1 Long-Term Care / Chronic Illness Benefit Riders - Carrier Offering Insurance Company Rider Rider Definition American General Terminal Illness Accelerated Benefit Rider - This rider provides an accelerated death benefit (living benefit) when insured is diagnosed with terminal illness (24 months or less to live). One-time accelerated benefit of up to 50% of the base policy death benefit (less policy loans and excluding riders). Maximum: $250,000. Automatically included. Subject to an administrative fee. Accelerated Access Solution (Chronic Illness Rider) - This premium paying rider provides income for qualifying chronic condition. There are three options available: 2% of AAS benefit per month; 4% of AAS benefit per month; IRS per diem maximum of the AAS benefit per month. Monthly benefit is capped at the maximum IRS daily rate at the time of claim. Permanency requirement removed for qualifying illnesses. 90 day elimination period. Annual recertification. Underwriting approval required. Indemnity. Charge: Varies by accelerated benefit amount option, issue age, gender and underwriting class. Brighthouse Financial Global Atlantic Acceleration of Death Benefit Rider - The Acceleration of Death Benefit Rider (ADBR) provides for a discounted payment of all or a portion of the amount that would normally be paid to the beneficiaries upon the death of the insured under the base policy. Such payment will be made once a physician certifies that the insured has been determined to be terminally ill, with 12 months (24 months in some states) or less to live. The payment under this rider is made to the policy owner(s), not the policy beneficiary and may be taxable or may adversely affect eligibility for benefits under state or federal law. Please consult a tax advisor to determine the effect prior to exercising the rider. This rider is available for all issue ages and risk classes, at issue and after issue, and may only be exercised once. There is no monthly charge for this rider. A onetime administrative fee, not to exceed $150.00, is charged at rider exercise. The minimum amount that can be requested under this rider is $20,000. The maximum amount that can be requested is the greater of: $250,000 or 10% of the eligible death benefit proceeds. If the policy proceeds are between $250,000 and $275,000, the policy owner may elect to accelerate the full amount. If less than the full amount is requested, the remaining policy death benefit must be at least $50,000. Minimums and maximums are subject to state law. If less than the full amount is requested, all future policy values, charges and premiums will be reduced to reflect the lower coverage. Accelerated Access Rider (Chronic Illness) - The Accelerated Access Rider provides an acceleration of the death benefit if the insured is certified as chronically ill by a licensed physician. The rider will pay out a portion of the policy death benefit to the policy owner should the insured meet all the eligibility requirements. The client must either: a) Be unable to perform without substantial assistance at least 2 of the 6 Activities of Daily Living b) Require substantial supervision to be protected from threats to health and safety due to severe cognitive impairment. The condition must have occurred during the 90 consecutive calendar days immediately preceding the certification. At least one condition is expected to be permanent. Written certification is required from a licensed physician every 12 months that the Insured is chronically ill. Rider is automatically included. No cost until benefits are received. Benefit is lesser of 2% of the eligible amount OR the IRS Per Diem Limit. John Hancock Terminal Illness Accelerated Benefit Rider - This rider allows you to access up to 50 percent of your policy s death benefit up to a maximum of $250,000 when the life expectancy of the insured is 12 months or less. This rider is automatically included at no extra charge on all new policies. A fee will be assessed when the rider is exercised. Accelerated Benefit Rider - This rider provides a "living benefit" if the insured is certified to be terminally ill with a life expectancy of one year or less. This provision allows the policy owner to receive 50% of the eligible death benefit to a maximum of $1 million. The benefit may be added at any time to a policy in good standing. There are no age restrictions to add or exercise this rider. There is no additional cost for this rider, unless it is exercised and its addition to a policy does does not affect the premiums due or any interest paid on the policy. Long-Term Care (LTC) Rider - This provides funds to help pay for qualified long-term care expenses by accelerating the death benefit. Maximum monthly benefit based on 1%, 2%, or 4% of the accelerated benefit amount elected at issue. The rider is available on level and increasing death benefit options. The Long Term Care pool can differ from the Death Benefit. A separate charge is deducted if this optional rider is elected. 1 of 6
