Background and Purpose Data Accuracy Program Adherence In compliance with section (2) F.S., and the Office of Early Learning (OEL) Grant Agreement, section 11, the coalition shall use the most current release of the OEL Single Statewide Information System. The coalition shall fully implement any system changes within 30 days of release, unless OEL specifies a shorter time period. The coalition shall use the most current release of OEL s single statewide information system, including a single point of entry and unified waiting list, to record, maintain and report on early learning programs and services. Grant Agreement 11(e) The coalition shall comply with data correction requests or data cleansing activities upon OEL s request. The Data Quality Unit runs edit and exception reports on a monthly or quarterly basis and places them on the Coalition Zone (SharePointCoalition ZoneCoalition LinkData Quality Edit Reports-Restricted). Reports may identify instances that could result in questioned costs because information is missing, invalid or inaccurate. The review team will review select edit and exception reports and abide by the guidelines and processes in this program guide. OEL will update the process(es) as OEL revises, adds or discontinues reports. OEL may identify instances during the review of these reports that will require coalition follow-up and review team validation. The annual edit report processes identified in this guide are to provide the coalition with technical assistance and guidance to ensure that data in the Enhanced Field System (EFS) is accurate. 4 II. Data Accuracy Review Process 1. The review analyst will retrieve selected reports (based on the selected scope for that review period) and assign each case a number for easy reference. 2. The review analyst will compile a list of cases for each report and send this list to the team lead. In some instances, the analyst will have to make a sample selection. For example, if the Active SR files with SSA/SSI income report lists 45 cases, the analyst will only select a sample of 20 cases. 3. For each selected report, the team lead will request that the coalition provide supporting documentation for all cases in the review analyst s sample selection list. OEL will send this information with the request for SR, VPK and PV files. 4. Once OEL receives supporting documentation, the review analyst will analyze it for errors and record this information in the OEL Comments section within the respective report spreadsheet. The review analyst must doc cite all observations, even if the analysis reveals no errors. The review analyst should be sure to describe the errors in detail and provide clear suggested corrective actions for the coalition in the OEL Comments section of each spreadsheet. 5. The review team lead will send the reports with analyst comments to the respective coalition prior to the 10-day preliminary findings review period so the coalition can review, correct and provide supporting documents to show corrections (i.e., EFS screenshots). 6. The review analyst will analyze the coalition s supporting documentation and note the coalition s final observations in the OEL Final Observations section of the spreadsheet and DA scorecard. The analyst will complete the OEL Final Observations section to clearly identify which files the coalition corrected and which files still require follow-up. 7. If available, the review analyst will review the subsequent report and compare it to the initial report to determine if the coalition has corrected the files in error. (Note: Any corrections the coalition makes in the current month will not appear in OEL s EFS system until after the 20th of the subsequent month; therefore, the analyst should request screen prints from the coalition in order to verify corrections made). 8. The review analyst will make a note on the DA scorecard of any new files listed on the subsequent report so that the coalition is aware of subsequent errors. The review analyst will not analyze the new files in error as the coalition has not had time to review and correct these files. 9. If there are any cases the coalition did not correct during the 10-day preliminary findings review period, the review analyst may request that the review team lead contact the coalition to attempt to resolve remaining cases include these cases in the AS draft report. 10. The review analyst will send the DA scorecard to the review team lead for review. The lead analyst will then send the scorecard to the review supervisor for final review. 11. The review analyst, review team lead and review supervisor must complete the DA scorecard prior to the exit conference/courtesy review period. The review team lead will upload the scorecard to SharePoint for the coalition s review. 12. During the courtesy review period, the coalition will be able to fix any outstanding errors. The DA scorecard should reflect all changes/corrections made during this time. 13. The coalition will need to correct all files in its corrective action plan that were not corrected during the courtesy review period. 14. During the CAP review, the review analyst will look at the new report on Coalition Zone to determine if the coalition corrected all files and if the coalition is in compliance before OEL accepts the CAP. OEL will place notations about final CAP corrections in the CAP plan and not on the DA scorecard since the DA scorecard will be final by this time. Page 4 of 23

