1 Page 1 Dear Participant, We are pleased to announce the 2018 updates to the Oregon Homecare Workers Benefit Trust and the Oregon Homecare Workers Supplemental Trust-Approved by that Trusts Board of Trustees. Please note that all benefits listed below are only provided to eligible Participants for as long as they are eligible. Benefit Trust Dental Benefits. Dental benefits for the 2018 plan year will continue to be provided by the Benefit Trust through Kaiser Permanente. Your Trust has heard participants and worked to continue to remove barriers to accessing care. We are excited to share the upcoming enhancements to your dental benefits. Starting January 1, 2018, there will no longer be a deductible for services and your annual maximum benefit will increase to $2,500 a year. Benefits for routine filings, simple tooth extraction and periodontics (gum disease & root planing) will also be enhanced to remove the co-insurance portion you were previously responsible for. Participants both inside and outside the Kaiser Service Area can take advantage of these enhanced benefits beginning January 1, Refer to your updated Summary of Benefits from Kaiser Dental for more information. Vision Benefits. Vision benefits for the 2018 plan year will continue to be provided by the Benefit Trust through Ameritas. Ameritas and your Trust heard participants here as well, and are excited to share the upcoming enhancements to your vision, hearing and Lasik benefits. Beginning January 1, 2018, the eye exam reimbursement benefit will increase to $100 and the vision materials maximum, Lasik benefit and hearing materials benefit will also increase. Refer to your updated Summary of Benefits from Ameritas for more information.

2 Page 2 Employee Assistance Program ( EAP ) Benefits. EAP benefits for the 2018 plan year will continue to be provided by the Benefit Trust through Reliant Behavioral Health with no plan changes. Your EAP benefits include 24-hour crisis help, up to five (5) face-to-face counseling sessions for every 12 months per issue, online mental health consultations, health coaching, access to discounted legal and financial services, mediation services, home ownership services and identity theft protection services. Refer to your flyer from Reliant Behavioral Health for more information. There are no benefit changes to Paid Time Off at this time. You will not be receiving new Dental or Vision cards for As a reminder, you cannot use your Benefit Convenience Card for Dental, Vision or EAP out of pocket expenses. for more information please visit the Trust website at orhomecaretrust.org or contact the Trust s Healthcare Enrollment Team at (Portland Metro Area) or toll free Supplemental Trust The Supplemental Trust does not provide medical insurance. The information in this section assumes you are eligible to receive Supplemental Trust assistance. If you are unsure if you are eligible, or have any questions about the information in this section, please refer to your Plan Booklet. If you are currently enrolled or are planning to enroll into an individual medical plan through your State s Health Insurance Marketplace (aka Exchange), or if you are currently enrolled or enrolling into Medicare, you will want to review this letter very closely as there are several important updates that will apply to you. Exchange Trust-Approved Plan Options. 1 Participants that reside in the following zip codes will transition from Kaiser PPO to Kaiser HMO on January 1, 2018 and may receive new cards: 97401, 97402, 97403, 97405, 97408, 97409, 97419, 97424, 97426, 97431, 97437, 97438, 97440, 97448, 97451, 97452, 97454, 97455, 97461, 97475, 97477, 97478, 97487, Kaiser Permanente will be sending you details regarding your coverage and a card if you are not already enrolled in Kaiser.