2 Lincoln Financial Accelerated Benefits Rider - The Accelerated Benefits Rider pays out a portion of the death benefit if you were to fall terminally ill or require permanent nursing home care. It may be elected after policy issue. This version of the rider will pay 50% of the death benefit for Terminal Illness and 40% of the death benefit for Nursing Home Confinement, subject to an overall maximum of $250,000 on all policies in force with us, in accordance with the terms of the rider. There is a one-time charge for this rider when it is exercised. Accelerated Benefits Rider with Critical Illness - The Accelerated Benefits Rider with Critical Illness pays out a portion of the death benefit if you were to fall terminally ill, require permanent nursing home care, or need help during a critical illness or condition. In the instance of critical illness, the portion of the death benefit payable is 5% (not to exceed a total of $25,000) upon the occurrence of the first critical illness covered by the rider. There is a one-time charge for this rider when it is exercised. If you elect this rider, there is a monthly Cost of Insurance Charge for this rider which will be part of the Monthly Deduction made under the Policy. LifeEnhance Accelerated Benefits Rider - This rider pays a benefit if you were to become permanently chronically ill or are diagnosed as terminally ill (life expectancy of 12 months or less). For Chronic Illness: Written Certification or Written Re-certification by a Licensed Health Care Practitioner that the Insured is Chronically Ill. Chronic Illness means that the insured has been certified within the preceding 12 months, by a Licensed Health Care Practitioner as: Being unable to perform (without substantial assistance from another individual) at least 2 Activities of Daily Living (ADLs) for a period of at least 90 days due to a loss of functional capacity. ADLs include: bathing, continence, dressing, eating, toileting and transferring OR Requiring substantial supervision from another individual to protect the insured from threats to health and safety due to severe cognitive impairment and According to certification from a Licensed Health Care Practitioner, services are likely to be needed for the rest of the insured s life. For Terminal Illness: Written Certification by a Licensed Health Care Practitioner that the insured is Terminally Ill. Two benefit payment options for chronic illness: 1) Monthly benefit option: lower of 2% of the original benefit amount or the IRS per diem limit times the number of days in the month (may increase annually); payable for 12 months at a time. 2) One-time lump sum payment discounted for mortality and interest; exercising this option will terminate the rider and the policy upon benefit payment. Terminal illness benefit included in rider: One-time lump sum payment lesser of 50% of the death benefit amount or $250,000. Minnesota Life / Securian Accelerated Benefit Agreement - This rider allows access to a portion of the death benefit in the event of terminal illness. The insured must have a terminal condition, caused by sickness or accident, which directly results in a life expectancy of 12 months or fewer. Payment is the lesser of 75 percent of death benefit or $1 million. The accelerated benefit will be paid as a lien. The entire amount of the lien will be due and payable at the death of the insured. There is no upfront charge for this agreement. Rider is not automatically included. for Chronic Illness Agreement - The CIA pays a monthly benefit for certified chronically ill individuals. It s an acceleration of the policy s death benefit, so it reduces the death benefit and surrender value. Issue ages: Maximum CIA face amount: Amount selected at time of issue. 10%-100% of the life insurance base face amount (not to exceed $5 million). Cost: Flat rate per $1,000 of face amount. Rate varies by age and underwriting class. Maximum monthly CIA benefit: The lesser of 2 or 4 percent of the CIA face amount or IRS per diem limit. Elimination Period: 90 days. The policyowner is eligible for CIA benefits upon the insured s being certified as a chronically ill individual and satisfying the elimination period. Policy will not lapse while on claim. If the insured is certified chronically ill, a written request for chronic illness benefit payment must be sent to our home office. Once notice of claim is received, Minnesota Life/Securian Life will send forms for verifying eligibility and filing proof of loss, as well as a statement demonstrating the effect of the chronic illness benefit payments on the policy s cash value, death benefit, premium, cost of insurance charges and policy. Chronic illness benefit payments will not be made if the requested proof of loss is not received. At the end of the 90-day elimination period, recertification by a licensed health care practitioner that the insured is chronically ill may be required. This recertification may be required annually. Mutual of Omaha for Terminal and Chronic Illness Rider - Terminal Illness Rider: The rider provides an accelerated death benefit if the insured provides evidence that their life expectancy is 12 months or less. The requested benefit amount may not exceed the lesser of $1,000,000 or 80 percent of the current death benefit. Chronic Illness Rider: The rider provides an accelerated death benefit if the insured is unable to perform 2 of 6 Activities of Daily Living (ADLs) for 90 consecutive days, as certified by their physician, and requires substantial supervision to protect him or herself from threats to health and safety due to severe cognitive impairment. The requested benefit amount may not exceed the lesser of $1,000,000 or 80 percent of the death benefit at the time of the first accelerated death benefit payment requested. Both riders are automatically included. There is no premium or cost of insurance charge for this rider. However, there will be a $100 charge for each acceleration made. In addition, United of Omaha will reduce the Terminal Illness Benefit requested by an actuarial discount. The actuarial discount will be determined as of the date of the requested acceleration and will not be greater than 6.00% 2 of 6
