1 The Patient Setup Screen is used to create a new patient chart or modify an existing patient s chart. After opening the Patient Setup screen you will have the ability to choose the correct Patient Tab on the left based on the information you need to add or modify. You must have information entered on the Demographics Tab in order to proceed to other tabs. After completing each tab, click Save Not all tabs are utilized by every Practice and may not be visible. before moving onto the next tab. 1
2 Demographics Patient Tab is the default tab when opening the Patient Setup screen. The items marked with a red asterisk must be filled out in order to save the tab. The Responsible Party checkbox is used if the patient is his/her financial guarantor/responsible party. Otherwise the Responsible Party Patient Tab must be used. Signature on File and Signature Date are required for filing claims with OfficeEMR. After completing the necessary information select the purple save icon located at the top of the screen. 2
3 The Insurance Tab is where a patient s primary, secondary and tertiary payer is added if applicable. Click Same as Patient if the patient carries the insurance; otherwise add in the Insured Information with the correct Name and Relationship. If not Same as Patient, be sure to add in the Address Information on the right. The Patient Information button can be utilized to quickly pull in the patient s address if the Insurance holder has the same information. Policy Information will include selecting the Payer along with entering the Member ID (the insurance type should fill in automatically after selecting payer). Fill in a Group Name or Group/Policy number if appropriate. The option is available to add in the patient s Copay, Percentage or Deductible amount if necessary. Click the purple save icon to complete changes. Click the White Sheet of Paper at top of screen to add additional Payers To modify an existing Payer, select the correct one in the Insurance Information window Move Payers using the up and down arrows to dedicate which Payer is the primary, secondary or tertiary insurance. Add an Effective End Date to Payers that are no longer ACTIVE 3
4 The Responsible Party tab contains the individual or individuals that are responsible for any of the charges (copays, general payments or etc.). This Tab could already be filled out if you selected Same as Patient on the Demographics Patient Tab. Otherwise add the necessary members who are responsible for the charges. Remember to click the purple save icon. Click the White Sheet of Paper at the top of the screen to add additional Responsible Parties Use the up and down arrows to adjust who is the Primary Responsible Party 4
5 The Emergency Contact tab allows a Primary and Secondary Contact to be added in case of an emergency. Add as much information as necessary and be sure to select the purple save icon. 5
6 The Comments Tab allows a member to create internal comments with the option of setting it as alert. To create a comment: 1. Type a comment in the textbox at the bottom of the screen. 2. Click the purple save icon. This is an internal comment and will only be visible through the comments screen. To create an alert: 1. Type a comment in the textbox or select a previous comment from the Existing Comments section. 2. Select the Alert checkbox. 3. Next add in an Effective Date and an Expiration Date if it should no longer be an alert after a specific date. (Don t add an expiration date if you never want it to expire) 4. Then add in the Alert Type. This will dedicate where the alert message will show up whenever opening a patient chart. (Multi-select by holding Ctrl on your keyboard and selecting the choices) 5. Lastly be sure to click Save. There can be more than one alert created. Remove an alert by select from the Existing Comments and unclicking the Alert checkbox. 6
7 The Employer tab is only visible for Practices utilizing the Employer Module. Here a practice has the ability to add a patient s Employer that will facilitate when scheduling the patient. 7
8 The Sliding Fee tab is an additional tab that will not be visible for all Practices. This will pertain to Practices that utilize a sliding fee schedule. The Patient Name along with what specific schedule is selected. Then the patient s income and number of members in the household are added. Lastly the purple save icon selected. 8
9 The Immunization Registry tab is not visible for all Practices. This tab is utilized to add the necessary data that can be sent to the appropriate state registry. 9
