Introduction A.1 About this Guide This document is intended to be used as a guide for quick reference on the standard benefits offered to the Visa Infinite Cardholders and their beneficiaries, as defined below. Here, the cardholder will find information on emergency services, travel assistance, as well as our diverse portfolio of insurance and coverage offered to protect purchases and Visa Infinite cardholders, including travel accident insurance, baggage delay or loss insurance, purchase protection, among others. The full version of the insurance policy (SUSEP Process No ), executed between Visa International Service Association ( Visa ) and Chartis Latin America, I.I. (SUSEP codes No /00 International Travel, /00 In-Country Travel, / Purchase Protection, / Extended Warranty, / Civil Responsibility, /2012 Special Condition: Auto Rental) is operated in Brazil by Chartis Seguros Brasil S/A, CNPJ/MF No / (hereinafter referred simply as Chartis ). Visa acts as merely the policyholder in the insurance agreement and acts before Chartis on behalf of the insureds, having no obligation to make any payments in their favor. The registration of the insurance policy referred to in this guide with SUSEP does not imply, by the local authority, inducement or recommendation to its commercialization. The benefits referred to in Section C hereof, namely, Compensation for Medical Emergencies, Travel Assistance, and Visa Concierge are offered to the Visa Infinite cardholders and their beneficiaries by AXA Assistance USA, Inc. ( AXA ). The compensation for medical emergencies provided in Section C will be offered during an eligible trip as defined in Section C. Other benefits offered in Section C have worldwide coverage and those benefits stem from a relationship of services rendered which exists between Visa and AXA. Visa is merely taker of the services of AXA and acts before it on behalf of the beneficiaries, with no obligation to making any payments in their favor. 4 4 A.2. Visa Infinite Visa Infinite is Visa s most complete and exclusive card, developed for special customers. It is intended for affluent consumers who are frequently travelling abroad and require the full availability of services, insurance and credit. Visa Infinite offers the inclusion of all the benefits Visa offers. The Visa Infinite Card is accepted in more than 30 million merchants, in 150 countries and territories, and can be used to withdraw cash in local currency in more than one million ATMs. For more information on the benefits provided by your Visa Infinite Card, please access or contact by phone the following numbers through toll free: (Brazil), (USA and Canada), or collect calls: (other countries). A.3. Definitions of the Benefits described in this Guide* (* = specific restrictions may apply. Please refer to the specific section of the benefit in question) BENEFICIARIES: The Visa Infinite CARDHOLDER, his/her spouse and children considered ECONOMICALLY DEPENDENT, minor of 23 years of age, under - the benefits provided for in Section C, subject to the provisions of applicable laws. CUSTOMER SERVICE: Communication channel and available facilities (equipment, systems and staff) which purpose is to make the interface between Visa and customers. It can be accessed through the following telephone numbers: (Brazil), (USA and Canada) and (other countries), and can always be made by collect call. INSURANCE COMPANY: Chartis Seguros Brasil S/A, CNPJ/MF No / INSURED: An active CARDHOLDER, his/her spouse and unmarried dependent children under age 23, under the benefits provided for in Section B. SEVERE WEATHER CONDITIONS: any weather condition that causes a delay in the arrival time and/or departure of a given MEANS OF TRANSPORTATION, as originally contracted with a TRANSPORTATION COMPANY.

5 5 SPOUSE: person related to the CARDHOLDER by marriage or common-law marriage (união estável), under the applicable laws in force. ECONOMIC DEPENDENTS: CARDHOLDER s single children and under 23 years old who cumulatively: (a) reside with the CARDHOLDER or are under his/her responsibility; and (b) are financially supported by the CARDHOLDER. This definition includes the CARDHOLDER s stepchildren. DISMEMBERMENT: loss of members of the body, as well as of sight, speech or hearing. TRANSPORTATION COMPANY: company that operates a MEANS OF TRANSPORTATION, including an AIRLINE COMPANY. AIRLINE COMPANY: company listed in the Official Airline Guide (OAG) or in the ABC World Airways guide and that has a license, certificate or similar authorization for the regular air transportation issued by the competente authorities of the country in which the aircraft is registered and, in accordance with such authorization, maintains and publishes flight schedules and service fees to passengers for flights between airports at regular and specific schedules. EQUIPMENT FAILURE: any sudden and unforeseen damage to the equipment of a TRANSPORTATION COMPANY that causes a delay or interruption of the scheduled COVERED TRIP. benefits presented. CLAIM FORM: document to be supplied by CUSTOMER SERVICE, upon notification of an incident or request for benefit, which must be filled out and returned together with the required documents within the deadlines for each of the MEANS OF TRANSPORTATION: any means of transport operated by a TRANSPORTATION COMPANY under a valid license for paid transportation of passengers, including: AIR: aircraft operated by an AIRLINE COMPANY, duly authorized by the competent authorities to operate regular scheduled flights; LAND AND SEA: all those included in this definition, WITH THE EXCLUSION OF (i) rental vehicles, except those that have been rented by a

6 6 TRANSPORTATION COMPANY for paid transportation of passengers; (ii) taxis, (iii) car rental companies transfers, hotels or parking outside the boundaries of the airport; TRANSFERS used immediately before boarding or after disembarking from an aircraft operated by an AIRLINE COMPANY, provided that they are connected to the COVERED TRIP; and TRANSFERS included in the COVERED TRIP. CARDHOLDER: individual who holds an active and valid Visa Infinite Card, issued in its name in Brazil, as holder or additional holder. COVERED TRIP: an itinerary, in any part of the world, including the Insured's country of residence, regardless of origin or destination. INSURED JOURNEY: an itinerary in any part of the world that is outside the Insured's country of card issuance with a minimum of 150 km from the Insured's place of residence. Both must satisfy any of the following characteristics: full amount of the tickets were paid with a valid Visa Infinite Card; or the tickets were purchased by redeeming points from a loyalty program, provided that the boarding fee and possible taxes due have been paid with a Visa Infinite Card; or if no boarding fee or taxes are levied, just the tickets purchased through redemption of points obtained upon utilization of the Visa Infinite Card. The DEFINITION OF INSURED, COVERED TRIP AND INSURED JOURNEY DO NOT APPLY TO SECTION C OF THIS GUIDE. THE COMPENSATION FOR MEDICAL EMERGENCIES AND TRAVEL ASSISTANCE BENEFIT UNDER SECTION C ARE ONLY APPLICABLE TO ELIGIBLE TRIPS (AS DEFINED UNDER SECTION C) WHICH ARE OUTSIDE THE BENEFICIARY'S COUNTRY OF RESIDENCE AND THE COUNTRY OF CARD ISSUANCE. A.4. GENERAL PROVISIONS OF THE PROGRAM HOW DO I SUBMIT A REQUEST? The CARDHOLDER, the one designated by the CARDHOLDER or the CARDHOLDER s representative, as applicable, should contact the CUSTOMER SERVICE immediately but, in any case, within the time limit established for each of the benefits provided, even if not yet in possession of the documents that will support the CLAIM FORM. Delayed communication may result in the loss of benefit.

7 7 The CUSTOMER SERVICE s operator will answer any of the CARDHOLDER s questions and send a CLAIM FORM. Upon receiving the CLAIM FORM, the CARDHOLDER, the one designated by the CARDHOLDER or the CARDHOLDER s representative, as applicable, must fill out the form and return it along with the required documentation required for each offered benefit. These documents must be submitted up to 90 days (depending on the program) from the initial contact with the CUSTOMER SERVICE to the addresses below: If the request is for the benefits provided for in Section B of this guide (Insurance Offered): Claims Administrator: Visa Card Benefits Administration (VCBA) Maipú 255- Piso 17 C1084 ABE, Buenos Aires, Argentina If the request is for the benefits provided for in Section C of this guide (Other Benefits): AXA 122 S. Michigan Avenue Suite 1100 Chicago, Illinois LOSS OF BENEFITS The INSUREDS shall use their best efforts to avoid or reduce any loss or damage to the insured property under this program. INSUREDS/BENEFICIARIES will lose their right to the benefits and payment of any compensation under the terms of this program if they make any claim they know to be false or fraudulent in any respect. CONVERTING AMOUNTS INTO FOREIGN CURRENCY The foreign currency amounts mentioned in this guide will be converted into Brazilian reais based on foreign currency s sale rate (PTAX 800) published by the Central Bank of Brazil at the day the credit card cycle is closed for the month after the incident occur and is reflect in the Account statement. In the event this information is not available, the FX rate to be used will be the one from the day before the payment of the claim.

