4 LG Service Advantage FAQs Questions? Contact your LG Sales Representative. 1. What are the benefits of LG Service Advantage Plans and what do they include? The LG Service Advantage plans are offered as either Extension (EXT) plans or Date of Purchase (DOP) plans. Both plans are inclusive of the items listed below: Mechanical breakdown of the product after expiration of the LG manufacturer s warranty Shipping/logistics of product transportation Toll-free customer support is available 24 x 7 x 364 by calling % coverage We provide 100% parts and labor coverage on all covered repairs. Full-Term coverage LG Service Advantage plans protect your customer s investment for the full-term of the service contract. The plan is fully transferable (Giving it away? Selling it? It s still covered). No Lemon Policy Experience four documented failures on the same component and receive a new replacement product. LG Service Advantage is supported by a national network of LG-authorized technicians. Power Surge Coverage This service plan protects against operational or mechanical failure of a covered product if a failure occurs while properly connected to a surge protector approved by Underwriter s Laboratory. Your customer s surge protector may be collected by Service Net for examination. In addition to the above features, Date of Purchase (DOP) contracts also support: De-installation of defective unit / Reinstallation of operational unit (if purchased) Guaranteed Restore Times 24, 48 & 72 hr. restore options available 2. When does coverage begin? Date of Purchase (DOP) warranties begin on the date the product is purchased and/or installed. Extended (EXT) warranty coverage begins the day after the manufacturer s warranty expires. 3. How quickly can customers expect service? The LG Service Advantage toll-free number is available 24 hours, 7 days a week. For extension contracts (mechanical breakdown coverage begins after expiration of LG manufacturer s warranty), your customer s products are serviced through depot repair. We will send a box to the customer, cover shipping to our depot repair facility and ship the repaired unit to the customer. The average transit time, one-way, is approximately three business days. Upon receipt of the product, average repair time is 3-5 business days pending parts availability. The LG Service Advantage Date of Purchase Plans offer onsite service. We will set up onsite service, dispatch technicians and ship advanced exchange inventory for customers. Regular dispatch business hours are Monday through Friday 7:00 a.m. - 7:00 p.m. local time. 4. 5 LG Service Advantage FAQs Questions? Contact your LG Sales Representative. 5. Are LG Service Advantage plans transferable? Customers may be eligible to transfer an LG Service Advantage plan. Pre-authorization is required. Please refer to the Service Contract Comprehensive Coverage sample at the end of this guide for complete details of the Terms & Conditions. 6. What benefits do your customers receive from LG Service Advantage since the product has a warranty from the manufacturer? LG Service Advantage extends the life of the products; therefore, any costs associated with the repair are covered by the plan, not your customer. 7. How long after hardware purchase can I sell LG Service Advantage? The best time to offer LG Service Advantage is during the quote process; however, you can sell contracts within the first 60 days of product purchase. After 60 days, aftermarket plans may be purchased up to 30 days prior to the expiration of the manufacturer s warranty. Please contact your LG Sales Representative for aftermarket price quotes. 8. What information must I provide LG to activate the customer s Service Advantage contracts? LG requires you to provide the end-customer contact information and equipment serial numbers in order to activate your customer s service contracts. Failure to do this could result in a delay of service. The easiest way to do this is to follow the instructions found on the LG Service Advantage Reseller Order Submission Form and include a copy of the completed form when submitting your purchase order to LG. This form is not required if both the service plans and equipment purchases are part of the same purchase order and directly purchased from LG. 9. When will the customer receive the LG Service Advantage Terms & Conditions? Your customer may receive the LG Service Advantage plan in one of two ways. If a valid address was provided, Service Net will a copy of the contract within 24 hours of receipt of the sale from LG. Additionally, customers may receive the service contract via postal service within business days. The Certificate of Coverage, Terms & Conditions and Serial Number backup file (if applicable) must be stored in a safe place for future assistance with service events. See the sample documents tab for examples. 