3 Page 3 For medical insurance, you can enroll into any plan you would like to on your individual State Exchange, assuming you qualify to do so. However, if you are a Supplemental Trust eligible participant, in order to receive benefits from the Trust, you must select the Trust-Approved plan applicable to your area. You can review the medical plan summaries online at orhomecaretrust.org/resources as they become available. You will want to review the updated plan summary for 2018 to understand the plan changes made by medical insurance carriers, such as increased deductibles and outof-pocket responsibilities for individuals enrolling into these plans. Your insurance carrier, if you are currently enrolled, will send you a summary letter notifying you about these plan changes, yearly premium increases and whether or not your plan will be offered next year. The Trust will need to know your monthly premium information for 2018 to update or start your benefits on your Benefit Convenience Card. You will want to check your monthly premium bill and payments to ensure that you are paying your premium payments in full, after the advanced premium tax credit, on time every month. Again, if you would like to be eligible for Supplemental Trust assistance, you will want to enroll into the Trust-Approved plan applicable for your area. Some counties have more than one Trust-approved plan. USE THE INFORMATION ON THE FOLLOWING PAGES OR THE OREGON COUNTY REFERENCE GUIDE ENCLOSED TO FIND THE COUNTY OR AREA WHERE YOU LIVE AND THE TRUST- APPROVED PLAN(S) FOR YOUR AREA. SOME CURRENTLY TRUST-APPROVED PLANS WILL NO LONGER BE OFFERED ON THE MARKETPLACE IN OPEN ENROLLMENT FOR COVERAGE IN 2018 IS VERY SHORT FROM NOVEMBER 1, 2017, TO DECEMBER 15, 2017, SO REVIEW THIS INFORMATION AS SOON AS POSSIBLE. SOME COUNTIES HAVE MORE THAN ONE TRUST- APPROVED PLAN. If you reside in Oregon, the Trust-approved plans for 2018 are as follows: Participants that reside within the Kaiser service area: the Kaiser Permanente Silver $2,500 Deductible plan. On Exchange: KP OR Silver 2500/30 Plan ID 71287OR If you are currently enrolled in the KP OR Silver 2000/30 plan, you should be automatically re-enrolled into this plan. Refer to the letter Kaiser sent you about your plan and premium for next year to update the Trust with your updated premium information.

4 Page 4 Participants that reside in the part of Clackamas County not covered by Kaiser: the PacificSource Silver $2,500 Deductible Legacy Health Network (LHN) plan. On Exchange: PacificSource Oregon Standard Silver Plan LHN, Plan ID 10091OR Participants that reside in Crook, Deschutes or Jefferson County: the PacificSource Silver $2,500 Deductible SCN (Central Oregon Smart Choice) plan. On Exchange: PacificSource Oregon Standard Silver Plan SCN, Plan ID 10091OR For participants that are currently enrolled in the BridgeSpan Oregon Standard Silver $2,500 Deductible plan, that plan is ending on December 31, 2017, and you are being automatically enrolled by the Health Insurance Marketplace into a new plan beginning January 1, You will need to set up automatic premium payments with your new insurance carrier, check to see if your current providers are covered by your new insurance carrier and network, and become familiar with the costs associated with services. In light of this change, the Trust is offering multiple plans as Trust-Approved plans for each area if they are available so that you may choose a plan that is best for you and your individual situation. Please note, as the date of this letter, the main insurance rating agency, A.M. Best, has rated Moda Health at a B-. This rating reflects its independent opinion of the financial strength and operating performance of an insurer. There are 15 rating categories, ranking from A++ to F. The higher the rating, the more likely it is, according to A.M. Best, that the carrier will meets its obligations to policyholders over a long period of time. A B- rating is assigned to companies that have a fair ability to meet their current obligations to policyholders, but are financially vulnerable to negative changes in underwriting and economic conditions. 5 Page 5 instability and could potentially face issues covering the cost of future outstanding claims. The Supplemental Trust has decided to continue to offer MODA so you can decide whether to stay with MODA or change to another Trust-approved plan. The fact that MODA is offered to you does not mean that the Trust is making a favorable decision about MODA - IT IS YOUR DECISION AND YOU SHOULD CARFEULLY CONSIDER THIS INFORMATION. Valley Insurance Professionals can assist you with selecting and enrolling into the plan that best meets your needs but you must act quickly between November 1, 2017 and December 15, 2017, if you want a plan other than the plan that you are being automatically enrolled into. o Participants that reside in Baker, Gilliam, Grant, Harney, Klamath, Lake, Malheur, Morrow, Sherman, Umatilla, Union, Wallowa or Wheeler Counties and are currently enrolled in the Trust-Approved BridgeSpan plan will automatically be enrolled in the Providence Oregon Standard Silver $2,500 Deductible Signature Network, Plan ID 56707OR This is the Trustapproved plan for these counties. BridgeSpan is no longer available on the exchange. o Participants that reside in Benton (outside of the Kaiser service area), Clatsop, Douglas, Lane, Lincoln, Linn (outside of the Kaiser service area), or Marion (outside of the Kaiser service area) Counties and are currently enrolled in the Trust-Approved BridgeSpan plan will automatically be enrolled in the Providence Oregon Standard Silver $2,500 Deductible Choice Network, Plan ID 56707OR This is the Trust-Approved plan for these counties. BridgeSpan is no longer available on the exchange. o Participants that reside in Coos, Curry, Hood River (outside of the Kaiser Service area), Jackson, Josephine, Tillamook or Wasco Counties and are currently enrolled in the Trust-Approved BridgeSpan plan will automatically be enrolled in