8 8 Section B. Insurance Offered This guide contains a summary of the insurance policy executed between Visa and the INSURANCE COMPANY with respect to the insurance coverage offered to CARDHOLDERS. In case of discrepancies between this guide and the policy, the provisions established in the accepted policy and in full force in the jurisdiction in which your Visa Infinite Card was issued will always prevail. In Brazil, the insurance coverage is offered by Chartis Seguros Brasil S/A. Should VISA cancel or refuse to renew any of the insurance coverage offered to CARDHOLDERS, the financial institution that issued the Visa Infinite Card will notify the CARDHOLDERS at least 60 days in advance. Should the INSURANCE COMPANY not renew or cancel any of the coverage provided, CARDHOLDERS will be notified at least 60 days before the expiration of the policy. In the event that a substantially similar coverage comes into force, with no interruptions whatsoever, the notification will not be required. When applicable, the INSURANCE COMPANY will reimburse the INSURED directly, once the process has been completed. No payment will be made until CUSTOMER SERVICE has received all required documentation. If a payment is made, the INSURANCE COMPANY will have the right to be reimbursed for the amounts paid to other parties or persons. Any party or person to whom the INSURANCE COMPANY has made a payment must transfer to it of its/his/her rights to credit that said party or person might have against any other party or person in connection with the portion of the compensation paid by the INSURANCE COMPANY. Said party or person must make all necessary efforts to preserve these rights and refrain from performing any act that may harm them. B.1. Travel Accident Insurance When using Means of Transportation Introduction WE RECOMMEND THAT YOU READ SECTION A. INTRODUCTION" ABOVE BEFORE READING THE ITEM BELOW. IN PARTICULAR, THE READING OF ITEM "A.3. DEFINITIONS FOR THE BENEFITS DESCRIBED IN THIS GUIDE" AND ITEM "A.4. GENERAL PROVISIONS OF THE PROGRAM" IS FUNDAMENTAL FOR THE UNDERSTANDING OF THE CONCEPTS USED HEREINBELOW, AS WELL AS FOR KNOWLEDGE OF THE EXCLUSIONS THAT APPLY TO THIS BENEFIT.

9 9 Basic benefits INSUREDS will be automatically covered worldwide, up to the limit of this coverage, against accidental bodily injuries that are the sole cause of death or DISMEMBERMENT during a COVERED TRIP, while using a MEANS OF TRANSPORTATION operated by a TRANSPORTATION COMPANY, including boarding or disembarking, or the use of transfers. The full fare must be charged to a Visa Infinite Card. AS PROVIDED BY ARTICLE 109 OF DECREE-LAW 2.063/40 AND ARTICLE 8 OF SUSEP S RESOLUTION 302/2005, INSUREDS DO NOT INCLUDE ECONOMIC DEPENDENTS WHO ARE FOURTEEN (14) YEARS OLD OR YOUNGER FOR PURPOSES OF THE COMPENSATION FOR DEATH PROVIDED UNDER THIS INSURANCE. CARDHOLDERS will also be entitled to such coverage when riding a means of transportation, such as buses, provided that carrier is licensed to carry passengers to or from a commercial aircraft, if this means of transport is included in the price of the airline ticket or if the airport provides said means of transportation within its facilities. This insurance is provided at no cost to CARDHOLDERS. In the event the bodily injuries cause death or DISMEMBERMENT, the benefits will be paid as described below: Damage Due Percentage of the benefit Accidental death 100% Loss of both hands and/or both feet, sight of both eyes, speech and hearing in both ears, one hand and one foot or even one hand or foot and sight in one eye Accidental loss of a hand or foot, sight in one of the eyes, speech or hearing in both ears 100% 50% Accidental loss of thumb and index finger on the same hand 25% TO BE UNDERSTOOD AS: Loss of Hand or Foot: depending on the case, loss of limbs beyond the wrist or below the knee; Loss of Sight: total and irreparable loss of vision; Loss of Thumb and Index Finger: loss of limbs beyond the joints at the palm of a hand; and Loss of Speech or Hearing: total and irreparable loss of speech or hearing in both ears. Should the INSURED suffer multiple injuries as a result of a single accident, the CARDHOLDER AND/OR BENEFICIARY/IES will receive the greatest individual benefit applicable. The benefit for the death of the INSURED will be paid to the first surviving BENEFICIARY, as follows: Spouse or, if none;

10 10 Children, in equal shares or, if none; Parents, in equal shares or, if none; Siblings, in equal shares of, if none; or Executor or trustee appointed by the local courts. In the event of injuries resulting from the fact that the INSURED has inevitably been exposed to the accident, benefits will be paid as if they resulted from the injury. The injury must occur within 365 days following the accident. Except for cases in which the INSURED is declared presumably dead by a court of law, the INSURANCE COMPANY will only pay the death benefit if the INSUREDbody fails to be located within one year following the forced landing, shipwreck, sinking or destruction of the MEANS OF TRANSPORTATION in which the INSURED was carried as a passenger, at which time his/her death will be recognized. THIS INSURANCE DOES NOT COVER LOSSES CAUSED BY: EMOTIONAL TRAUMA, PHYSICAL OR MENTAL ILLNESS, PREGNANCY, CHILDBIRTH OR MISCARRIAGE, BACTERIAL OR VIRAL INFECTION (OTHER THAN BACTERIAL INFECTION CAUSED BY ACCIDENT OR BY ACCIDENTAL INGESTION OF A SUBSTANCE CONTAMINATED BY BACTERIA) OR ANY OTHER PHYSIOLOGICAL DYSFUNCTION OF THE INSURED; SUICIDE, ATTEMPTED SUICIDE OR SELF-INFLICTED INJURIES; WAR, DECLARED OR NOT, EXCLUDING GUERRILLA ACTIONS; and INJURIES SUFFERED BY THE INSURED WHILE TRAVELING BY TAXI OR WHILE ENTERING OR EXITING SAID VEHICLE. This is a summary of the insurance policy. For more information on the insurance policy agreement executed between Visa and the INSURANCE COMPANY, including the definitions and exclusions applicable to this benefit and procedure for submitting a claim, please see the Introduction section of this guide. This insurance is effective from July 1, 1997, from the date on which the issuer of your card contracted this coverage or from the date on which your card was issued, whichever occurred last, and will terminate on the expiration date of the policy or on the date on which you cease to be a HOLDER of this CARD, whichever comes first.

Delayed communication may result in the loss of the benefit.

12 12 B.2. Accident Insurance during Stay (Insured Journey) Introduction WE RECOMMEND THAT YOU READ SECTION A. INTRODUCTION" ABOVE BEFORE READING THE ITEM BELOW. IN PARTICULAR, THE READING OF ITEM "A.3. DEFINITIONS FOR THE BENEFITS DESCRIBED IN THIS GUIDE" AND ITEM "A.4. GENERAL PROVISIONS OF THE PROGRAM" IS FUNDAMENTAL FOR THE UNDERSTANDING OF THE CONCEPTS USED HEREINBELOW, AS WELL AS FOR KNOWLEDGE OF THE EXCLUSIONS THAT APPLY TO THIS BENEFIT. INSUREDS will be automatically covered up to the value of benefit against accidental bodily injuries that are the sole cause of death or DISMEMBERMENT arising from an accident that occurs during a INSURED JOURNEY. The full fare must be charged to a Visa Infinite Card. AS PROVIDED BY ARTICLE 109 OF DECREE-LAW 2.063/40 AND ARTICLE 8 OF SUSEP S RESOLUTION 302/2005, INSUREDS- DO NOT INCLUDE ECONOMIC DEPENDENTS WHO ARE FOURTEEN (14) YEARS OLD OR YOUNGER FOR PURPOSES OF THE COMPENSATION FOR DEATH PROVIDED UNDER THIS INSURANCE. Coverage begins when the INSURED disembarks from the MEANS OF TRANSPORTATION at the INSURED JOURNEY s destination, and expires as soon as the -INSURED boards a MEANS OF TRANSPORTATION bound for the final destination, as designated in the ticket. THIS INSURANCE DOES NOT COVER ACCIDENTS THAT OCCUR DURING TRAVEL TO AND FROM AN INSURED JOURNEY WHILE IN, ON, OR ALIGHTING FROM the MEANS OF TRANSPORTATION. This coverage is meant to complement and not duplicate the coverage provided by the Travel Accident Insurance on your Visa Infinite Card. What is the benefit? Should an accidental bodily injury result in death or DISMEMBERMENT within a period of 365 days following the incident, including loss of vision, speech or hearing, the following benefits will be paid: Damage Due Percentage of the benefit Accidental death 100% Loss of both hands and/or both feet, sight of both eyes, speech and hearing in both ears, one hand and one foot or even a hand or foot and sight in one eye Accidental loss of a hand or foot, sight in one eye, speech or hearing in both ears 100% 50% Accidental loss of thumb and index finger on the same hand 25%