10. What information is needed to schedule service? The customer will need the serial number (located on the front side of the contract, Certificate of Coverage or Serial Number backup file) ready when calling LG Service Advantage Customer Support. Customers should keep their contracts in a safe place for easy access. Service plans can be cancelled for full refunds within 60 days of purchase, less any claims, taxes or fees paid on the contract. How does your customer request service? If deemed appropriate, we will dispatch a technician for smaller screen sizes. LG Service Advantage provides coverage throughout the lifetime of a product. 16. Who is responsible for the shipping if the product needs to be returned for replacement or repair? LG Service Advantage covers all of the shipping and logistics costs associated with the repair of customer s product. In the event the customer purchased Advanced Exchange coverage, the operational unit will arrive within the designated timeframe as noted in the contract coverage and the defective unit will be shipped in the same box back to the repair facility. 17. What areas are covered? LG Service Advantage provides warranty coverage in the 50 U.S. states (including Hawaii and Alaska) and Puerto Rico. *Limitations and exclusions apply. 32 SERVICE CONTRACT This document sets forth the entire Contract between the Service Contract Administrator hereinafter referred to as We, Us and Our, and, the Purchaser as You and Your. No representation, promise or condition herein shall modify these terms. Service Net Warranty, LLC ( Service Net ) is contractually obligated to You to provide service under this Contract. 1. WHAT IS COVERED. We will furnish labor, parts, and/or replacement equipment (or pay for same) necessary to repair operational or mechanical breakdowns of the Product(s) as originally provided to You and covered by this service plan or provide You a replacement Product or buyout in cases where repair is not possible. The service plan also covers parts and accessories that are necessary for Your Product(s) to function otherwise these parts and accessories are excluded (see below). This Plan covers the de-installation and reinstallation of your Display for service purposes so long as de-installation/reinstallation has been purchased as well as the shipping/transit costs associated with Your Product(s) repair based upon the purchased service level. Power Surge and Spike: This service plan protects against operational failure of a covered Product if a failure occurs while connected to a surge protector accepted by the Underwriter s Laboratory. Your surge protector may be collected by Us for examination. 2. SWAP COVERAGE. Swap coverage is available for LG Displays and other approved components. Swap coverage will provide immediate replacement of eligible product(s) as a result of mechanical breakdown. The replacement product(s) will be managed by You or Us, depending upon the swap coverage services purchased. If managed by You, You must make the swap inventory immediately available upon service provider s arrival onsite for diagnostics and repair. If managed by Us, We will manage the swap inventory and coordinate with the service provider to transport and replace the defective product(s) with operable product(s). 3. DE-INSTALLATION/REINSTALLATION. De-installation/reinstallation is available for LG displays and other approved components. De-installation/reinstallation will provide installation services of an eligible product(s) if de-installation/reinstallation option is purchased. An authorized service technician will arrive onsite within the previously designated response time per service contract service level agreements and will remove the defective product(s) from its location and reinstall an operable product(s) in its place. 4. GUARANTEED RESTORE TIMES. Guaranteed restore times are available for LG displays and below and other approved components. Next business day guaranteed restore times for displays over 42 are subject to size and logistics restrictions and availability and will be determined on a case by case basis. Guaranteed restore times will provide You with a guaranteed arrival time of a service technician to perform the functions listed within your original contract purchase. If you have purchased Swap Coverage, the guaranteed restore time within your service level agreement includes an immediate product(s) restoration. Restoration is defined as immediate replacement of defective product(s) with operable product(s). Guaranteed restore times are defined as: a. Next Business Day Product(s) and/or Service technician will arrive onsite within the next business day if claim approved prior to 3:00 p.m. local time. b. 2nd Business Day Product(s) and/or Service technician will arrive onsite within two business days if claim approved prior to 3:00 p.m. local time. c. Third Business Day Product(s) and/or Service technician will arrive onsite within three business days if claim approved prior to 3:00 p.m. local time. Guaranteed Restore Times may be altered at which time notification will be sent to You. Alterations to