2 Accidental Death & Dismemberment A Direct Response Insurance Program Administered by Affinion Benefits Group Our Accidental Death & Dismemberment (AD&D) plan is a Customer Appreciation program designed to strengthen ties with your customers and to generate incremental revenue. The program offers $1,000 of AD&D insurance at no cost to your customers you pay the premium as well as the opportunity to purchase additional coverage at affordable group rates. The program is marketed with a tried-and-true direct mail solicitation program, including, over time, the implementation of multiple acquisition, cross-sell and upsell direct response events. PRODUCT SUMMARY AND OFFER AD&D insurance covers your customers in the event of loss of life, limb, sight, speech, or hearing, due to a covered accident. No one can be turned down for coverage. Because no underwriting is involved, physical examinations and lengthy application forms are not required. The AD&D program offers your customers the opportunity to receive $1,000 of AD&D insurance at no cost as long as they maintain their account with you. Also, they may purchase up to $300,000 of additional insurance at affordable group rates. Customers may choose individual or family coverage. CALL TOLL FREE The mailing refers customers to this toll-free number if they have any questions about the insurance program, want to cancel coverage, or need to file a claim. They may call weekdays, 7:00 a.m. to 8:00 p.m., Saturdays, 8:30 a.m. to 5:00 p.m., CST. If you have a question, please call and identify yourself as a financial institution employee and ask to speak to a representative about the Customer Appreciation AD&D program.

3 Program Facts PROGRAM OVERVIEW This direct mail customer appreciation promotion offers your customers $1,000 Accidental Death & Dismemberment insurance (AD&D) at no cost to them. You pay the premiums. This complimentary coverage is available to all eligible account holders age 18 or older. (The $1,000 benefit reduces to $500 at age 70 or older.) We will pay the premiums for this coverage as long as the customer maintains their account with your financial institution and for as long as you sponsor the program. In addition to the $1,000 no-cost offer, customerss are eligible to purchase from $10,000 to $300,000 of additional AD&D coverage at low group rates. Family coverage is also available which pays a percentage of the additional coverage amount to the spouse and dependent children of the insured. HOW THE CUSTOMER BECOMES INSURED There are no medical exams or lengthy health questionnaires to qualify for this insurance. In order to activate this insurance coverage, the customer must simply complete the enrollment form included in their package and send it to the Plan Administrator before the deadline indicated. Once enrolled, customers will receive a personalized Insurance Coverage Document in the mail. HOW PREMIUMS ARE PAID Again, there is no cost to the insured for the first $1,000 of coverage. If additional coverage is selected, the premiums will be automatically collected as follows: Checking customers: the premiums will be electronically debited from the checking account quarterly Mortgage customers: the premiums will be electronically added to the monthly mortgage payment Credit Card customers: the premiums will be electronically charged to the credit card account quarterly The premium amount and toll-free number for service and inquiries will be shown on customers statements. 4 Accidental Death & Dismemberment Quick Facts PRODUCT DESCRIPTION Covers insured in the event of loss of life, limb, sight, speech or hearing due to a covered accident ACCOUNT TYPES DDA, Credit Card, Mortgage ELIGIBILITY Insured and spouse: 18 and older Dependent children: under 19, or under 23 if enrolled in accredited institution of higher learning No termination age; however, coverage reduces 50% at age 70 or older FAMILY COVERAGE Spouse: Spouse with children 50% supplemental coverage amount; spouse without children 60% supplemental coverage amount Child(ren): Each dependent child 20% supplemental coverage amount; each dependent child of a single parent 25% supplemental coverage amount ITEMIZED COVERAGE Loss of Life 100% of benefit; Loss of Both Hands or Both Feet or Entire Sight of Both Eyes 100%; Loss of Speech and Hearing 100%; Loss of One Hand and One Foot 100%; Loss of Either Hand or Foot and Entire Sight of One Eye 100%; Loss of Entire Sight in One Eye 50%; Loss of Either Hand or Foot 50%; Loss of Speech or Hearing 50%; Loss of Thumb and Index Finger on Same Hand 25% ADDITIONAL FEATURES Anti-Inflation Benefit: For every two years Additional Coverage remains continuous, it will be increased by 5%. This provision can continue to increase coverage until it reaches 125% of the original amount. Common Carrier Accidental Death: Provides benefits for accidental loss of life from a covered injury while traveling as a fare-paying passenger in or on a public conveyance operated by a licensed common carrier. Benefit is two times the amount of supplemental coverage. Educational Assistance Benefit (available with Family Coverage): If primary insured dies from a covered accident, this benefit pays an educational benefit for each