3 National Life Accelerated Benefit Rider for Critical Illness and Critical Injury - ABR is a no additional premium rider that allows for the death benefit, under certain circumstances, and, on a discounted basis, to be accelerated during lifetime, for critical illness or critical injury. There is no restriction placed on the use of the benefit received. The amount of benefit available for ABR payout is based on a discount factor of the death benefit amount. The amount accelerated will reduce the death benefit, the cash surrender value and the loan value. The critically ill or critically injured client can request a full acceleration or partial acceleration of the policy s death benefit in the form of a lump sum not to exceed the lifetime benefit maximum of $1,000,000 per insured. There is no annual limit. The level of discounting applied to the death benefit varies depending on the insured s age and the severity of the critical illness or critical injury. Accelerated Benefits Rider for Chronic Illness - ABR is a no additional premium rider that allows for the death benefit, on a discounted basis, to be accelerated during lifetime, for chronic illness. There is no restriction placed on the use of the benefit received. A chronically ill individual is one who has been certified, within the past 12 months, by a licensed health care practitioner as being unable to perform, without substantial assistance, at least 2 out of 6 activities of daily living ADLs for a period of at least 90 days due to a loss of functional capacity. The amount of benefit available for ABR payout is based on a discount factor of the death benefit amount. The amount accelerated will reduce the death benefit, the cash surrender value and the loan value. For Chronic Illness, National Life will accelerate (or convert to reduced paid-up insurance), income tax-free, the discounted death benefit, not to exceed a maximum amount of $1,500,000 under all contracts made over the entire lifetime of the Insured. Nationwide Financial Accelerated Benefits Rider for Terminal Illness - ABR is a no additional premium rider that allows for the death benefit, on a discounted basis, to be accelerated during lifetime, for terminal illness. There is no restriction placed on the use of the benefit received. The accelerated payment can be made if the Insured is suffering from a terminal illness that will result in death within 24 months (see carrier for state specifics). The amount of death benefit available for ABR payout is based on a discount factor of the death benefit amount. The amount accelerated will reduce the death benefit, the cash surrender value and the loan value. For Terminal Illness, National Life will accelerate, income-tax free, the discounted death benefit, not to exceed a lifetime maximum of $1,500,000 per insured. There is no annual limit and the benefit can be received as a lump sum if desired. ABR Terminal does not have a waiting period. for Terminal Illness Rider - This rider provides for an accelerated death benefit to the owner in the event that the insured has a non-correctable terminal illness that results in the insured having a remaining life expectancy of 12 months or less. Death benefits, cash values and loan values will be reduced if an accelerated death benefit is paid. The rider is added to the policy at the time a claim has been approved. Conditions for eligibility: Policy must have at least two years remaining from the rider effective date until the maturity date (age 120); Policy must have been in force for two years; The specified amount must be at least $50,000 on the rider effective date, i.e. after processing; The adjusted death benefit of the policy must be at least the minimum specified amount. The maximum amount that can be accelerated is 50% of the base policy specified amount. The benefit payment (before charges are deducted) must be at least $10,000 and may not exceed $250,000. There is no cost associated unless the claim has been approved. At that time an administrative charge will be taken. Long-Term Care Rider - This indemnity style rider provides monthly benefits for nursing home care, home health care, assisted living and adult day care. It will provide for a monthly accelerated payment of a portion of the death benefit under the base policy whenever qualifying conditions and the elimination period have been met. The rider can be added after the policy is issued. This rider has a monthly charge based on the insured s age, sex, health