6 III Data Accuracy Review Criteria Office of Early Learning The numbering scheme below corresponds with the DA scorecard and the AS monitoring report. A. School Readiness (SR) Edit and Exception Reports (DA-A) 1. SR Duplicate Name Report (7) Identifies children (by name) who received more than one SR payment during the same service period. It is possible to have two payments within the same month with no payment error (for example, a child moved in the middle of the month from one coalition to another). It is possible to have two payments within the same month with a payment error. Reports are run based on child s name. Determine if the coalitions made reimbursements for the same days. Determine which coalition made the duplicate payment, if any. Both coalitions will need to submit the following information for the child and time period in question: Sign in and out sheets. Attendance certification. Documentation of payments made. Child s birth certificate. Parent s identification. Document any questioned costs. Questioned costs apply to open and closed cases. Encourage collaboration between coalitions to determine the cause of the duplicate name payment. The coalitions must submit documentation to verify any corrective action or to validate that no error occurred. 2. SR Duplicate SSN Report (7) Identifies children (by SSN) who received more than one SR payment during the same service period. It is possible to have two payments within the same month with no payment error (for example, a child moved in the middle of the month from one coalition to another). Encourage collaboration between coalitions to determine the cause of the duplicate SSN payment. The coalitions must submit documentation to verify any corrective action or to validate that no error occurred. 3. Child s age => 13 (18) Identifies children who are equal to or older than 13 and receiving SR services. Ensures the coalition is not serving ineligible children and that children are not in the wrong billing group. Rule 6M-4.200, FAC, provides SR services for children under the age of 13. If a child who is at least 13 years of age but not older than 19 years of age and is physically or mentally incapable of self-care or under court supervision was determined to be eligible before July 31, 2013, the child should be served in the BGSNT billing group. Determine if DOB is accurate for children the report identifies. Determine if eligibility was determined before July 31, Determine if the coalition is serving SPTN children in the correct billing group. Determine if the coalition coded the level of care correctly. Determine if the coalition made reimbursement for ineligible children. Document any questioned costs. Questioned costs apply to open and closed cases. 4. Active SR files with SSA/SSI income (19) Assists with identifying if the coalition properly included/excluded SSA/SSI income. Files listed on this report are not necessarily in error. The report lists those clients that the coalition entered in EFS as receiving SSI. The coalition provides SR services to a child who is an SSI recipient or who receives SSA benefits because of a disabled parent. The coalition should not include child s SSI in the income calculation, but should include SSA. Services coded under BG8-29 (prior to August 1, 2013). The coalition provides SR services to a child living with a parent/guardian who is an SSI and/or SSA recipient. The coalition should include SSA/SSI in total income. SSA is an insurance program and the coalition should include it (except as specified below for child-only cases). Several household members can receive this, so ensure that the coalition has included all appropriate income for all household members. If the coalition is serving the child in a Child Only billing group and the adult receives SSA/SSI, the coalition should not include the income because the family unit is child only or sibling group only. SSI maximum amount is $8, annually for 2012 and $8, for SSI and SSA disability = BG8-ECON. SSA non-disability = BG8-ECON. Page 7 of 23

8 If the SSA/SSI report contains more than 20 cases, select a sample of 20 cases from the edit report. If the report contains fewer than 20 cases, then use all listed cases. Review Award Letter or Benefits Verification Letter. Ensure documentation is less than a year old. Determine if the coalition properly included/excluded income appropriately based upon categorization of SSA/SSI. If the coalition inappropriately included/excluded income, the parent fee may be incorrect. (See Attachment for additional guidance for identifying SSA/SSI) Review the income information for the Parent/Child in EFS. Determine if the coalition used the correct income codes in EFS: SSB = Code for SSA (adult & child). Always include income. (The coalition should not use this code for SSA disability.) SSAD = Code for SSA disability (adult & child). Always include income. SSIC = Code for SSI of a child. Income is always excluded. SSI = Code for SSI of an adult. Income is always included. Determine if billing group and eligibility code is accurate by referring to the most recent OCA standard codes. Document any questioned costs. Questioned costs apply to open and closed cases. Note: Analyst must doc cite the SSI/SSA award letters for all