1 Acceptable Records of Production ATTENTION: Failure to follow these Acceptable Records of Production requirements will have a negative impact on any future payable loss you may be eligible to receive. You the producer/farmer (insured) are responsible for providing separate records of production for each Unit (Exposure) of coverage. Units are generally established by Farm Serial Number (FSN) and/or Section/Township/ Range. These records (or, the lack thereof) are used to calculate your average yield history. These records are then used to determine your insurance coverage for each year, and/or the loss payment that you may receive. Harvested production is determined by acceptable records of third-party sales, commercial storage, farm stored production and/or acceptable harvested production records from precision farming technology. Failure to follow RMA s requirements for separation of Units will in some cases result in production and Units being re-determined for claim purposes. Acceptable Third-Party Sales and/or Commercial Storage Records For all crops, acceptable third-party sales and/or commercial storage records must contain the following: Name and address of the buyer or the commercial storage facility, insured s name, load or ticket number, crop, gross weight, tare weight, date weighed, and unit and/or field identification from which the production was harvested that can be correlated to the unit numbers. Loads cannot be split between units as this constitutes commingled production. Production Harvested and Stored on the Farm You the producer/farmer (insured) are responsible for providing separate records of production for each Unit that is stored and notifying the insurance company for measurement when production from another Unit, crop year, or uninsured acreage is to be added to existing production in a single storage structure. There are three options for maintaining separate records of harvested production for each unit that is stored. One option is to store each unit in a separate storage structure. A second option is weighing production prior to storage. This method requires the production to be weighed on an acceptable scale and documented on a weight ticket prior to storage. The adjuster will measure the total production in the storage structure. The total production from the weight tickets will be compared to the adjuster s measurement of production in the storage structure. Acceptable scale types are non-portable on-farm scales, commercial elevator scales, or grain carts. The grain cart must produce printed tickets showing the weight, or have an integrated display panel to show the weight of the production in the cart or the grain cart is equipped with scales integrated with Bluetooth (or other wired or wireless) interface that is capable of electronically recording and storing weight records on a field-by-field basis from which the insured can print all individual load tickets and detailed summary, including all of the required information listed below. If a producer used multiple grain carts, but not all were equipped with the system described herein, the adjuster must verify the production by other means (i.e., bin measurements, sales records, etc.). To be acceptable, each individual scale weight ticket or record for each load must be available and must provide at least the insured s name, crop, the gross weight, per load, of the conveyance with production and the gross weight of the conveyance without production (only the gross weight, per conveyance, of the production is required if the production is weighed using a grain cart that meets the criteria listed above for acceptable grain cart records), date weighed, load number (if the scale used does not print a number, the insured must apply a number), identification and location of farm-storage structure in which the load(s) from each field are stored and/or satisfactory explanation of disposition of the production if any or all of the production is no longer stored at the time of inspection; and unit

2 and/or field identification from which the production was harvested that can be correlated to the unit numbers for the crop stored. The insured must hand-write any of the required information listed if the scale being used is not capable of printing a ticket or the required information. A summary record of scale weight tickets/records is not acceptable. The insured may record the moisture percentage of each load at the time production is weighed. If the insured s scale or weight tickets/records are unacceptable, the insured s records of weighed production cannot be used. The adjuster measured and calculated production will be used and the procedures for commingled production apply. The third option for maintaining separate records of harvested production for each unit that is farm stored is to follow the criteria outlined in A, B and C below. This document may be used as your authorization from the insurance company to keep your own load records and bin markings. A. Measurements Prior to Insured Commingling Production 1) For the purpose of indemnities, insurance providers generally cannot accept the insured s determinations of separate unit production when production has been commingled with other units, uninsured acreage production, or different crop years. When a loss situation is probable, the insured should request measurement of existing production in a storage structure PRIOR TO adding production from other units, uninsured acreage, etc. 2) The insured s load records, bin markings, and combine monitor records may be used to determine the proportion each unit contributed to the total production. Commingled production may be authorized in accordance with the requirements in B and C below. B. Criteria Needed to Accept Insured s Production Records or Storage-Structure Markings Used in Lieu of the Insurance Company s Pre-Measurement of Production that is to be Kept Separate 1) If structure markings, load records or combine monitor records are used to keep production separate, the loads are recorded as outlined in C below. 2) Prior year s production in the structure must be measured by the company prior to the current year s production being added. 3) The total production within the structure must be accounted for; i.e., all production stored in the storage structure can be identified separately by unit number or field I.D., uninsured acreage, etc. C. Insured s Instructions for Load Records, Storage Structure Markings, Printed Records from Combine Monitors Used in Lieu of the Insurance Company s Pre-Measurement of Production that is to be Kept Separate 1) Loads - Maintain a contemporaneous*(daily) ledger, by crop, recording loads of production identified by unit and field number, date of harvest, identity of the conveyance* used to transport the grain to the bin and the estimated bushel volume per conveyance. *Contemporaneous Occurring or originating during the same time. *Conveyance- Anything in which agricultural commodities are transported. This may include combine hoppers, commodity bins, grain carts, grain wagons, farm truck, semi trailer, railcar, or barge. 2) Storage Structure Markings - Identify the depth of such production by marking the storage structure with a permanent marker. Write the unit number(s) (by field ID) from which the production was harvested, and date and initial the mark. Also, on the storage structure, identify and mark the depth of uninsured-acreage production separately from insured-acreage production when the storage structure will contain both. 3) Records from Non-Precision Farming Technology System Combine Monitors printed records from combine monitors must show the location of field, name of crop, date, and number of pounds or bushels of the crop. Insureds must also identify the field ID and unit number on the records. If production