and coverage amount specified for the long-term care rider. LTC rider has a 90 day elimination period. It need not be continuous, but must be met within a 730 day period. This is a lifetime elimination period, meaning once the elimination period is met, it never has to be met again, no matter how many separate instances there are. While the insured is receiving the monthly benefits, the policy will not lapse and monthly deductions for the cost of the LTC rider will be waived. The total pool of money available for long-term care benefits is chosen at the time of purchase and may be as much as 100% of the total death benefit amount, os little as 10% of the total death benefit or anywhere in between. At the time of death the beneficiaries will receive the greater of: Gross death benefit less the cumulative LTC benefit paid less partial surrenders/loans; 10% of the original specified amount (feature not available in NY or VI) less any loans or withdrawals. 3 of 6
4 Principal Chronic Illness Death Benefit Advance Rider - Pays an accelerated death benefit upon the insured showing proof of chronic illness defined as either being unable to perform at least two of six Activities of Daily Living or having a cognitive impairment. Each accelerated benefit reduces the policy s death benefit by the amount of the benefit. Each of the following is reduced proportionately, as applicable: accumulated value, surrender charge, loan balance, no-lapse guarantee. Automatically added at issue for fully underwritten cases with issue ages There is no cost for this rider at issue. If the rider is exercised, a reduction factor is applied to the requested accelerated benefit amount. A $150 administrative fee per claim is also applied. Lifetime maximum accelerated benefit: Lesser of 75% of the death benefit on initial election, or $1 million. Annual maximum accelerated benefit: Lesser of 25% of the death benefit on initial election, or the annualized per diem acceleration as defined in the rider divided by the accelerated benefit reduction factor. Annual minimum accelerated benefit: Lesser of 10% of the death benefit on initial election, or $10,000. Minimum residual death benefit: Greater of 25% of the death benefit on initial election, or $10,000 face amount or death benefit. Terminal Illness Death Benefit Advance Rider - Pays an accelerated benefit upon insured showing proof of a terminal illness as defined in the policy rider form. The accelerated benefit that may be received is the lesser of the Accelerated Benefits Cap shown on the current data pages, or 75% of the eligible face amount minus any outstanding policy loans, unpaid loan interest and previously paid accelerated benefit, not to exceed $1 million. The accelerated benefit is considered a lien against the policy and accrues with interest. At the time of death, the beneficiary receives the policy death benefit minus the benefit advance and applicable interest. The minimum amount of any payment is $500. It is available to all policy owners and covers the insured. There are no special underwriting requirements. It may be added at any time. There is no cost to have this rider on the policy. A one-time administrative charge of up to $150 may be imposed when the rider is exercised. Protective Life Terminal Illness - Included at no extra charge, this can accelerate up to 60% of the policy s death benefit or $1 million, whichever is less, if the insured has a qualifying terminal illness. A lien equal to the accelerated payment is established against the policy and accumulates interest, resulting in a reduction in the amount of the death benefit and the amount available for loans and withdrawals. ExtendedCare 2017 Rider (Chronic Illness) - This rider advances a portion of the base policy s death benefit if the insured is certified by a licensed health care practitioner within the past 12 months as chronically ill - defined as either unable to perform at least two Activities of Daily Living without substantial assistance for at least equal to the Elimination Period, or requiring substantial supervision for protection from health and safety threats due to severe cognitive impairment. An individual must be certified as being chronically ill for at least 12 months. This rider is available only at issue, and there is an Elimination Period. The Elimination Period (waiting period) is 90 days. This is the period of time which must pass from written certification of the insured s Chronic Illness status. A Benefit Period lasts 12 months. Before each Benefit Period, the insured must select the amount to be paid for each month of that Benefit Period, subject to monthly minimums and maximums. Each month within a given Benefit Period the insured will receive the same ADB payout amount (subject to exhaustion of the lifetime maximum), but a new payout amount can be selected for the next Benefit Period. During a Benefit Period, we will waive the monthly deductions required to maintain the policy. If the insured is certified as chronically ill for three consecutive Benefit Periods, the monthly deductions will be waived for as long as the policy is in force. Benefits under this rider may be taxable as income. Benefits received under this rider may impact the recipient s eligibility for Medicaid or other government benefits. 4 of 6