income-eligible cases. For protective services clients, OEL does not require the coalition to provide an award letter, but the coalition must show an attempt to do so. Review the case notes to determine attempts. If the coalition corrects a case, the analyst must also doc site an EFS screenshot of the corrected income screen or SR Total Family Income exceeds 200% (29) Identifies families who are potentially over income and ineligible for the program (does not apply to BG1). SR services are limited to 200 percent of the Federal Poverty Guidelines based on family size. Protective services clients (BG1) are not subject to the income requirements; however, income should be accurate to determine the parent fee. Review client s income documentation (SR-100 and documentation that supports all income listed on SR-100). Using the client s income documentation, determine if the coalition accurately reported the parent s income information in EFS. Review the income information under the Interview tab and EligibilityMaintain screen for the applicable period listed on the report. The analyst must doc cite screenshots of the EFS information. Determine if correct Federal Poverty Levels are in place. Compare the coalition s most current sliding fee scale to the OEL-approved current year sliding fee scale to ensure that amounts listed correspond to the coalition s sliding fee scale (see Attachment B). An error with FPL could cause the client to appear to be over income when the client is actually within the eligible range. For income eligible categories, determine if the coalition made reimbursement for ineligible clients. If the coalition made reimbursement for ineligible clients, the coalition will be responsible for repayment of those funds to OEL. Document any questioned costs. Questioned costs apply to open and closed cases. 6. BG3 with no TANF income (27) Identifies families in an incorrect billing group. (includes BG3R-RCG, BG3- Page 8 of 23

9 TCAN, BG3-TCAW, BG3-28A and WRC-RCI) This report is currently not being reviewed by OEL. Office of Early Learning Billing groups BG3R-RCG, BG3-TCAN, BG3-28A and WRC-RCI are for services provided to families receiving temporary cash assistance; should have TCA cash amounts in EFS. The billing group BG3-TCAW is for services provided to families receiving TCA who are working; should have TCA cash amounts in EFS and employment income. Report indicates the following codes: T = TANF income not included. E = Employment income not included. B = TANF and employment are not included. Determine if family receives TCA. If the family receives TCA, review EFS data entry. If the family does not receive TCA, determine the correct billing group. Document any questioned costs. Questioned costs apply to open and closed cases. Note: Questioned costs typically would not apply; errors identified in this report are usually limited to incorrect data entry of TCA amount in the wrong field or incorrect billing group. 7. Gold Seal Edit Report (1) Identifies providers receiving a Gold Seal payment who have a missing certificate or error with the certificate end date. DQ The Gold Seal report includes detailed accreditation and Gold Seal information on SR providers who may need their accreditation information updated to remain in the Gold Seal category. To receive the Gold Seal differential payment a provider must have a current Gold Seal accreditation. Request the Gold Seal Certification for each provider listed. The analyst may also check a provider s Gold Seal accreditation status using DCF s Provider Search function. The analyst must doc cite this information. Using the Gold Seal Certification, determine if the provider was Gold Seal accredited for the period indicated on the report. Review the provider s accreditation information in EFS (to locate this, click CCR&R at the top of the EFS homepageprovideradd/maintainsearch for providerdetailqualityaccreditation). Note: the analyst may choose to doc cite this information in EFS, but the report reflects the same information. After the coalition corrects the information, the analyst must doc cite screenshots of the provider s accreditation. (Note: It typically takes a month for EFS to reflect changes; therefore, the analyst must request EFS screenshots from the coalition). Document any questioned costs. Questioned costs apply to open and closed cases. Limit questioned costs to the Gold Seal differential payment. Page 9 of 23

10 8. Purpose for Care Report (16) Identifies active enrollments with employment as the purpose for care without a corresponding employer listed. EFS should include information supporting the purpose for care. The coalition must list an employer if employment is the purpose for care. Request documentation from the coalition to support the child/client s purpose for care. If the coalition cannot support the purpose for care, the client may be ineligible. Determine if a data entry error exists. Document any questioned costs. Questioned costs apply to open and closed cases. 