2 Instruction for Reporting to TMT TROUBLE REPORT (For NYK owned vessel) MARTM Revision: 4 Date: 2005/07/01 Page No.:1 TROUBLE REPORT Trouble Report should be submitted to us after the occurrence of the trouble/damage by FAX and/or (To: Martm and Martanker) as soon as possible. The first report should be informed us the estimation LOT (Loss of Time) using this form. After the second information, it is allowed to be reported us without using this form at any time. Final trouble report should be submitted to us after the trouble has been settled with Fixed LOT. 1. TROUB TROUBLE REPORT AA) A Trouble Report should be submitted to us when any of the following troubles, Incidents, and/or damage has occurred: 1) Loss of Time against a vessel s schedule that have taken hour to manage & repair for trouble more than 1 hour. 2) Major Damage that affects a vessel s seaworthiness. 3) Any trouble occurring with critical machinery, such as M/E, D/G, Boiler, Steering gear, Windlass, Winch, Navigational Equipment, and such. 4) Any trouble that hinders a vessel s safe navigation or safe machinery operation. BB) A Trouble Report (First Notice) that reports an outline of the trouble including an estimated LOT(Loss of Time) should be submitted to us as soon as possible when any one or more of the above troubles have occurred. Afterward, please inform us of any further progress in the situation by Trouble Reports (Interim) as follow-up information in due course. CC) A Trouble Report (Final) should be submitted to us when the trouble has been resolved. This report should include the fixed LOT & Stopping Time, the cause of the trouble, suitable countermeasures taken, and so on. DD) A Trouble Report should be submitted to us with this form which is made by EXCEL sheet without any modification by WORD and other application software in order to easy mention on the Status form of Trouble Report by copy & paste some mentioned words and phrases in the column of Trouble Report.

Form of TROUBLE REPORT Refer to the attached SAMPLE. 1) Serial No. : this should describe the ship s name, the year of occurrence, and the number(first, second, third ) of the reported trouble. 2) Time & Date of Occurrence : in this column should be mentioned not only the date, but also the Local Time & GMT. 3) Place of Trouble : the position is to be mentioned if the trouble occurs at sea. 4) ETD & ETA : they are to be mentioned if the trouble might affect them. 5) Loss of Time : LOT is to be mentioned if the vessel s schedule might be affected by treatment of the trouble. 6) Stopping Time : this should mention the actual amount of time affected by treating the trouble. 7) PRIORITY check box 8) URGENT check box 9) If the following countermeasures are required for treatment of the trouble, mark a check in the appropriate row: (1) Revision of Planned Maintenance System (2) Docking Repair (3) Need Continuous Observation (4) Need Maker s Investigation 10) Equipment Code and Cause Code are to be mentioned with attached Code Tables. 11) Class Status column( including Due date) A recommendation and its due date are to be mentioned if a recommendation is indicated by Class Survey. 12) Operational Restriction (including Cylinder Cut Operation check box) The situation restricted is to be mentioned in if the operation might be restricted by troubles. 13) Expected Date for the following items on REMARKS column (1) Delivery of Spares (2) Completion of the treatments (3) Closure of TR

9 Instruction for Reporting to TMT Maiden Voyage Report for newly built vessel (For NYK owned vessel) MARTM Revision: 4 Date: 2005/07/01 Page No.:8 Maiden Voyage Report for newly built vessel Maiden Voyage Report should be submitted to the NYK Line Technical Group (Maritime Technology team) by mail within 6 months (from3 to 6 months of services) after delivery of the vessel. 1. The purpose of this report. a) To verify that the vessel s performance on her maiden voyage meets the relevant specifications and requirements. b) To make possible feedback (on the findings) to the next newly built vessels. 2. Procedure a) NYK Line as the owner will send the Maiden Voyage Report with the example to the newly building at a shipyard directly. b) After making of the report, Master and Chief engineer submit it to the NYK Line Technical Group (Maritime Technology Team) directly. * Write the report in English in principle. However, if the Master and Chief engineer are Japanese, it is possible to make the report in Japanese along with English. 3.Contact Details Please do not hesitate to contact us if you have any questions. 10 September 27, 2004 MARTM/ To All Ship Managers Dear Sirs: Re: Revision of the TROUBLE REPORT form and introduction of the new STATUS OF TROUBLE REPORT As always, we are very grateful for everything you do to facilitate the safe operation of ships under your management. We would like to inform you of a revision to the TROUBLE REPORT form (From FORM No.R to R-004-8) and introduce the new STATUS OF TROUBLE REPORT form as per the attached FORM No. R The details are as follows; 4. New Form STATUS OF TROUBLE REPORT Please refer to the attached SAMPLE & GUIDANCE for the new form (FORM NO.R-005-1). When you receive this MARTM letter, the new reporting system is to be commenced for this entire fiscal year. Therefore, the first report, the one submitted after the end of September, should cover the first two fiscal quarters (from April 1 to September 30) and also any items pending from the previous fiscal year.