6 Page 6 the MODA Health Oregon Standard Silver $2,500 Deductible (Beacon), Plan ID 39424OR This is the Trust-Approved plan for these counties. BridgeSpan is no longer available on the exchange. For Coos, Curry, Josephine, Tillamook or Wasco Counties, the other Trust- Approved plans for these counties are the Providence Oregon Standard Silver $2,500 Deductible Signature Network, Plan ID 56707OR or the MODA Health Beacon Be Prepared Silver $3,000 Deductible, Plan ID 39424OR For Hood River (outside of the Kaiser Service area) or Jackson Counties, the other Trust-Approved plans for these counties are the Providence Oregon Standard Silver $2,500 Deductible Choice Network, Plan ID 56707OR or the MODA Health Beacon Be Prepared Silver $3,000 Deductible, Plan ID 39424OR For participants currently enrolled in the MODA Health Beacon Be Prepared plan, you will be automatically enrolled into the MODA Health Beacon Be Prepared Silver $3,000 Deductible, Plan ID 39424OR This is a Trust-approved plan. If you choose to do so, you can instead enroll into one of the other Trust-Approved plans available in these counties. o Valley Insurance Professionals can assist you with selecting and enrolling into the plan that best meets your needs but you must act quickly between November 1, 2017 and December 15, 2017, if you want a plan other than the plan you are being automatically enrolled into. If you reside in Washington, the Trust-approved plans for 2018 are as follows: Participants that reside within the Kaiser service area: the Kaiser Permanente Silver $2,500 Deductible plan. On Exchange: KP OR Silver 2500/30 Plan ID 23371WA If you are currently enrolled in the KP OR Silver 2000/30 plan, you should be automatically enrolled into this plan. Refer to the letter Kaiser sent you about your plan and premium for next year.

7 Page 7 Participants that reside outside of the Kaiser service area: the Trust-Approved plan for 2018 will be decided in the coming weeks. Once that information is available it will be posted on the Trusts website. If you reside in Idaho or California, the Trust-approved plan for 2018 will be decided in the coming weeks. Once that information is available it will be posted on the Trusts website. Premium Payments. If you are covered under one of the Trust-Approved plans through the Exchange and are eligible for benefits under the Supplemental Trust, the Trust will continue to reimburse your net premium, which is the part of your individual premium that is left after applying the maximum federal Advance Premium Tax Credit ( APTC ) you are entitled to receive. If you are enrolled as an individual, you can continue to use your Benefit Convenience Card to pay your monthly premiums. If you enrolled in family coverage under a Trust-approved Exchange plan, the Trust will not pay your premium directly. Instead, you will need to pay the premium directly to the insurance carrier and then submit a copy of your premium bill to the Trust administrative office for reimbursement of the premium amount relating to your individual coverage. In order to receive monthly premium reimbursements from the Trust in 2018, you will need to submit a new Reimbursement Form and proof of premium payment to the Trust administrative office for every month you are requesting premium reimbursement(s). Please note, the Trust will reimburse you only your portion of the premium (and not the cost for family coverage). You will need to submit proof from your medical carrier or the Marketplace of your portion of the net premium amount after your APTC. You can fill out the Reimbursement Form online or request the paperwork be mailed to you. If you are currently enrolled in the Marketplace and would like to continue to receive benefits from the Trust and are otherwise eligible you need to complete steps to update your information with the Trust Administrative Office to continue to receive benefits. Visit the 2018 Open Enrollment page on the Trust website to learn more and review the enrollment process checklist for 2018 to make sure you do not miss any steps in the process.