9. SR Redetermination Report Identifies SR child enrollments that do not show an updated status in EFS after a client s scheduled redetermination date. (ss (7), (5), (6), F.S.; CCDF Plan 2.3.6) A redetermination should be conducted prior to the end of the current eligibility period. Prior to the end of an eligibility period, the coalition should redetermine each case and update the eligibility/redetermination status in EFS. A redetermination may result in reauthorization of SR services or termination of services. The coalition should generate and monitor the overdue redetermination review report within EFS to track redeterminations. - Run overdue redetermination report from EFS. Go to client on the main EFS page Select Counselor Aids then redetermination by county Enter the start and end (thru) date that you would like to review based on the select sample review period Report review period should be six months prior to the first month of the eligibility review period. Select sort by redetermination date Enter the county you would like to review (note: this is only necessary to do for Orange, Osceola or Seminole) Select Execute Report - Investigate any files included on the overdue report. Review the report to determine if cases are overdue for redetermination Separate SR child results from the VPK child overdue review report Look up cases individually as needed in EFS Determine if case is actually overdue for redetermination or only appears on the report due to an EFS error. B. Voluntary Prekindergarten (VPK) Edit and Exception Reports (DA-B) 1. VPK Provider Payment Rate (15) Identifies providers who the coalition paid with the incorrect VPK rate during the reimbursement period. DQ Page 10 of 23

11 Each year the county of service provides the VPK standard rate for payment to each coalition. Determine if reimbursement rate is correct. If the rate was incorrect, the coalition will need to correct the rate and provide an adjustment. Document any questioned costs. Questioned costs apply to open and closed cases. 2. VPK Instructor Credential Edit Report (4) Identifies instructors of current classes where an instructor has one or more errors based on the criteria provided below. DQ The VPK law requires a minimum instructor credential for each lead VPK instructor. An instructor may appear on the report more than once if the instructor is assigned to multiple classes. The report identifies: Invalid instructor name. No value for instructor credential. Non-standard or inactive code for instructor credential. No value for highest degree. Non-standard or inactive code for highest degree. Determine if the instructor meets the qualifications for VPK. Determine if instructor information input in EFS is incomplete. If the instructor is in multiple classrooms on the same day, review to ensure the times are different for each class. 3. VPK Calendar Edit Report (3) Ensures calendars do not exceed 540 hours for school year program and 300 for summer program. DQ DQI report indicates classroom with hours that exceed 540 for school year and 300 for summer program. Calendars may exceed the allowable program hours due to the provider schedules. Payment may not exceed the allowable program hours. Determine if the coalition has made payment for classroom hours that exceed the maximum total hours. Coalition should run a CC045 report for hours claimed and make corrections as necessary. Reviewer should ensure the coalition did not make payment for excess hours. Review twice a year: End of summer. End of school year. Page 11 of 23

VPK Dual Enrollment Report (6) Identifies children who are enrolled in VPK in one coalition and have been determined eligible for VPK by a second coalition. (Based on SSN search) DQ A VPK child is only eligible for one enrollment. Exceptions exist for good cause and extreme hardship (Rule 6M , FAC). Enrollments should never be concurrent. Determine status of child in each coalition. Determine if the coalition terminated enrollment but did not enter into EFS. Determine if the coalition made duplicate or unallowable reimbursement. Document any questioned costs. Questioned costs apply to open and closed cases. 5. VPK Redetermination Report - Identifies VPK child enrollments that do not have program end dates entered into EFS VPK clients cannot be redetermined, however, a coalition is responsible for verifying that VPK child enrollments have program end dates entered into EFS. To run overdue reports for VPK from EFS Go to client on the main EFS page Select Counselor Aids then redetermination by county Enter the start and end date that you would like to review based on the select sample review period (six months beginning with the most recent month available). Enter the county you would like to review. (Note: This is only necessary to do for Orange, Osceola or Seminole.) Select Execute report. Investigate any VPK files included on the overdue report Look up cases individually as needed in EFS Identify the number of overdue redeterminations C. Edit and Exception Reports for Both SR and VPK (DA-C) 1. Active Providers Report (14) (formerly called Provider Accreditation report) Based upon the Office of Early Learning Grant Agreement requirement 11 (a): On a monthly basis, the coalition shall ensure that it includes all legally operating early learning and school-age child care providers (including, but not limited to, all licensed and license exempt centers; faith-based providers; licensed, registered and large family child care homes; school-age care providers; SR providers; VPK providers; Head Start providers; Early Head Start providers; nanny/au-pair agencies; and summer camp providers) in the coalition s service area in OEL s single statewide information system.