In the event of injuries resulting from the fact that the -INSURED has inevitably been exposed to the accident, the benefits will be paid as if they resulted from the injury. The injury must occur within 365 days following the accident. Except for cases in which the INSURED is declared presumably dead by a court of law, the INSURANCE COMPANY will only pay the loss of life benefit if the -INSURED s body fails to be located within one year following the forced landing, shipwreck, sinking or destruction of the MEANS OF TRANSPORTATION in which the INSURED was carried as a passenger, at which time his/her death will be recognized. Should the INSURED suffer multiple injuries as a result of a single accident, the CARDHOLDER AND/OR BENEFCIARY (IES) will receive the greatest individual benefit applicable. The INSURED S death benefit will be paid to the first surviving BENEFICIARY, as follows: Spouse or, if none; Children, in equal shares or, if none; Parents, in equal shares or, if none; Siblings, in equal shares or, if none; Executor or trustee appointed by the local courts. What is not covered by the insurance? 1. INTENTIONALLY SELF-INFLICTED INJURIES; 2. SUICIDE OR ATTEMPTED SUICIDE IN A SANE MENTAL STATE: 3. DISEASES, UNLESS OTHERWISE PROVIDED FOR IN THE INSURANCE POLICIES; 4. BACTERIAL INFECTIONS, EXCEPT FOR BACTERIAL INFECTIONS IN INJURIES OR IF DEATH IS RESULTED BY ACCIDENTAL INGESTION OF A SUBSTANCE CONTAMINATED BY BACTERIA; 5. DAMAGES ARISING OUT OF ACTS OF WAR, DECLARED OR NOT; 6. ACCIDENTS THAT OCCURRED WHILE A PASSENGER IS (I) OPERATING, LEARNING HOW TO OPERATE, OR SERVING AS A CREW MEMBER OF AN AIRCRAFT;

14 14 7. DAMAGES IN WHICH ONE OF THE CONCURRENT CAUSES HAS BEEN THE PRACTICE OF, OR ATTEMPT TO PRACTICE, AN ILLEGAL ACT ON BEHALF OF A BENEFICIARY; 8. PARTICIPATION IN ANY CONTACT SPORT OR TEAM PROFESSIONAL, SEMI- PROFESSIONAL OR COLLEGE SPORT; 9. PARTICIPATION IN EXTREME SPORTS, AS WELL UNDERSTOOD BY SPEED COMPETITIONS WHERE THEY USE MOTOR VEHICLES OR BIKES, PARACHUTING, SKYDIVING, HANG GLIDING, BUNGEE JUMPING, SKI, SCUBA DIVING, CLIMBING, SPELUNKING, OR MOTORCYCLE RACING ABOVE 100CC S; 10. DAMAGES OCCURRED WHILE THE INSURED WAS SERVING IN THE ARMY, NAVY OR AIR FORCE, IN ANY COUNTRY; 11. DAMAGES WHEN UNDER THE INFLUENCE OF DRUGS, ALCOHOL OR OTHER DRUGS, EXCEPT AS PRESCRIBED BY A QUALIFIED PROFESSIONAL AND TAKEN TO TREAT DRUG ADDICTIONS, AS INDICATED IN THE MEDICAL PRESCRIPTIONS; 12. DEPRESSION, ANXIETY, MENTAL OR NERVOUS DISORDERS OR TREATMENTS KNOWN AS "REST CURES"; 13. PREGNANCY AND PREGNANCY-RELATED CONDITIONS, INCLUDING FERTILITY TREATMENT OR OTHER MEDICAL ISSUES RELATED TO A WOMAN S INABILITY TO CONCEIVE; 14. INSURED TRAVELING AGAINST A PHYSICIAN S RECOMMENDATIONS; 15. ANY INJURY, ILLNESS, DEATH, LOSS OR EXPENSE, DIRECTLY OR INDIRECTLY ATTRIBUTABLE TO THE HIV VIRUS AND/OR HIV-RELATED CONDITIONS, INCLUDING AIDS AND/OR ANY MUTATIONS OR VARIATIONS THEREOF OR SOMEHOW CAUSED BY IT; 16. ANY EXPENSES COVERED BY POLICIES AND INSURANCE POLICIES RELATED TO WORKERS COMPENSATION OR EMPLOYEE RISKS. 17. ELECTIVE PLASTIC AND COSMETIC SURGERY, EXCEPT WHEN MEDICALLY REQUIRED AS THE RESULT OF DAMAGES; 18. ANY DAMAGE RESULTING IN A HERNIA; 19. ANY PRE-EXISTING CONDITION. General Provisions of the Program This insurance is effective from July 1, 1997, from the date on which the issuer of your card contracted this coverage, or from the date on which your card was issued, whichever occurred last, and will terminate on the expiration date of the policy contracted with the INSURANCE COMPANY, or on the date on which you cease to be a CARDHOLDER, whichever comes first.

Delayed communication may result in the loss of benefit.

GENERAL PROVISIONS OF THE PROGRAM" IS FUNDAMENTAL FOR THE UNDERSTANDING OF THE CONCEPTS USED HEREINBELOW, AS WELL AS FOR KNOWLEDGE OF THE EXCLUSIONS THAT APPLY TO THIS BENEFIT. Basic benefits CARDHOLDERS can get Car Rental Insurance, at no additional cost, every time they rent a car using their Visa Infinite Card. The CARDHOLDER must decline the Collision Damage Waiver / Loss Damage Waiver (CDW/LDW) insurance or similar coverage offered by the car rental company. Basic requirements To be entitled to this insurance coverage, the CARDHOLDER must: Use the CARDHOLDER s Visa Infinite Card to book and pay the full cost of the car rental. All authorized drivers specifically mentioned in the car rental agreement are also covered. No one other than the CARDHOLDER or authorized drivers, as provided above, will have the right to any coverage, including the right to reimbursement or to submitting any insurance claims and/or damages under or arising from this coverage. Should the CARDHOLDER be unable to decline the car rental company s coverage, this coverage will be secondary to the coverage offered by the car rental company and the CARDHOLDER will have to pay any expenses that exceed the amounts of coverage offered by the car rental company and the damages for which the CARDHOLDER may be possibly held liable. Coverage The Car Rental Insurance covers up to the market value of the vehicle or the cost of repairs or replacement of the rented vehicle, provided that such vehicle is in the possession of the CARDHOLDER or authorized driver,

The following specific costs are included in the insurance policy: Damage due to collision; Theft of the vehicle and related costs; Costs arising from vandalism; Loss arising from accidental fire, provided that the associated responsibility is attributable to the CARDHOLDER or authorized driver. THE FOLLOWING SPECIFIC COSTS AND CLAIMS ARE NOT INCLUDED IN THE POLICY RELATED TO THIS COVERAGE: ANY OBLIGATION ASSUMED BY THE CARDHOLDER OR AUTHORIZED DRIVER(S) UNDER OTHER AGREEMENTS; INJURIES TO ANY PERSON OR DAMAGES TO ANY OBJECT INSIDE OR OUTSIDE THE RENTED VEHICLE; LOSS OR THEFT OF PERSONAL BELONGINGS; PERSONAL LIABILITY; EXPENSES ASSUMED OR PAID BY THE CAR RENTAL COMPANY OR BY ITS INSURANCE COMPANY; THE COST OF THE INSURANCE COVERAGE ACQUIRED THROUGH THE CAR RENTAL COMPANY; ANY OPERATION AND VEHICLE COSTS IN VIOLATION OF THE TERMS OF THE CAR RENTAL AGREEMENT; ANY LOSSES RESULTING FROM INTENTIONAL ACTS, OR FROM THE ADMISSION OF GUILT ON THE PART OF AN AUTHORIZED DRIVER WHO IS PROSECUTED OR FOUND GUILTY BY A COURT OF LAW OF BEING UNDER THE INFLUENCE OF DRUGS OR INTOXICATING SUBSTANCES, OR AS A RESULT OF ILLEGAL ACTIVITIES OR SMUGGLING; GRADUAL WEAR AND TEAR DUE TO NORMAL USE OR MECHANICAL PROBLEMS; LOSSES ARISING FROM ANY TYPE OF HOSTILITY (INCLUDING WAR, INVASION, REBELLION OR INSURRECTION)

19 19 FINES AND PENALTIES IMPOSED UPON THE CARDHOLDER OR AUTHORIZED DRIVER AND ANY EXPENSES RELATED TO CRIMINAL ACTIONS OR PROCEEDINGS; LOSSES RESULTING FROM TECHNICAL/PROFESSIONAL SPECIALIZED SERVICES PROVIDED TO THE VEHICLE AND UNRELATED TO ITS LOCOMOTION; LOSSES OCCURRING WHILE THE INSURED VEHICLE WAS PARTICIPATING IN COMPETITIONS, SPORTING EVENTS, BETS OR RACES; LOSSES RESULTING FROM IONIZING RADIATIONS OR RADIOACTIVE CONTAMINATIONS, FROM ANY TYPE OF RESIDUAL NUCLEAR COMBUSTION; PROPERTY AND PROFIT LOSSES NOT DIRECTLY RESULTING FROM THE MATERIAL AND BODILY INJURIES DAMAGES COVERED BY THE INSURANCE COMPANY; MORAL DAMAGES; and DAMAGES CAUSED BY POLLUTION OR CONTAMINATION OF THE ENVIRONMENT. For more information on the insurance policy agreement executed between Visa and the INSURANCE COMPANY, including the definitions and exclusions applicable to this benefit and procedure for submitting a claim, please see the Introduction section of this guide. What happens if the car rental company insists that the CARDHOLDER buy its insurance? The CARDHOLDER should ask the car rental company s representative to call thecustomer SERVICE using the toll free number assigned to the country, or make a collect call from any country of the world. The CUSTOMER SERVICE will provide a confirmation of the coverage or a coverage certification. Where and when is coverage effective? This insurance becomes effective when the CARDHOLDER rents a vehicle eligible for the coverage; this coverage is available 24 hours a day and will remain in effect while the vehicle is in the possession of the CARDHOLDER or any of the drivers named in the car rental agreement. The coverage will terminate as soon as the rental car is returned to the car rental company. Vehicles rented for rental periods of more than thirty-one (31) days are not covered from the thirty-second (32nd) day on.

