The registration process is as follows: Please read and complete the enclosed policies and return them with your: a. Deposit - $25 per person if registering prior to May 16, 2016 b. Registrations made after May 16 th will require the full cost of Kamp upon registration c. Policy/Lice/Health Services Agreement form signed and dated d. Registration form detail the number of people registered and record your contact information NOTE: A hardcopy of these registration forms NEED NOT be submitted if you choose to complete the registration process online at However, payment is still required in order to activate your registration request. Registrations for which no payment is made will not be reserved and will be marked as pending. Once we have received these items we will register your group for Kamp. We will send you a confirmation noting your acceptance and/or wait list status (if the session you request is full.) You will then receive materials needed to assist you and your group to prepare for KAA. It is important that you contact us if you do not receive a confirmation or any further registration materials after your registrations are complete. Remember to copy ALL forms in this packet or any you send to us in the future for your records. Please keep in mind that all registration is on a first come, first served basis an early response is most ideal. We look forward to leading kids across America to the cross of Jesus Christ! 2 Kids Across America and Higher Ground 2016 Policy/Lice/Health Services Agreement Form All group leaders who enroll for the 2016 Kamp season are required to sign the following policies. Please read them carefully, sign and return with your group registration form. Your registration request will not be processed without this contract signed. A. ATTENDANCE 1) Registration will not be completed without either an online or hardcopy representation of all the following items: a. signed policy agreement b. registration form and c. required payment. 2) KAA Kampers must be between the ages of 9-18 years old and Kaleos must be no younger than 20 years old. If spaces are not filled with participants of the correct ages, the tuition fee will be lost. Acceptable Kamper and Kaleo ages are as follows: a. KAA 1: ages 9-11 c. KAA 3 and Higher Ground: ages b. KAA 2: ages d. Kaleos: ages 20 and up 3) The standard policy is 1 Kaleo to 10 Kampers. Requests above this ratio will initially result in the additional Kaleo(s) being placed on a wait list until space is available. One Kaleo is desired per Kamp where you have Kampers registered even if there are less than 10 Kampers in attendance within that Kamp. a. If there are enough Kaleos available within your group, there must be one scheduled for each Kamp where Kampers are registered. Registration reserves the right to move Kaleos accordingly. b. Higher Ground does not require Kaleos to be enrolled. 4) Each group may register a total of 40 Kampers per Kamp for an overall total of 120 Kampers. 5) KAA/HG is not a conducive environment for group members who are pregnant or have disabilities which require individualized care and/or extreme changes in Kamp procedures. Such disabilities include but are not limited to: Down syndrome, autism, sickle cell anemia, persons requiring use of a wheel chair or those with severe cognitive delays. Persons with such medical situations are prohibited from attending KAA. 6) Only those Kampers/Kaleos who are approved by KAA Registration may attend. No additional group members should attend otherwise a penalty will be incurred. 7) No Kamper may attend KAA more than once per summer other than those between ages who decide to attend Higher Ground as well. 8) With the exception of Higher Ground, all attendees must be registered under a group of 8 or more Kampers and 1 Kaleo. 9) It is the group leader s responsibility to monitor the group registrations to know if group members are enrolled, waiting or pending and to know if the enrollment is correct. B. WAIT LIST 10) As space becomes available, Registration will send out a confirmation fax/ which must be signed by the group leader and returned by the stated deadline if the space is to be accepted and enrolled. 11) Payments for spaces received from the wait list must be paid by their due dates otherwise the space(s) will be cancelled and offered to another group. Payment due dates are as follows: a. May 16 th if the spaces are offered/accepted prior to this date b. Within 10 days if the spaces are offered/accepted after May 16 th c. Opening Day if the spaces are offered/accepted less than 10 days before Kamp begins 12) Do not bring Kampers who were not enrolled from waiting in the hope that space will become available. If no offer has been made, there is no space for your kamper. C. PAYMENTS/CHARGES 13) Before May 16, 2016, a $25 deposit per person is due at the time of registration. Spaces must be paid in full upon registration after this date. Registration will not be processed either online or in our offices if the required payments are not made. 14) The full cost of Kamp is $175 per person for KAA Kampers and Kaleos attending Session 5. For all other weeks the cost is $165. Pricing for HG1,HG2 and HG3 are $165, $325 and $665 respectively. 15) Multiple checks or checks from multiple sources cannot be accepted all at one time. Parents/supporters should make payment to your organization/church so that one payment may be made to Kids Across America Kamps. Please note that the deposits and tuition fees are not tax deductible. 16) The final payment is due no later than May 16 th 5pm CST. If payment is not received on time, your spaces will be reduced to show a paid in full balance. Registration will decide which spaces will be cancelled and will not accept a change form in this situation. 17) Any fees due for previous years of Kamp must be paid before registration for the current year can be processed. Monies applied toward the current year s registration will first be applied to the previous year to cover the outstanding balance, then to the new registrations.

