The Accidental Death and Dismemberment (AD&D) plan offered by ALPA helps lessen an already stressful time for your family by providing a lump-sum payment of up to $500,000 ($350,000 for retired members) in the event of an accidental death or catastrophic loss (such as loss of limbs, eyesight, or other catastrophic losses). Designed To Fit Your Needs and Risks As a Pilot ALPA offers a plan with your life in mind: No physical exams! No denials! If you re an active or retired ALPA member in good standing under age 70, you cannot be turned down for coverage. The plan pays a benefit regardless of any other insurance you may have and is income tax free under current federal tax laws. The plan provides lump-sum benefits for injuries resulting in the loss of life or the loss of a hand, foot, eye, speech, or hearing. Family coverage is available. 4 ALPA Insurance Plans: By Pilots, for Pilots Overview of AD&D Insurance Coverage for ALPA Members AD&D Benefit: Guardian will pay the benefits described below if a member suffers an irreversible covered loss due to an accident that occurs while he or she is insured. The loss must be a direct result of the accident, independent of all other causes. And it must occur within 365 days of the date of the accident. Covered Losses: Benefits will be paid according to the coverage amount the member has elected and only for losses defined in the policy. Covered Loss Benefit (% of Insurance Amount) Loss of Life 100% Loss of a hand 50% Loss of a foot 50% Loss of sight in one eye 50% Loss of thumb and index finger of same hand 25% Catastrophic Loss Benefits Covered Loss Benefit (% of Insurance Amount) Quadriplegia (total paralysis of upper and lower limbs, bilaterally) 100% Loss of speech and hearing, both ears 100% Loss of cognitive function 100% Comatose state, in excess of one month 100% Hemiplegia (total paralysis of upper and lower limbs, unilaterally) 50% Paraplegia (total paralysis of both lower limbs) 50% Loss of speech or hearing (both ears) 50% For covered multiple losses due to the same accident, Guardian will pay 100% of the insurance amount. Guardian will not pay more than 100% of the insurance amount for all losses due to the same accident, except under the Common Carrier, Seatbelt and Airbag Benefit, and the Repatriation Benefit provisions. 4

6 ALPA Insurance Plans: By Pilots, for Pilots Additional Features and Benefits of the Plan Repatriation Benefit For covered loss of life due to an accident which occurs at least 75 miles from the member s home, Guardian will pay an extra sum of up to $5, for costs to prepare and transport the body to a mortuary chosen by the member or an authorized agent. Common Carrier If the member or his or her covered dependent s loss is due to an accident that occurs while riding as a fare-paying passenger in, or being stricken by, a public conveyance, Guardian will increase the benefit payable. Guardian pays two times the amount that otherwise applies to such loss, to a maximum of $100, Seatbelt and Airbag Benefits If a member or his or her covered dependent dies as a direct result of a motor vehicle accident while properly wearing a seatbelt, his or her benefit will increase by $10, And, if the member or his or her covered dependent dies as a direct result of a motor vehicle accident while both properly wearing a seatbelt and sitting in a seat equipped with an airbag, his or her benefit will increase by an additional $5,000.00, for a total increase of $15, Loss of Cognitive Function This term means if a member or his or her covered dependent incurs a significant decline or loss in intellectual aptitude. Such loss must result from an accidental injury and must be supported by clinical proof or standardized tests that precisely measure decline in the areas of: (1) short-term memory; (2) orientation to time, place, and person; (3) deductive or abstract reasoning; and (4) judgment as it relates to awareness of safety. Common Disaster If loss of life benefits are payable under this plan for both the member and his or her spouse, Guardian will increase the benefit payable due to the death of the 6