the time could be caused by unforeseen issues such as technician unavailability, environmental or natural disasters, lockouts, strikes or other labor issues with any service or logistics partners, acts of God, acts of war. 5. TERM OF SERVICE PLAN. You have coverage under Your service plan during the time listed on the front of this Service Contract. All plan coverage commences on the original product(s) installation date and will expire on the date notated on your purchase receipt and/or your fulfillment material. This plan is inclusive of the base warranty; it does not replace the base warranty, but provides certain optional benefits during the term of the base warranty. After the base warranty expires, this Plan continues to provide the base benefits, as well as certain optional benefits listed within the Plan s Terms & Conditions. 6. PAYMENT TERMS. The purchase price for Your Service Contract is due in full before Your coverage under Your service plan will begin. If You do not pay the full amount, We, at Our sole option, may collect any unpaid portion of the purchase price prior to providing You coverage under the service plan covered by this Service Contract, unless otherwise specified. 7. TO OBTAIN AUTHORIZATION FOR SERVICE. Call the LG Commercial Display Product Support toll-free number listed on the front of this Service Contract. Have this Service Contract, model number(s) and serial number(s) of Your Product(s), and the original Product(s) receipt available. 8. ACCESSIBILITY OF PRODUCT. If onsite service is required, You agree to make the Product(s) reasonably accessible to the repair person. Any service fees incurred by Us will be applied to the maximum liability of this Contract. 9. LIMIT OF LIABILITY. This Service Contract will expire once the expiration date of Your Service Contract has been reached. 10. IMPORTANT NOTE. Repairs recommended by the repairing facility not necessitated by mechanical breakdown are not covered unless specifically authorized by Us. We reserve the right to inspect the items to be covered, or the items covered, as the case may be, prior to coverage or during SAMPLE the coverage period. Model number, serial number and original date of installation of all Products to be covered must be provided to execute application for service. If You request a service call for a noncovered repair, You will be responsible for all costs associated with the repair. If the Product(s) is found to be performing to the manufacturer s specifications, it will be returned to You. See Service and Place of Service for requirements of Your obligations under this Service Contract regarding swap inventory and scheduling of service. 11. TIME FOR SERVICE. Service will be performed during the hours of 7:00 a.m. to 7:00 p.m. local time Monday through Friday, excluding holidays, unless otherwise specified. Any additional costs above the service providers authorized hourly rate (premium or overtime charges) or after hours service which have not been previously authorized will be at Your expense with exception of health related or severe weather related emergencies. 12. SERVICE AND PLACE OF SERVICE. To request service, please provide the following: (i) LG model number(s), (ii) LG serial number(s), (iii) installation date (copy of installation order or purchase receipt may be required), (iv) contents or components outside of the core product(s) (v) full address and contact information for the property where the defective product(s) is located, and (vi) any special requirements or setup pertinent to execution of service call (i.e. two-person service call, product installed in high or not easily accessible locations, etc.) Some calls will include a fault diagnosis to clarify the problem prior to any service. Service will be provided by a manufacturer-authorized service center. If you purchased or are otherwise entitled to onsite service but, due to environmental and/or technical requirements certain repairs cannot be completed at Your location, the cost to transport and/ or ship Your Product for service will be covered under Your service plan. Guaranteed Restore Times and De-Installation/Reinstallation service costs of installed product(s) will be covered if specific services are purchased per your service agreement. If your contract includes a Guaranteed Restore Time, you must be able to receive the necessary swap inventory during regular business hours, unless otherwise specified, and secure inventory until the service technician arrives. In order to comply with contracted service levels, certain remote or non-standard installations may require prior review. Location will be required to provide a lift truck onsite should installation exceed more than nine (9) feet. 13. PARTS AND SUBCONTRACTING. Parts used to repair equipment may either be new or refurbished at Our sole option. Service may be performed by subcontractors. 