3 18) A $5 discount per person will be granted on tuitions which are paid entirely by check/e-check, money order or cash or a combination of these methods. Payments either partial or whole made with any other form of payment will be manually billed the full cost of $165/$175/$325/$665 per person at a later date after the initial registration. Credit balances from the previous year may be applied toward new registrations. 19) Registrants will be charged a late fee of $100 if arriving after 5 p.m. on Opening Day. 20) Registrants who leave prior to the end of Vespers program or before 9 a.m. the last day of Kamp will receive an early departure fee of $100 per group. 21) Medical charges are applied after the completion of a session and are incurred for the issue of items such as feminine products, ace bandages etc. and the retrieval of prescriptions for group members. These fees offset the cost of treatment at the Health Station and must be paid prior to registration for the new Kamp year. These charges are not able to be billed through insurance. 22) A group leader may not begin a new group nor attend with another group unless all accounts from previous years have been settled and show a zero balance. D. CANCELLATIONS 23) All cancellations, whether for individual spaces or the entire group, must be made in writing and submitted to our offices. Complete cancellations must be made no later than April 1 st 5pm CST to avoid penalty. 24) Financial penalties will be incurred in the following cases: a. Cancellations made after April 1 st 5pm CST (our cancellation deadline). The $25 deposit for each cancelled space will be forfeited between April 2 nd and May 16 th f or all areas except HG3. b. Cancellations made after May 16 th (our payment deadline). The full tuition price will be forfeited for any cancellations made between May 17 th and Opening Day. No refunds will be issued for complete cancellations. 25) KAA reserves the right to change a Kamper s Kamp placement as we deem appropriate at any time before or after Kamp begins. 26) Initial group registrations completed after April 1 st will receive a fee of $100 or 10% of the registration fee as a service charge for cancellation of their session. The cancellation fee amount will depend upon the amount of the registration fee. Any remaining funds will be commuted to the upcoming year and will not be refunded. E. FORMS/VERIFICATIONS 27) Kamper/Kaleo information must be entered online at least 1 month prior to your Opening Day. If the information is not entered by 5pm CST of your designated date, a $25 fee will be issued per form. (Please refer to your handbook for due dates.) a. There are no penalties for changing the information only for not having it entered on time. b. These items must be submitted to the Health Services Staff on Opening Day i. A signed copy of all Kamper/MAR (Medication Record) forms for any Kamper who takes medication. ii. A signed Diabetic Policy for those who take insulin. iii. A signed Seizure Policy for those who have a history of convulsions 28) hours prior to your Opening day, a verification form will be faxed/ ed to you. It is important that the names, birth dates, genders, and Kamps be verified and returned to us by 3pm CST the Friday before Opening Day and again by 10am on Opening Day. One verification form will be sent to you but you must verify with us twice. F. INSURANCE 29) The safety and well-being of your Kampers/Kaleos is our number one priority. We are requiring that all Kampers/Kaleos who attend KAA have adequate medical coverage. Please see the group leader handbook for more information on insurance. 30) The group/ministry is liable for every group member in attendance to Kamp. It is the group leader s responsibility to make sure that each group member is covered by insurance. 31) We require groups/ministries to provide a group/ministry insurance plan in order to cover those group members who do not have individual/private coverage. We recommend but do not require that you use Brotherhood Mutual, Special Markets Insurance or TravelGuard s individual insurance. For contact information please see the group leader handbook. 32) The tiers of coverage are as follows: 1) individual insurance/medicaid 2) group insurance. If an accident occurs, the individual coverage will be filed first then the group/ministry s insurance. G. LICE AND HEALTH SERVICES POLICIES 33) Each group member is to be checked for lice and treated prior to departing for Kamp. Please use the following link for further assistance and discounts: 34) Group members who have a fever or appear to be ill should be asked to remain behind and/or replaced.

4 35) Upon your arrival to Kamp, our Health Center Staff will check your group members for lice and health concerns. Those who have lice, fevers or other health concerns will not immediately be allowed to enter Kamp until the matter is resolved. 36) Since lice are spread very quickly from one person to another, those within the same vehicle in which the infected person is traveling will be detained and all persons treated for lice at the cost of $20 per person. 37) It is the Kaleos responsibility (with the assistance of our Health Center Staff) to wash all the clothes and bedding of their group members when lice are found. A small laundry facility will be provided at Kamp; however if there are multiple loads of laundry it may be necessary to go into the nearest town to use a larger capacity laundry facility. 38) You are required to contact the Registration Department if your potential Kamper/Kaleo has a medical condition that needs specialized treatment, assistance, or has a behavior disorder. You must get permission from the Health Services Director in order for the Kamper/Kaleo to attend Kamp. 39) Although we try to give you as much notice as possible; it is possible that you may not be notified that a Kamper cannot attend Kamp until a day or two before your session begins. In such cases there will be no penalty for the cancelation of the space. 40) Please be aware that the closest hospital is 1 hour away from our Kamp. Our Health Center Staff will attend to regular day-to-day medical needs but is not equipped for major medical situations. 41) If you have a group member who has a questionable condition, call the Registration Department as soon as possible. We will then contact the Health Services Director. Please do not bring that person without consulting with our Health Services Director or registration staff. 42) The cost of lice treatments and any medical supplies will be billed to the account after your session ends. 43) I, the group leader, agree to discuss the Lice and Health Services Policy Agreement with my Kampers parents and Kaleos in order to thoroughly abide by and understand these Kamp policies. 44) Inhalers will be kept primarily by the Kamper in KAA 2 and 3 or the Kounselor in KAA 1 and placed with the Health Center Staff as a secondary measure as needed. 45) Kampers/Kaleos with severe Asthma must submit an action plan and must have adequate medication on hand in order to attend. If these conditions are not met that person will be sent home at the group's expense. I have read, understand and agree to comply with the above stated Policies.
