2 SUMMARY (voice) (cell) The Saint Louis Science Center Foundation ( SLSCF ) is seeking bids from qualified Contractors interested in providing sealing and striping of surface parking asphalt at the Science Center s 5050 Oakland service lot and drive and the James S. McDonnell Planetarium lot and drive located in Forest Park (Attachment A). The Science Center s 5050 service lot is used for shipping and receiving, contractor parking and waste pickup as well as a pedestrian walkway for SLSC staff to enter the building. There are 6 contractor parking spaces and 2 SLSC Security parking spaces. There are 2 entrances to the service lot. The main drive entrance is off of Oakland Avenue, west of the 5050 building and the other entrance is from the main parking lot and is gated. The service lot is a heavily trafficked area during business hours. The Planetarium drive and parking lot is located in the southeast corner of Forest Park. There is only one entrance into this lot for both visitors and SLSC staff as well as waste pickup. There are 96 parking spaces, 6 which are for disabled. SCOPE OF SERVICES This purpose of this RFP is to establish an agreement with a qualified Contractor to provide asphalt sealing and parking striping based on this scope of services: 1) Barricade/stanchion off areas to be sealed on the day prior to the work to be performed, 2) to keep vehicles from entering on the day(s) of the work; Thoroughly clean all areas to be sealed, using heavy-duty blowers and push brooms where necessary; 3) Apply two coats of sand-filled, emulsion sealer to the areas as per manufacturer s specifications. First coat to be squeegee-applied (by either machine or hand), second coat spray-applied; Sealer to contain between four (4) to six (6) lbs. of sand per gallon of sealer and not to be cut more than ten-percent (10%) with water; 4) Restripe the completed areas as per the existing layout, with directional arrow/wording 5) specifications; Disabled stalls to be restriped and relabeled in adherence to the latest Americans with 6) Disabilities Act guidelines. Remove barricades/stanchions from the work areas once completed. PROJECT PARAMETERS The following is assumed: a. b. There will be one (1) prime contractor, one (1) bid package and one (1) period of work; The work area is as described in the above Summary and depicted in Attachment A; c. d. The period of work is limited to Sunday, September 16 Friday, September 21. Work may need to be conducted at night and on weekends; e. Traffic signage, blockage and safety measures are the responsibility of the Contractor; f. Permit application services, as required, are the responsibility of the successful Contractor; g. Alternatives to the Scope of Work may be proposed but must be accompanied by rationale and cost. RESPONSE PARAMETERS Page 2 of 7

The Board of Commissioners and the Board of Trustees for the Saint Louis Science Center request that no Contractor contact them or any member of Science Center management or staff, other than as detailed in this RFP, in conjunction with this RFP. Any contact with a member of the Commissioners, Trustees or staff will be grounds for rejection of a response to this RFP. b. Prepare and submit the bid as follows: i. Submit one (1) original printed document and one (1) electronic copy. ii. Provide the bid on company letterhead. iii. The attached Signature Page must be completed and included in the bid response. c. Any questions regarding this RFP must be submitted in writing, either via postal mail or at the address listed above, no later than 12 pm CST on July 10, 2018 to ensure timely response. d. A single representative from your firm should be chosen to communicate with the Science Center. It is his or her responsibility to distribute communications to other members of your organization. e. Contractors are advised that any information submitted as part of this RFP becomes public information and may be released without further notification. Any information that the Contractor considers confidential should not be submitted with the proposal. RESPONSE CRITERIA 1. RELEVANT EXPERIENCE Provide the name, address, phone number, and title of a client reference for three similar projects. 2. APPROACH & MANAGEMENT a. Describe your plan for sequencing the work to enable the lot to be accessible and safe for visitors. b. Describe your traffic and safety plan. c. Describe any other factors that should be factored into considering your firm for this project. 3. PROJECT SCHEDULE Based on the Scope of Work and Project Parameters, provide a proposed schedule that accomplishes the work within the available work period. 4. FEES AND REIMBURSABLE COSTS State the fixed fee, both locations separate, the services as outlined in this RFP. Both locations will be awarded to one (1) bidder. 5. INSURANCE Please note the SLSC insurance requirements in the following Terms And Conditions (#16) a. Submit your insurance certificate for general liability, auto liability, umbrella liability and worker s compensation and employer s liability insurance. b. Identify and provide evidence of your current bonding capacity. 6. MBE/WBE PARTICIPATION It is SLSCF s goal to promote MBE/WBE participation in this Project. Note the qualifications of your firm in this regard, and describe the plans that would be utilized to promote such participation with any consultants or sub-contractors. Page 3 of 7