15 Guidance for "Status of Trouble Report" 1. Purpose The purpose of this report is to help us to better understand the status of each Trouble Report and the cost required to address each trouble. 1) To ensure the closing of the trouble by entering the submitted date of Final Report 2) To understand percentage of repair cost relating with the trouble and total cost for corrective maintenance by entering Repair Budget 3) To better understand the outline of Special Works due to the trouble on the Dock Repair Form (Form No.D ) and to study a suitable repair plan in advance, by entering Docking Repair on Repair Status Column 4) To better understand the status of Class recommendation and Insurance claim relating with the trouble 5) To understand the performance of Crew and SI against the trouble by Repair Status and Special Comment columns 6) To better evaluate the ship management company by figures providing the annual number of troubles and the percentage of troubles being settled 2. Reporting Procedure 1) Please send the report every quarter (at the end of June, September, December, and the following March). The report submitted in March should be an Annual Report. 2) In the Annual Report, please attach the report form for the next year and transfer items pending from the current year. 3) Please report updates and revisions when they occur. Although official reporting is to be carried out four times a year, the status record should be revised in a timely manner and reported to us as soon as possible. 4) Please send the report form by Recording Procedure Please refer to the following instructions and the attached Sample to fill in the form "Status of Trouble Report". (1) How to make the form for each vessel 1) To begin with, make a copy spread sheet of the blank status form enclosed this file for each your managed vessel. 2) Fill in the vessel name in the tub section at the bottom of the spread sheet. 3) Fill in the particulars for each vessel in the yellow rows on the spread sheet. a) Vessel Name; b) Name of Ship Manager; c) Name of SI; d) Contact Address; e) Budget for Running Repair; f) Budget for Spare Parts; g) Next Dock Schedule 4) Fill in pending cases from the previous year. The serial number column should be highlighted in blue to distinguish it from current year cases. 5) Fill in current year cases thereafter. (2) How to fill in each column (see "Sample") 1) Serial No. This should be the same serial number as that on the Trouble Report. This will allow us to identify the year & the vessel name. 2) Equipment Code Please use the equipment codes attached. 3) Title of Trouble Please copy this from the Trouble Report. 4) First Date and Final Date The First Date is the date on which the trouble occurred and the Final Date is the date on which the Final Report was submitted. 5) Stopping Time This should be actual stopping time of M/E & other for repair or detention period. 6) Cost of Repair Please enter an estimate for repair cost. When actual cost is confirmed, please enter Final Parts Cost and Final Repair Cost in the appropriate columns separately. If the repair work is a Guarantee Claim (G) or a Stevedore Damage Claim (S), please enter the appropriate letter, "G" or "S," in the Estimate column.

16 7) Repair Status In the upper row of this column please enter the status of the repair work, and in the lower row of this column please enter the date on which the repair work was carried out or is planned. Upper Row: Temporary Repair, Investigation by Maker, Repair by Maker, Observation, etc. Lower Row: If you plan repair work at the dock, please highlight this row in purple. 8) Class Status Please enter Recommendation(in the upper row) and its due date(in the lower row) or Completion of survey(in the upper row) and its date(in the lower row). 9) Insurance Status If the repair work is associated with an insurance claim, please enter the type of insurance (H&M or P&I) in the upper row of this column. In the lower row of this column please enter the date on which the claim was settled. Repair Cost associated with an insurance claim should not be added to the Cost of Repair. 10) Unsettled Account In the upper row of this column please enter the Accident Code noted on the SU form. In the lower row of this column please enter the current settlement cost. When the claim settlement is finalized, please enter the final claim amount from the insurer and highlight this column in red. Please provide a comment column and enter the recoverable amount, deductible amount, and portion of repair cost. 11) Total Trouble Recovering Time & Costs The rows highlighted in yellow contain budgeted values input at the beginning of the year. The other columns are calculated automatically. 12) Annual Report on the Status of Troubles Please enter the appropriate number for each item. You should note in parentheses the number pending from the previous year. 13) Comments Please feel free to provide comments. (3) How to close a case by highlighting the relevant column (see "Sample") 1) Please close a case by highlighting it in blue when a Final Date (the date on which you submitted the Final Report) is input. However, in the following cases you should highlight in blue from the Serial No. column to the Final Date column. Please do not highlight any of the other columns and you should not treat this case as completely closed. 2) The Cost of Repair columns are to be highlighted in blue as closed when actual final costs are entered in the Final Spare & Final Repair columns. 3) The Repair Status column should not be highlighted in blue if this column is highlighted in purple for repairs to take place at dock. 4) The upper and lower rows of the Insurance Status column should be highlighted in red after inputting the date on which the claim was settled.