14. UNABLE TO REPAIR. If We determine that We are unable to repair Your Product(s) due to the unavailability of functional parts, service or technical information, We will resolve the claim, at Our discretion. Service Net will make every reasonable effort to replace defective product(s) with identical model(s); however, Service Net reserves the right to replace with comparable specifications. In the event identical model(s) with comparable specifications is unavailable, You will receive reimbursement of the original purchase price You paid for Your Product(s) under this service contract All Our contractual obligations to You under this Service Contract are fulfilled upon expiration of the term of Your Service Contract. Upon replacement or reimbursement of Your Product(s) original purchase price minus sales tax and claims already paid under the conditions set forth above, the original product(s) and all originally included components become Our property. 15. LIMITATIONS OF COVERAGE THIS CONTRACT DOES NOT COVER: A. Any Product located outside the continental United States, Alaska, Hawaii and Puerto Rico. B. Software and components not previously authorized in original contract purchase. C. Service required as a result of any alteration of the equipment, or repairs made by anyone other than a participating servicing dealer, an authorized service provider, its agents, distributors, contractors or licensees, or the use of supplies other than those recommended by the manufacturer. D. Damage or other equipment failure due to causes beyond Our control including, but not limited to, repairs necessary due to operator negligence, the failure to maintain the equipment according to the owner s manual instructions, abuse, vandalism, theft, fire, flood, wind, freezing, power failure, inadequate power supply, unusual atmospheric conditions, animal or insect damage, acts of war or acts of God E. Any utilization of equipment that is inconsistent with either the design of the equipment or the way the manufacturer intended the equipment to be used. Any installation that prevents normal service. Any and all cases in which the manufacturer of the equipment would not honor any warranty regarding the equipment. F. Non-functional parts such as, but not limited to, plastics, finishes, porcelain or enamel parts, knobs and dials, handles (unless critical to the function of the Product), trim, accessory items such as water or electrical connections and venting equipment and decals. Expendable or lost items, such as, but not limited to ear buds or head phones. G. Consumable items are defined as any part that is considered consumable by the manufacturer or any item that is designed to be consumed (wear out) during the life of the Product, regardless if it is consumer replaceable or not. Consumable items include, but are not limited to: remote control, batteries, mounts or other hardware used in the installation of the product, telephone, cabling or other lines connecting to the equipment. H. In-warranty parts not provided or shipped by the manufacturer. Operational or mechanical failure covered by manufacturer s warranty, manufacturer s recall, improper construction, or factory bulletins, (regardless of whether or not the manufacturer is doing business as an ongoing enterprise). I. Consequential damages as a result of malfunctioning of or damage to an operating part of the covered equipment, or damages as a result of any repairs or replacements under this agreement. Damages caused by delays in rendering service or loss of use during the period that the Product is at the authorized service center or otherwise awaiting parts are not covered. You are responsible for creating back-ups of all Your data and software on a regular basis. J. Damage or failure caused by animals or insects. K. Damage or failure caused by bodily fluids, including but not limited to urine and vomit. L. Operational or mechanical failure which is not reported prior to expiration of this Contract or within 30 days of Product failure. M. Normal, periodic or preventative maintenance and/or checkups, including but not limited to customer education, adjustments, cleanings, and convergence. N. Loss or damage as a result of violation of existing federal, state or municipal codes including repairs to Products not complying with said codes. O. Pre-existing conditions (incurred prior to the effective date of coverage), known to You. This includes situations where the Product was not taken out of the box or utilized prior to manufacturer warranty expiration and a failure is discovered upon removal or use during Our coverage. P. Equipment where the serial plate attached to the equipment is removed, defaced or made illegible. Q. Display screen imperfections, including normal wear and tear or cosmetic damage, markings or retained images on glass panel resulting from viewing fixed images (including, among other things, non-expanded standard 4:3 pictures on wide screen TVs, video game images, or onscreen data in stationary and fixed location). R. Damage resulting from unauthorized repair; software virus; or