, business property liability insurance premiums, dues for professional associations, interest on debt, accounting fees; - other fixed contractual business expenses, which are normal and customary in the operation of the business: wages, fees or other compensation for any employee if that employee is involved in administrative support and is not engaged in any revenue producing or sales generating activities for the business and was continuously employed in the business for a period of not less than six months prior to Your date of Disability. Focus On Ability vs. Disability The Fundamental Series Disability Income Protection offers quality income replacement benefits designed to replace your lost income while you are unable to work as the result of an injury or illness. Also included in this policy is the return-to-work assistance benefit that is designed to help you transition back to work as you recover from your disability. 3 The following is a list of benefits that are included in The Fundamental Series Disability Income Protection package. Income Replacement Benefits Total Disability Totally Disabled or Total Disability means that: a) Due directly to Injury or Illness (if Illness Coverage was purchased), You are unable to perform the important duties of Your Regular Occupation; and b) You are not engaged in any gainful occupation; and c) You are receiving Physician s Care. After Disability benefits have been payable for 36 Months during any period of Disability, then Total Disability means that: a) Due directly to Injury or Illness (if Illness Coverage was purchased), You are unable to engage in any Reasonable occupation for which the You are, or may reasonably become, fitted by education, training or experience; and b) You are receiving Physician s Care. No period of Disability will be considered as due to Injury if it begins more than 120 days after the date of the Accident. The availability of work does not affect the determination of Total Disability. Partial Disability The built-in Partial Disability Benefit is 50% of the Total benefit and may be payable for up to 6 months Partially Disabled or Partial Disability means that: a) You are not Totally Disabled; and b) You are engaged in Your Regular Occupation or any gainful occupation; and c) Due directly to continuing Injury or Illness (if Illness Coverage was purchased), You are unable to perform either: i) One or more important duties of Your Regular Occupation; or ii) The important duties of Your Regular Occupation at least one-half of the time normally required; and d) You are receiving Physician s Care. No period of Disability will be considered as due to Injury if it begins more than 120 days after the date of the Accident. The availability of work does not affect the determination of Partial Disability. Recurrent Disability If after a period of Disability for which We paid You Loss of Income Benefits ends, You become Disabled again within 6 Months from the same or a related medical cause, We will recommence paying You a Loss of Income Benefit from the first day of the recurrence of Your Disability. We will consider this Disability to be a continuation of the previous period of Disability in determining the Maximum Benefit Period. Each period of Disability separated by 6 Months or more will be considered as a separate Disability, even if such Disabilities are due to the same or related causes. 4 Waiver of Premium We recognize the significant strain a disability will have on your finances. In addition to providing a source of income during disability, we will also waive any premiums that become due while you are receiving disability benefits from us. Accidental Death and Dismemberment If you sustain an accidental injury or you die as a result of your injury, you or your beneficiaries receive a lump sum benefit. Focus on Ability vs. Disability Return to Work Assistance Benefit Starting from the first day of injury or illness, we may provide some assistance in returning you to work or enhancing your ability to work. Our assistance may include such services as: - medical investigation and/or treatment; - physical rehabilitation; - psychiatric and/or psychological; - rehabilitation; - job placement; - vocational evaluation; - financial and/or business planning. In addition, we may provide work site modification which could include such things as: - ergonomic furniture and/or equipment; - mobility enhancing equipment; - visual and/or audio equipment If we approve the services prior to the costs being incurred, we may pay some or all of the costs of these services or approved item(s), provided you are not entitled to payment of the costs from any other sources. Integration of benefits To ensure benefits are coordinated with other sources of income received during disability, an integration of benefits provision has been included in this policy. Benefits for disability will be reduced by the sum of the benefits for disability, current earnings, and other income or disability benefits. Other income benefits include disability benefits from provincial or federal government plans such as: CPP/QPP and Workers Compensation legislation or insurance intended to provide similar benefits, as well as automobile insurance, any group or association long-term or short-term disability or weekly indemnity insurance or plans and salary continuation plans. 5 Specific Conditions Limitation Any period of Disability that results, directly or indirectly from Degenerative Disc disease is limited to 20 days and there is a policy lifetime maximum of 120 days. The following conditions are covered by the Fundamental Series policy; however, coverage is limited to the maximum benefit period(s) outlined below: - periods of disability resulting directly or indirectly from a soft tissue injury are not limited; however, there is a cumulative maximum of 36 months for all periods of disability related to soft tissue injuries. Residency Limitation While you reside or travel outside of Canada, the United States, the United Kingdom or Australia, no benefits will be paid and premiums will not be waived for any disability. No benefits will be paid and premiums will not be waived, for any disability or illness that commences while you have been residing or travelling outside of Canada, the United States, the United Kingdom or Australia, for longer than 60 days. Unemployment/Minimal Hours Limitation If you sustain an Injury or have an Illness during any period that you have been unemployed for more than 60 days, Regular Occupation shall be deemed to mean Reasonable Occupation. If you become disabled due to Illness during any period that you have been unemployed for more than 60 days, the maximum benefit period for Total Disability under the Loss of Income Illness Coverage will be 60 Months. Leave of Absence Limitation There are special limitations that apply if at the start of disability, you are on a scheduled leave of absence. In these instances, benefits will be payable providing: - as a result of injury or illness, you are unable to work in a Reasonable Occupation suited to your education, training or experience and prior economic status; and - you are under physician's care and receiving appropriate treatment. However, after the date you are scheduled to return to work, you will be considered disabled if: - as a result of an injury or illness, you are unable to perform the essential duties of your occupation; - you are not working in any gainful occupation; and - you are under physician's care and receiving appropriate treatment. Reasonable Occupation means any occupation in which you could reasonably expect to earn an annual income equal to or greater than the Reasonable Occupation Income shown in the same row of the Benefit Determination Chart that shows Your Maximum Eligible Monthly Benefit. 6 Exclusions We will not cover disabilities that result directly or indirectly from: - sickness or disease, unless you purchased Illness coverage - intoxication by drugs, alcohol or otherwise - abuse of prescription or non-prescription drugs or alcohol, or other substance abuse - a suicide attempt or other intentionally self-inflicted harm - any act of declared on undeclared war, any riot or insurrection or any other form of public disturbance - infections or illness that physicians commonly associate with Acquired Immune Deficiency Syndrome (AIDS) or the Human Immunodeficiency Virus (HIV), if you tested positive for AIDS or HIV before coverage begins - injuries that occur while committing or attempting to commit a crime. We will not pay benefits: - while you are in a jail or otherwise imprisoned - for any disability resulting from a normal pregnancy or childbirth This plan will, however, pay benefits for disabilities caused by complications of pregnancy or childbirth. 30 Day Free Look If the owner is not satisfied for any reason with this policy, it may be cancelled for a full refund if it is returned to us, or the agent through whom it was purchased, within 30 days of its delivery to the Owner.
