Critical illness Return of premiums T10, T20 and T75 must not exceed the amount of cancer insurance Yes return of 100% of all paid premiums upon death or 50% at age 75, less any claim payments. 3 CancerGuard No medical exam cancer insurance Introduction CancerGuard is a simplified issue cancer insurance product for people from birth to age 65, who look for a cancer and critical illness coverage without any need for a medical exams or blood tests. CancerGuard provides coverage as high as $150,000. No medical exams Few questions to answer on current health status Three available terms: 10 years, 20 years or up to age 75 Renewable contract without proof of insurability at the end of terms 10 and 20 years Three optional coverages: Critical illness Prevention Return of premiums No policy fee even if multiple coverage is chosen. Main product features CancerGuard is a simplified issue insurance product that offers 3 levels of coverage. A few questions are required to determine the eligibility to coverage. Clients complete a three-level questionnaire to qualify for better premium rates and coverage as they proceed. 4 ELIGIBILITY STEPS Eligibility to simplified issue cancer insurance is determined by answering few questions about current health status and family history. The questions asked are divided into three levels: Standard, Preferred and Preferred PLUS rates. If client answers "yes" to any question of the first level, they are not eligible to the product. For any other level, by answering "yes" to any question, they become qualified for the preceding level only and are limited to the maximum sum insured available for that step. Of course, if all answers are "no", they qualify for the preferred PLUS rate. Please refer to the questionnaire section to better understand how it determines your eligibility. Clients who do not wish to purchase the critical illness optional coverage do not need to answer the questions related to critical illness (questions 2B, 4B AND 5B). It is not allowed to obtain a better rate or a higher sum insured for the critical illness coverage, but the reverse is possible, given the answers. 4
5 ELIGIBILITY QUESTIONNAIRE If a client answers «yes» to any question in the first level, they are not eligible. For the other levels, by answering yes to any question, they become qualified for the preceding level only and are limited to the maximum insurance amount available for that level. LEVEL 1 STANDARD RATE ANY "YES" ANSWER: Not eligible ALL NO'S: Qualifies for STANDARD RATE up to $50,000 Can also proceed to level 2 for PREFERRED RATE LEVEL 1 STANDARD RATE ($5,000 to $50,000) 1) In your lifetime, have you ever been diagnosed with acquired immunodeficiency syndrome (AIDS) or tested positive for the human immunodeficiency virus (HIV)? 2) Do you have, have you ever had, have you noticed signs or symptoms for which you have not consulted a physician yet, or are you waiting for a test or test results for any of the following illnesses or medical conditions: a. Leukemia, lymphoma, malignant tumour or any form of cancer? To be answered only if the primary insured is applying for critical illness optional coverage b. Chronic neurodegenerative diseases, type 1 (insulin dependent) diabetes, type 2 diabetes, congenital heart abnormality, angina, angioplasty, coronary artery bypass surgery, heart attack, heart failure, cardiomyopathy, heart valve disease, stroke (cerebrovascular accident), transient ischemic attack (TIA), any other cerebrovascular disease, any other disorder of the heart or blood vessels, abnormal electrocardiogram (ECG), chronic renal failure or polycystic kidney disease? YES NO LEVEL 2 PREFERRED RATE ANY "YES" ANSWER: Eligible for level 1 STANDARD RATE ALL NO'S: Qualifies for PREFERRED RATE up to $100,000 Can also proceed to level 3 for PREFERRED PLUS RATE LEVEL 2 PREFERRED RATE ($5,000 to $100,000) 3) In the past two years, have you had an application for critical illness or cancer insurance declined or deferred by any insurance company, including ia Excellence? 4) Have two or more members of your immediate family (father, mother, brothers and sisters) suffered from or been diagnosed before the age of 60 with: a.