electrical wiring and connections; damage caused during delivery or removal, improper installation, or setup including, but not limited to packing, unpacking or assembly, user-facilitated minor adjustments and settings outlined in the Product s owner s manual, external antenna or local reception problems, inaccessible products or parts, negligence, misuse or abuse whether willful or not. S. Non-failure problems that do not require parts and intermittent issues. Subsequent trip charges may need to be paid by You if a second no failure found diagnosis is determined based on the same problem. T. De-installation/reinstallation of Your Product(s) unless a de-installation/reinstallation service option is purchased. U. Misuse, abuse, rentals, unauthorized repairs and/or parts by others, parts designed for replacement during the life of the Product such as bulbs and filters, squeaking or other noises. V. Coverage only applies to the repair or replacement of a covered unit unless the covered unit is part of a custom installation Contract. Repair of resolution (pixels) failure must match the factory minimum standards of failed pixels before an authorized repair occurs. Sealed system failures are considered a manufacturer s defect and are not covered under this program. Installation/de-installation of Plasma Televisions is not covered, unless de-installation/reinstallation service option is purchased. W. Products sold second hand including but not limited to floor models (unless covered by a full manufacturer s warranty on your date of installation) and demonstration models. X. Any cost recoverable under any other warranty, guarantee, or under an insurance policy (in such case, this Contract will cover any applicable deductible). Y. Damage which is not reported prior to the expiration of this Contract. 16. NO LEMON GUARANTEE. During the term of this Contract, when three service repairs, with three separate claim numbers, have been completed on the same part, and that same part requires repair under a fourth claim number, as determined by Us, Your Product will be replaced with a Product with comparable specifications by Us, not to exceed the original retail purchase price. In the event a comparable replacement cannot be located, a buyout, not to exceed the current market value of a Product with comparable specifications, will be provided. This does not include repairs necessary during the manufacturer s warranty period, rework/callback service required after initial service, during the warranty of work period provided by the Service Company, or previous service Contract terms. Once you have received Your Product replacement or buyout all contractual obligations under this Contract have been fulfilled 17. DEDUCTIBLE. No deductible applies to this Contract. 18. RENEWABILITY. This Contract may be renewable. Please contact LG Commercial Display Product Support for further information. 19. CANCELLATION AND REFUND. You may cancel this Contract at any time for any reason. If You cancel this Contract within sixty (60) days of the date of installation You will receive a refund of the full purchase price less any claims paid. If You cancel this Contract thereafter, You will receive a prorated refund of Your purchase price less any claims paid (unless otherwise noted on the front of this Service Contract.) To cancel your contract, you must submit written notification, including business name, contact name, contact number, contact address, contract number(s) and serial number(s) to: Service Net, c/o LG Commercial Display Cancellations, 650 Missouri Avenue, Jeffersonville, IN STATE VARIATIONS. Certain states have specific conditions; conditions listed on the front of this form may apply to You. 21. TRANSFERABILITY. You may be eligible to transfer this Agreement to any person. If Service Net assigns another insurance carrier with an AM Best industry rating of A- or better under this Contract directly or indirectly, such new insurance carrier will carry the liability under this Contract. 23. RIGHT TO RECOVER FROM OTHERS. If We make any payment, We are entitled to recover what We paid from other parties. By accepting settlement of a claim, You transfer to Us Your right to recovery against any other party. 24. COVERAGE AND TERM. This is not an insurance policy. As the Administrator, We will assist You in understanding Your warranty and coverage benefits from the day Your Plan becomes active. If Your Product needs repair for operational or mechanical failure, You are required to call the toll free number listed on the front of this Contract or submit Your claim in writing to LG Commercial Display Product Support c/o Service Net, 650 Missouri Ave., Jeffersonville, IN With any correspondence, please provide Your daytime phone number and claim number if applicable. The expiration date and price of this Contract are listed on the face of this Contract. There are some limitations of coverage. You should review the limitations of coverage paragraph for details.
