1 THE CATAUMET CLUB CAMP 2015 SUMMER DAY CAMP Dates: June 22 - August 14 Location: 145 Scraggy Neck Road, Cataumet, Massachusetts For over 50 years, campers have enjoyed a fun, safe, 8-week day camp experience on the shores of Squeteague Harbor. Each week, campers receive instruction in tennis, sailing and swimming as well as participating in games, crafts and competitions. Give your child a summer to remember this year! Counselors are CPR and First Aid Certified. Sailing Instructors are US Sailing certified. A variety of boat types are used (Optimists, Sunfish, 420 Sailboats, and rowboats.) The swimming program is run by a Red Cross Certified Water Safety Instructor. Tennis instructors are USTA Ten and Under Tennis Certified. Tennis lessons are taught in small groups on the Club s three clay courts by a seasoned tennis staff. Group instruction and competition are included. There are also games, crafts, rainy day activities, overnight camping trips and day trips. No more than 50 campers are enrolled in any one week. This camp must comply with regulations of the Massachusetts Department of Public Health and is licensed by the local board of health. 2 THE CATAUMET CLUB CAMP SCHEDULE OF EVENTS (additional details to be provided later) June 22 Camp begins August 14 Final Night Celebration and Performance Awards Other events include: Tennis matches/swim competitions with other clubs Sailing regattas* with other clubs *Extra fees may apply for participants REQUIRED EQUIPMENT Equipment & safety gear, in good working condition, is required: USCG life vest (suitable for child s weight) plastic whistle (for life vest) tennis racquet 2 new cans of tennis balls (10 and Under Balls for <11) bathing suit tennis sneakers (smooth soles suitable for clay courts) All items should be clearly and permanently labeled. sweatshirt water bottle towel sunscreen water shoes (closed toe shoes must be worn while boating and swimming)

3 The following forms are legal documents and must be signed and mailed to us at the time of registration. Again, your registration is not complete until we have received these forms: Parent Medical Form Fill out and sign Parent Medical Form. In addition, you need to obtain a record from your physician s office with his/her signature that includes an exam within the last 2 years, a health history, and an up-to-date record of immunizations. Children without sufficient medical records cannot start camp until the records are received. State licensing mandates strict adherence to these requirements. (We understand that the doctor s records will be coming later and that delay will not affect registration.) 6 THE CATAUMET CLUB CAMP CAMPER NAME: RELEASE OF LIABILITY AND WAIVER AGREEMENT By signing this agreement, the participant/guardian (we) affirm(s) having read it. In consideration of my participant s participation in the Cataumet Club Camp program, I acknowledge, appreciate and agree that: 1. We risk bodily injury, including disability and death as well as risk of damage to or loss of property. 2. We knowingly and freely assume all risks, both known and unknown. 3. We, for the participant, and on behalf of heirs, assigns, personal representatives, and next of kin, hereby release, hold harmless and promise not to sue the Cataumet Club Inc. or any of its officers, directors, agents or employees with respect to injuries or loss of property from our participation even though such injury or loss of property may arise out of negligence or carelessness on the part of the persons or entities mentioned above. I have read this Release of Liability and Waiver Agreement, fully understand its terms and sign it freely and voluntarily.
