2 Logging In Enter this URL into your internet browser: harmony.benselect.com You will see this screen on the landing page after it has loaded: Your personal login credentials are as follows: In the Employee ID or Social Security Number field- you will enter your full Social Security Number, with no spaces or dashes (e.g., would be entered as ). Your Pin Number is the Last Four Digits of your SSN, followed by the last Two digits of your birth year (e.g., If you were born in 1970, your Pin in this continued example would be ) Once you have entered this information, click the Log In button to continue.

3 After you have successfully logged in, you will be brought to the home page of your enrollment site. This is the screen you will see: This screen gives you a recap of your current benefits- hit the Next button to begin your enrollment.

4 The next screen will be your Personal Info screen. You may update any personal information on this screen After you have made your changes please click Next button. If there are no changes to be made just click Next.

5 The following screen is the Dependents screen. You may update your dependent info here. To add a dependent not listed, please select the plus sign on the screen and the following screen will pop up so you can add a dependent:

6 Click save and this will bring you back out to the main dependent screen. You should see the new dependent now. If your changes include the deletion of a dependent then you would just select the x next to the row of the dependent you would like to delete. It will ask you if you are sure you want to delete and just click OK. After all dependent changes are made you can click the Next button.

7 This next screen reviews your Employment information. Click Next to continue. These next screens will be the actual enrollment screens for your benefits.

8 On this screen you will see all of your options for benefit elections. Any coverages that you are currently enrolled in will show under each benefit. Continued--

10 You will begin by going through each benefit. You can select the option of Quick Enroll if you wish to keep the exact same coverage that you currently have. If you would to review the benefit because you would like to change or look at other coverages you would select the Review tab. Some of the benefits do not have a Quick Enroll option because you have to re-elect or waive the coverage. For example if you are eligible for Life Insurance you will have to review the benefit to re-elect your beneficiaries. Just click the Review tab to walk through the screens. 11 The Flexible Spending account benefit also does not have a Quick Enroll tab. You have to re-elect your FSA and Dependent Care elections every year. To re-elect your FSA monies you would enter a dollar amount in the Amount per pay period field and click the Calculate button. If you choose not to participate in the FSA you would simply click the I wish to CANCEL this coverage radio button.

13 For example to look at one of the Colonial products you would click review and then you will see this screen: Click Next and you will be directed to the harmony system where you can look at and enroll in that particular coverage. Click on Get a Quote to further see your options of coverages and prices. You can finish the process to purchase a product, or you can click the button that says My benefits and that will take you back to your original enrollment screen.

14 Once you have waived or enrolled in all of the benefits on your enrollment screen you will have to Sign and Submit. Take a look and make sure your elections look correct and click Next

15 This will bring you to the screen where you will enter your PIN to sign and submit. Enter your PIN at the bottom and click Sign Form (Your Pin number is the last four or your ss# and last two of your birth year.)

16 You have completed your enrollment once you see the following screen: Please click the Logout button at the top of your screen.

22 You can use this coverage more than once Subsequent Diagnosis of a different Specified Critical Illness If you receive a benefit for a Specified Critical Illness, and later you are diagnosed with a different Specified Critical Illness, we will pay the percentage of the original face amount. Subsequent Diagnosis of the same Specified Critical Illness If you receive a benefit for a Specified Critical Illness, and later you are diagnosed with the same Specified Critical Illness (except those listed below), we will pay 25% of the original face amount. (Critical illnesses that do not qualify are: Cancer, Coronary Artery Bypass Graft Surgery, Carcinoma in Situ, and Occupational Infectious HIV or Occupational Infectious Hepatitis B, C or D.) Dates of Diagnoses of Specifed Critical Illnesses must be separated by at least 180 days. Cancer Vaccine Benefit We will pay this benefit if you or a covered family member incurs a charge for and receives any cancer vaccine that is FDA approved for the prevention of cancer, while your policy is in force. You re paid regardless of any other insurance you may have with other insurance companies. Benefits are paid directly to you (unless you specify otherwise). When am I considered totally disabled? Totally disabled means you are: l Unable to perform the material and substantial duties of your job; l Not working at any job; and l Under the regular and appropriate care of a doctor. What if I want to return to work part-time after I am totally disabled? You may be able to return to work part-time and still receive benefits. We call this Partial Disability. This means you may be eligible for coverage if: l You are unable to perform the material and substantial duties of your job 20 hours or more per week, l You are able to work at your job or any other job for less than 20 hours per week, l Your employer will allow you to work for less than 20 hours per week, and l You are under the regular and appropriate care of a doctor. The total disability benefit must have been paid for at least one full month immediately prior to your being partially disabled What if I change employers? If you change jobs or leave your employer, you can take your coverage with you at no additional cost. Your coverage is guaranteed renewable to age 70 as long as you continue to pay your premiums when they are due. What is a pre-existing condition? A pre-existing condition is when you have a sickness or physical condition for which you were treated, received medical advice, or had taken medication within 12 months before the effective date of your policy. If you become disabled because of a pre-existing condition, Colonial Life will not pay for any disability period if it begins during the first 12 months the policy is in force. Can my premium change? You may choose the amount of coverage to meet your needs (subject to your income). You can elect more or less coverage which will change your premium. A covered accident is an accident. A covered sickness means an illness, infection, disease or any other abnormal physical condition, not caused by an injury. You can choose to have your policy paid up when you reach age 65 or 95. Accelerated Death Benefit If you are diagnosed with a terminal illness, you can request up to 75 percent of the policy s death benefit, up to $150,000. Guaranteed Purchase Option If you are age 55 or younger when you purchase the policy, you have the option to purchase additional whole life coverage without having to answer health questions at three different points in the future. You may purchase up to your initial face amount, not to exceed a total combined maximum of $100,000 for all options. $3,000 Immediate Claim Payment This payment can help meet immediate needs, such as funeral costs, by providing an initial death benefit payment of $3,000 to the designated beneficiary. Dependent Coverage You may purchase up to $10,000 in term life coverage for each of your eligible dependent children and pay one premium. You can add this additional coverage to either the primary or the spouse policy, but not both. Waiver of Premium Benefit Rider Your premiums on the whole life policy and any riders attached to it will be waived if you become totally disabled before the policy anniversary following your 65th birthday and you satisfy the six-month elimination period (the amount of time until benefits are payable). EXCLUSIONS AND LIMITATIONS If the insured commits suicide within two years (one year in ND) from the coverage effective date or the date of reinstatement (not applicable in AR), whether he is sane or insane (not applicable in AZ), we will not pay the death benefit.