8 Page 8 If you would like to receive benefits from the Trust and are otherwise eligible and you are enrolling into your individual medical insurance through the Health Insurance Marketplace for the first time, please visit the 2018 Open Enrollment page on the Trust website to learn more and review the enrollment process checklist for 2018 to make sure you do not miss any steps in the process. You must inform the Trust of your new premium information any time your medical premium changes to ensure that your Benefit Convenience Card is funded correctly. Your monthly premium may change for your 2018 coverage. You can update the Trust with your new premium information online, over the phone or by requesting the paperwork be mailed to you. If you are enrolled for automatic premium payments with a medical insurance carrier that will also be your medical insurance carrier in 2018, your automatic premium payments should continue automatically. If your medical insurance carrier is changing, you will need to set up monthly premium payments with your new medical insurance carrier. Continue to monitor your monthly premium payments with your medical insurance carrier and make sure they are paid in full by the due date so that you do not lose your medical insurance due to non-payment. You will not receive a new Benefit Convenience Card for 2018 if you already have one unless your Card is expiring. If your Card expires, you will be sent a new Card automatically to the address on file with the Trust. It is important that you alert your payroll vendor to any address changes so that the Trust receives that information. If you receive a new Card, make sure to update your automatic payments with your individual insurance carrier with the new Card information. Exchange and Medicare Plan Deductibles, Copayments and Coinsurance. The Board of Trustees is excited to announce an increase on the out-of-pocket reimbursement amount from $4,000 to $5,000 beginning January The Trustees know that there continue to be significant increases to the out-of-pocket expenses on individual medical plans.

9 Page 9 We encourage you to review your individual medical plan summary of benefits for 2018 to understand the costs associated with care, so that you understand the importance of seeking in-network care with your medical insurance plan. Please note, networks can change, and you will want to confirm your current providers are in-network for your medical insurance plan for 2018 before seeking services. In-network means that the provider you visit is covered by your medical insurance plan. To determine if the provider you visit is covered in-network on your Trust-Approved plan, you will need to check with your insurance carrier online or over the phone. Out-of-network costs are significant and are not covered by your Benefit Convenience Card. We are pleased to advise that, if you are enrolled in a Trust-approved qualified health plan and are eligible for coverage under the Supplemental Trust, the Trust will pay up to $5,000 per calendar year of your covered out-of-pocket deductible, copayment and coinsurance expenses relating to any claims you incur that are covered under your Trust-approved Exchange or Medicare plan. This is an increase from last year s limit of $4,000. The process has not changed. You may pay for covered deductible, copayment and coinsurance expenses at the point of service using the Benefit Convenience Card issued to you by the Trust. You may be asked to show proof of your expenses, so keep your Explanation of Benefits ( EOB ), bills, and all receipts (especially your prescription receipts since prescription expenses do not appear on the EOBs issued by your insurance company). If you have not received your Benefit Convenience Card yet and have a qualified out-of-pocket expense, please submit a completed Reimbursement Form along with a copy of your EOB to the Trust. You can learn more about this at the Trust website at Medicare Reimbursement Renewals. For 2018, if you are currently receiving Part B reimbursements, you do not need to resubmit a reimbursement form to continue to receive that Part B premium benefit as long as you remain eligible. You do need to submit a reimbursement form to the Trust Administrative Office for any Supplemental/Advantage or Part D premium benefit in The monthly reimbursement for Supplemental/Advantage or Part D premiums will continue to be up to $44 for Reimbursement Submission.

