(g) Repayment of private housing loans for which application can be made for payment of the Public Service Housing Subsidy. (h) Contributions to "Die Suid-Afrikaanse Polisie Bystandsfonds" for White and Non-White members. (i) (j) Contributions to the South African Police Widows 'and Orphans' Fund for White and Non-White members. Contributions to "Die Suid-Afrikaanse Kultuurvereniging van die Suid- Afrikaanse Polisie" (AKPOL). (k) Deductions for the purchasing of National Saving Certificates. (l) Premiums payable to the Highveld Accident and Motor Group Insurance Scheme for the South African Police. (m) Repayment of housing loans to the Corporation for Economical Development Limited, by Bantu members of the South African Police. (n) "Premiums for approved short-term insurance." THIS FORM IS NOT SUBJECT TO STAMP DUTY (a) No stop-order will be accepted in the event where a monthly repayment for a garnishee order is deducted from the salary of the proposer. The commanding officer must acquaint himself with the circumstances prior to this countersigning to stop-order. (b) Except in the event of a submission, alternation or cancellation of a stoporder, correspondence in connection with a query which cannot be solved to the satisfaction of a depositor by the local postmaster, shall be directed directly to the Senior Accountant, Savings Bank Branch, General Post Office, Bloemfontein. STOP-ORDERS FOR INSURANCE PREMIUMS (5) (a) It appears that proposers, either through ignorance or through incorrect information furnished by representatives, sometimes do not appreciate the full implications of the policy for which stop-order facilities are desired. (b) In this respect members who are delegated to authorize acceptance of stop-orders, have a particular responsibility towards proposers, especially inexperienced junior members. It is therefore essential that members who have to sign the certificate, shall acquaint themselves with the terms and conditions of the policy for which stop-order facilities are desired in order that they may satisfy themselves that the proposer fully appreciates the benefits and commitments which the policy will carry. (See SO(F) 29(6)(b)).

5 SO(F) 29-5 (c) Where it appears that a representative has misled the proposer, or committed any other malpractice, the matter must be reported to the Commissioner. (6) Subject to the approval of the Commissioner a stop-order for insurance premiums excluding the South African Police Assurance Fund, the Group Life Assurance Schemes and the Bancura Group Accident Assurance Scheme for the South African Police, may only be considered under the following conditions: (a) (i) A member should at least have completed two years service. SO(F) 29(6)(a)(i) replaced by FO(G) 23/1988 (ii) The provisions of paragraph (6)(a)(i) is not applicable to insurance companies with whom the South 'African Police has agreements. Stop order facilities, for insurance companies, are granted to members after two months employment. SO(F) 29(6)(a)(ii) paragraph is inserted by SFO(F) 23/1988 (b) Stop-orders for insurance premiums are restricted to life, endowment, accident, illness and pension annuity Policies. SO(F) 29(6)(b) words amended by FO(G) 23/1988 (c) A stop-order in the form of an irrevocable cession of a portion of a member's salary will operate as such for a period of twelve months and during such time it may only be cancelled with the written consent of the insurance company. After expiry of the twelve months the stop-order will operate as a request, by virtue of clause 6 of the form of stop-order in Standing Orders (F) 29(1). (d) A stop-order shall reach the Commissioner not later than the first day of the month in which the first premium must be deducted and not later than two months after the date of proposal. (e) Except in the case of a commissioned officer no stop-order shall be accepted which would have the effect of bringing deductions for insurance premiums up to an amount in excess of fifteen persent of his/her basic salary (including persionable allowances, but excluding all other allowances). Deductions for insurance premiums of the South African Police Assurance Fund, Retirement annuities, the Group Life Assurance Schemes and the Bancura Group Short Term Insurance for the South African Police are not taken into account when determining the stated fifteen persent.

6 SO(F) 29-6 SO(F) 29(6)(e) single words are amended by FO(G) 9/1991 (f) A stop-order which is likely to cause financial hardships to a member, his family of his creditors will not be accepted. (g) A member whose stop-order has been cancelled will, for a period of twelve months after date of cancellation, not be permitted to submit a new stoporder, except where the lapsed police is revived, the amount of insurance is increased, or in the event of a conversion of an existing policy to a different type of policy. (h) Provided that insurance companies are prepared to accept it and subject to the conditions in subparagraphs (a), (b), (c), (d), (e), (f) and (g) a member who desired to pay insurance premiums by deductions from his salary, but who does not wish to complete a stop-order for an irrevocable cession for twelve months, may make application in the form of a request, as specified in SO(F) 29(1). (i) In the case of Constables, the certificate on the stop-order, either in the form of an irrevocable cession for twelve months or in the form of a request, shall be signed by a Officer, Station Commander or Unit Commander. (See SO(F) 29 (5)(b)). SO(F) 29(6)(i) words amended by FO(G) 23/1988 (j) A stop-order, irrevocable for twelve months, will only be accepted in the form specified in "A" and a stop-order (request) in the form specified in "B". The insurance is based upon an annual premium payable in advance, but the company and I have agreed, subject to the conditions of the said policy, that the said premium shall be paid by me in twelve monthly instalments of R... each. 3. As security for due payment of the amounts indicated in clauses 1 and 2 for the said twelve months, I hereby irrevocably cede, assign and transfer to the company all my rights and title to and interest in my monthly pay which is or will become due to me as employee, to the extent of: (1) The sum of R... for each month of my service until such instalments have been fully paid, and (2) the sum of R... for the month of... only, in respect of the stamp duty as aforesaid. 4. So far as it may be necessary in law, this document when produced to and approved by the Commissioner, will serve as notice to him that the company is entitled to claim and obtain payment as aforesaid in terms of clause 3 hereof. 5. The conditions of clauses 3 and 4 will be fully valid from the date hereof and will remain valid until the said twelve instalments have been deducted by the officer referred to in clause 4, unless the company during such period notifies the officer in writing that it agrees to the earlier withdrawal of this document as from a certain date, in which event it shall be fully valid until such date only. 6. Although the irrevocable cession of a portion of my monthly pay will continue only until the said twelve monthly instalments have been fully paid, I request the officer referred to in clause 4 to continue thereafter to deduct from my pay the monthly instalments of premium as they become due until I shall have notified to him in writing my desire that further deductions be discontinued. 7. Nothing in this document shall be deemed to vary the policy in any respect. 8. I declare that the owner of the above-mentioned policy has not received any payment or advance from the company or any other person, nor been offered any other consideration as an inducement to effect the insurance referred to herein. 9. I declare that I have also read the under-mentioned certificate and understand that commencement of the payments to the company in respect of the amounts mentioned herein will constitute sufficient proof that the cession of a portion of my salary in favour of the company and my requests to the employer have been accepted and that no other notice of acceptance is expected by me or will be given
