4 We are committed to: Dedicated Customer Service No phone tree! When you call Flores, a live voice will answer your call. Each employer is assigned a dedicated account manager. Your dedicated account manager is Jeff Whittaker. Innovative Technology Participant website, Downloadable app, E-Receipt SMS/IVR communication tools

5 After you enroll Enroll in FSA during Open Enrollment. Prior to 1/1/2018, you will receive a Participant ID Notice via or mail. Save your notice! You ll use this to create a log-in to our web portal, flores247.com where you can manage your account. If you enroll in the Medical FSA, you will also receive a Flores Benefits Mastercard. Cards will be loaded with your entire Medical FSA election and you can begin using it on your benefit eligibility date. Your card will be valid for several years, so please do not destroy your card after your balance is spent. When you re-enroll the next year, the same card will be reloaded. If you need extra cards, you can order them on flores247.com under the Maintenance tab. Save receipts for services purchased with the debit card in case you receive a debit card documentation request.

6 "No Wait" Dependent Care Process If you enroll in Dependent Care FSA, you can take advantage of our no-wait dependent care claim process and receive automatic reimbursements for dependent care expenses every pay period! Why is it called No-Wait? If you file a claim for your yearly dependent care expenses at the beginning of the year, each pay period your reimbursement will be issued to you automatically, meaning you don t have to wait to see your money hit your account!

7 Steps to file a claim 1. Download Go to the "Document Library" after logging in to to download a Dependent Care claim form or obtain this form from your HR department. 2. Complete Complete the claim form with the service dates, dependent name(s), the service provider's name and 9 digit Tax-ID number. 3. Substantiate Submit either an itemized statement or have your provider sign in the "Provider Signature" box on the form to verify you will incur the amount claimed. 4. Submit Submit your claim via upload to or the E-Receipt app, fax, or mail.

9 2018 FSA Plan Details

10 Each pay period, your contribution to your account is deducted from 40% your paycheck before taxes and deposited into your FSA. A Flexible Spending Account (FSA) allows you save between 25 and 40 percent on eligible medical and dependent care expenses.

11 Medical FSA Plan Basics You can contribute up to $2,650 to a Medical FSA for 2018 to pay for your eligible healthcare expenses, as well as expenses for your spouse and dependents. Services must take place between 1/1/2018 and 12/31/2018*. Eligible expenses include**: Medical: Copays, deductibles, and co-insurance Prescriptions Vision: Exams, glasses, contacts, Lasik Orthodontia payments Dental: Cleanings, fillings, and crowns Prescribed OTC medications Medical supplies *If you enroll in FSA after 1/1/2018, your medical FSA balance will be available on your benefit effective date. **If you choose to enroll in a qualified High Deductible Health Plan with an HSA, you are not eligible to participate in a Medical FSA plan.

12 Access your balance Reimbursements are issued by check, direct deposit, or by using the Flores Benefits Mastercard. The entire Medical FSA balance is available on 1/1/2018. Claims deadline The IRS stipulates that the FSA is a use-itor-lose-it account in exchange for allowing you to access the entire FSA balance before all funds have been contributed. If you have not spent your full balance by the end of the plan year and are enrolled on 12/31/2018, you may rollover up to $500 to the next plan year's balance. Contributions remaining in excess of $500 will be forfeited. All claims must be submitted by March 31, 2019 in order to be reimbursed.

13 Dependent Care Basics You can contribute up to $5000 to a Dependent Care FSA for You can use these funds for eligible childcare expenses for any child(ren) under the age of 13 so that you, and your spouse if married, may go to work. Eligible expenses include: Daycare or preschool Before and after school care Day camps Ineligible expenses include: Overnight camps Tuition for kindergarten and above Educational expenses Fees not applied toward the care of the child All claims must be submitted by March 31, 2019 in order to be reimbursed.