3 from the combine monitor records has been adjusted for moisture by the insured or automatically by the combine monitor, this recorded amount will be the amount compared against the adjuster s measured and calculated production. Acceptable Farm Management Records from Precision Farming Technology Acceptable Precision Farming Systems must include at least the following components: GPS technology integrated with planter monitors, combine monitors, yield mapping software; planting and harvesting summary reporting that reflects total harvested production; and calibrations performed per manufacturer s requirements. For planted acreage records from automated planter monitoring systems to be acceptable as determined acres, the insured must provide the following information in conjunction with production data as stated in harvested production records section below: Insured s name; unit number; FSA farm/tract/field ID number (optional); legal description of acreage; and a print out from the precision farming technology system with the following information: crop name; acres planted; and electronically produced maps of planted acreage and acreage summary records. These records must show required discernible breaks between units or practices except as stated in (1) below. If the insured planted overlapping rows within the planted acreage, the Insurance Company must determine if the automated planter monitor records adjusted for overlapping planted rows. If the system did not adjust for the overlapping planted rows, the automated planter monitoring system cannot be used for determined acres. Insurance Company approved precision farming technology system automated planter records may be used to separate optional units on center pivots irrigation system for irrigated circles and non-irrigated corners without discernible breaks in the planting pattern provided the insured can: provide records showing the variable rate planting populations; document the automated planter monitoring system used; provide the acres planted and practice for each optional unit; provide production records by optional unit and practice; and provide the required information listed above. If the automated planter monitor acreage records provided by the insured are not reasonable, or the Insurance Company has reason to question the records, the insured must provide the precision farming technology systems, yield monitor systems raw data and any additional records requested by the Insurance Company. If the Insurance Company determines the planted acreage records are not acceptable, the automated planter monitoring system cannot be used for determined acres. The production records from the precision farming technology system s yield monitor may still be used. Acceptable harvested production records from precision farming technology to establish total production. The insured should maintain alternate production records by unit in the event the precision farming production records are determined to be unacceptable. Production records from precision farming technology systems may be used in lieu of settlement sheets and bin measurements provided all of the requirements are met. For the production records to be acceptable, the insured must provide the following information, in conjunction with planting data as stated above: The insured must have calibrated the yield monitoring system for each insured crop and crop year, in accordance with the owner s manual specifications. The insured must provide documentation showing the weighed average sensor calibrations for the crop and crop year. The sensor calibrations must not exceed three percent when compared to the actual weighed production harvested from the acreage used to calibrate the sensor. If the initial sensor calibration difference exceeds three percent when compared to the actual weighed production harvested from the acreage used to calibrate the sensor, additional calibration samples must have been taken until the results were within tolerance. If after calibrating the yield monitoring system as stated above, the average sensor calibrations for the crop and

