3 IDENTITY CAMP POLICY ADMISSION Identity Camp is a Christian camp open to students entering the 6th-8th grades. CANCELLATION/REFUND POLICY FULL REFUND Written notice of cancellation received a minimum of 14 days prior to the first day of camp PARTIAL REFUND Written notice of cancellation minimum of seven (7) days prior to first day of camp Doctors note for any illness or injury seven (7) days prior to first day of camp (50% refund) Illness or Injury occurring at camp, refunds will be prorated but will not exceed 50% of registration fee. Written request required. NO REFUND Cancellations occurring six (6) days or less before first day of camp Disciplinary action or homesickness that results in a student being sent home Cell Phones Cell phones are not permitted at Identity Camp. Parents may leave a message with a staff member and they will be contacted at their earliest convenience. Cell phones discovered on students will be taken. *Identity camp staff members are not responsible for lost, stolen or damaged phones. DISCIPLINE POLICY Any behavior deemed excessive may result in the student being asked to leave camp. Parents will be required to pick the student up from the campsite. No refunds will be given if a student is removed from the camp for disciplinary reasons. REGISTRATION COST The entire balance of the registration cost is due when registering your student(s). No refund will be given if the student leaves camp due to homesickness, discipline, family vacations, etc. SPENDING MONEY Students will have the opportunity to purchase soft drinks and candy if they choose.
4 WHAT TO BRING Bed linens for twin bed (include pillow) or sleeping bag Toiletries (soap, shampoo, deodorant) Towels Insect repellent Sunscreen Bible, notebook, pen Clothes for 5 days* Bathing suit Ladies (1 piece) Guys (no speedos) Light jacket or sweatshirt Tennis shoes & flip flops *Clothing Shorts, jeans, and t-shirts are best) Female Students -- Allow your arms to hang to your side, your shorts or skirts must reach your fingertips. No spaghetti strap shirts, or garments that leave a midriff or cleavage exposed, including see-through garments or t-shirts with the sides cut will be allowed. Parents will be contacted for inappropriate clothing. WHAT NOT TO BRING Cell phones Matches, lighters Tobacco products Alcohol, illegal drugs IPads, ipods (all electronics) Energy drinks Leave all valuables at home. There is no place to store them safely throughout the week. FIRST AID The Identity camp nurse will treat students with minor injuries. In the event of more serious injury or condition, students will be transported to the nearest emergency room for treatment and parents will be notified. MEDICATION Any student requiring medication while at camp MUST complete the Authorization for the Administration of Prescription Medication form. ALL medication, prescription or over-the counter (Tylenol, Benadryl, etc.) MUST be turned in to the camp nurse. Parents will be contacted for medication found in a student s possession. 5 Church of the King Identity Camp 2016 June LIABILITY RELEASE AND MEDICAL CONSENT FORM LIABILITY RELEASE The undersigned individual and, where applicable, the minor or child described below over whom the undersigned individual represents that he/she has legal authority (collectively Participants ) enter into this Camp Agreement ( Agreement ) with Church of the King, Incorporated ( COTK ) in the context of a church camp sponsored by COTK ( Camp ). Participants participation in activities related to Camp, including but not limited to sporting events and activities; physical exercise; outdoor and other recreational activities and games; physically challenging activities of various kinds; use of various types of equipment, sporting equipment, devices, and/or dangerous or potentially dangerous things; transportation to and/or from Camp and/or any and all activities arising from or related to participation in Camp; and all other activities, whether active or passive, arising from or related to Camp, including but not limited to merely being present (collectively Activities ) is conditioned upon and in consideration of acceptance of the terms of this Agreement. Participants understand and agree that participation in Activities is strictly voluntary, that participation in Activities may be dangerous or inherently dangerous, that bodily injury up to and including serious bodily injury and even death may result from participation in Activities, and that participation in Activities is at Participants own risk. Participants understand and agree that it is a part of Camp for COTK and COTK s agents, employees, volunteers, affiliates, directors, officers, assigns, successors, and licensees (collectively Released Parties ) to record audio and/or video and/or photographic footage ( Footage ) of Participants during Camp, while Participants are participating in Activities, and/or during interviews during which Participants may be invited to provide comments, testimonies, a performance, creative material, singing, or other contributions to the Footage. COTK and/or any of the Released Parties may use Footage by making edits, alterations, adaptations, enhancements, and synchronizations (collectively Edits ) to it; by broadcasting it before a group or audience of people; by publishing or broadcasting it over the Internet in any way; by creating CDs, DVDs, and/or any other copies of it in any format and/or in any medium, which may be given away or provided for free for any purpose and/or may be sold for a fee or price collected by COTK and/or any of the Released Parties (collectively Uses ). Participants consent and agree to the taking of the Footage, consent and agree to all of the Uses of the Footage, and consent and agree that COTK shall exclusively own all worldwide right, title, and interest in and to the Footage, the Edits, and the Uses and all results thereof, and further assign to COTK all right, title, and interest that Participants may have in and/or to the Footage, the Edits, and the Uses and all results thereof. Participants shall never be entitled to any