4 TERMS AND CONDITIONS It is the policy of the Saint Louis Science Center that no person shall, on the grounds of race, creed, color, religion, sex, age, national origin, ancestry, sexual orientation, disability or veteran status, be excluded from consideration for employment, denied employment with or be subject to discrimination of any kind. 1. Alternate proposals will be considered for an award. St. Louis Science Center reserves the right to make final determination of actual equivalency or suitability of such bids with respect to requirements outlined herein. The bids submitted, and any further information acquired through interviews, will become and is considered to be a part of the final completed purchase order or contract. If there is any variance or conflict, the bid specifications will control. 2. The Purchase Order/Contract will be awarded to that responsible bidder whose bid, conforming to the Request for Proposal, will be most advantageous to the Saint Louis Science Center, price and other factors considered. 3. The Saint Louis Science Center reserves the right to reject any or all bids and to waive informalities and minor irregularities in bids received, and to accept any portion of bid or all items bid if deemed in the best interest of the Science Center to do so. 4. A signed Purchase Order or Contract negotiated with the successful bidder results in a binding contract without further action by either party. 5. Late or unsigned bids will not be accepted or considered. It is the responsibility of bidder to ensure that the bid arrives to the person and office identified on the cover sheet prior to the time indicated. 6. No bid shall be opened until after the date and time set for receipt of the bids. No public opening of bids shall be scheduled. 7. The bid price shall be exclusive of any Federal or State taxes from which the Saint Louis Science Center is exempt by law. 8. Any interpretation, correction or change of the Bidding Documents will be made by Addendum. Interpretations, corrections and changes of the Bidding Documents made in any other manner will not be binding, and Bidder shall not rely upon such interpretations, corrections or changes. The Saint Louis Science Center s representative will not be responsible for oral clarification. 9. The Saint Louis Science Center promotes the purchase/leasing of energy efficient materials and reduced-toxic level products where availability, quality and budget constraints allow. Bidders are expected whenever possible to provide products, materials and/or equipment that promote sustainability and utilize post-consumer recycled-content materials. Page 4 of 7

Insurance Requirements The successful bidder shall maintain and pay the cost of the following insurance policies, written by companies licensed in the State of Missouri and rated B+ or better in the most current issue of Best s Insurance Reports: 1. A general liability policy for premise operations, independent contractors, completed operations, personal injury and contractual liability. The amount of liability insurance shall be combined single-limit for two million dollars ($2,000,000) 2. An all-risk property insurance policy for physical damage to the property of the Saint Louis Science Center, with coverage of no less than five million dollars ($5,000,000) on a replacement-cost basis. 3. The foregoing policies shall name the Saint Louis Science Center as additional insured and Consultant shall provide the Science Center Foundation with certificates of insurance verifying that all required insurance policies, including worker s compensation, coverages, and limits are in effect. 4. In addition, all employees of Consultant will be covered by Worker s Compensation Insurance to comply with the laws of the State of Missouri day notice of cancellation to the Saint Louis Science Center for all insurance policies (10 days for non-payment of premium). 6. Note: Required insurance documentation must be submitted with the signed Purchase Order/Contract within thirty (30) days of conditional award of contract by the Saint Louis Science Center. Please note that failure, for whatever reason, to provide the required documentation within 30 days of contract award shall disqualify the selected firm and void the contract. Page 5 of 7