dependent child enrolled as full-time student in an institution of higher education on the date of your death. (Benefits will also be paid for dependent children who are in 12th grade on date of your death but who enroll in an institution of higher education within 1 year.) The benefit for each qualifying student is 2% of supplemental coverage paid for each year the student qualifies, up to a maximum of 4 years. If no children eligible, an additional 2% of the coverage will be paid to beneficiary. Adaptive Home and Vehicle Benefit: Provides coverage for a loss (other than of life) under the AD&D benefit, the plan will pay a benefit of 2% of Additional Coverage, the expense incurred, or $2,500 (whichever is less) for the one time cost of adapting (under defined circumstances) the covered person s home to make it more accessible and/or vehicle to make it more drivable or ridable. 5 Customer FAQ Q. Do I automatically receive the $1,000 no-cost basic insurance? A. No. You must complete and return the enrollment form. Q. Do I have to buy additional coverage in order to get the $1,000 basic? A. No. The $1,000 AD&D insurance coverage is provided as our gift to you, at no cost, as long as you maintain your account with us. Q. Does this cover on-the-job accidents? A. Yes. The coverage is effective 24 hours a day, worldwide. Q. How much additional coverage may I choose? Can I get insurance for my spouse and family, too? A. AD&D insurance has coverage levels to fit your budget, from $10,000 to $300,000. Spouses and unmarried children under the age of 19 years (age 23 if full-time students) may be insured under the family plan. If the family plan is selected, spouses are insured for 50%. Dependent children are each insured for 20%. If your household does not have any dependent children, the spouse s coverage rises to 60% of the additional coverage selected. If you do not have a spouse, dependent children s coverage rises to 25%. Q. Do I have to take a medical exam or answer a lot of health questions? A. No. There are no medical questions or physical exams required to qualify. Q. I bought additional coverage, and I received something called added benefits. What does this mean? A. You automatically receive four added benefits that are included with your insurance. (1) Common Carrier Accidental Death: Pays twice the face value of your additional coverage. (2) Educational Assistance: Pays a benefit to each dependent child who is enrolled as a full-time student in an institution of higher learning. (3) Anti-Inflation: After two years of continuous coverage, benefit increases a specific percentage until it reaches 125% of the original amount. (4) Adaptive Home and Vehicle: Provides coverage for a loss (other than of life) under the AD&D benefit, the plan will pay a benefit of 2% of Additional Coverage, the expense incurred, or $2,500 (whichever is less) for the one time cost of adapting (under defined circumstances) the covered person s home to make it more accessible and/or vehicle to make it more drivable or ridable. Your Insurance Coverage Document will explain all of the specifics of these added benefits (including exclusions, limitations and terms of coverage). Q. Is there an age cutoff? A. There is no termination age; however, coverage reduces 50% at age 70 and older. Q. When does my coverage begin? A. The effective date is shown on your Insurance Coverage Document. Q. What does common carrier mean? A. The common carrier benefit applies when you pay a fare to travel on common or public forms of transportation. This includes commercial airlines, trains, buses, cabs, ferries, and more. Q. How long does it take before I receive my insurance coverage document? A. It takes about 30 days from the time the enrollment form is received to process and issue the Insurance Coverage Document. Q. What if I decide to cancel? A. Your coverage features a no-risk 30-day guarantee. If you choose not to keep your insurance, simply call toll free , and your insurance will be cancelled immediately. Q. How will I be billed? A. 1) Because you are a checking customer, the premiums will be automatically debited from your account quarterly. This transaction will be reflected on your statement. 2) Because you are a credit card customer, the premiums will be charged to your account quarterly. This transaction will be reflected on your statement. 3) Because you are a mortgage customer, the premiums will be added to your monthly mortgage payment. Q. How do I file a claim? A. Call toll free at weekdays, 7:00 a.m. to 8:00 p.m., Saturdays, 8:30 a.m. to 5:00 p.m., CST. Q. How long will it take to process my claim? A. The claims are processed within five days of the Plan Administrator receiving the appropriate information and documentation in their offices in Nashville, TN. Once processed, the claim is then forwarded to the underwriter for determination of benefits. Q. Who is the plan administrator? A. The Plan Administrator is Affinion Benefits Group, a division of Affinion Group, the nation s largest provider of direct response insurance programs. What happens if your customer asks a question, and you are unsure of the answer? Call! Trained representatives are available to assist both employees and customers.