4 crop year still exceed three percent when compared to the actual production harvested from the acreage used to calibrate the sensor, the insured may utilize the precision farming system post-harvest calibration of yield maps created by the system. The insured must provide documentation of the actual production based on acceptable weight records used to post calibrate the system and yield maps. The annual calibration report, from the yield monitor system or documentation from the insured, must include all calibrations and adjustments performed, by crop, for the crop year, including the date each calibration/adjustment was performed and the difference from the previous setting. The annual calibration report must be provided to the Insurance Company or RMA. The records should also include: insured s name; unit number; FSA farm/tract/field ID number; Legal description of acreage; and a print out, by unit, of the following precision farming technology information: crop name; acres harvested; date harvested; total production (unadjusted for moisture); average moisture content (moisture must be adjusted in accordance with the crop provisions); and yield maps and acreage/production summary records. These records, generated from the system, must show separate production records were maintained by unit and/or practice. These maps must be reviewed to identify harvested and unharvested acreage. If the map indicates unharvested acreage, a visual inspection is required to determine if crop appraisals are needed. If the production and yield map records provided by the insured are not reasonable or the AIP has reason to question the production and/or yield map records, the insured must provide the precision farming technology or yield monitor systems raw data and any additional production records requested by the Insurance Company. If after reviewing the systems raw data, the precision farming production records are determined to be not acceptable, production and quality determination must be determined using the applicable RMA procedures.

5 Acreage Reporting Use this Checklist as an aid to ensure your acreage is reported timely and accurately with your agent. Initial Agent Insured My Acreage Reporting Dates are: Identify the specific fields where the following apply: Did you have acreage that was prevented from planting? Identify the fields & report claim. Was a crop replanted? Report the original acres and original date to agent. Do you insure a landlord or tenant s share? Indicate which acres and the appropriate share percentage of each person. Are you sharing any crop on this policy? Indicate the complete name of the person/entity with whom you are sharing. Was any New Breaking ground planted? Identify the New Breaking land. CLU reporting is required. A New Breaking Insurability Request may be required. Was there Conservation Reserve Program (CRP) Acreage involved? Identify the CRP acreage. CLU reporting is required. Is there uninsured land? Identify the necessary acreage type (see page 2 of your acreage report for acreage type codes). Is there a 1st Crop/2nd Crop situation? (see back of form for explanation). For every line on the acreage report, be sure to verify: The crop planted, the date the crop was planted, and your share of this crop. Your legal descriptions and CLU. The Common Farm Name of the field to aid you in reviewing your information. Your Tax ID number at the top of the acreage report. Remember that even if there were no acres planted, a properly signed and dated acreage report must be submitted. Complete the Coverage Summary (pg 1) of your acreage report to assure all acres were reported. You have signed and dated your acreage report. Once your acreage report has been processed, you will receive a Summary of Coverage (SOC) detailing your coverage. Review your SOC for accuracy. Accurate reporting of your acres is your responsibility.

7 Payment Requirements ALL premiums, fees, interest and other charges must be paid prior to the termination date. An open or pending claim does NOT eliminate your requirement to have all premiums, fees, interest, and other charges paid prior to the termination date. Failure to pay the policy timely will cause you to become ineligible for ANY federally insured policy and may make you ineligible for Farm Service Agency benefits. Premium payment for Multiple Peril Crop Insurance policies are due and payable at the premium due date. The company sends billing statements for each month that a premium is due, up until the termination date. ANY unpaid premium on ANY federally insured policy, as of a termination date, will terminate that policy. It cannot be expected that a claim for indemnity on any policy will pay a premium for a policy in it s entirety by the termination date. Reasoning is: 1) the indemnity may not pay the full amount of the premium, and 2) the claim may not be settled by the termination date. Any unpaid premium on any policy will also terminate any and all other policies that are associated with you at the policy(s) termination dates. You will remain ineligible for coverage until all debts are paid on your federally insured policies. Ineligible CAT policies may be subject to U.S. Department of the Treasury collections.

