6 Insurance for what is most precious your ability to provide for your family! The reality is that too many Canadians do not have the financial means to see them through an unexpected loss of income. SOLO TM Disability Income pays you monthly income replacement benefits when you are not able to work due to an illness or an accident. This benefit is based on your monthly income and is paid to you for a period of time that you select based upon your financial needs. With SOLO Disability Income, you can focus on getting better!
7 Protects your income in the event of a disability due to an accident or illness Disability Income designed for your reality Monthly benefits from $400 to $9,000 For the first 36 months of your disability, up to $1,200 of your monthly benefit is guaranteed, regardless of any benefit you may receive from the government or other insurance Benefits start after the waiting period, but if you are hospitalized or having day surgery, the benefit will be paid starting on the first day An additional lump sum payment equal to 5 times the monthly benefit in the event of a critical illness such as cancer, stroke or heart attack (25 critical illnesses/ conditions covered) An extra 5% of the monthly benefit will be deposited into a Desjardins Insurance RRSP or savings account of your choice, with the 13th cheque Life insurance equal to 5 times your monthly benefit should you die during a period of disability Features: Waiting period of 14, 30, 60, 90, 120 or 730 days Benefit period of 2 or 5 years, or up to age 65 Premiums adjusted every 10 years or level premium to age 65 Coverage may be extended past age 65 Disability income coverage can be converted into business expense coverage
8 Select from a menu of options: 3 Cost of living: monthly benefit will be increased based on the Consumer Price Index 3 Partial disability: receive monthly benefit payments even during a partial disability or a gradual return to work 3 Residual disability: receive partial benefit payments in case you do not fully recover 3 Refund of premium: refund of 50% of the premiums paid when the contract matures, minus any benefits received 3 Future insurability: option to increase the benefit amount as your income increases, regardless of your state of health 3 Regular occupation period extender: be eligible to continue receiving your monthly benefit, after 24 months of total disability even if you are able to perform another type of occupation SOLO Disability Income allows you to cover your expenses and rehabilitation costs without dipping into your hard earned savings or putting yourself into debt that you may never get out of. Considering how important you are to your family and/or business, you have every reason to protect yourself against the unexpected.

SOLO TM Business Expense pays you a monthly benefit to help cover your overhead costs when you are unable to work due to an accident or illness. By covering expenses such as payroll, rent and property taxes, it allows you to take the time you need to recover. A little planning can go a long way; when you recover and return to work, wouldn t it be great to not have to work twice as much to repay expenses incurred during your absence. Medical and paramedical care: Eligible expenses: Healthcare professionals and alternative medicine practitioners such as massage therapists, chiropractors, physiotherapists and homeopaths Hearing aids, orthotics, insulin pumps, etc. Diagnostic services such as CT scans or MRIs Reimbursement up to $250,000 for all care received, for the term of the contract Vision Care: reimbursement up to $150 per period of 24 consecutive months Travel insurance (30 days): maximum benefit of $5,000,000 per individual
22 2. Health Plus (Enhanced Plan) Same coverage as the basic plan, but with higher allowable reimbursement amounts for eligible medical and paramedical care. Reimbursement of up to $350,000 for all care received, for the term of the contract. If the enhanced plan is chosen, a prescription drug option must also be added. Optional modules (riders) Prescription drugs Atlantic, Ontario and Western provinces Basic Plan: reimbursement of 70% of the first $4,285. Maximum: $3,000 per individual per contractual year. Enhanced Plan: reimbursement of 70% of the first $4,285 and 90% afterwards. Maximum of $25,000 per individual per contractual year. Quebec Reimbursement of 100% of dispensing fees not covered by the RAMQ, including the deductible and coinsurance. Maximum of $25,000 per individual per contractual year. Dental Benefit 2 Basic Plan: Preventive services (examination, cleaning, etc.): reimbursement of 80% of expenses. Basic care (fillings, gingival curettage and root planing [deep scaling below the gum line] and other minor restoration): reimbursement of 50% of expenses. Annual maximum per person for preventive services and basic care: $700 for all consultations. 2 Available in combination with one of the prescription drug options.
23 Enhanced Plan: Preventive services: reimbursement of 100% of expenses. Basic care: reimbursement of 60% of expenses Annual maximum per person for preventive services and basic care: $750 for the first year, $750 for the second year, and $1,000, starting in the third year. Major care (root canals, periodontics, oral surgery, unerupted teeth extractions, dentures, bridges and overlays, veneers, onlays): reimbursement of 60% of expenses. Annual maximum per person: $500, effective the third year. Comprehensive orthodontic care : reimbursement of 60% of expenses. Annual maximum per person: $1,000 for the term of the contract, starting in the third year. Hospitalization Basic Plan: Reimbursement of room costs up to the cost of a semi-private room (maximum of $150 per day). Cash benefit of $25 per day (30-day maximum), starting on the fourth day of hospitalization if a private or semi-private room is not available. Enhanced Plan: Reimbursement of the cost of a room up to $200 per day. Cash benefit of $50 per day (maximum 60 days), starting on the fourth day of hospitalization if a private or semi-private room is not available. SOLO Healthcare is an excellent complement to our other SOLO products. Multi-coverage discount Receive a 5% multi-coverage discount when combining SOLO Healthcare coverage with any other eligible insurance product for an annual premium of $400 or more. 3 3 Certain conditions apply.

This is why SOLO Disability Income includes critical illness coverage equal to 5 times your monthly benefit. It is paid on the diagnosis of one of the 25 illnesses covered by Critical Illness insurance, such as cancer, heart attack, stroke. You don t have to be disabled to be eligible, and you can still be working. You can use this amount as you wish, for example, to pay expenses incurred by your illness or hire replacement staff. 5 times your monthly benefit is only basic coverage, you should consider more You can purchase up to $2,000,000 in Critical illness insurance. You can add a premium refund option to get your premiums back if you remain in good health. It can be purchased jointly with your company, which can be tax-effective. Because life goes on during and after your recovery.
31 LONG-TERM CARE INSURANCE If you loose your independence, you can still retain control and live life the way you want! In the next 20 years, one in five Canadians will be at least 65 years old and 50% of those 65 and older will need long-term care. Source: New England Journal of Medicine. Getting out of bed, bathing and dressing are some of the little things that may become impossible to do without help one day. If you do need long-term care, who would you like by your side to help you every day? Long-Term Care insurance a solution that s within your reach: You ll receive a non-taxable monthly benefit if you can no longer perform at least two of the following activities: bathing, dressing, toileting, transferring, maintaining continence or eating, or if you re suffering from a cognitive impairment that threatens your health or safety (e.g., Alzheimer s disease).
