This is to help mitigate the University s administrative costs in undertaking this procedure. Please note that all requested enclosures are a requirement of the application process. We will be unable to consider your application until all requested documentation is received. Please provide a current Statement of your Company s Accounts, copies of the last 2 years Audited Accounts for your Company and a copy of your most recent Annual Report. This evidence is required for the application to be approved. (If you are a Subsidiary Company, please provide, in addition, the same documents but for your Parent Company) Please confirm that none of the following circumstances, as set out in Article 20 of EU Directive 93/36/EEC, apply to your Company: Being Bankrupt, or being Wound-Up, or in the Hands of Administrators, or being the subject of Proceedings for a Declaration of Bankruptcy, or Compulsory Winding-up. All contractors must register with the Contractors Health and Safety Assessment Scheme (CHAS) who will conduct the assessment. To apply for a CHAS assessment, please visit Once successful you will receive a confirmation letter of compliance. Please include a copy of this letter with your application as proof of compliance. This letter is required for the completion of your application Please ensure that all company employees who are likely to work on the University estate will be made aware of the Code of Safe Working Practices for Contractors document which should accompany this application.