royalties, payments, compensation, remuneration, benefits, or any other consideration of any kind arising from or relating to the Footage, the Edits, or the Uses or any results thereof. Participants hereby and forever waive and release the Released Parties from any and all damages, injuries, causes of action, fees, attorney fees, losses, claims, demands, and suits whatsoever arising from or relating to Camp, Activities, Footage, Edits, and Uses and the results thereof, as each defined in this Agreement. Participation Agreement Person & Property Church of the King ( COTK ) welcomes the participation of: ( Participant ) for the following COTK event/s, function/s, or activity/ies: ( Event ) taking place on the following date/s:. This Participation Agreement Person & Property ( Agreement ) is an agreement between COTK and Participant. Participation in association with the Event is voluntary, may be discontinued at any time for any or no reason, and is at Participant s own risk. Participation might cause or provide occasion for serious bodily injury or even death. For example, participation may involve physical activity that is strenuous or dangerous to Participant, or the possibility that other persons may accidentally or intentionally cause harm to Participant that COTK does not or cannot prevent. Participant (or the undersigned) promises to COTK that to the best of Participant s knowledge (or that of the undersigned), after reasonable investigation, Participant does not have any medical, physical, emotional, or psychological condition that could reasonably be anticipated to hinder Participant s ability to participate in association with the Event in a way that is safe for Participant and other participants as well. Participation may involve presence around or use of various kinds of property that might cause or provide occasion for serious bodily injury or even death. Participant takes all property, such as buildings, land, personal/movable property, and vehicles, that is owned, operated, controlled, or supervised by COTK as Participant finds it, as is, with all known and unknown flaws, defects, hazards, and dangers. Participant is free to provide Participant s own transportation, and assumes all risks of traveling in association with the Event, such as the risks associated with traveling with COTK employees or volunteers or other participants, and traveling in vehicles owned or operated by COTK employees or volunteers or other participants. During participation in association with the Event, a medical or health condition, need, or emergency might arise that may suggest or require an appropriate response in order to avoid possible harm or increased harm to Participant ( Medical Situation ). Participant or a person apparently acting on behalf or for the benefit of Participant may also provide COTK or its officers, directors, employees, volunteers, agents, mandataries, or affiliates (collectively Workers ) with instructions, devices, procedures, or recommendations to be used in the event of some Medical Situation. Participant authorizes the Workers to take any and all reasonable actions that they in good faith deem necessary or beneficial to Participant under the circumstances of any Medical Situation, and also to decline to personally take action in response to any Medical Situation. Any actions taken by the Workers for the benefit of Participant will be, at COTK s election, at Participant s cost. Participation may involve the possibility that property owned, controlled, or used by Participant could be damaged, lost, or stolen in association with the Event. COTK assumes no responsibility for any such property. Participant releases and indemnifies COTK and all entities related to or affiliated with COTK, together with all of their respective officers, directors, shareholders, employees, volunteers, agents, mandataries, assigns, licensees, attorneys, and affiliates from any and all damages, injuries, liabilities, causes of action, fees, losses, claims, demands, attorney s fees, court costs, and suits whatsoever arising from or relating to any personal injury, death, or property damage that may occur in association with Participant s participation in the Event, and any breach of this Agreement by Participant or the undersigned. Participant agrees that any disputes about this Agreement or participation in the Event will be resolved only in accordance with biblical principles. Disputes will be resolved within the church and not through lawsuits in the courts according to 1 Corinthians 6:1-11, and in private, then with one or two witnesses if necessary according to Matthew 18: If Participant is a minor or child under the age of 18, all persons with legal authority over Participant concerning the matters discussed in this Agreement, such as Participant s parents, guardians, or tutors, must sign this Agreement on behalf of Participant. Persons signing for Participant promise that they have full legal authority to enter into this Agreement on behalf of Participant without need for any additional authorizations beyond those of the undersigned. 11 FAQ ' S What Should I Bring? There is an extensive list of what to bring on pages 3-4 of the Camp Policy. Please make sure to bring bed linens for a twin bed or a sleeping bag, and a pillow. One-piece swimsuits are required for ladies and speedos are prohibited for men. Leave all valuables at home and bring a little spending money if you want extra snacks/soft drinks. When is the Registration Fee Due? The entire balance of the registration cost is due upon registration of student(s). Please see Camp Policy page 2 for more information. Can My Student Take Their Cell Phone? No cell phones are permitted at camp. Parents may leave a message with a staff member in case of emergency. Any cell phones discovered on students will be confiscated. Please see Camp Policy page 2 for more information. What About My Child's Medication? All students requiring medication MUST fill out the Authorization for the Administration of Prescription Medication form and turn all medication in to the camp nurse. Please see Camp Policy page 4 for more details.