8 Crop Insurance Plan Summary The products and product topics summarized in this outline are not all-encompassing and do not substitute for the policy provisions. See the policy provisions and/or contact your company for a complete description of available coverages and their terms and conditions. Yield Protection (YP) (Plan 01) YP provides protection against a loss in yield due to unavoidable, naturally occurring events. For most crops, that includes adverse weather, fire, insects, plant disease, wildlife, earthquake, volcanic eruption, and failure of irrigation water supply due to a naturally occurring event. Like the APH (Actual Production History) plan of insurance, YP guarantees a production yield based on the individual producer s APH. Unlike the APH plan of insurance, a price for YP is established according to the crop s applicable commodity board of trade/exchange as defined in the Commodity Exchange Price Provisions (CEPP). The projected price is used to determine the yield protection guarantee, premium, and replant payment or prevented planting payment, and to value the production to count. The coverage and exclusions of YP are similar to those for the APH plan of insurance. An indemnity is due when the production to count is less than the yield protection guarantee. The main crops covered under this plan including barley (including malting types), canola/rapeseed, corn, cotton, grain sorghum, rice, soybeans, sunflowers, and wheat. Revenue Protection (RP) (Plan 02) RP provides protection against a loss of revenue caused by price increase or decrease, low yields, or a combination of both (for corn silage and rapeseed, protection is only provided for production losses.) This coverage guarantees an amount based on the individual producer s APH and the greater of the projected price or harvest price. Both the projected price and harvest price are established according to the crop s applicable commodity board of trade/exchange as defined in the Commodity Exchange Price Provisions (CEPP). While the revenue protection guarantee may increase, the premium will not. The projected price is used to calculate the premium and replant payment or prevented planting payment. An indemnity is due when the calculated revenue (production to count x harvest price) is less than the revenue protection guarantee for the crop acreage. Crops covered under this plan include barley (including malting types), canola/rapeseed, corn, cotton, grain sorghum, rice, soybeans, sunflowers, and wheat. Revenue Protection with the Harvest Price Exclusion (RP HPE) (Plan 03) RP HPE is similar to RP, however, RP HPE coverage provides protection against loss of revenue caused by price decrease, low yields or a combination of both. Unlike RP, the revenue protection guarantee for RP HPE is based on the projected price only and it does not increase based on a harvest price. Crops covered under this plan include barley (including malting types), canola/rapeseed, corn, cotton, grain sorghum, rice, soybeans, sunflowers, and wheat. Area Yield Protection (AYP) (Plan 04) AYP coverage is based on the experience of the county rather than your individual farm. Maintaining your actual production history is now mandatory and may be used by RMA as a data source to establish and maintain the area programs. AYP indemnifies you in the event the final county yield falls below your trigger yield. The Federal Crop Insurance Corporation (FCIC) will issue the final county yield in the calendar year following the crop year insured. Since this plan is based on county yields and not individual yields, you may have a low yield on your farm and not receive a payment. Area Revenue Protection (ARP) (Plan 05) Like other area plans, ARP is based on the experience of the county rather than individual farms. Coverage is provided against loss of revenue due to a county level production loss, a price decline, or a combination of both. Upside harvest price protection is included which increases the policy protection at the end of the insurance period if the harvest price is greater than the projected price and if there is a production loss. ARP will pay a loss when the final county revenue is less than the trigger revenue which is calculated using the higher of the projected price or harvest price. Area Revenue Protection with the Harvest Price Exclusion (ARP HPE) (Plan 06) Like AYP and ARP, ARP HPE is based on the experience of the county rather than the individual farm. Maintaining your actual production history is now mandatory and may be used by RMA as a data source to establish and maintain the area programs. An ARP HPE policy provides protection against loss of revenue due to a county level production loss, price decline or a combination of both. This plan only uses the projected price and does not provide upside harvest protection. An indemnity is due under ARP HPE when the final county revenues published by FCIC are less than the trigger revenue. Since this plan is based on county revenue and not individual revenue, you may have a loss in revenue on your farm and not receive a payment under ARP HPE Actual Production History (APH) (Plan 90) The APH plan of insurance provides protection against a loss in yield due to nearly all natural disasters. For most crops, that includes drought, excess moisture, cold and frost, wind, flood, and unavoidable damage from insect and disease. Like YP, the APH plan of insurance guarantees a yield based on the individual producer s actual production history. Unlike YP, the available price elections are established by the Risk Management Agency. An indemnity is due when the value of the production to count is less than the liability. Of the small grain crops, only oats, rye, flax, and buckwheat remain covered under the APH plan of insurance for the 2015 crop year. Current as of July National Crop Insurance Services