In place of the spouse s benefit, Guardian will pay 100% of the member s Voluntary Accidental Death and Dismemberment insurance amount, to a maximum of $250, But the following conditions must be met: Insurance must be in force on the date of the accident; and Both the member and his or her spouse must die due to injuries (a) sustained in the same accident or (b) sustained in separate accidents that occur within the same 24-hour period. Exposure and Disappearance Coverage Exposure: If the member or his or her covered dependent suffers a covered loss shown in the Table of Covered Losses (Page 4) due to an accidental bodily injury caused by being unavoidably exposed to the elements, Guardian will pay the amount that otherwise applies to the loss. Disappearance: The member or his or her covered dependent will have a presumed accidental bodily injury due to an accident if (a) he or she is riding in a public conveyance that is involved in an accident; (b) as a result of the accident, the public conveyance is wrecked, sinks, is stranded, or disappears; (c) the member s or his or her covered dependent s body is not found within 365 days of the date of the accident; and (d) the accident occurs while the member or his or her covered dependent is covered by this policy. If covered loss benefits are deemed payable under Exposure or Disappearance, the covered loss benefit is only paid once, not in addition to the Exposure or Disappearance payments. Hijacking and Air Piracy If a member suffers a covered loss shown in the Table of Covered Losses (Page 4) due to an accidental bodily injury that occurs during the hijacking, air piracy, or unlawful seizure or attempted seizure of an aircraft, Guardian will pay the amount which otherwise applies to the loss. 7

8 ALPA Insurance Plans: By Pilots, for Pilots Terrorism Coverage If a member suffers a covered loss shown in the Table of Covered Losses (Page 4) due to an accidental bodily injury that occurs during an act of terrorism, Guardian will pay the amount which otherwise applies to the loss. Dependent Child Education Benefit If a member or his or her covered spouse (if dependent spouse coverage is elected) suffers a specified loss due to an accidental bodily injury, Guardian will pay an education benefit on behalf of a qualified dependent, subject to plan terms. Spousal Education and Retraining If a member or his or her covered spouse (if dependent spouse coverage is elected) suffers a specified loss due to an accidental bodily injury, Guardian will pay a spousal education and retraining benefit, subject to plan terms. Day Care Expense Benefit If a member or his or her covered spouse (if dependent spouse coverage is elected) suffers a specified loss due to an accidental bodily injury, Guardian will pay a Day Care Expense Benefit, subject to plan terms. 8

9 ALPA AD&D Insurance When Coverage Begins Coverage will become effective on the first day of the month after your completed application is received. In order to obtain plan benefits for claims occuring on or after the effective date, premiums must be current. When Coverage Ends Except as explained in the When Active Service Ends section of this policy, a member s insurance will end on the last day of the month following the date a member s membership ends for any reason; the date a member no longer meets the eligibility criteria for his or her class under this plan; the date the group plan ends or is discontinued for a class of members to which the member belongs; or the last day of the period for which required payments are made for the member. 9

10 ALPA Insurance Plans: By Pilots, for Pilots Exclusions or Limitations Guardian will pay no benefits for any loss caused directly or indirectly by any of the following: willful self-injury, suicide, or attempted suicide; sickness, disease, mental infirmity, or medical or surgical treatment; taking part in a riot or other civil disorder or in the commission of or attempt to commit a felony; declared or undeclared war or act of war or armed aggression; while a member of any armed force; while driving a motor vehicle without a current, valid driver s license; by legal intoxication (this includes, but is not limited to, the operation of a motor vehicle); voluntary use of any poison, chemical, or prescription or nonprescription drug or controlled substance unless (1) it was prescribed for the insured by a doctor and (2) it was used as prescribed. In the case of a nonprescription drug, Guardian does not pay for any loss resulting from or contributed to by the insured s use in a manner inconsistent with package instructions. A controlled substance is anything called a controlled substance in Title II of the Comprehensive Drug Abuse Prevention and Control Act of 1970, as amended from time to time. How Accidental Death Benefit is Paid The applicable principal sum will be paid if, within one year of a covered accident under the group policy, bodily injuries suffered by the insured as a direct result and from no other cause than that from the accident result in loss of life. The accident must happen while the insured is covered under the group policy. 10
