1 Medical Care & Coverage Information for Student-Athletes The University of Washington Athletic Training Department emphasizes the importance of injury prevention, as well as the need to appropriately manage injuries sustained while participating in athletics, to assure the long term health and well-being of Husky student-athletes. In consultation with team physicians, the athletic training staff provides medical services for athletic injuries including evaluation, diagnosis, management and rehabilitation. Participation in intercollegiate athletics at the University of Washington is a privilege and not a right. In that regard student-athletes at the University of Washington are expected to carry health insurance or be covered by their parent/guardian s health insurance plan. Student-athletes not covered by their parent/guardian s health insurance plan, or the Student Health Insurance Plan, are urged to purchase health insurance to protect themselves from both a health and financial perspective. Medical Insurance Information To assist the University of Washington Athletics Department (ICA) in providing medical services for your son/daughter, ICA requires that current health/vision/dental insurance plan information be provided annually prior to participation. The University of Washington Athletics Department is self-insured and will act as secondary medical coverage for injuries and illnesses sustained as a direct result of participation in Official Team Activities, Practices, Conditioning and Competitions. This means that injuries and illnesses not related to athletics participation (car accidents, serious illnesses, appendicitis, pregnancy tests, acne medications, STD tests, etc) are not covered. All medical claims for your son or daughter must be submitted to the primary insurance plan first. Remaining balances from bills related to conditions resulting from official participation will be covered by ICA. Post Eligibility Medical Coverage Medical coverage for injuries sustained as a direct result of participation in official activities is limited to 4 years from the date of graduation or separation from the University, or when the athlete turns 26 (whichever occurs first). Coverage provided by ICA requires preauthorization (described below) prior to any services being rendered. Coverage will be limited to injuries sustained during official participation and documented in the student-athlete s medical record. Coverage will exclude pre-existing injuries sustained prior to becoming a student-athlete at the
2 University of Washington. Upon graduation/separation your son/daughter will be required to complete an Exit Physical and review the medical conditions occurring during their participation at the University of Washington. Only those conditions occurring after July 1, 2015 and listed and reviewed during the Exit Physical will be eligible for the 4-year medical care and coverage. If eligible, medical services for approved conditions will follow the same procedures listed above and the student-athlete s primary health insurance plan will be billed first and ICA will act as secondary coverage. ICA reserves the right to require approved medical care to be rendered by UW Team Physicians. The University of Washington Athletics Department (ICA) is not responsible for any costs associated with pre-existing injuries or injuries sustained prior to the student-athlete becoming a student-athlete cleared for participation with the University of Washington. Any medical bills for such injuries will be the responsibility of the student-athlete and/or parent/guardian. Benefits provided through NCAA Catastrophic Insurance program are not subject to the 4-year care and coverage plan but are subject to limits outlined by the NCAA Catastrophic Insurance program. Proof of Insurance Proof of current health insurance is required prior to participation. To meet this requirement both of the following need to be completed and submitted to the Athletic Training staff: Completed Insurance Policy section of the Athlete Portal in Presagia Sports o Uninsured student-athletes must indicate this by selecting uninsured in the drop down menu of Insurance Company & Insurance Type Copy of the front and back of current insurance card(s) o These photos will be uploaded to the electronic medical record for the studentathlete. Your son/daughter WILL NOT be cleared to participate until proof of their health insurance information is completed. It is the responsibility of the son/daughter or the parent/guardian to provide updated health insurance information to the Athletic Training staff. 3 Additional Student-Athlete Benefits NCAA Catastrophic Injury Insurance Program The UW ICA is a participant in the Catastrophic Injury Insurance Program (CIIP) sponsored by the NCAA. The CIIP is an insurance program that covers the student-athlete who is catastrophically injured while participating in a covered intercollegiate athletic activity. The policy has a $90,000 deductible and provides benefits in excess of any other valid and collectible insurance. Cheer and spirit squad members are only covered by this plan when practicing or participating for NCAA sponsored events and refraining from prohibited moves. The UW cheer coach is certified in accordance with NCAA policy. For a Benefit Summary of the Catastrophic Injury Policy sponsored by the NCAA, please contact the Program Coordinator at the Athletic Training Room at or visit the NCAA and search catastrophic injury insurance program. Approved Vision Care Routine eye exam during their annual physical exam. Evaluation and treatment of vision injuries resulting from participation in official ICA activities, practice, training & competition. Corrective eyewear necessary to participate in approved ICA official activities, practices, training & competitions. Lost or broken eye glasses or contact lenses will be replaced or repaired by ICA only if the loss or damage occurs as a direct result of participation in an intercollegiate sport. Otherwise the replacement/repair is the responsibility of the student-athlete. ICA will pay a MAXIMUM OF $250/YEAR for corrective eyewear Vision appointments must be approved by the ICA Athletic Training Room and will be billed to primary insurance plans first, before ICA will provide secondary coverage. Approved Dental Care Dental injury directly related to official ICA activities, practices, training & competitions. Dental appointments must be approved by the ICA Athletic Training Room and will be billed to primary insurance plans first, before ICA will provide secondary coverage.
4 Exclusions & Other Important Information The following are examples that are specifically excluded from coverage under ICA Insurance Policy: ANY NON-AUTHORIZED CHARGES. Charges incurred for non-sport related injuries/illnesses. Emergency room visits for non-sport related injuries/illnesses. Surgery for any non-sport related injury/illness. Surgery or treatment for any type of congenital disorder or illness, any pre-existing condition prior to becoming an University of Washington student-athlete (e.g. an injury sustained in high school athletics, appendicitis, etc). Pregnancy tests, STD tests, acne medications, etc. Other Important Information ICA is not responsible for missed appointment charges. ICA is not responsible for late fees or collection fees. ICA is not responsible for replacing lost or stolen durable medical equipment such as orthotics, braces, etc. ICA is not responsible for replacing lost or stolen prescription medication.