13 Compare DCF Master Facilities and Homes Listing to Provider Listing Report from EFS to ensure the coalition has included all legally operating providers. 2. OCA Standard Codes Edit Reports (11) Identifies differences in the standard codes in EFS as compared to the new release of the OEL Standard Codes Reports. This should also read: Identifies OCA, billing group, or eligibilities that should be added or removed. The coalition should update Standard Codes within 30 days of release. Determine if Standard Codes are current and accurate. 3. CCRR Standard Codes Edit Reports (8) Identifies CCRR standard codes of which the coalition should add, remove\ or correct the description.. The success of OEL programs depends on the availability, integrity and confidentiality of data. Information has integrity when it is timely, accurate, complete and consistent. Pursuant to section (2)(n), F.S., the coalition shall, within 30 days of the release of any system changes, use the most current release of OEL s single statewide information system, including a single point of entry and unified waiting list, to record, maintain and report on early learning programs and services. Review local EFS Standard Codes to ensure the coalition/contractor is using the latest version of the OEL Standard Codes release for CCRR. 4. Review local EFS Standard Codes to ensure the coalition/contractor is using the latest version of the OEL Standard Codes release for CRLV. Compare the CRLV codes in the coalition s EFS database to the latest version of the OEL Standard Codes release for CRLV codes to ensure that the coalition is using the correct codes in EFS. D. SR Edit and Exception Reports Requiring Further by the Coalition (DA-D) OEL developed these reports in response to the Office of Child Care Program Integrity and Accountability Webinar Series, based on lessons learned from other states. Files these reports identify are indicators that there is a need for additional verification. These reports do not indicate non-compliance based on the data reported. However, OEL should evaluate files these reports identify for questioned costs as applicable. 1. Care Authorized exceeds FTFT (26) Identifies whether a child is at a provider for longer than 12 hours a day. Purpose for care should support provided hours of care. Determine if purpose for care supports hours of care authorized. Document any questioned costs. Questioned costs apply to open and closed cases. E. DA Previous Corrective Actions (DA-E) A coalition is required to submit a corrective action plan (CAP) response to OEL s written notices of findings of noncompliance within 30 days and to implement the CAP response. 1. Did the coalition implement DA corrective actions that the previous OEL-issued Accountability Section review report lists? (Early Learning Grant Agreement (8) and (31)) Note This criterion is only applicable if a coalition had previous period DA findings that required follow-up. A coalition shall submit a CAP response to OEL s written notices of findings of non-compliance within 30 days and shall implement the CAP response. Assessment activity Review the previous OEL-issued AS review report to determine if the coalition implemented DA corrective actions. Access the Program Integrity SharePoint homepage and select a coalition for review under Coalition Links. Next, select Accountability Post Reviews and click on Close Out under Documents. Finally, select Page 14 of 23

15 the previous review year to access the final CAP close-out document. Note Restrict the analysis to policies that the coalition had not implemented or specific file errors that the coalition had not corrected by the previous review s closeout date. The analysis should not cover instances where the analyst observed the same type of file errors as in the previous review or an evaluation of errors that are merely data entry errors, unless the number of new errors is significant. 