20 20 What type of insurance coverage is provided? "Primary" coverage is offered. Should the CARDHOLDER be unable to decline the car rental company s coverage, this coverage will be secondary to the coverage offered by the car rental company and the CARDHOLDER will have to pay any expenses that exceed the amounts of coverage offered by the car rental company and the damages for which the CARDHOLDER may possibly be held liable. What vehicles are covered? Most cars are covered. SOME OF THE RENTED CARS NOT COVERED INCLUDE: LUXURY OR EXOTIC CARS, INCLUDING BUT NOT LIMITED TO ASTON-MARTIN, BENTLEY, BRICKLIN, DAIMLER, DELOREAN, EXCALIBUR, FERRARI, JENSEN, LAMBORGHINI, LOTUS, MASERATI, PORSCHE AND ROLLS-ROYCE; VERY OLD VEHICLES (MORE THAN 20 YEARS OLD OR LEFT TO BE MANUFACTURED IN THE LAST 10 YEARS); TRUCKS, PICK-UPS FOR COMMERCIAL USE, MOTORCYCLES, MOPEDS, MOTORIZED BIKES, LIMOUSINES AND RECREATIONAL VEHICLES. 4x4 JEEPS ARE COVERED, PROVIDED THAT THEY ARE BEING DRIVEN ON ROADS MAINTAINED BY THE GOVERNMENT. MINIVANS TYPE VEHICLES THAT ARE USED TO CARRY MORE THAN 10 PASSENGERS (INCLUDING DRIVER). MINIVANS TYPE VEHICLE NOT DESIGNED TO TRANSPORT PASSENGERS. Should the CARDHOLDER have any questions regarding a specific vehicle, he/she should contact the CUSTOMER SERVICE. What should the CARDHOLDER do in the event of an accident or theft? If the CARDHOLDER is involved in an accident or the rented vehicle is stolen, he/she should contact the CUSTOMER SERVICE immediately. A representative will answer any questions the CARDHOLDER or the car rental company representative may have, and will send a CLAIM FORM to the CARDHOLDER.

21 21 The car rental company might require that the CARDHOLDER pay for the damages with his/her Visa card. Should this occur, the INSURANCE COMPANY will reimburse the CARDHOLDER directly for the amount, after the claim has been processed. The CARDHOLDER, the one designated by the CARDHOLDER or the CARDHOLDER s representative must contact the CUSTOMER SERVICE immediately or within a maximum of 30 days after the date of the accident or theft, even if not yet in possession of the documentation that will support the CLAIM FORM. Delayed communication may result in the loss of the benefit. What does the CARDHOLDER need from the car rental company in order to submit a CLAIM FORM? Upon the occurrence of an accident or return of the rented car on which a claim is being made, the CARDHOLDER must request the following documents from the car rental company: A copy of the accident report and a request form indicating the costs for which the CARDHOLDER is responsible; A copy of the car rental agreement; A copy of the repair estimate and final repair invoice; If available, two photographs of the vehicle showing the damages; For claims including loss of use charges by the car rental company, a copy of the daily utilization record for the loss period, if available; A copy of the police report or accident report (if any). How is the claim submitted? Within the timeframes stated in the CLAIM FORM provided by the VISA representative, the CARDHOLDER, the one designated by the CARDHOLDER or the CARDHOLDER s representative must fill out and sign the CLAIM FORM and send it to the address provided therein along with: A copy of the sales receipt or document showing that the full cost of the booked and rented car was charged and paid with CARDHOLDER s Visa Infinite Card; All of the documentation provided to the CARDHOLDER by the car rental company (see the list of documents) must be submitted to the CUSTOMER SERVICE within the timeframes established in the CLAIM FORM.

22 22 The claim will remain open during a period of 12 months following the date of loss. During that period, the CARDHOLDER must support the claim with all of the required documents. No payment will be made until our CUSTOMER SERVICE has received all the necessary documentation at its offices. What else should the CARDHOLDER do? In normal circumstances, the claim will be paid within a period of 15 days from the date our CUSTOMER SERVICE receives all required documentation. However, as soon as the claim is paid, all rights against any person in connection with the incident will be transferred to the INSURANCE COMPANY. This means that the INSURANCE COMPANY will have the right to file suit on its own and on behalf of the CARDHOLDER. The CARDHOLDER will assist the INSURANCE COMPANY in all manners, as reasonably requested by the INSURANCE COMPANY, in order to help the INSURANCE COMPANY enforce any rights or recourse the INSURANCE COMPANY may have, which includes signing all of documents required by the INSURANCE COMPANY to file a suit on behalf of the CARDHOLDER. This Car Rental Insurance will be applied to all car rental agreements in force before the date of cancelation or non-renewal of this policy, provided that all of the other terms and conditions of the coverage are met. GENERAL PROVISIONS OF THE PROGRAM" IS FUNDAMENTAL FOR THE UNDERSTANDING OF THE CONCEPTS USED HEREINBELOW, AS WELL AS FOR KNOWLEDGE OF THE EXCLUSIONS THAT APPLY TO THIS BENEFIT. The INSUREDS will be automatically covered, up to a maximum of US$500 each, for reasonable unavoidable expenses incurred for the emergency replacement of essential items during a COVERED TRIP if, during said COVERED TRIP, the TRANSPORTATION COMPANY delays delivery of their baggage, misdirects it or temporarily loses it for over six (06) hours, or if baggage delivery is delayed as a result of TRANSPORTATION COMPANY s financial collapse. The full fare must be charged to a Visa Infinite Card. All claims must be submitted to the TRANSPORTATION COMPANY. Conditions ALL CLAIMS MUST BE SUPPORTED BY WRITTEN PROOF FROM THE TRANSPORTATION COMPANY STATING THAT BAGGAGE DELIVERY WAS DELAYED, PLUS ORIGINAL RECEIPTS FOR THE ESSENTIAL REPLACEMENT ITEMS PURCHASED AS A RESULT OF THE LUGGAGE BEING DELAYED. THE BAGGAGE MUST BE DELAYED FOR OVER SIX (06) HOURS DURING a COVERED TRIP; CONFISCATION OR SEIZURE OF BAGGAGE BY CUSTOMS OR OTHER GOVERNMENT AUTHORITY WILL NOT BE GROUNDS FOR A BAGGAGE DELAY CLAIM. What should the INSURED do if the TRANSPORTATION COMPANY loses the INSURED s baggage while it is under its care, control, and custody? The first thing the INSURED should do is contact the TRANSPORTATION COMPANY, preferably before leaving the baggage claim area, or within seven (07) days ofinsured s arrival, and submit a CLAIM FORM supported by the baggage claim receipts. How do I submit a claim? After receiving and filling out the CLAIM FORM, the INSURED must submit this to our CUSTOMER SERVICE, along with the following documentation: Copy of bank or card statement, as well as a copy of the ticket issued by the TRANSPORTATION COMPANY for the portion of the trip for which the luggage has been delayed, so as to prove that the full fare has been paid for with a Visa Infinite Card; Written proof from the TRANSPORTATION COMPANY that the baggage was delayed, and Any additional information to support the claim. The INSURED, the person designated by the -INSURED or the -INSURED s representative must contact our CUSTOMER SERVICE immediately or within no more than 30 days after the date of arrival, even if all the documentation that will support the CLAIM FORM is not yet available. Delayed communication may result in the loss of benefit. General Provisions of the Program This insurance is effective from December 1, 2003, from the date on which your bank has contracted this coverage, or from the date on which your card was issued, whichever occurred last, and will terminate on the expiration date of the policy contracted with the INSURANCE COMPANY, or on the date you cease to be an INSURED, whichever comes first.