11 Production Reporting Use this checklist as an aid to ensure your Production Report is completed timely and accurately with your agent. Initial Agent Insured My Production Reporting Date is the earlier of 45 days after the Sales Closing Date or Acreage Reporting Date, unless specified otherwise in the Actuarial Special Provisions. The Production Reporting Dates are: Identify the specific fields where the following apply: Verify the complete share name is listed. Is the correct legal description listed? The SSN is a 9-digit number issued by the (SSN) United States Social Security Administration to an individual for purposes of section 205(c)(2)(A) of the Social Security Act. Employer Identification An EIN is an acceptable identification number for crop insurance purposes. Also known as a Federal Number (EIN) Employer Tax Identification Number, the EIN is used to identify a business entity. RMA Assigned Number (RAN) Substantial Beneficial Interest (SBI) Each person type requires either a(n) SSN and/or EIN, or RAN to be reported. Insurance will not be provided to persons who fail to report their SSN, EIN, or RAN, as applicable, by the Sales Closing Date. An incorrectly reported identification number may result in a voided policy. Each identification number is to be used only by the person to whom it was assigned, and in accordance with the person type procedure. A RAN is an acceptable number for crop insurance purposes. A RAN is issued by RMA to those individuals who demonstrate that they are entitled to Federal benefits in accordance with the Personal Responsibility and Work Opportunity Act of 1996 (PRWORA). A RAN may also be assigned to a Bureau of Indian Affairs allotment that does not have an ID number. All persons (both foreign and domestic) with an interest of at least 10% in the insured/applicant must be listed and the application identification number provided on the policy/application. A policy will be void if it does not include all person with an SBI in the applicant or insured.

14 Claims Responsibility If you believe you may have a loss or your crop has sustained damage: Report a loss to your agent immediately For a planted crop, when there is damage or loss of production, you must give us notice, by unit, within 72 hours of your initial discovery of damage or loss of production (but not later than 15 days after the end of the insurance period, even if you have not harvested the crop) For crops for which Revenue Protection is elected, if there is no damage or loss of production, you must give us notice not later than 45 days after the latest date the harvest price is released for any crop in the unit where there is a revenue loss In the event you are prevented from planting an insured crop that has prevented planting coverage, you must notify us within 72 hours after the final plant date or when you determine you will not be able to plant during the late plant period You must obtain consent from your crop adjuster before you Destroy any of your insured crop Put any of your insured crop to an alternate use Replant any of your insured crop Put the acreage (the land) to another use Abandon or stop caring for any portion of the insured crop It is your responsibility to report your loss within 72 hours and provide evidence to establish your loss. The company is responsible to determine your loss using the evidence available. When in doubt, notify your agent immediately.

15 Unit Structure In crop insurance, a unit is an exposure of insurance. It is the description and the identity of what is covered. Specifically, in crop insurance, it is how you maintain your harvest records of your crops that determines how much and where your crops are covered. It is the most important factor of your crop insurance policy. Basic Units: Determined by Crop, County, and with whom you share. A change in the percentage of share does not qualify for a separate basic unit Production should be kept separate for each share arrangement by crop Basic units can consist of more than one legal location Variable discount or 10%, depending on crop Optional Units: Basic units divided further by legal location or Farm Serial Number (FSN) Must be elected by the Production Reporting Date Production records must be kept separate by legal location or Farm Serial Number (FSN) Irrigated and Non Irrigated MAY qualify for separate units IF production between the practices is kept separate Optional units may consist of more than one legal location or Farm Serial Number (FSN), if the field crosses section lines Enterprise Units: All insurable acreage of the crop in the county, regardless of share in one unit Enterprise Units may be available by irrigated and non-irrigated practices Must be elected by Sales Closing Date Qualifications must be met to be eligible; determined after the acreage report processed Significant premium reduction Only planted acres are utilized when calculating the discount factor See back of form for a flow chart of Enterprise Unit Qualification Whole Farm Units: All insurable acreage of all crops in the county in which you have a share Must be elected by Sales Closing Date Crops must be insured with the same company and at the same coverage level At least two insured crops are required At least two insured crops must have planted acres that constitute 10% or more of the total planted acreage liability of all crops in the Whole Farm Unit How are losses determined? Basic Units - All acres of a crop by share will be added together to determine the indemnity. Optional Units Each legal location or Farm Serial Number will be determined separately to calculate the indemnity. Acreage must be in separate sections or FSN s to qualify as an Optional Unit. Enterprise Units All acres of the crop in the county (regardless of share) will be added together to determine the indemnity. One field may be under the guarantee, while another field may be over the guarantee which may affect an indemnity. Whole Farm Units All production from all acres of all crops in the county will be added together to determine the indemnity.
