Return of Premium Rider Need: Designation of Beneficiary Form to designate anyone other than the Estate and in some states, the additional Coverage Selection page is required. 10 year or Decreasing after age 65 options available at an additional cost. If no contingent selections were made on the application, the policy for the approved applicant will be issued with half of the benefit multiplier submitted for the shared plan. When premium is submitted, the modal premium for the approved applicant will be applied and the balance of the overpayment from the declined applicant will be refunded. In the event the next premium is due based on the policy effective date, we will retain this premium rather than refunding. If one applicant is declined and one is approved, the policy will be issued based on the contingent benefits reflected on the Coverage Selection page. The second applicant names the first insured at time of application and the first insured submits a Coverage Change Request (form provided by the company). The second policy will be issued with Shared Rider and the first policy will have Shared Rider added. The first policy will be reissued with a current effective date to match the second policy. Note: Both applicants must select identical benefits to be eligible for Shared Rider. Customer Service Address Change Return Mail EFT/Bank Changes Refunds Can be taken over the phone, written request or web service. If notified by Post Office via return mail that a policyholder's or Third Party Designee's address has changed, we will update address and resend the item. Note: If return mail is received with no updated address label for the policyholder, we will research to locate an address and resend the item to the updated address. If return mail is received with no updated address label for the Third Party Designee, we will send a letter to the policyholder requesting the updated information. If already on EFT, to change draft date, call or send a CCR. EFT draft dates cannot be set between the 28th and 31st of the month. To establish EFT or change banking information, submit an updated EFT form 5 days prior to the next scheduled draft. You may also call Customer Service or visit our website to update the information. Note: EFT can only be from a checking account, NOT a savings or investment account. Premiums will be drafted from the account one month after the premium due date. When processing refunds for ANY reason, refunds will be held for 21 days from premium receipt. Third Party Notification Waiver of Premium 1035 Exchanges Ability to add or delete a third party, an insured can have up to four (4) third party notifiers. Policies are put on waiver only at the direction of the Claims Department. If Tax Qualified, Refund of premium occurs only if full modal premium, any excess is held until waiver ends.