The INSUREDS will be covered automatically for the maximum limit of the benefit (see table below) for any baggage that is lost due to theft or misdirection while in the care, custody and control of a TRANSPORTATION COMPANY licensed to transport passengers for a fare, during a COVERED TRIP, provided that the full travel fare has been paid for with a Visa Infinite Card. All claims must be submitted to the TRANSPORTATION COMPANY. What is the maximum amount of reimbursement? The INSURED will be reimbursed up to the maximum amount of the benefit (see table below) for any amount that exceeds the amounts paid or owed by: the TRANSPORTATION COMPANY responsible for the loss, and other valid insurance coverage, if any. Will the coverage still apply if the TRANSPORTATION COMPANY pays a reimbursement? Yes. Any valid claims for lost baggage and personal belongings in excess of the reimbursement paid by the TRANSPORTATION COMPANY will be covered up to the maximum limit of the benefit. What if the INSURED has another valid insurance? The benefit will complement any other INSURED s valid and collectible insurance. If at the time of the occurrence of any loss there is other valid and collectible insurance in place, the INSURED will be reimbursed only for the excess of the amount of loss, over the amount of other insurance and any applicable deductible. How is the amount of reimbursement determined? The INSURANCE COMPANY may choose to: pay for the cost of replacement, or

The INSURED will be reimbursed for up to the maximum amount of the benefit (see table below) for the cost of replacement of the lost baggage and personal effects that is in excess of all other valid and collectible insurance or reimbursement paid or owed by the TRANSPORTATION COMPANY, as long as the baggage loss is confirmed by the TRANSPORTATION COMPANY.

29 29 Once the loss of luggage has been confirmed by the TRANSPORTATION COMPANY and the claim submitted to the TRANSPORTATION COMPANY has been analyzed, the amount of loss in excess of the TRANSPORTATION COMPANY s reimbursement or any valid insurance will be reimbursed to the INSURED up to the maximum amount of the benefit. To this end, the INSURED, the person designated by the INSURED or the INSURED s representative, depending on the case, must notify our CUSTOMER SERVICE immediately or within no more than 30 days after the loss of baggage is confirmed, even if all the documentation that will support the CLAIM FORM is not yet available. How do I submit a claim? After receiving and filling out the CLAIM FORM, the INSURED must submit this to our CUSTOMER SERVICE, together with the following documentation: Copy of bank or card statement, as well as a copy of the ticket issued by the TRANSPORTATION COMPANY for the itinerary in which the baggage has been lost, so as to prove that the full fare has been paid for with a Visa Infinite Card; The documentation related to the final analysis of the claim submitted to the TRANSPORTATION COMPANY, showing that the TRANSPORTATION COMPANY will no longer take any measures to locate the baggage, and therefore recognizing that the loss is irreversible; and Any additional information to support the claim. Who and what is covered? The INSURED will be covered automatically up to US$2,500 for travel and/or accommodation expenses related to a COVERED TRIP for which INSUREDS are legally liable and are not recoverable from any other source if a COVERED TRIP is cancelled between the date of payment of travel and/or accommodation expenses and the date of commencement of the COVERED TRIP in the event of: Serious injury, serious illness, or unexpected death of INSURED, or INSURED s traveling companion or immediate family member; If the BENEFICIARY s place of residence or business has been rendered uninhabitable for 10 days or less prior to the COVERED TRIP as a result of accidental damage; or If the INSURED s presence is being required by the Police following burglary or attempted burglary of the INSURED s place of residence or business. Who is considered a Traveling Companion? A traveling companion is any person who is not an immediate family member of the INSURED with a reservation to accompany the INSURED on the COVERED TRIP.

The INSURED s spouse, children, stepchildren, siblings, parents, parents-in-law, grandparents, grandchildren, legal guardian, adoptive children or stepchildren, stepfather or stepmother, aunts, uncles, nephews and nieces who live in INSURED s country of residence. What is considered a Serious Injury or Illness? The INSURED will be covered for any injury or illness that appears or is contracted between the date of payment of the travel and/or lodging expenses and the date of commencement of the COVERED TRIP, and that is diagnosed as life threatening to the INSURED by a legally qualified physician and that requires medical treatment. What is considered a pre-existing condition? In the case of an injury, a pre-existing condition means a condition that requires medical treatment or for which medical treatment, care or advice has been provided or that has appeared for the first time before the date on which the insurance has become effective. In the case of an illness, a pre-existing condition means a condition that occurred up to 90 days prior to or on the date on which the insurance would have become effective, for which the INSURED has sought or been advised to seek medical treatment by a licensed physician or whose symptoms would have led a prudent person to seek medical diagnosis, care or treatment. A condition will not be considered to be pre-existing if it has been under control for a period of 90 days (with no symptoms or has required no adjustments in treatment or medication) through the use of prescription medications and for which there is no medical restriction for traveling. The CARDHOLDER is provided Extended Warranty Protection, which doubles the free warranty period established under the original manufacturer's written warranty, providing up to one additional year of warranty on eligible products with manufacturer s warranties lasting three years or less. The item must be completely paid for with a Visa Infinite Card. How do I get a repair? To repair an item, the CARDHOLDER should contact our CUSTOMER SERVICE. Are gifts included under this coverage? Yes, as long as the gift has been paid for in full with a valid Visa Infinite Card and meets the terms and conditions of the program What items are covered by this protection? In order to be eligible for this coverage, the full price of the products must have been paid with a valid Visa Infinite Card and the items must have an original manufacturer s written warranty valid in the country in which they were purchased. This extended warranty protection will begin on the expiration date of the original warranty, or depending on the case, on the expiration date of the additional warranty contracted, as provided by Article 2, II, of the SUSEP Directive 388/08. What items are not covered? Boats, automobiles, and any other motorized vehicles and their motors or equipment Any costs other than those specifically covered under the terms of the original manufacturer's written warranty or other eligible warranty. Real estate and items that are intended to become part of the real estate; Items purchased for resale, or for professional or commercial use;

If an additional warranty is contracted at the time of purchase, this benefit will only apply after the expiration of said additional warranty. To submit a claim, the CARDHOLDER must provide proof that the item was purchased with a Visa Infinite Card, together with a copy of the original manufacturer s written warranty, as well of any other applicable warranty How do I submit a claim? The CARDHOLDER, the person designated by the CARDHOLDER or the CARDHOLDER s representative must contact our CUSTOMER SERVICE immediately or within no more than 30 days after noticing a defect in the product, even if all of the documentation that will support the CLAIM FORM is not yet available. Delayed communication may result in the loss of benefit. The CUSTOMER SERVICE will request some preliminary information, provide the contact information for an authorized repair shop and send the appropriate CLAIM FORM. The CLAIM FORM must be filled out, signed and returned with all the supporting documentation within the timeframes established in the form. Unless otherwise provided for, the date of the loss will be the date on which our CUSTOMER SERVICE was first notified of the problem.

39 39 Items given as gift will also be covered. However, the recipient of the gift must provide all of the documents required for supporting the claim. What documents must the CARDHOLDER submit with the claim? The CLAIM FORM, duly completed and signed; Detailed receipt, proving that the item was purchased with a Visa Infinite Card; Detailed sales receipt issued by the store; A copy of the original manufacturer's written warranty, as well as a copy of any other applicable warranty; Product description and serial number, and any other documentation that may be necessary or relevant to support the claim. This includes packaging and, depending on the case, copies of repair records and receipts; The original repair request form; and Proof that the CARDHOLDER is current with all payments to his/her card s issuer at the time of the claim; All claims must be fully supported and corroborated. How will the CARDHOLDER be reimbursed? The Extended Warranty Protection will pay the authorized repair facility directly for repairs, or if this is not possible, will reimburse the cardholder once a claim has been filed and approved, up to a maximum of US$5,000 per incident per carholder account, with a maximum annual limit of US$25,000 per cardholder account for all claims. Under no circumstances shall the amount of reimbursement exceed the amount of purchase. The protection only covers valid and reasonable repairs made at the manufacturer's authorized repair facility.