16 Attachment A Identifying SSI/SSA Payments SOCIAL SECURITY ADMINISTRATION PROGRAMS It is imperative that SR eligibility workers recognize the Social Security income types and determine whether the Social Security Administration (the Administration) categorized the income as SSI or SSA. The two programs the Social Security Administration administers that affect SR eligibility are: SSA Retirement, Survivors, and Disability Insurance. SSI Supplemental Security Income. The coalition cannot use SSI and SSA income verification letters if the verification date is older than one year. Overview of the SSI and SSA Programs SSA SSI Benefits for individuals who worked and earned sufficient credits to qualify. May appear as Retirement, Survivors, and Disability Insurance (RSDI), OASDI, SSD or SSDI. Aged, disabled or blind individuals who have little to no income and did not earn sufficient work credits receive payments. Appears as Supplemental Security Income (SSI). There is no cap placed on income in order to receive benefits (unlike SSI). Qualified wage-earner must be disabled or have reached retirement age. Survivors and dependents commonly receive benefits from the qualified wage-earner s account. In 2012, the maximum payment rate was $698/month for an individual ($1,048/month for a couple). In 2013, the maximum payment rate is $710/month for an individual ($1,066/month for a couple). The Administration makes payments on the 1 st of the month. The Administration may lower payments based on income. Once eligible, SSI recipients qualify for food stamps and Medicaid. This list is not exhaustive and individual cases may vary. SSA CLAIM NUMBERS Example A: Claim Number: Mildred receives survivor s benefits on the account of her deceased husband, Baxter. Mildred s SS# is The first part of her claim number is The claim number is not based on Mildred s SS#. It is most likely Baxter s SS#. Example B: Claim Number: C1 If the child s claim number matches the mother s SS#, the child is drawing from her account. The specialist should expect to see SSA income reported for both. If not, ask questions and make clear case notes. Originally, the Administration assigns the C1 suffix to the youngest child. Example C: Claim Number: C3 This means there may be two other children drawing SSA with C1 being the youngest child. If these three children are in the home, the specialist should question the SSA income of all three children. The coalition must include all SSA in household income. If this child s claim number matches the parent in the home, that usually means the parent also receives SSA and the specialist should ask for the parent s letter. KEY POINTS ABOUT SSA INCOME The coalition must always count SSA in the household income for everyone counted in the household size, including the child. SSA Income Benefit Receipt Dates To determine whether income is SSA, review the verification letter. This letter usually contains the date the Administration pays benefits. Birth Date On Benefits Pay Date 1 st -10 th Second Wednesday Page 17 of 23

18 11 th -20 th Third Wednesday 21 st -31 st Fourth Wednesday For example, if the person was born on June 16, the verification letter may state, Your monthly benefit will be paid on the third Wednesday of every month. If the Administration approved SSA benefits before May 1, 1997, the payment date will be on the 3 rd of the month. KEY POINTS TO IDENTIFY SSI INCOME Award/verification letters refer to the cash as a payment, not benefit. The monthly payment is almost always $710/month. If the amount is less, be aware that the applicant likely reported other income to the SS office. If the amount is more, the income is most likely SSA, and the specialist can verify by further reviewing the award/verification letter. Verification letters usually state that the Administration makes payments on the 1 st of each month. Coalitions should never count in the household income the SSI amount children receive in the program; coalitions should always count the SSI of adults. If the income appears to be SSI, you can ask whether the family member receiving SSI is on Medicaid. Those on SSI, qualify for Medicaid (nationwide). Florida SS offices almost always start both at the same time. To verify the income type, review the verification letter. When the parent receives SSI for the child, list the child s name in EFS (coded as SSIC) and reflect the child s name in the income worksheet as the person receiving benefits. If AI, AS, BI, BS, DI, DS or DC follows the Social Security claim number, the income is SSI. Persons on SSI usually qualify for food stamp income, which coalitions must identify as excluded on the income worksheet. SSI and SSA Codes in EFS SSB SSAD SSA of Adult & Child SSA Disability of Adult & Child Included income. Paid for reasons other than disability, such as retirement or survivor benefits. Included income. Disability benefits are payable to disabled workers, widow(er)s or unmarried children of disabled workers. SSIC SSI of Child Excluded income.
