1 KNOWLEDGE Builders WORKDAY: ENROLLING IN BENEFITS This Knowledge Builder will walk an employee through enrolling in benefits for themselves and their family members. Try It Out Follow the steps below to enroll in Benefits in Workday: 1. In Workday, click Inbox 2. Select Change Benefit task from your inbox. The task may be labeled Change Benefits for Life Event, Benefit Change New Hire, or Benefit Change Employment Status Change. Remember There are instructions at the top of each page as you are making your benefit elections. Some pages have hyperlinks that will take you to additional information or required forms, such as Evidence of Insurability forms.

If you are enrolling in medical, dental and/or vision benefits, click Elect and then select the Coverage you would like (single, employee + spouse, family, etc.) by clicking on the prompt in the Coverage Section. 4. If you are covering dependents, click the prompt button in the Enroll Dependents section 5. Click Create. 6. Click Add My Dependent From Enrollment. TIP: If you wish to use your dependent as a beneficiary for your life insurance, select Yes when asked if you want to use your dependent as a beneficiary
3 7. Follow the prompts to enter information about your dependent. Items marked with a red asterisk are required fields. NOTE: The dependents address defaults to the employee s address. If your dependent has a different address, please see the Learn More section. 8. Click OK when you have added your dependent s information. 9. Repeat Steps 4-8 for each dependent you wish to cover for medical, dental and/or vision. TIP: You can select your newly created dependent by clicking the Prompt Button in the Enroll Dependents, click Dependents, click the name of the dependent. 10. Continue to enroll or waive dental and vision insurance. Click Continue to go next screen
4 11. You may be prompted to enter your dependents Social Security Number. If you do not have this information, you can enter a reason why the number is not available and then click Continue. Note: The Affordable Care Act (ACA) requires that everyone have qualifying insurance, qualify for an exemption, or potentially pay a tax penalty. As part of the ACA, employers must file an annual report with the IRS that includes Social Security numbers for all employees, spouses, and dependent children enrolled in your medical insurance. One purpose of the report is to show the IRS that employees and their dependents have qualifying health coverage so they won't be subject to ACA's individualmandate tax penalty. Therefore, it is important to provide your dependent s Social Security Number. 12. Next, enroll or waive coverage in the Flexible Spending Accounts. If you are enrolling in the FSA plan, you can enter your annual election in the How much do you want to contribute for the total year? or you can enter the amount you wish to come out of each pay check. Please keep in mind that FSA deductions come out of 24 pay periods for the year (not 26 pay periods). 13. Click Continue
5 14. Life Insurance, Accidental Death & Dismemberment insurance and Long Term Disability are available to employees scheduled to work 30+ hours per week. Depending on your scheduled hours, you may not have any elections to make on this screen. You can elect or waive life insurance coverage for yourself, spouse, and/or child(ren). You can also enroll in Accidental Death & Dismemberment insurance for yourself, spouse and/or child(ren). Lastly, you may be eligible to elect to buy additional Long Term Disability coverage. 15. Click Elect for the benefit plan you would like to enroll in. 16. Click the Prompt Button and select the amount of coverage you want. (This is not available for Long Term Disability). 17. Click Continue 18. Review the information regarding the 403(b) plan and click Continue
6 19. Next, enter your Beneficiaries for life and AD&D insurance, if applicable. Click the Prompt Button to select or create your Beneficiary. See the Beneficiary Knowledge Builder if you need help setting up your beneficiary. 20. Indicate if the beneficiary is a Primary or Contingent beneficiary and assign a percentage to each beneficiary. 21. Click Continue 22. Review the information regarding the Employee Assistance Plan and click Continue. 23. On the last page, review your benefit elections. Pay attention to any messages regarding the need to complete an Evidence of Insurability form
7 TIP: If you need to make changes to your elections, click Go Back. If you need more time to make your elections, click Save for Later and the changes you have made will be saved. Your elections will not be finalized until you submit them. Make sure you go back into the system and submit your elections by the Submit By date shown at the top of the screen. 24. When you are satisfied with your elections, scroll to the bottom of the page, review the information in the Electronic Signature and click I Agree. 25. Click Submit to submit your benefit elections. 26. Click Print for a PDF copy of your Benefit Confirmation Statement. Then click Done. Learn More The address for your dependents will default to your mailing address. To change the address for one or more of your dependents, follow the steps below: A. Go back to your home page by clicking Workday at the top of the screen
8 B. Click Benefits and select Dependents in the Change column. C. Click Edit for the dependent whose address needs to be changed. D. Scroll down to the Address section and click on the Pen and Paper icon. E. Click the X next to Use Existing Address F. Enter the dependents new address and click Done. Remember, items with a red asterisk are required fields. G. Click Submit