40 40 In either case, the INSURANCE COMPANY's payment, replacement or repair made in good faith will fulfill the INSURANCE COMPANY's obligation under the program coverage. Should the item need to be repaired, the supplier may, at its own discretion, opt to replace the item. Does the CARDHOLDER need to submit the claim directly to the CARDHOLDER s insurance company? If the CARDHOLDER has purchased a service or extended warranty contract, the Visa Extended Warranty Protection is secondary to that coverage and will only cover the amount in excess of the primary coverage, up to the limit of the secondary coverage. If a product paid for entirely with a Visa Infinite Card is damaged, stolen or lost within 45 days of the date of purchase, there is a good chance that the cardholder is protected. To get coverage Visa s Purchase Protection Insurance covers products paid for entirely with a Visa Infinite Card, whether bought for the cardholders own use or as gifts; The purchases do not need to be registered with Visa. Type of coverage provided Coverage is secondary. Should the cardholder or the recipient of the gift have any other insurance that would cover the eligible purchases (such as homeowner s, renter s, or auto insurance), Visa s Purchase Protection will pay the amount not covered by the primary insurance up to the amount paid by cardholder for the product with his/her card, subject to the limits expressed below; and What is covered Most products paid for entirely with a Visa Infinite Card are covered for a full 45 days from the date of purchase indicated on the bank or credit card statement. Coverage is limited to the cost of the item (excluding delivery and transportation costs) up to US$5,000 per cardholder account per 12 (twelve) month period. In the event of loss, the INSURANCE COMPANY will pay for the smallest of the following amounts: a) the amount of the Protected Purchase charged to the CARDHOLDER s card; or the actual cost of repair of the Protected Purchase or cost of replacement of the Protected Purchase for an item of equal kind and quality. The CARDHOLDER must keep all of the receipts and invoices of all products bought with a Visa Infinite Card. Simply pay for the entire cost of the product with a Visa Infinite Card and, if within 30 days of the date you purchased the product you see either a print advertisement or non-auction internet advertisement for the same product (same manufacturer, model number and model year) for a lower price and in the same country* in which you bought your product with a Visa Infinite Card, you will be refunded the difference, up to US$250 per item (up to a maximum of US$1.000 per CARDHOLDER ACCOUNT per twelve-month period). A maximum of 4 claims on any one type of item may be submitted per CARDHOLDER per twelve-month period. The BENEFICIARIES will use the benefit, either when traveling together or separately. What is considered a MEDICAL EMERGENCY? A sudden, unforeseen and urgent medical condition that requires immediate medical services or treatment or surgical treatment to relieve acute pain and suffering during an ELIGIBLE TRIP and which may prevent the ELIGIBLE TRIP s completion, for which the BENEFICIARY seeks treatment, and for which medical services are paid for in accordance with the terms and conditions of these benefits. How do you get the MEDICAL EMERGENCY benefit? Should the BENEFICIARY need the services of a physician or a medical service provider, our CUSTOMER SERVICE is there to help him/her. The BENEFICIARY needs to contact our CUSTOMER SERVICE immediately or within 30 days followingthe date on which the MEDICAL EMERGENCY or ACCIDENT occurred. What is the amount of the MEDICAL EMERGENCY benefit? Each BENEFICIARY is entitled to receive up to US$ 30,000 ( 30,000 in European Union countries participating in the Schengen Treaty) for the USUAL, REASONABLE AND CUSTOMARY CHARGES incurred for included medical expenses in the event of a MEDICAL EMERGENCY or ACCIDENT during the course of an ELIGIBLE TRIP. All medical expenses included are in excess of any other valid type of insurance that the BENEFICIARY holds. The benefit will not apply in the

54 54 case of CHRONIC ILLNESS or PRE-EXISTING CONDITIONS. Treatment must be received upon the recommendation of a LICENSED PHYSICIAN and provided during the course of an ELIGIBLE TRIP. When are the benefits offered? The benefit will be granted for incidents which have occurred up to the 60 th (sixtieth) day of an ELIGIBLE TRIP. Should the BENEFICIARY be hospitalized, the benefit will be extended until the earlier of the following, whichever comes first: - the maximum limits of the benefit are reached; or - the BENEFICIARY is discharged from the hospital. What are the main characteristics of the MEDICAL EMERGENCY benefit? This benefit is valid anywhere in the world, except for the COUNTRY OF RESIDENCE of the BENEFICIARY or the COUNTRY OF CARD ISSUANCE; The benefit is valid for an ELIGIBLE TRIP for up to sixty (60) consecutive days; The benefit of compensation for Medical Emergencies will be offered up to the maximum limit of US$ 30,000 per BENEFICIARY-; There is no limitation on the number of ELIGIBLE TRIPs; and Benefits in the Schengen area: when the BENEFICIARY travels to any European Union country participating in the Schengen Treaty during an ELIGIBLE TRIP, the maximum benefit for medical expenses will be automatically increased to up to in said country. The CARDHOLDER, the person designated by the CARDHOLDER or the CARDHOLDER s representative must contact our CUSTOMER SERVICE immediately or within no more than 30 days of the occurrence of the ACCIDENT or MEDICAL EMERGENCY. Delayed communication may result in the loss of the benefit. Will the BENEFICIARY have to pay for all of his/her medical expenses? In the event that the BENEFICIARY contacts our CUSTOMER SERVICE due to an ACCIDENT or MEDICAL EMERGENCY, CUSTOMER SERVICE will coordinate the services provided and when possible, pay all of the included medical expenses directly, which will allow the BENEFICIARY to avoid using his/her funds to pay for included medical expenses and have to request reimbursement.

55 55 Payment by CUSTOMER SERVICE for all included medical expenses on behalf of the BENEFICIARY is only possible if CUSTOMER SERVICE is notified at the point of service and certain documentation is provided as requested by CUSTOMER SERVICE, which enables our CUSTOMER SERVICE to contact the hospital or clinic directly and process the payment, so that the CARDHOLDER is not required to make any payments. However, certain restrictions apply based upon the location and the requirements of the medical provider. In the event that the BENEFICIARY is unable to, or chooses not to, contact CUSTOMER SERVICE, he/she should contact a LICENSED PHYSICIAN or a licensed and authorized hospital, pay for the services provided and submit the itemized bills for the included medical expenses to CUSTOMER SERVICE to claim reimbursement. What is included in the Compensation for Medical Emergencies benefit? The expenses covered by this benefit are the medical and prescription expenses incurred by the BENEFICIARY during an ELIGIBLE TRIP following the recommendations of a LICENSED PHYSICIAN. Subject to the benefit limit, as described above, the benefit includes expenses related to: Services of a physician; Hospitalization and use of operating room; Anesthetics (including their administration), x-rays, or treatments and lab exams; Ambulance services, medications, therapeutic services and supplies; and Emergency Dental Care resulting from an ACCIDENT -, up to a maximum amount of US$ 1,000. What hospitals are approved to provide the services? Hospitals approved to provide the services are those that: Hold a valid license (if required by law); Have as their primary business the treatment and care of ill or injured people; Maintain a staff of one or more physicians available at all hours; Provide 24-hour nursing services and have at least one registered nurse at all hours; Have organized facilities for diagnostics and surgery, whether on location or in facilities made available to the hospital as a result of a previous negotiation; and

56 56 Are not, except incidentally, a clinic, nursing home, retirement home or place of convalescence for the elderly or a facility operated as a center for treating drug and/or alcohol addiction. What is not included? Limitations and Exclusions 1. ANY CHRONIC ILLNESS OR PRE-EXISTING CONDITION AND THEIR CONSEQUENCES OR CONSEQUENCES RELATED THERETO AND/OR ANY RELAPSE OR PERIODS OF CONVALESCENCE. FOR PURPOSES OF THIS LIMITATION AND EXCLUSION, CONSEQUENCES OR CONSEQUENCES RELATED THERETO ARE ANY COMPLICATIONS OR EFFECTS RELATED TO THE PRE- EXISTING CONDITION OR CHRONIC ILLNESS THAT MAY HAVE ANY CONTRIBUTION TO THE MEDICAL EMERGENCY. SOME EXAMPLES INCLUDE, BUT ARE NOT LIMITED TO, ADHESIONS FROM A PRIOR RUPTURED APPENDIX CAUSING A BOWEL OBSTRUCTION; FRACTURE ACROSS AN OLD FRACTURE SITE; ACUTE BRONCHITIS IN A PATIENT WITH CHRONIC BRONCHITIS; EMBOLIC STROKE IN A PATIENT WITH STABLE ATRIAL FIBRILLATION; HEART ATTACKS IN THE PRESENCE OF HIGH BLOOD PRESSURE OR HIGH CHOLESTEROL. 2. ANY MEDICAL EMERGENCY RESULTING FROM A AN ELIGIBLE TRIP UNDERTAKEN AGAINST MEDICAL ADVICE OR OCCURRING WHILE A PRE- EXISTING CONDITION WAS BEING TREATED ABROAD. 3. TREATMENT, COMPLICATIONS OR SIDE EFFECTS RESULTING FROM THE INGESTION OF DRUGS FOR MENTAL OR NERVOUS ILLNESS. TREATMENT OF MENTAL ILLNESS OR NERVOUS DISORDERS. CONSULTATION AND TREATMENT DETERMINED BY A PSYCHIATRIST. ILLNESS, INJURIES AND EXPENSES ARISING FROM THE USE OF NARCOTICS, DRUGS OR MEDICATION TAKEN WITHOUT A PRESCRIPTION. TREATMENT OF ALCOHOLISM OR ANY TYPE OF DRUG ADDICTION AND ANY COMPLICATIONS ARISING THEREFROM INCLUDING, BUT NOT LIMITED TO, SERVICES OR PROGRAMS FOR TREATING ALCOHOLISM AND DRUG ADDICTION. 4. ANY COMPLICATION DUE TO PREGNANCY, CHILDBIRTH, MISCARRIAGE OR ABORTION, INCLUDING THOSE THAT OCCUR DURING THE FIRST TRIMESTER OF PREGNANCY. 5. TREATMENT OF ANY ILLNESS OR SELF-INFLICTED INJURY, SUCH AS SUICIDE OR ATTEMPTED SUICIDE, AND ANY CONSEQUENCES THEREOF, WHICH OCCURRED WHEN THE BENEFICIARY WAS IN A SOUND OR UNSOUND STATE OF MIND. 6. BEING UNDER THE INFLUENCE OF DRUGS OR NARCOTICS, EXCEPT WHEN THEY HAVE BEEN PRESCRIBED BY A LICENSED PHYSICIAN.

