2 FARM LIABILITY FORMS Liability Grant FL Farm Liability Form U Additional Exclusions and Provisions - Liability Insurance Asbestos and Lead provisions, ACV definition Business Activities Additional Insureds Enhancements FL Business Activities Provides liability coverage for business activities conducted by the insured on their premises. ALL business activities must be specifically described and listed on the form to trigger coverage See Rating Step 3 for list of acceptable business activities. Only those business activities listed in the underwriting guidelines may be listed on this form. FL Additional Insured Executors, Administrators, Trustees and Beneficiaries $50 per entity, flat, fully earned when issued FL Additional Insured Farm Liability $50 per entity, flat, fully earned when issued Designed to provide additional interest coverage for entities from which the insured leases land, persons or organizations that exercise financial control over the insured and/or non-residential coowners of a non-farm residence Naming an independent operator-manager of a farm/ranch owned by or leased to the insured as an additional insured is prohibited. Use of section 3.a. of form FL0450 prohibited. Vendors - if you are trying to provide vendor s coverage via FL0450 the premium charge is based on actual receipts tied to the vendor(s) and you apply the products rate per $1000 of receipts instead of a flat $50 charge You must schedule all A.I. entities on this form FL Additional Residences Rented to Others if you have farm related dwelling occupied by others Modifies the rental of premises exclusion List all dwellings leased or rented to others to trigger liability coverage FL Amendatory Endt Heat / Smoke / Overspray $100 flat charge, do not pro-rate Contractors providing herbicide or pesticide services to the insured must provide evidence of insurance of at least $300,000 limits.