5 Prudential BenefitAccess Rider (Chronic Illness) - The insured may accelerate all or part of the policy s death benefit if the insured is chronically ill or terminally ill, provided all eligibility requirements are met. The BenefitAccess Rider is available for extra charges and additional underwriting requirements and limits may also apply. Chronic Illness Portion: Chronically ill means the insured has been certified by a licensed health professional as being, or expected to be, unable to perform (without substantial assistance from another individual) at least two activities of daily living due to a loss of functional capacity that is expected to last for the remainder of the Insured s life, or requiring substantial supervision for protection from threats to health and safety due to severe cognitive impairment that is expected to last for the remainder of the insured s life. A physician or licensed health care practitioner must certify, at the time the Insured applies for benefits and every 12 months thereafter, that the insured is chronically ill as defined in the Rider. We reserve the right to independently assess the insured. Chronic illness benefits are subject to limits, such as a maximum monthly benefit payment that is based in part on a percentage of the lifetime benefit amount and on the per day benefit limitations set by the Internal Revenue Service. This rider is not Long-Term Care insurance (LTC) and it is not intended to replace LTC. The rider may not cover all of the costs associated with chronic illness. When we approve a chronic illness claim, we will permanently waive future rider charges. We will also waive policy charges and will do so permanently once the insured receives 25 monthly chronic illness benefit payments. Interest on outstanding loans will continue to accrue. BenefitAccess Rider (Terminal Illness) - The insured may accelerate all or part of the policy s death benefit if the insured is chronically ill or terminally ill, provided all eligibility requirements are met. The BenefitAccess Rider is available for extra charges and additional underwriting requirements and limits may also apply. Terminal Illness Portion: Terminally ill means the insured has a medical condition that is reasonably expected to result in the insured s death within 6 months or less. If the insured is terminally ill and meets all other requirements, such as providing a physician s certification, they may be able to accelerate all of the death benefit in a single sum. The insured may choose instead to accelerate a portion of the death benefit. Payments under this portion of the rider will be reduced by a discount interest rate and are subject to a $150 processing fee. If the insured use the terminal illness portion of the rider, they can no longer use the chronic illness portion. Obtaining benefits under the terms of the rider will reduce or eliminate the death benefit. If a death benefit remains, the contract fund and other values inside the policy may be adjusted. Unless all future charges are waived under this or another rider, then taking benefits under this rider this could shorten protection under a No-lapse Guarantee and require additional premiums to continue the policy. Outstanding loans will reduce payments under the rider. Benefits paid under the BenefitAccess Rider are intended to be treated for federal tax purposes as accelerated life insurance death benefits under IRC 101(g)(1)(b). Tax laws related to the receipt of accelerated death benefits are complex and benefits may be taxable in certain circumstances. Receipt of benefits may affect eligibility for public assistance programs such as Medicaid. Clients should consult tax and legal advisors prior to initiating any claim. Living Needs Benefit Rider - This rider is available under this policy at no additional premium to the policyowner. Under the rider, the policyowner may receive a portion of the death benefit prior to the insured s death if the insured has been confined to an eligible nursing home for at least six consecutive months, and is expected to be permanently confined, or is terminally ill, and has a life expectancy of six months or less. Living Needs Benefit amounts are calculated based on the policy death benefit and the Living Needs Benefit assumptions in effect when a claim is made. When a claim is paid under this rider, the death benefit is reduced for early payment, and a processing fee of up to $150 is deducted. If more than one policy is used for the claim, each policy will have a processing fee of up to $150. The maximum amount the policyowner can receive may approximate 70 to 80 percent of the death benefit for the Nursing Home option and 90 to 95 percent for the Terminal Illness. The Living Needs Benefit is not a health, nursing home or long-term care insurance benefit and is not designed to eliminate the need for insurance of these types. Portions of the Living Needs Benefit payment may be taxable, and receipt of the accelerated death benefit may affect eligibility for public assistance programs. The federal income tax treatment of payments made under this rider depends upon whether the insured is the