57 57 7. TREATMENT AND/OR COMPLICATIONS DUE TO THE HUMAN IMMUNODEFICIENCY VIRUS (HIV) OR ACQUIRED IMMUNODEFICIENCY SYNDROME (AIDS), AND AIDS RELATED COMPLEX (ARC). TREATMENT OF SEXUALLY TRANSMITTED DISEASES. 8. CHIROPRACTIC TREATMENT; HOMEOPATHIC TREATMENT; ACUPUNCTURE, OCCUPATIONAL THERAPY, PHYSICAL THERAPY. 9. ANY TREATMENT OR SERVICE PROVIDED BY REHABILITATION INSTITUTIONS OR TREATMENT CENTERS, GERIATRIC INSTITUTIONS OR TREATMENT CENTERS, AND BY INSTITUTIONS THAT ARE NOT HOSPITALS INCLUDING, BUT NOT LIMITED TO, THERMAL BATHS, SPAS AND HYDROTHERAPY CLINICS. 10. ANY DENTAL OR ORTHODONTIC TREATMENT, EXCEPT IN THE EVENT OF EMERGENCY DENTAL CARE. 11. DIAGNOSTIC EXAMS OR TESTS THAT ARE PART OF A ROUTINE PHYSICAL EXAMINATION OR SCHEDULED TREATMENT, INCLUDING, BUT NOT LIMITED TO: VACCINES, ROUTINE EYE AND EAR EXAMS, OPTOMETRY AND VISION CORRECTION, EYEGLASSES, CONTACT LENSES, HEARING AIDS AND ANY KIND OF MAINTENANCE OR ADJUSTMENT THEREOF, PROSTHETICS, PURCHASE OR RENTAL OF HUMIDIFIERS, ATOMIZERS, WALKERS OR CANES, INHALERS, EXERCISE EQUIPMENT OR SIMILAR EQUIPMENT. 12. COSMETIC PLASTIC SURGERY, EXCEPT FOR RECONSTRUCTION SURGERY PERFORMED AS A RESULT OF AN ACCIDENT OR MEDICAL EMERGENCY. 13. ORGAN TRANSPLANTS OR THE TRANSPORT THEREOF; 14. FOOT TREATMENTS INCLUDING, BUT NOT LIMITED TO, CALLUSES, CORNS, FLAT FEET, WEAK ARCHES, CONGENITAL DISEASE AND ANY TYPE OFPROSTHESIS. 15. TREATMENT OF SEXUAL DYSFUNCTION OR SEXUAL ISSUES. SERVICES OR CARE RELATED TO MALE OR FEMALE STERILIZATION. SERVICES OR CARE RELATED TO BIRTH CONTROL.

58 ANY TREATMENT THAT IS NOT RELATED TO AN ACCIDENT OR A MEDICAL EMERGENCY. 17. ANY KIND OF SERVICE OR CARE RECEIVED BY THE BENEFICIARY RELATED TO WEIGHT CONTROL OR THE TREATMENT OF OBESITY, WHETHER IN THE FORM OF DIET, LIQUID INJECTION, MEDICATION OR SURGERY OF ANY KIND. 18. TREATMENT OF ANY MEDICAL EMERGENCY, ACCIDENT OR INCIDENTAL EXPENSES RESULTING DIRECTLY OR INDIRECTLY FROM THE BENEFICIARY S INVOLVEMENT IN WARS, REVOLUTIONS, CIVIL UNREST, CONSPIRACIES, RIOTS, PUBLIC DISTURBANCES, OR ANY CRIMINAL OR ILLEGAL ACTIVITY, WHETHER AS A PRINCIPAL, AID, OR IN ANY OTHER CAPACITY, INCLUDING, BUT NOT LIMITED TO, PHYSICAL FIGHTS INITIATED BY THE BENEFICIARY. 19. HOTEL, RESTAURANT OR TAXI EXPENSES. 20. ANY COSTS OF MEDICAL CARE AND/OR MEDICAL TREATMENT INCURRED BY THE BENEFICIARY AFTER THE END OF THE ELIGIBLE TRIP, RELATED OR UNRELATED TO AN ACCIDENT OR MEDICAL EMERGENCY THAT APPEARED OR WERE DIAGNOSED WHILE THE BENEFICIARY WAS TRAVELING. 21. WORK-RELATED DISEASES OR THEIR CONSEQUENCES. 22. ANY TREATMENT OR SERVICE THAT IS NOT SPECIFICALLY PROVIDED FOR UNDER THIS BENEFIT. 23. CIRCUMSTANCES RESULTING FROM OR IN ANY WAY RELATED TO FORCE MAJEURE, SUCH AS STRIKES, ACTS OF SABOTAGE, CIVIL OR INTERNATIONAL WARS, RIOTS, TERRORISM, ETC. 24. MEDICAL RECORDS FEES. 25. THE COSTS OF A FUNERAL. 26. ANY MEDICAL TREATMENT, MEDICAL CHECK OR MEDICAL SERVICE KNOWN BEFORE HAND, OR MEDICAL EXPENSES INCURRED WHERE TRAVEL HAS BEEN UNDERTAKEN FOR THE SPECIFIC PURPOSE OF SECURING MEDICAL TREATMENT. 27. TREATMENT OF A MEDICAL EMERGENCY OR FROM AN ACCIDENT RESULTING FROM THE PRACTICE OF A PROFESSIONAL SPORT OR OF ANY SPORT OR ACTIVITY THAT MAY POSE A RISK TO THE BENEFICIARY S LIFE, RESULTING OR NOT FROM THE BENEFICIARY S OWN IRRESPONSIBILITY, LACK OF SKILL, OR LACK OF EXPERIENCE, INCLUDING, BUT NOT LIMITED TO: SKYDIVING, PARAGLIDING, HANG-GLIDING, RAFTING, DRIVING UNLICENSED OFF-ROAD VEHICLES, MOUNTAINEERING, EQUESTRIAN SPORTS, WINTER SPORTS NOT

59 59 PRACTICED IN A RESORT AND UNDER NORMAL SAFETY CONDITIONS, CAR RACING, MOTORCYCLE RACING AND ANY TYPE OF RACES EXCEPT FOR FOOTRACES, AND UNDERWATER ACTIVITIES. 28. TREATMENT OF A MEDICAL EMERGENCY OR FROM AN ACCIDENT RESULTING FROM THE FAILURE TO ABIDE BY LOCAL LAWS, RULES, REGULATIONS OR NORMAL SAFETY STANDARDS. The BENEFICIARY is also entitled to the following services worldwide: Emergency Medical Evacuation: up to US$ 100,000 to include expenses incurred during the course of an ELIGIBLE TRIP should the BENEFICIARY S MEDICAL EMERGENCY require an emergency medical evacuation, due to a lack of appropriate medical facilities at the site where the ACCIDENT or MEDICAL EMERGENCY occurred. CUSTOMER SERVICE will coordinate and pay for USUAL, REASONABLE AND CUSTOMARY EXPENSES incurred for emergency medical evacuation to the nearest medical facility equipped to provide the adequate emergency treatment; Repatriation of Mortal Remains: up to a maximum of US$ 100,000 for USUAL, REASONABLE AND CUSTOMARY EXPENSES in the event of the BENEFICIARY s unexpected death during an ELIGIBLE TRIP. Our CUSTOMER SERVICE will secure the proper governmental authorizations and pay for the USUAL, REASONABLE AND CUSTOMARY EXPENSES to preserve and bring the BENEFICIARY S mortal remains and the required COUNTRY OF RESIDENCE casket for repatriating the mortal remains to the BENEFICIARY s COUNTRY OF RESIDENCE. THE COSTS OF A FUNERAL ARE EXCLUDED; Early Return: up to a maximum of US$ 10,000 to pay for the additional charge imposed by the TRANSPORTATION COMPANY for a ticket change requested for the early return of the BENEFICIARY due to the unexpected death, or MEDICAL EMERGENCY of his/her traveling companion or immediate family member (ascendant or descendent up to the first generation and collateral up to the second generation). If the ticket is not reimbursable and there is no way to change the flight, this benefit will cover an economy class ticket whose cost is lower than the maximum benefit indicated herein. CUSTOMER SERVICE must coordinate and make all the necessary arrangements to bring the BENEFICIARY home; Trip Interruption: up to a maximum of US$ 1,000 for the BENEFICIARY S loss of unused pre-paid travel and/or lodging expenses (that are not eligible for reimbursement, either in cash or credit), in the event that an ELIGIBLE TRIP is interrupted due to an ACCIDENT or MEDICAL EMERGENCY of the BENEFICIARY or BENEFICIARY s traveling companion or immediate family member (ascendant or descendent up to the first generation and collateral up to the second generation); Convalescence: up to a maximum of US$ 1,000 (US$ 200 per day) to meet the costs of a hotel room if it is determined that the BENEFICIARY needs convalescence as a result of an