17 G uidance for "S tatus of T rouble R eport" 1. 目的本報告システムは 以下の手法により当方及び管理会社担当者が各 TroubleReportの対応進捗状況及びトラフ ルによるコの大略を把握し易くするために実施する 1)FinalReport の提出日を記載し 消し込みを実施する事によりトラフ ル対応を確実に完結させる 2) 確定コストの記載により 修繕費予算の消費状況及び修繕費に於けるトラフ ルに因る事後保全費用割合が大略把握出来る 3) トラフ ルに因る入渠時 SpecialWorksの事前把握及び検討 4)Class 案件及び保険案件把握の容易化 5)RepairStatus 及びSpecialComment 欄によるCrew 及びS/Iのトラフ ル対応に対する認識の把握 6) 各船の年間トラフ ル発生件数及び解決率等統計テ ーターの把握により 管理会社評価の一助とする 2. 報告方法 1) 本報告 Formは 原則として年度間 4 回 ( 6, 9, 1 2, 3 月末日締め ) とし 3 月末の報告を年次報告とする 2) 年次報告には次年度 Formを添付し 此れにUn-FinalReportsを記載し継続させる 3) 上記 1) に定めた如く原則年 4 回の報告だが 管理会社は随時このForm でStatusの変化を報告することが出来る 4) 本 Formの提出は の添付 Fileにて実施する 3. 記載方法本 ''Status of Trouble Report''FORM 記載に際しては以下及び添付 SAMPLEを参照 (1) 各船毎のForm 作成添付 StatusFormを必要数コヒ ーし 貴管理船毎に以下を入力して各 Sheetを作成する 1)Index タグに船名を記載 2) 以下の黄色網掛け空欄に各船毎 Dataを記載 1 船名 2 管理会社名 3S/I 氏名 4 連絡先 5 修繕費予算額 6 部品費予算額 7 次回入渠予定日 3) 前年度以前発生したUn-FinalReport の記載 ( 本年度分と識別の為 SerialNo. 欄に青色網掛け実施 ) 4) 以下に従い今年度に発生したTroubleReport につき記載 (2) 各欄の具体的記載方法 ( サンフ ル参照 ) 1) Serial No. 既存のTroubleReportのSerialNo. を記載する 新たにNo. を作成する場合には年度が識別出来る事 2) Equip.Code 従来から使用している別添のEquip.Code 表に従う事 3) Title of Trouble TroubleReportの同一欄を記載 (Trouble Report 改訂版はエクセルである為 コヒ ー & ヘ ーストで容易に出 4) 1st date 及び Final date 1stdate はトラフ ル発生日 Final dateはfinalreport 提出日を記載 5) Stopping Time スケシ ュールの遅延時間に関係なく Actualの停船期間 Detention 期間を記載する 6) Cost for Repair Estimateには予想コストを記載 コスト確定後は部品費 修繕費に分けてFinalSpare,FinalRepair 欄に記載 Guarantee Claim 又はStevedore damage Claimの場合はコスト記載は不要 Estimate 欄に G" 又は 7)RepairStatus 原則として上段には実施した内容 実施中及び現状 下段には実施日及び実施予定を以下の様に記載 上段 ; TemporaryRepair Maker'sInvestigation Maker'sRepair Observation 等下段 ; DockRepair 予定の場合は紫色網掛けでマーキングする 8)ClassStatus 原則として上段には受検完了或いはRecommendation 有りを特記 下段には受検完了日或いはRecommendat の有効期限を記載 9) Insurance Status H&M,P&I 保険案件である場合は上段に記載 Settleした場合は下段にその日付を記載 保険案件としてこの欄に記載した場合はCost for Repair 欄の記載は記載しない 1 0) Unsettled Account 保険案件としてUnsettledAccount-ClaimReceivables(Form:SU ) に記載されたAccidentCod 継続案件の場合は下段に現在確定したCost を記載 最終的に保険交渉がSettleした場合には最終請求保険金額記載のうえ赤色網掛けにてClosed 表示 更にCommentを挿入して 免責額 受領保険金額 修繕費に廻る金額を記 11) Total Trouble Recovering Cost 年次期初にYearRepairBudeget 欄の黄色網掛け部 FinalSpare 及びFinalRepairに期初予算部品費及他の欄は自動的に計算され記入される 12) Annual Report of Trouble Status 報告時の各項目件数を記載し 年度末に年度集計し完結させる 尚 ) 内には前年度より引継いだ未解決案件の件数 13) Special Comment 管理会社から幣方への特記事項欄として自由利用 (3 ) 各 TroubleReportの消し込み方法 ( サンフ ル参照 ) 1)FinalReport を提出した時点でそのトラフ ル案件は青色網掛けでClosed 表示とする 但し 以下の場合はFi 迄の消し込みとしClosed 案件とはならない 2)FinalSpare&FinalRepair 欄に確定コストが記載されるまで Cost for Repair 欄の消し込みは実施しな 3)RepairStatus 欄にDockRepairの紫色網掛け表示を付した場合には原則として消し込みは実施しない 4) 保険案件については継続案件については上段のみ赤色網掛けとし Settleした時点で下段にSettleした日付のうえ 赤色網掛けで消し込む
