Your current health 31 care coverage through Access Health CT ends December 31, You should have already received a letter in the mail from us about renewing your current coverage for YOUR LETTER SAID... WHAT YOU NEED TO DO You can renew your health coverage without doing anything, AND you have no changes in your family or income YOU DON T NEED TO DO ANYTHING. If your plan still meet your needs, Access Health CT will enroll you in the 2016 version of your plan that will start on January 1, You can renew your health coverage without doing anything BUT you have changes in your family, income, address or others*... 4 TIPS FOR CHOOSING YOUR PLAN The Right Coverage for You Think about the type of health care coverage you and your family need during the year, and compare with what the plan covers. Essential Health Benefits. ALL the plans include the following: Preventive services - like yearly checkups, shots etc. Lab Tests Prescription Drugs Pediatric services, including dental and vision care for kids Care before and after your baby is born Emergency room visits Mental health and substance abuse treatment Treatment in the Hospital - known as Inpatient Care Services and devices to help you recover after an illness or injury Outpatient Care - the care you get without being admitted to a hospital Get your money s worth! After choosing a plan, take advantage of your preventive benefits at NO ADDITIONAL COST. Learn What s Important Don t just look at the monthly payment (known as a premium), these are also very important: 1. What you owe when you get covered services before your insurance company begins to pay (known as deductible); 2. What you may owe on top of the amount your insurance company pays (known as deductibles, copays and coinsurance); and 3. Which doctors are in your plan s network. Understand What You re Really Buying Plan levels, known as metal levels, go from Bronze (lowest) to Platinum (highest): Bronze: Pay more out-of-pocket for medical services, but may also have the cheapest monthly premium payment. Need Dental coverage? Access Health CT also offers several dental plans to meet your needs. Platinum: Lowest out-of-pocket costs, but may also have the most expensive monthly premium payment. 2

5 ADDING A PERSON TO YOUR COVERAGE If you need to add a member of your family be sure to have the following information: Social Security numbers Visa, green card or other immigration documents Most recent tax return Employer information and any health plans they offer Current insurance coverage For a complete list of documents visit Learn.AccessHealthCT.com. GET HELP PAYING FOR YOUR COVERAGE Let s Start With The Basics There are two ways to get help paying for health care coverage through Access Health CT. When you apply, we ask you for your Modified Adjusted Gross Income* (or MAGI), household size and home address. Based on that, when you enroll in a plan, you and other members of your family may be eligible for: Advanced Premium Tax Credits These are paid directly to your insurance company to reduce your monthly payment. You can also wait until you file your federal income taxes to claim these tax credits. Cost Sharing Reduction Plans These are special plans available only to individuals and families who qualify. These plans lower the amount you have to pay out-of-pocket for deductibles, coinsurance and copayments. Not familiar with health insurance terms? 6 GET HELP PAYING FOR YOUR COVERAGE continued Reminder: 2015 Federal Tax Filing If you received tax credits, Access Health CT will send you a letter by the end of January 2016, that shows the amount of credits paid to your insurance company to help lower your monthly costs. You will need the information on this letter to complete your 2015 Federal Tax returns. * you IMPORTANT: If your filing status is different than what you applied for in a health plan (example: fewer dependents or higher MAGI) you may have to pay back some or all tax credits received by April 15, 2016 (tax filing deadline). AVOID A POTENTIAL TAX PENALTY You Can Avoid a Tax Penalty If You 1. Have a gap in coverage that is less than 3 consecutive months in a year 3. Have a household income that is below the amount that you need to make to be required to file taxes 2. Apply for and are given an exemption from Access Health CT 4. Have other coverage like Medicaid, Medicare or an employer plan If you do not enroll to get health care coverage in 2016 you could pay a $695 Tax Penalty* * Per person ($ per child under 18), up to a maximum of three people in your tax household, or up to 2.5% of your family s Modified Adjusted Gross Income (or MAGI) if that amount is higher. Make sure you file your 2014 federal income taxes before renewing your coverage for 2016! It could impact the financial help you receive through Access Health CT. Importance of reporting changes and updating your information When you don t update your information, this could mean: Tax credits may be delayed/ canceled You may have to pay a penalty You may have to pay back some or all of the tax credits at tax time Log on to your account at AccessHealthCT.com and click on Report a Change. 4