60 60 included hospitalization, due to an ACCIDENT or MEDICAL EMERGENCY. The BENEFICIARY will receive or be reimbursed for the costs that exceed the USUAL, REASONABLE AND CUSTOMARY CHARGES of a hotel room only in case the BENEFICIARY has been hospitalized for five (05) consecutive days or more; Emergency Family Travel: up to a maximum of US$ 10,000 if the BENEFICIARY is hospitalized for more than 5 days, provided that said hospitalization is included in this protection, while undertaking an ELIGIBLE TRIP with no companion. Arrangements will be made and the cost of a round- trip economy class air ticket will be paid for so that a family member or a person designated by the CARDHOLDER may stay with the CARDHOLDER. OTHER EXPENSES INCURRED BY THE FAMILY MEMBER OR BY THE PERSON DESIGNATED BY THE CARDHOLDER WILL NOT BEINCLUDED Maximum benefit will not exceed the amount of US$ 10,000 per 12-month period; Return of an Elderly Traveling Companion or Minor: up to a maximum of US$ 10,000 to include the cost of a BENEFICIARY, or the cost for a person who will accompany the minor or elderly dependent during the trip back home, in the event that the BENEFICIARY is hospitalized in a foreign country, except the COUNTRY OF RESIDENCE or COUNTRY OF CARD ISSUANCE for more than five (05) days during an ELIGIBLE TRIP; Emergency Dental Care: up to a maximum of US$ 1,000 to include the cost of emergency dental treatment, understood as an accidental injury to healthy and natural teeth, that requires immediate treatment for temporary relief of pain and acute suffering; Out of-pocket Prescription Expenses: up to a maximum of US$ 1,000 to meet prescription expenses prescribed by a LICENSED PHYSICIAN which are incurred for treating an ACCIDENT or MEDICAL EMERGENCY; and Legal Expenses: reimbursement, after a determination that the benefit is included, up to a maximum of US$ 1,000 to include legal expenses, should the BENEFICIARY incur this type of expense during an ELIGIBLE TRIP as the result of a false arrest or wrongful detention by any government or foreign power. What happens if the MEDICAL EMERGENCY is so severe that it results in death or requires an Emergency Medical Evacuation? In the event of the BENEFICIARY s death during an ELIGIBLE TRIP, our CUSTOMER SERVICE will secure all of the necessary permits and pay the costs of transporting the mortal remains and the necessary casket for the return of the BENEFICIARY s remains to his/her COUNTRY OF RESIDENCE, up to a maximum of US$ 100,000. THE COSTS OF A FUNERAL ARE EXCLUDED. If a LICENSED PHYSICIAN certifies that the seriousness or nature of the BENEFICIARY s illness or injury requires an Emergency Medical Evacuation, and provided that this evacuation is approved and authorized by CUSTOMER SERVICE upon consultation with the attending physician, the BENEFICIARY may count on up to US$ 100,000 for expenses incurred in the evacuation during the course of an ELIGIBLE TRIP.

61 61 Does the Emergency Medical Evacuation need to be pre-approved? Yes, the evacuation must be approved and authorized by CUSTOMER SERVICE upon consultation with the LICENSED PHYSICIAN in charge, who will certify that the ACCIDENT or MEDICAL EMERGENCY requires the Emergency Medical Evacuation. The BENEFICIARY must obtain preapproval from our CUSTOMER SERVICE, who will then make arrangements for the Emergency Medical Evacuation. Under what conditions is the provision for Emergency Medical Evacuation granted? The BENEFICIARY s medical condition must require immediate evacuation from the place of ACCIDENT or MEDICAL EMERGENCY to the nearest hospital equipped to provide the medical treatment he/she needs; and/or if after being treated at a local hospital, his/her medical condition requires transport to the nearest qualified medical facility or to his/her COUNTRY OF RESIDENCE for furtherhospitalization. What type of transportation will be provided in the event of an Emergency Medical Evacuation? Any means of land, water or air transportation needed to transport the BENEFICIARY during an Emergency Medical Evacuation will be provided. Special transportation includes, but is not limited to, air ambulances, land ambulances and/or private cars. Does the Repatriation of Mortal Remains require pre-approval? Yes, first and foremost, our CUSTOMER SERVICE must be contacted, and provided that the claim is approved, CUSTOMER SERVICE will make all of the arrangements for the Repatriation of Mortal Remains. Is pre-approval required in the case of an Early Return? Yes. Before using the Early Return benefit, the BENEFICIARY must obtain pre-approval from our CUSTOMER SERVICE, who will then make arrangements for his/her transport. How can the CARDHOLDER obtain a certificate of benefits during an ELIGIBLE TRIP to European countries that participate in the Schengen Treaty? The CARDHOLDER must call our CUSTOMER SERVICE to request a certificate. This certificate will be sent by fax or at no additional cost to the CARDHOLDER. General Definitions USUAL, REASONABLE AND CUSTOMARY CHARGES: means that the benefit to the BENEFICIARY will include medical treatment, supplies and services that are medically necessary, provided that they do not exceed the typical or standard charges for similar medical

62 62 treatment, supplies or services in the community or the locality where the medical treatment is provided. CHRONIC ILLNESS: means any diagnosed illness or abnormal physiological condition that exists over time and is unresolved or recurrent (or thought to be resolved or cured). Some examples include, but are not limited to: cancers; fractured bones, whether repaired or not: diabetes: hypertension: atherosclerosis, coronary artery disease; osteoporosis; chronic lung disease; asthma; and chronic adhesions after abdominal infection or surgery,or any other illness or condition that falls into the definition of chronic illness, determined by a physician. COUNTRY OF RESIDENCE: means the country in which the BENEFICIARY keeps his/her effective, fixed and permanent home and main place of residence and to which the BENEFICIARY intends to return. COUNTRY OF CARD ISSUANCE: means the country in Latin America or Caribbean where a BENEFICIARY had his/her VISA card issued by a bank. ELIGIBLE TRIP: an itinerary, outside of the BENEFICIARY s COUNTRY OF RESIDENCE and COUNTRY OF CARD ISSUANCE, which satisfies any of the following characteristics: full amount of the tickets were paid to a TRANSPORATION COMPANY with a valid Visa Infinite Card; or the tickets were purchased by redeeming points from a loyalty program, provided that the boarding fee and possible taxes due have been paid with a Visa Infinite Card; or if no boarding fee or taxes are levied, just the tickets purchased through redemption of points obtained upon utilization of the Visa Infinite Card. The benefits provided in this Section C are only valid for an Eligible Trip for up to sixty (60) consecutive days of such Eligible Trip. ACCIDENT: means a sudden, unexpected, uncontrollable and unexpected physical event that happens to the BENEFICIARY due to external, violent and visible causes during an ELIGIBLE TRIP. SCHENGEN CERTIFICATE: means that when the BENEFICIARY travels to any European Union country participating in the Schengen Treaty during an ELIGIBLE TRIP, the maximum benefit for medical expenses will be automatically increased to up to in the event of a MEDICAL EMERGENCY occurring at said country. MEDICAL EMERGENCY: a sudden, unforeseen and urgent medical condition that requires immediate medical services or treatment or surgical treatment to relieve acute pain and suffering.

63 63 PRE-EXISTING CONDITION: means any condition that occurs within one hundred eighty (180) days of the date on which the individual s benefit will become effective (inclusive) for which the BENEFICIARY has sought or has been advised to seek treatment by a LICENSED PHYSICIAN or for which there are symptoms that would lead a prudent person to seek diagnosis, care or treatment. LICENSED PHYSICIAN: means a professional with a degree in medicine or doctor of osteopathy qualified to provide medical services or perform surgeries in accordance with the laws of the country where these professional services are performed. THIS DEFINITION EXCLUDES CHIROPRACTORS, PHYSICAL THERAPISTS, AND HOMEOPATHIC AND NATUROPATHIC THERAPISTS. REWARDS PROGRAM: means a program developed/offered by an International Visa Card and its Issuing Bank allowing you to obtain value (mileage points, cash, etc.) and redeem rewards (goods, trips, etc.) under your eligible Visa Rewards Program or any frequent traveler's rewards program or additional tickets on which taxes and fees have been charged at the time of issuance of a ticket paid with an International Visa Card. How do I submit a claim? The CARDHOLDER, the person designated by the CARDHOLDER or the CARDHOLDER s representative must contact our CUSTOMER SERVICE immediately or within thirty (30) daysof the date of the ACCIDENT or MEDICAL EMERGENCY. Delayed communication may result in the loss of benefit. Timeframe for submitting documentation: No more than ninety (90) days from the notification date. The documents required for compensation of the medical emergencies benefit are: Copy of your Visa Infinite Card statement, copy of the ticket for air, sea or land transportation related to the JOURNEY and proof that it was entirely paid for with the CARDHOLDER S Visa Infinite Card. Copy of receipts provided by your medical service provider. Proof of travel dates Copy of the documentation pertaining to any other valid and collectible insurance. Copy of all medical reports, including the name and address of the medical facilities used and the physicians who provided the services. CLAIM FORM, duly completed. In the case of Repatriation of Mortal Remains, a notarized copy of the death certificate and itemized bills for the services contracted.