recipient of the benefit, and is considered "terminally ill" or "chronically ill". BenefitAccess Rider (Chronic & Terminal Illness) - This rider is an accelerated death benefit rider that advances up to 100% of the policy s death benefit. The death benefit can be accelerated in the event that the insured is certified as chronically or terminally ill by a licensed health care practitioner and otherwise qualifies under the terms of the rider. Accelerating the death benefit will reduce and may eliminate the death benefit on the policy. Satisfactory evidence must be received, including certification by a licensed health care practitioner, that the insured is chronically ill and is not expected to recover from the chronic illness. He or she will need to be recertified as chronically ill every 12 months to qualify. BenefitAccess is not Long-Term Care (LTC) insurance and it is not intended to replace LTC. The rider described below is available at issue for an additional premium and subject to additional underwriting, if the client qualifies. The rider is subject to state variations and may not be available in all states. Maximum monthly benefit is 2% of the death benefit, not to exceed: 1) IRS limit at time of claim; 2) IRS per diem limit at policy issue, compounded at 4% annually. Maximum total benefit $5M. One-time payout for terminal illness only. The Benefit Amount for the terminal illness benefit is the death benefit of the policy, adjusted by a discount factor. The amount paid to policyowners is reduced by the discount factor, which results in a net benefit that is between 90% and 95% of the death benefit. There is no waiting period. Once Prudential approves a BenefitAccess claim, the first benefit payment will be made on the monthly policy date on or following the date that Prudential received certification that the insured is chronically ill. 5 of 6
6 Transamerica Terminal Illness - This endorsement is issued automatically on each policy in states where it is available at no charge until the endorsement benefit is exercised. This endorsement allows the owner to access up to 75% of the available death benefit on the Insured (including BIR) or $500,000 of aggregate accelerated death benefit on the life of the Insured, whichever is less, prior to death of a terminally ill Insured. The minimum amount allowed is $10,000. The policy owner can request multiple accelerations, up to the maximum. There is an administration charge of $350 for each acceleration, deducted from the proceeds when the endorsement is exercised. The administration charge is subject to increase with the Consumer Price Index. Long Term Care (LTC) Rider - The LTC Rider is designed to provide a monthly benefit amount that accelerates the death benefit of the base policy once the insured is certified as a Chronically Ill individual and incurs monthly out of pocket expenses for Qualified Long Term Care services pursuant to a Plan of Care. The LTC Rider requires that monthly Proof of Loss documentation be provided to Transamerica to substantiate the out of pocket expense for the Qualified Long Term Care received. Care provided by family members is not eligible to be considered for reimbursement under the LTC Rider. The cost of an LTC Rider frequently costs less than for similar coverage amounts under a standalone long term care policy. And, if the LTC Rider benefits are not needed, a life insurance death benefit is still paid when the insured dies. Therefore, there is financial benefit under the policy whether or not long term care benefits are ever paid. Voya Financial Accelerated Benefit Rider - This rider allows a portion of the death benefit to be paid to the policyowner prior to the death of the insured in the event the insured becomes terminally ill or in the event of other catastrophic qualifying conditions. The accelerated benefit will be paid when carrier receives written proof that the insured has been diagnosed with a qualifying condition (during the lifetime of the insured). Minimum benefit is $10,000, maximum is the lesser of 50% of eligible death benefit or $1,000,000. Chronic Illness Rider - This Rider allows the insured to access their death benefit once they meet the eligibility requirements. Eligibility may vary by state. A licensed health care practitioner must confirm that without substantial assistance from another individual, the insured is expected to be permanently 1) Unable to perform at least 2 of these 6 activities of daily living for at least 90 days OR 2) is at risk to his or her health and safety due to severe cognitive impairment. When the insured becomes eligible for benefits, the money received can be used for any purpose. No evidence of expenses incurred is necessary. Once eligible, the insured may accelerate monthly or annual portions of the death benefit that are the lesser of 24% of the eligible death benefit per year (or 2% monthly) OR the IRS per diem limit. Maximum lifetime benefit equals $2 million ($25,000 residual death benefit required). Monthly benefit amounts may not be changed during a 12-month benefit period. The insured must submit annual proof of claims eligibility and pay an administrative charge of up to $300/year. This rider is only available at issue.