11 IMPORTANT INFORMATION FOR QUALIFIED HEALTH PLAN ENROLLED PARTICIPANTS REGARDING AVERAGE PREMIUM REIMBURSEMENTS, AUTOMATIC RE-ENROLLMENTS, IN- NETWORK COSTS, MEDICAL PLAN CHANGES, APPEALS AND PREMIUM PAYMENTS Average Premium Reimbursements If you are currently receiving an Average Premium reimbursement from the Trust, you must enroll in a Trust-Approved plan during open enrollment to continue receiving benefits from the Supplemental Trust. Refer to your Plan Booklet, pages Automatic Re-Enrollments If you are currently enrolled in a Trust-Approved plan in the Marketplace, your insurance carrier or the Marketplace will automatically re-enroll you into a new Marketplace plan for Your insurance carrier and the Marketplace, if you are currently enrolled, will send you summary letters notifying you about these plan changes. If you would like to be eligible for Supplemental Trust assistance, you must be enrolled into the Trust-Approved plan applicable for your area. You will need to fill out the annual enrollment paperwork and the Trust will need to know your monthly premium information for 2018 to update your benefits on your Benefit Convenience Card. You will want to check your monthly premium bill and payments to ensure that you are paying your premium payments in full, after the Advance Premium Tax Credit, on time every month. If you are satisfied with your 2018 plan and your information with the Marketplace is up to date, you will not need to enroll or renew with the Marketplace. If you have not updated your information with the Health Insurance Marketplace in the past two years, you will want to do so this year. Otherwise, you could lose any Cost Share Reductions or Advance Premium Tax Credits you would qualify for. This could increase the costs you pay in 2018 for medical and out-of-pocket expenses. If you do not have internet access through a computer or smart phone, you can call the Marketplace directly at and let the representative that answers know that you would like to update your information for 2018.

12 In-Network Costs We encourage you to review your individual medical plan summary of benefits for 2018 to understand the costs associated with care and the importance of seeking in-network care with your medical insurance plan. Please note, networks can change, and you will want to confirm your current providers and the services you seek are in-network for your medical insurance plan for 2018 before seeking services. Medical Plan Changes If you are enrolled in BridgeSpan for 2017, that plan is ending on the Marketplace outside of the Portland Metro Area and is not a Trust-Approved plan for You are being automatically enrolled into a new plan effective January 1, 2018 by the Marketplace. You will need to set up automatic premium payments with your new insurance carrier, check to see if your current providers are covered by your new insurance carrier and network, and become familiar with the costs associated with services. In light of this change, the Trust is offering multiple plans as Trust-Approved plans for each area if they are available so that you may choose a plan that is best for you and your individual situation. Refer to the enclosed letter for more information. If you have questions about which Trust-Approved plan may be the best fit for you and there are options, please review the Summary of Benefits available on the Trust website. If after reviewing you still have questions, you can schedule a phone enrollment appointment for assistance with this process. Note: Open enrollment is shorter this year, so act now!

13 If you are satisfied with your new 2018 plan and your information with the Marketplace is up to date, you will not need to enroll or renew with the Marketplace. If you have not updated your information with the Health Insurance Marketplace in the past two years, you will want to do so this year otherwise you could lose any Cost Share Reductions or Advance Premium Tax Credits you would qualify for, which could increase the costs you pay in 2018 for medical and out-of-pocket expenses. Appeals If you are currently approved for a particular plan by the Trust through the Appeals process and your insurance carrier and plan is still available in 2018, you do not need to submit a new appeal. If your appealed carrier and plan are not available in 2018, you do need to submit a new appeal to the Trust Administrative Office as soon as possible. Premium Payments You will get a bill from your insurance carrier once your enrollment or re-enrollment information is processed for The Trust will need to know your monthly premium information for 2018 to update your benefits on your Benefit Convenience Card. You will need to pay this premium bill to activate your insurance coverage. If you do not, you will not activate or re-activate your coverage and could be without medical insurance for the entire year. If you have set up automatic payments, and you are enrolled with the same insurance carrier for 2017 and 2018, your automatic payments can continue. If you are enrolled into a different insurance carrier for 2018, you will need to set up automatic payments with your new insurance carrier. Monitor your account and call your carriers to make sure that your premium

14 payments are getting paid by the due date so that your medical insurance does not get cancelled.
