2 Welcome to Poppie Dog Care! Thank you for choosing Poppie Dog Care to provide care for your pets. We believe every pet is different and deserves specialized care. That is why our mission is to provide quality and individualized care tailored to your pet s needs. We want to make caring for your pet easier and less stressful with our variety of pet services. No need to worry about midday vet appointments or last-minute trips out of town; Poppie Dog Care has you covered. And when you are home and want to bond with your pet, you can attend our oneon-one training sessions to bring you closer together. At Poppie Dog Care we believe pets are part of the family. As such they deserve the same quality love and attention you would give your children. For this reason, we like to update our Clients regularly on how their pets are doing and take as many photos as pawsible. You want to know your pet is in good hands while you are away, and we want to put your mind at ease! Enclosed is your Registration Packet for pet services. Please print a copy of this packet and complete all forms prior to your scheduled initial Meet and Greet. At the initial Meet and Greet, your Sitter will review the materials with you and answer any questions you may have. This Registration Packet contains: Services and Pricing Guide Client Information Form Pet Information Form Vet Authorization Form Key Handling Form Service Contract Please have your pet s vet records for proof of vaccines. In addition, please have two copies of your house keys ready to provide to your Sitter. One copy will remain with your Sitter while the second copy will be securely stored in our office for backup emergencies. 3 Services and Pricing Guide Dog Drop-In Visits ($15) Standard 30-minute visit with dog in the Client s home. This may include: feeding, watering, walking, playing. An Extra Pet Fee of $15 is applied for each additional dog. Dog Boarding ($35) Charged per night your dog is in the Sitter s home. Drop off is after 5am and pickup is before 10pm. A Sitter will care for your dog overnight and feed, water, let out and play with your pet. An Extra Pet Fee of $30 is applied for each additional pet. Dog House-Sitting ($30) Standard 24 hours in Client s home. A Sitter will drop in on your dog throughout the day at specified times to feed, water, let out and play with your pet. An Extra Pet Fee of $25 is applied for each additional pet. Doggie Daycare ($30) Between 5am and 10pm in either the Sitter s home or the Client s home. An Extra Pet Fee of $25 is applied for each additional dog. Dog Training ($50) Standard 1-hour visit. Training is tailored to the specific dog s needs. Client can choose to attend training sessions with their pet. An Extra Pet Fee of $40 is applied for each additional pet. Cat Drop-In Visits * ($15) Standard 30-minute visit with cat in the Client s home. This may include: feeding, watering, playing, cleaning litter box. An Extra Litter Box Fee of $2 is applied for each additional litter box. Cat Boarding * ($20) Charged per night your cat is in the Sitter s home. Drop off is after 5am and pickup is before 10pm. A Sitter will care for your cat overnight and feed, water, clean the litter box and play with your cat. An Extra Litter Box Fee of $5 is applied for each additional litter box. Cat House-Sitting * ($20) Standard 24 hours in Client s home. A Sitter will drop in on your cat throughout the day at specified times to feed, water, clean the litter box and play with your cat. An Extra Litter Box Fee of $5 is applied for each additional litter box Pet Taxi ($15) Cost is one way within a 5-mile radius of your home. Additional $1 per mile fee is applied if outside the 5-mile radius. Waiting times are charged at $15 per 30-minutes. An Extra Pet Fee of $5 is applied for each additional pet. Puppy Surcharge ($5 per day) An additional fee charged for dogs under the age of nine months for the following services: drop in visits, doggie daycare, house sitting, dog boarding. Holiday Surcharge ($10 per day) A non-refundable surcharge that is applied to any pet service scheduled on holidays or their corresponding weekends. Holidays: New Year s Eve/New Year s Day, Valentine s Day, Independence Day, Halloween, Thanksgiving, Christmas. Misc. Billing Fees: Late Payment Fee ($20), Returned Check Fee ($20) Cancellation Policy: All cancellations require 24-hour notice to guarantee you are not charged for the visit. Payment can be made before or during the service and must be paid in full by the end of the service. In the event that I cannot be reached, I authorize the following: In the event of illness or injury, I authorize Poppie Dog Care to seek appropriate medical treatment for my pet. I understand that every effort will be made to take my pet to the vet clinic specified on the emergency form if the situation permits however; Poppie Dog Care has authority to seek treatment at any veterinary clinic. Furthermore, I agree to reimburse Poppie Dog Care within 14 days of incident for veterinary fees and all related costs in any amount up to $ (please specify dollar amount per pet. 6 Key Handling Form At your initial Meet and Greet, please provide your Sitter with two sets of keys. One set will be for your Sitter and the other will be kept securely in our office for emergencies for the duration of the service. If applicable, please provide the Sitter with a key pass to enter and exit your building and any security codes. I have provided Poppie Dog Care with the following: Number of keys and doors they open: Home security system information Location of keypad Alarm code Time before alarm is triggered Special instructions for entering home I furthermore agree to and understand the following: Poppie Dog Care does not make backups of Client keys. Poppie Dog Care has permission to provide my keys to any Sitter that will be conducting services. If Client supplies only one key, Poppie Dog Care cannot respond to emergency situations in a timely manner. Client understands the risk that their pet may not be cared for as scheduled. Client further understands that if the services of a locksmith are required in order to access your home, Client is responsible for all locksmith charges. Client keys will be turned over to Client at the end of the service. Client can request Poppie Dog Care securely retain keys on file for future services. 7 Service Contract Client desires to engage Poppie Dog Care, its employees, members, agents and representatives ( Poppie Dog Care ) to obtain the care and services provided by Poppie Dog Care (the services ) for Client s pet(s) (the Pets ), and Poppie Dog Care agrees to provide Services in accordance with the terms and conditions of this Service Agreement (the Agreement ). In consideration of the following terms and conditions, and other good and valuable consideration hereby acknowledged by the parties hereto, Client and Poppie Dog Care agree as follows: 1. Client authorizes and engages Poppie Dog Care to perform the Services as set forth herein and in the price sheet provided to Client (the Price Sheet ) for the time period(s) as requested by Client ( Scheduled Period ). During any Scheduled Period, fees for Services will be calculated pursuant to the Price Sheet, which may be modified from time-to-time by Poppie Dog Care in its sole discretion. If Client determines that any Services scheduled during the Scheduled Period are no longer required, Client must notify Poppie Dog Care promptly, and in no event less than the period of time specified in the Price Sheet to avoid being charged for any such Services. 2. In the event of an emergency (e.g., injured pets, severe weather, natural disaster, etc.), Poppie Dog Care is hereby authorized to take all measures deemed necessary or advisable by Poppie Dog Care in its sole and absolute discretion in caring for Pets and Client s property (including without limitation emergency veterinary care for Pets and emergency repair services for Client s home) and Client agrees to defend, indemnify and hold harmless Poppie Dog Care, its respective employees, members, agents and affiliates from all liabilities, claims and expenses, including reasonable attorneys fees, that arise from or relate to such decisions. In the event of such an emergency, Client shall immediately reimburse Poppie Dog Care for expenses incurred. Furthermore, Client is responsible for providing keys to access their home or building. Any locksmith fees incurred as a result of providing faulty keys is the financial responsibility of the Client. 3. Client shall promptly pay all invoices from Poppie Dog Care and may be required to pay certain fees in advance as determined by Poppie Dog Care. Late fees, handling fees for returned checks and other fees shall be payable as set forth in the Price Sheet. Client shall pay interest charges at the lesser rate of one and one-half percent (1.5%) per month or the maximum rate permitted by law on past due invoices. Client will be responsible for all costs and fees associated with collection proceedings, including attorneys fees, for all amounts more than thirty (30) days past due. 4. Client represents and warrants that Pets are currently vaccinated in accordance with all local and state laws and regulations. Client agrees to indemnify, defend and hold harmless Poppie Dog Care, its respective employees, members, agents and affiliates from all liabilities, claims and expenses, including reasonable attorney fees, that arise from or relate to Pets behavior, including without limitation property damage, personal injury or death caused by Pets. 5. Poppie Dog Care provides no representations or warranties, express or implied, with respect to the services and disclaims any and all implied warranties, including the implied warranties of merchantability and fitness for a particular purpose. In consideration of the services and as an express condition thereof, the client expressly waives and relinquishes any and all claims and liabilities of any kind against Poppie Dog Care arising from or relating to the Services or this Agreement. Without limiting the foregoing, in no event shall Poppie Dog Care be liable for any special, incidental, indirect or consequential damages of any kind in connection with this Agreement, even if Poppie Dog Care has been informed in advance of the possibility of such damages; in no event shall Poppie Dog care s total and aggregate liability hereunder exceed the amount paid by Client to Poppie Dog Care hereunder. 6. Either party may terminate this Agreement at any time for any reason or no reason by providing the other party with notice of such termination. This Agreement constitutes the entire agreement between the parties in connection with the subject matter hereof and supersedes all prior and contemporaneous agreements, understandings, negotiations and discussions between the parties, whether oral or written. The validity, construction and performance of this Agreement shall be governed by and construed in accordance with the substantive law of the North Carolina, without regard to conflicts of law provisions. If any provision of this Agreement or the application of any such provision shall be held to be contrary to law, the remaining provisions of this Agreement shall remain in full force and effect to the maximum extent permissible.
